A novel mycovirus identified in clinical isolates of Cryptococcus neoformans.

Mycoviruses are widespread throughout the fungal kingdom and are known to infect diverse fungal taxa, including fungal species that are important plant and human pathogens. Although many mycoviruses have been found to have minimal effects on their host, several viruses have been found to modulate fungal physiology, and, as a result, impact fungal virulence. Screens for mycoviruses in clinically relevant fungi have identified numerous mycoviruses within several important human pathogens, but mycoviruses remain uncharacterized in the clinically relevant human pathogen Cryptococcus neoformans. Infection-specific long-chain fatty acid metabolism as a broad anti-enterovirus target.

Enteroviruses are arguably the most numerous group of viruses infecting humans. While most enterovirus infections are benign and self-resolving, their sheer number inevitably increases the chances of multiple complications. The diversity of enteroviruses means that the development of vaccines is only economically feasible against a select few, and no direct-acting or host-targeted anti-virals are approved to treat enteroviral infections, largely due to the rapid development of resistance against all experimental drugs. Here, we explored a universal property of enterovirus infection-a massive u

Human astroviruses: molecular virology.

SUMMARYHuman astroviruses (HAstVs) are important etiologic agents of pediatric gastroenteritis and are increasingly recognized as pathogens associated with extraintestinal disease, including encephalitis and meningitis, particularly in immunocompromised individuals. Despite their clinical relevance, widespread prevalence, and expanding genetic diversity, no approved vaccines or antiviral therapies are available. Progress toward effective interventions requires a comprehensive understanding of HAstV molecular biology, including mechanisms of cell entry, viral replication, and immune recognition

The hidden burden of viral meningitis in ethiopia: Etiology and clinical outcomes.

BACKGROUND: Viral meningitis remains a significant global public health problem, particularly in resource-limited settings where the diagnosis of viral etiology is mainly based on clinical assessment. This study aimed to identify the common viral etiologies of meningitis, describe their associated clinical features, and assess patient outcomes among individuals suspected of meningitis without evidence of bacterial meningitis. METHODS: We conducted a multicenter study involving 430 patients clinically suspected of meningitis. Clinical data and cerebrospinal fluid samples (CSF) were collected f

Respiratory Viral Positivity Is Associated with Decreased Risk of Serious Bacterial Infections in Febrile Infants.

INTRODUCTION: Febrile infants ≤ 60 days of age are at increased risk for serious bacterial infections (SBIs), often necessitating extensive diagnostic evaluation and hospitalization. Although respiratory viral infections are common in this population and may influence the likelihood of concurrent bacterial infection, the relationship between respiratory viral panel (RVP) results and SBI risk remains an important consideration in clinical decision-making. The objective of this study was to evaluate the association between RVP results and the occurrence of SBIs among febrile infants ≤ 60 days of

HIV-1 compartmentalization in the central nervous system during cryptococcal meningitis: Analysis of viral, fungal, and host factors.

Opportunistic infections in the central nervous system (CNS), such as cryptococcal meningitis (CM) have profound consequences for people living with HIV-1 (PWH), including blood-brain barrier impairment and neuroinflammation. To investigate how CM may shape the HIV-1 populations in the CNS, we characterized viral variants in the blood and cerebrospinal fluid (CSF) compartments in PWH with active CM. Thirty-nine participants with CM had paired plasma and CSF viral load measurements prior to the start of anti-retroviral therapy (pre-ART), with overall significantly higher HIV-1 viral loads in pl

The Utility of Using FilmArray Meningitis-Encephalitis PCR Panel (MEP) in Children Admitted With Meningitis and Encephalitis: Is It a Useful Stewardship Tool?

OBJECTIVE: FilmArray Meningitis-Encephalitis Polymerase Chain Reaction (PCR) Panel (MEP) allows for rapid detection or exclusion of pathogens causing meningitis and encephalitis (ME) in neonates and children. Studies showed its high sensitivity and specificity as a tool in diagnosing ME. We conducted a retrospective study in our institution to evaluate MEP usage in our pediatric population, assess the agreement between MEP and cerebrospinal fluid (CSF) culture, and measure the impact of MEP results on discontinuing empiric antimicrobials. METHODS: We reviewed electronic records of patients ag

Temporal dynamics of the cerebrospinal fluid proteome in tick-borne encephalitis and enteroviral meningitis.

BACKGROUND: Tick-borne encephalitis (TBE) is a severe viral infection of the central nervous system (CNS), causing acute meningoencephalitis and often long-term neurocognitive sequelae. The lack of specific treatment for TBE highlights the need to improve understanding of disease mechanisms and identify novel biomarkers. Enteroviral meningitis is typically a milder infection of the CNS and rarely causes encephalitis. OBJECTIVES: In this hypothesis-generating study, we aimed to characterise temporal changes in the cerebrospinal fluid (CSF) proteome in TBE and identify candidate biomarkers asso

Varicella zoster virus meningitis with a dermatomal rash in immunocompetent hosts: a systematic review and novel case report.

Varicella zoster virus (VZV) reactivation with central nervous system (CNS) involvement is an uncommon but important cause of lymphocytic meningitis in both immunocompromised and immunocompetent individuals. While cranial presentations, especially V1 involvement, are well documented, spinal-dermatomal VZV meningitis is rarely reported. We conducted a systematic review of PubMed, Scopus, and Embase using PRISMA guidelines to identify all cases of laboratory-confirmed VZV meningitis or encephalitis with a dermatomal rash in immunocompetent adults affecting spinal levels C1 and below. Echovirus 19 Molecular Epidemiology in Russia and Some Central Asian Countries in 1999-2024.

Echovirus 19 (E19) is a rarely reported non-polio enterovirus (NPEV), although it has been associated with a spectrum of severe syndromes, including encephalitis, meningitis, and multisystem hemorrhagic disease in infants. Moreover, Echovirus 19 has been detected in patients with acute flaccid paralysis (AFP), suggesting a potential association between viral infection and CNS damage. Here we describe molecular epidemiology of Echovirus 19 in Russia and Central Asia from 1999 to 2024 using data on poliomyelitis surveillance. Echovirus 19 accounted for no more than 4% of annual NPEV isolates, ho

Occurrence of Sensorineural Hearing Loss in Viral Meningitis and Encephalitis.

Hearing loss in viral neuroinfections is poorly investigated. The present study aimed to clarify if hearing loss can develop in viral meningitis and encephalitis. An OAE (oto-acoustic emission) was measured in adult patients with viral meningitis and encephalitis on the day of admission and 30-60 days after discharge. Frequencies were categorized as Low (1, 1.5, 2 kHz), Mid (3, 4, 5 kHz), Mid-high (6, 7, 8 kHz), and High (9, 10 kHz). An OAE was measured in 41 patients with viral meningitis and 8 patients with encephalitis. Six patients with viral meningitis (15%) experienced a decrease of > 5

Application of procalcitonin in pediatric emergency and critical care settings.

This expert consensus on the clinical application of procalcitonin (PCT) in pediatric emergency and critical care was convened by the Taiwan Society of Pediatric Emergency Medicine (TSPEM) and formulated by a committee of 14 leading experts. In pediatric emergency and critical care medicine, distinguishing between bacterial and viral etiologies remains a significant challenge, as clinical symptoms often lack specificity. While traditional biomarkers such as white blood cell count (WBC) and Creactive protein (CRP) provide reference values, they possess limitations regarding diagnostic timelines

In vitro inhibition of STAT5 signaling limits CD8⁺ T cell exhaustion and improves outcomes in adoptive T-cell therapy.

T cell exhaustion remains a barrier to adoptive cell therapies. Although γc cytokines are used to expand T cells, their impact on T cell states remains incompletely defined. Using an in vitro model of repetitive TCR stimulation, we found that high-dose IL-2 or IL-15 promoted exhausted CD8⁺ T cell (TEX) differentiation, with increased inhibitory receptor expression and loss of stem-like features. During chronic LCMV Clone 13 infection, high IL2Rβ expression marked virus-specific CD8⁺ T cells with features of terminally exhausted TEX cells, whereas IL2Rβ deficiency favored progenitor-like popula

Continuous Intravenous Acyclovir as Outpatient Parenteral Antimicrobial Therapy for HSV and VZV Neurological Infections: A Case Series With Pharmacokinetic and Cost Evaluation, and Review of Literature.

Varicella zoster virus (VZV) and herpes simplex virus (HSV) are major causes of viral meningitis and encephalitis. Standard intermittent intravenous acyclovir requires hospitalisation, reducing patient comfort and prolonging admissions. Continuous infusion may maintain therapeutic plasma and cerebrospinal fluid (CSF) concentrations while enabling treatment through Outpatient Parenteral Antimicrobial Therapy (OPAT). Objectives to evaluate the feasibility, pharmacokinetic target attainment, and cost reduction of continuous intravenous acyclovir for HSV and VZV neurological infections in an OPAT

Investigating CD8 T Cell Exhaustion in Chronic LCMV Infection by Flow Cytometry.

CD8 T cells and the process of T cell exhaustion are central topics in both basic immunology and translational research, including clinical applications. Here, we provide a comprehensive, step-by-step guide to investigate CD8 T cells in murine in vivo lymphocytic choriomeningitis virus (LCMV) infection models. We outline viral stock preparation, infection, and viral titer evaluation. We put an emphasis on flow cytometry as a method to investigate phenotypical and functional characteristics of CD8 T cell exhaustion, but the protocols provided will also facilitate imaging or transcriptomic analy
