The Utility of Using FilmArray Meningitis-Encephalitis PCR Panel (MEP) in Children Admitted With Meningitis and Encephalitis: Is It a Useful Stewardship Tool?

OBJECTIVE: FilmArray Meningitis-Encephalitis Polymerase Chain Reaction (PCR) Panel (MEP) allows for rapid detection or exclusion of pathogens causing meningitis and encephalitis (ME) in neonates and children. Studies showed its high sensitivity and specificity as a tool in diagnosing ME. We conducted a retrospective study in our institution to evaluate MEP usage in our pediatric population, assess the agreement between MEP and cerebrospinal fluid (CSF) culture, and measure the impact of MEP results on discontinuing empiric antimicrobials. METHODS: We reviewed electronic records of patients ag

Temporal dynamics of the cerebrospinal fluid proteome in tick-borne encephalitis and enteroviral meningitis.

BACKGROUND: Tick-borne encephalitis (TBE) is a severe viral infection of the central nervous system (CNS), causing acute meningoencephalitis and often long-term neurocognitive sequelae. The lack of specific treatment for TBE highlights the need to improve understanding of disease mechanisms and identify novel biomarkers. Enteroviral meningitis is typically a milder infection of the CNS and rarely causes encephalitis. OBJECTIVES: In this hypothesis-generating study, we aimed to characterise temporal changes in the cerebrospinal fluid (CSF) proteome in TBE and identify candidate biomarkers asso

Varicella zoster virus meningitis with a dermatomal rash in immunocompetent hosts: a systematic review and novel case report.

Varicella zoster virus (VZV) reactivation with central nervous system (CNS) involvement is an uncommon but important cause of lymphocytic meningitis in both immunocompromised and immunocompetent individuals. While cranial presentations, especially V1 involvement, are well documented, spinal-dermatomal VZV meningitis is rarely reported. We conducted a systematic review of PubMed, Scopus, and Embase using PRISMA guidelines to identify all cases of laboratory-confirmed VZV meningitis or encephalitis with a dermatomal rash in immunocompetent adults affecting spinal levels C1 and below. Data extrac

Cytomegalovirus (CMV) Central Nervous System Infection and Recurrent Posterior Circulation Ischemic Strokes in an Immunocompetent Patient.

Cytomegalovirus (CMV) infection in an immunocompetent adult is rarely symptomatic. Neurological manifestations are uncommon and are predominantly characterized by meningitis, myelitis, and acute polyradiculoneuritis. We report a rare case of an immunocompetent patient who presented with prolonged headaches complicated by recurrent posterior circulation ischemic strokes associated with intracranial stenoses of the basilar trunk and posterior cerebral arteries. Analysis of his cerebrospinal fluid (CSF) revealed lymphocytic meningitis with moderate elevation of the CSF protein level, normal CSF g

Echovirus 19 Molecular Epidemiology in Russia and Some Central Asian Countries in 1999-2024.

Echovirus 19 (E19) is a rarely reported non-polio enterovirus (NPEV), although it has been associated with a spectrum of severe syndromes, including encephalitis, meningitis, and multisystem hemorrhagic disease in infants. Moreover, Echovirus 19 has been detected in patients with acute flaccid paralysis (AFP), suggesting a potential association between viral infection and CNS damage. Here we describe molecular epidemiology of Echovirus 19 in Russia and Central Asia from 1999 to 2024 using data on poliomyelitis surveillance. Echovirus 19 accounted for no more than 4% of annual NPEV isolates, ho

Occurrence of Sensorineural Hearing Loss in Viral Meningitis and Encephalitis.

Hearing loss in viral neuroinfections is poorly investigated. The present study aimed to clarify if hearing loss can develop in viral meningitis and encephalitis. An OAE (oto-acoustic emission) was measured in adult patients with viral meningitis and encephalitis on the day of admission and 30-60 days after discharge. Frequencies were categorized as Low (1, 1.5, 2 kHz), Mid (3, 4, 5 kHz), Mid-high (6, 7, 8 kHz), and High (9, 10 kHz). An OAE was measured in 41 patients with viral meningitis and 8 patients with encephalitis. Six patients with viral meningitis (15%) experienced a decrease of > 5

Application of procalcitonin in pediatric emergency and critical care settings.

This expert consensus on the clinical application of procalcitonin (PCT) in pediatric emergency and critical care was convened by the Taiwan Society of Pediatric Emergency Medicine (TSPEM) and formulated by a committee of 14 leading experts. In pediatric emergency and critical care medicine, distinguishing between bacterial and viral etiologies remains a significant challenge, as clinical symptoms often lack specificity. While traditional biomarkers such as white blood cell count (WBC) and Creactive protein (CRP) provide reference values, they possess limitations regarding diagnostic timelines

In vitro inhibition of STAT5 signaling limits CD8⁺ T cell exhaustion and improves outcomes in adoptive T-cell therapy.

T cell exhaustion remains a barrier to adoptive cell therapies. Although γc cytokines are used to expand T cells, their impact on T cell states remains incompletely defined. Using an in vitro model of repetitive TCR stimulation, we found that high-dose IL-2 or IL-15 promoted exhausted CD8⁺ T cell (TEX) differentiation, with increased inhibitory receptor expression and loss of stem-like features. During chronic LCMV Clone 13 infection, high IL2Rβ expression marked virus-specific CD8⁺ T cells with features of terminally exhausted TEX cells, whereas IL2Rβ deficiency favored progenitor-like popula

Continuous Intravenous Acyclovir as Outpatient Parenteral Antimicrobial Therapy for HSV and VZV Neurological Infections: A Case Series With Pharmacokinetic and Cost Evaluation, and Review of Literature.

Varicella zoster virus (VZV) and herpes simplex virus (HSV) are major causes of viral meningitis and encephalitis. Standard intermittent intravenous acyclovir requires hospitalisation, reducing patient comfort and prolonging admissions. Continuous infusion may maintain therapeutic plasma and cerebrospinal fluid (CSF) concentrations while enabling treatment through Outpatient Parenteral Antimicrobial Therapy (OPAT). Objectives to evaluate the feasibility, pharmacokinetic target attainment, and cost reduction of continuous intravenous acyclovir for HSV and VZV neurological infections in an OPAT

Investigating CD8 T Cell Exhaustion in Chronic LCMV Infection by Flow Cytometry.

CD8 T cells and the process of T cell exhaustion are central topics in both basic immunology and translational research, including clinical applications. Here, we provide a comprehensive, step-by-step guide to investigate CD8 T cells in murine in vivo lymphocytic choriomeningitis virus (LCMV) infection models. We outline viral stock preparation, infection, and viral titer evaluation. We put an emphasis on flow cytometry as a method to investigate phenotypical and functional characteristics of CD8 T cell exhaustion, but the protocols provided will also facilitate imaging or transcriptomic analy

Transmission of mutated SARS-CoV-2 variants is favored by relatively prolonged infections due to delayed immunity.

SARS-CoV-2 evolution enhanced viral fitness and immune evasion, extending the COVID-19 pandemic and resulting in millions of excess deaths. Viral diversity is generated within infected individuals, yet the timing and interplay of viral and immunological forces that drive transmissible evolution are incompletely understood. We developed a multi-scale within host phylodynamic (WiPhy) model of SARS-CoV-2 infection which couples viral replication, innate and acquired immune responses, and viral mutation. We then validated the model against quantitative viral and phylodynamic metrics. Model output

A Multisystem Presentation of West Nile Neuroinvasive Disease: Lessons in Diagnostic Complexity.

West Nile virus (WNV) is an uncommon but serious case of encephalitis and meningitis, often presenting with nonspecific symptoms that can mimic a wide range of infectious, metabolic, and endocrine disorders. We present the case of a 61-year-old male with multiple comorbidities who arrived with 5 days of fever, chills, profuse diarrhea, black stools, and worsening confusion. Initial evaluation in the emergency department revealed hyponatremia, transaminitis, and concern for tick-borne illness, prompting empiric doxycycline. However, the patient denied any tick bites. Given his known thyroid nod

Safety first, science second: Healthcare workers' conditional trust and willingness to participate in Lassa fever vaccine trials in Nigeria.

BACKGROUND: Lassa fever poses a persistent occupational risk to healthcare workers (HCWs) in Nigeria. While HCWs are pivotal to vaccine trial success, their decision-making in settings shaped by historical research controversies remains poorly understood. This study explored the conditions under which HCWs at Aminu Kano Teaching Hospital (AKTH), northern Nigeria, considered participation in Lassa fever vaccine trials acceptable. METHODS: We conducted a descriptive qualitative study using semi-structured in-depth interviews with 20 physicians, nurses, and midwives at AKTH between September and

Halo-Pelvic Traction in Severe Adult Spinal Deformity: A Systematic Review and Meta-Analysis.

UNLABELLED: Early-stage Hodgkin's lymphoma (ESHL) is highly curable, usually with a combination of chemotherapy and radiation. Real-world data may show differences in survival and prognostic factors when compared to clinical trials. There is limited published literature on ESHL from India. The data on the baseline characters, treatment, and outcomes of patients with ESHL (stage IA, IB, and IIA) were obtained from five institutions' medical records and entered in a common database. Event-free survival (EFS) and overall survival (OS) were estimated using the Kaplan Meier method, and cox-regressi

Efficacy, Immunogenicity, and Safety of an Investigational Maternal Respiratory Syncytial Virus Prefusion F Protein-Based Vaccine.

BACKGROUND: In this phase 3 trial of an investigational maternal respiratory syncytial virus prefusion F protein-based vaccine (RSVPreF3-Mat), a higher rate of preterm birth was observed in the vaccine (6.8%) versus the placebo group (4.9%). Trial enrollment and vaccination were stopped. Results of investigations into this safety signal were reported previously. Here, we describe end-of-trial efficacy, immunogenicity, and safety results. METHODS: Women 18-49 years old were randomized 2:1 to receive 1 dose of RSVPreF3-Mat (n = 3557) or placebo (n = 1771) at 240/7-340/7 weeks' gestation. Primar

Designing of a multi-epitope vaccine targeting enterovirus D68: An integrated immunoinformatic and reverse vaccinology approach.

Enterovirus D68 (EV-D68) is an enterovirus known for causing respiratory infections, as well as flaccid myelitis, meningitis and encephalitis. Despite the efforts, no licensed vaccine against EV-D68 is currently available. Vaccine development efforts are ongoing; however, the process is complex and requires extensive clinical validation. In contrast, immunoinformatic is a rapidly expanding area with the potential to significantly influence the therapeutic interventions and vaccine development for infectious diseases. Herein, immunoinformatic and reverse vaccinology strategies were utilized to
