Cyclosporin-A (CsA) was given to 22 patients who received allogeneic bone marrow transplants as therapy for aplastic anemia and hematologic malignancies. The drug was given daily for 180 days starting with the day of marrow infusion. Engraftment was not impaired and myelotoxicity was not observed. Cutaneous graft-versus-host disease (GVHD) developed in five patients and all either spontaneously resolved or promptly responded to therapy with steroids. Total body irradiation is being used with increasing frequency for the treatment of lymphopoietic malignancies and in preparation for marrow transplantation. Acute toxicities include reversible gastroenteritis, mucositis, myelosuppression and alopecia. Two patients with small cell carcinoma of the lung treated by chemoradiotherapy developed, diffuse interstitial pneumonitis after 7 and 21 months of treatment while in complete remission of their malignant disease. One patient died after an acute respiratory distress syndrome, but the second improved after steroid therapy and discontinuation of cytotoxic therapy. Pulmonary toxicity appeared to be related to cyclophosphamide therapy, but radiation therapy could have potentiated cyclophosphamide toxicity as it has been shown in bleomycin lung disease.

Aplastic anemia is the most severe hematologic side-effect. All chemotherapeutic agents, with the exception of bleomycin and L-asparaginase, may induce aplasia, but the degree of hematotoxicity varies according to the drug. Radiogallium imaging plays a growing role in the investigation of acute and chronic inflammatory diseases of the lung. We studied a series of patients undergoing radiation treatment for carcinoma of the breast to evaluate the usefulness of imaging with 67gallium citrate in the detection of early radiation changes in the pulmonary parenchyma. In a series of 12 patients there was no correlation between the clinical and radiographic manifestations of radiation pneumonitis and the gallium scan.
