Transmission of mutated SARS-CoV-2 variants is favored by relatively prolonged infections due to delayed immunity.

SARS-CoV-2 evolution enhanced viral fitness and immune evasion, extending the COVID-19 pandemic and resulting in millions of excess deaths. Viral diversity is generated within infected individuals, yet the timing and interplay of viral and immunological forces that drive transmissible evolution are incompletely understood. We developed a multi-scale within host phylodynamic (WiPhy) model of SARS-CoV-2 infection which couples viral replication, innate and acquired immune responses, and viral mutation. We then validated the model against quantitative viral and phylodynamic metrics. Model output

A Multisystem Presentation of West Nile Neuroinvasive Disease: Lessons in Diagnostic Complexity.

West Nile virus (WNV) is an uncommon but serious case of encephalitis and meningitis, often presenting with nonspecific symptoms that can mimic a wide range of infectious, metabolic, and endocrine disorders. We present the case of a 61-year-old male with multiple comorbidities who arrived with 5 days of fever, chills, profuse diarrhea, black stools, and worsening confusion. Initial evaluation in the emergency department revealed hyponatremia, transaminitis, and concern for tick-borne illness, prompting empiric doxycycline. However, the patient denied any tick bites. Given his known thyroid nod

Safety first, science second: Healthcare workers' conditional trust and willingness to participate in Lassa fever vaccine trials in Nigeria.

BACKGROUND: Lassa fever poses a persistent occupational risk to healthcare workers (HCWs) in Nigeria. While HCWs are pivotal to vaccine trial success, their decision-making in settings shaped by historical research controversies remains poorly understood. This study explored the conditions under which HCWs at Aminu Kano Teaching Hospital (AKTH), northern Nigeria, considered participation in Lassa fever vaccine trials acceptable. METHODS: We conducted a descriptive qualitative study using semi-structured in-depth interviews with 20 physicians, nurses, and midwives at AKTH between September and

Halo-Pelvic Traction in Severe Adult Spinal Deformity: A Systematic Review and Meta-Analysis.

UNLABELLED: Early-stage Hodgkin's lymphoma (ESHL) is highly curable, usually with a combination of chemotherapy and radiation. Real-world data may show differences in survival and prognostic factors when compared to clinical trials. There is limited published literature on ESHL from India. The data on the baseline characters, treatment, and outcomes of patients with ESHL (stage IA, IB, and IIA) were obtained from five institutions' medical records and entered in a common database. Event-free survival (EFS) and overall survival (OS) were estimated using the Kaplan Meier method, and cox-regressi

Designing of a multi-epitope vaccine targeting enterovirus D68: An integrated immunoinformatic and reverse vaccinology approach.

Enterovirus D68 (EV-D68) is an enterovirus known for causing respiratory infections, as well as flaccid myelitis, meningitis and encephalitis. Despite the efforts, no licensed vaccine against EV-D68 is currently available. Vaccine development efforts are ongoing; however, the process is complex and requires extensive clinical validation. In contrast, immunoinformatic is a rapidly expanding area with the potential to significantly influence the therapeutic interventions and vaccine development for infectious diseases. Herein, immunoinformatic and reverse vaccinology strategies were utilized to

Spectrum of Pediatric Acute Febrile Encephalopathy in Hilly Areas of Northwestern Himalayas.

BACKGROUND AND AIM: The etiological profile of acute febrile encephalopathy (AFE) varies across different geographic areas and in different seasons across the world. Limited literature in children suggests central nervous system (CNS) infections to be the most common cause of AFE in India and developing countries. Most of these studies have been done in the plains of India, and no study has been done in the Himalayan region of northwestern India, where most of the vectors causing AFE in other parts of India, like mosquitoes, are not found. Therefore, this leads to a lacuna in available data on

The Use of CSF Multiplex PCR Panel in Patients with Viral Uveitis.

Background/Objectives: Polymerase chain reaction (PCR) testing of ocular fluids is an essential diagnostic method for identifying infectious causes of uveitis. However, multiplex PCR kits specifically developed for ophthalmic use are not commercially available in many regions, including Korea. Given the biochemical similarity between cerebrospinal fluid (CSF) and aqueous humor, this study evaluated the diagnostic utility of a commercially available CSF multiplex PCR panel for detecting herpesviruses in patients with suspected viral uveitis. Methods: We retrospectively reviewed the medical reco

Pathological characteristics of coxsackievirus A14 in neonatal mice model.

In recent years, with the increasing incidence of other non-EV-A71 enteroviruses, the research on other enteroviruses has become more and more important. Coxsackievirus A14 (CVA14), A member of the enterovirus group A, causes hand-foot-mouth disease (HFMD), aseptic meningitis and acute abdominal pain. In this study, we established the first ICR mouse model of CVA14 infection. The CVA14 HE27 strain was selected to infect neonatal mice through a preliminary experiment, and the infection model was established according to three different conditions of infection dose, route and age. It was found t

Lineage-matched Oropouche virus mRNA-LNP vaccines confer complete, cross-protective immunity in mice.

UNLABELLED: Oropouche virus (OROV) is an emerging orthobunyavirus responsible for recurrent and increasingly widespread outbreaks across South and Central America, including a recent epidemic that elevated global concern. In addition to its known neurotropic potential-manifesting as meningitis and encephalitis-OROV is now associated with vertical transmission and adverse pregnancy outcomes, underscoring its growing clinical and public health relevance. The rapid geographic expansion and extensive genetic diversification of circulating OROV lineages highlight the need for vaccine platforms that

Neurological Complications Associated With COVID-19 Compared to Other Viral Infections: A Systematic Review of Current Evidence.

Neurological complications have become one of the most concerning features of COVID-19, yet clinicians still lack a clear comparison between these findings and what is seen in other viral infections. Understanding where SARS-CoV-2 fits, whether it behaves like influenza and dengue or follows an entirely different pattern, is essential for diagnosis, management, and planning long-term care. We conducted a systematic review following Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines (PROSPERO: CRD420251064831). Mumps Virus: Replication, Immune Response, and the Changing Landscape of Vaccine Effectiveness.

Mumps virus (MuV) is a single-stranded, negative-sense RNA virus of the Family Paramyxoviridae. MuV is a highly contagious human pathogen that causes primarily mild symptoms, including hallmark swelling of the parotid glands. Severe cases can occur, leading to neurological complications, including deafness, meningitis, and encephalitis. The mumps vaccine, now included in combination with measles and rubella vaccines (MMR), was first made available in the 1960s. After its introduction, mumps incidence dropped dramatically to less than 500 cases annually in the US. However, even with long-standi

Genetic Diversity of the Non-Polio Enteroviruses Detected in Samples of Patients with Aseptic Meningitis in the Ural Federal District and Western Siberia.

Human non-polio enteroviruses (NPEVs) cause a plethora of infections in humans, ranging from mild to severe neurological diseases including aseptic meningitis. NPEVs are the leading cause of aseptic meningitis in both children and adults worldwide. In Russia, reports of NPEV infections have surged, especially in the post-COVID era starting in 2022, with elevated infection rates into 2023. A comprehensive examination of the whole genome is crucial for understanding the evolution of NPEV genes and for predicting potential outbreaks. This study focused on identifying the circulating NPEV strains

A Mysterious Case of Autoimmune Hepatitis Triggered by Herpes Zoster Virus Versus Drug-Induced Liver Injury.

Autoimmune hepatitis triggered by Herpes Zoster infection is a relatively rare condition and a diagnosis of exclusion, as there are other more common causes of autoimmune hepatitis. While evaluating a patient with suspected autoimmune hepatitis, common causes such as viral hepatitis, Epstein-Barr virus, and medications like amoxicillin-clavulanic acid, statins, nitrofurantoin and methyldopa must be ruled out. Furthermore, autoimmune processes like thyroiditis, type 1 diabetes mellitus, rheumatoid arthritis, and ulcerative colitis should be considered. Herpes Zoster virus is known to cause dise
