Acute Pazopanib-Associated Interstitial Lung Disease in a Patient With Ulcerative Colitis and Metastatic Sarcoma: A Case Report.

Pazopanib is used in the treatment of advanced renal cell carcinoma and soft tissue sarcoma. While pneumothorax is a recognized pulmonary complication, interstitial lung disease (ILD) typically occurs after months of therapy. We present a rare case of acute onset ILD occurring within 72 hours of initiating pazopanib in a 43-year-old male with metastatic sarcoma and a history of ulcerative colitis. The patient developed rapidly progressive hypoxemic respiratory failure shortly after starting pazopanib. Extensive infectious and autoimmune workups were negative. Imaging findings, combined with a

Lung organoids for respiratory diseases: overcoming translational hurdles in drug discovery and safety assessment.

Respiratory diseases, encompassing chronic inflammatory conditions, interstitial fibrotic disorders, acute infectious diseases, and pulmonary malignancies, represent a profound global health burden with unacceptably high morbidity and mortality rates. Historically, the pharmaceutical pipeline for respiratory therapeutics has suffered staggering attrition rates during clinical development. This is primarily due to the fundamental inability of conventional two-dimensional cell cultures and in vivo animal models to faithfully recapitulate the complex three-dimensional architecture, multicellular

N-acetylcysteine: evidence based consensus document on the therapeutic advantages in respiratory diseases (NECTAR).

BACKGROUND: N-acetylcysteine (NAC) is a key precursor of glutathione (GSH), the lung's principal antioxidant. First developed as a mucolytic, NAC is now recognized for broader antioxidant, anti-inflammatory, immunomodulatory, and anti-biofilm effects, prompting its use as an adjuvant in treatment of chronic respiratory conditions. This document consolidates existing evidence to this effect, and adds insights from practicing clinicians to guide about use of NAC in clinical practice. METHODS: An expert working group (EWG) comprising ten pulmonologists and a multidisciplinary drafting committee

Diffuse lung diseases ascribed to drugs: a nationwide observational study over 37 years using the French Pharmacovigilance Database.

BACKGROUND: Drug-induced interstitial lung disease (DI-ILD) is a heterogeneous subgroup of interstitial lung disease (ILD). The number of molecules involved is increasing with time. Due to their low incidence, DI-ILDs may be detected only after a drug has been marketed, notably through adverse drug reaction (ADR) reports to pharmacovigilance centres. The aim of our study was to describe drug-induced diffuse lung disease cases notified to and recorded by the French Pharmacovigilance Database (FPVD), reported clinical pictures and the potentially causal drugs. METHODS: This retrospective study

Acute renal injury induced by ensartinib in a lung adenocarcinoma patient treated with sequential ALK inhibitors: A case report and literature review.

INTRODUCTION: Lung cancer ranks among the foremost causes of mortality associated with cancer. Ensartinib is a highly effective oral anaplastic lymphoma kinase (ALK) inhibitor utilized in the treatment of ALK-positive lung adenocarcinoma. This report presents a case of acute renal failure attributed to the administration of ensartinib. This is the first case report documenting the nephrotoxic effects of ensartinib, specifically manifesting as acute tubulointerstitial nephritis (ATIN) and IgA nephropathy. CASE PRESENTATION: This report details the case of a 52-year-old man diagnosed with ALK-p

[A CASE OF IDIOPATHIC PULMONARY FIBROSIS WITH SUSPECTED DRUG-INDUCED LUNG INJURY FOLLOWING ACUTE EXACERBATION AFTER NINTEDANIB ADMINISTRATION].

A 74-year-old man was treated with nintedanib for idiopathic pulmonary fibrosis (IPF). Thirty-six days after starting to take nintedanib, he admitted to our hospital due to respiratory failure with ground-glass opacities and mediastinal emphysema on chest CT. Acute exacerbation of IPF was suspected. Because lung injury due to nintedanib was suspected, nintedanib was discontinued. Although he was treated with pulse corticosteroid therapy, interstitial pneumonia and mediastinal emphysema deteriorated. Despite continuing corticosteroid therapy, he died 21 days after admission due to respiratory f

Drug-induced Lung Injury Caused by Azacitidine That Was Not Treated with Drugs.

An 83-year-old male was hospitalized with pneumonia while receiving azacitidine for myelodysplastic syndrome. Despite treatment with antimicrobial agents, his pneumonia worsened. Bronchoscopy revealed organizing pneumonia (OP). This condition was thought to be a drug-induced lung injury caused by azacitidine. As the patient had no subjective symptoms, antimicrobial therapy was discontinued and the patient was monitored closely. Thereafter, the pneumonia gradually improved. Drug-induced lung injury caused by azacitidine, which has few subjective symptoms, can be treated without steroids. Howeve

Concurrent interstitial pneumonia and acute hepatitis induced by mesalazine.

Ulcerative colitis includes gastrointestinal and, in some cases, extraintestinal symptoms, potentially affecting any organ. Diagnosing such extraintestinal involvement can be challenging, especially when considering alternative aetiologies, including drug-induced toxicity. We report the case of an octogenarian male recently diagnosed with ulcerative colitis and undergoing treatment with mesalazine who was diagnosed with non-specific interstitial pneumonia due to respiratory symptoms. Subsequently, he presented with mucocutaneous jaundice, prompting a comprehensive evaluation, including a liver

Daptomycin and acute lung injury: recognising drug-induced eosinophilic pneumonia.

Daptomycin-induced eosinophilic pneumonia (DIEP) is a rare medication adverse event that can lead to severe respiratory distress and clinical decompensation. Herein, we report a man in his 70s who was being treated with outpatient intravenous daptomycin for foot osteomyelitis and presented with fevers and shortness of breath. His clinical exam and imaging favoured an infectious pneumonia. However, the patient's respiratory and clinical status continued to decline despite broad-spectrum antibiotics. A diagnosis of DIEP was established by elevated peripheral eosinophil count and >25% eosinophils

Pulmonary Complications in Connective Tissue Disease-Associated Interstitial Lung Disease.

Interstitial lung disease (ILD) associated with connective tissue disease (CTD) is a challenging entity burdened with multiple risk factors and undesirable events. ILD can occur at any time and progress regardless of the underlying disease activity. Apart from the established difficulties in choosing an appropriate treatment strategy for both pulmonary and extrapulmonary involvement, such patients require a holistic and multidisciplinary approach. In an attempt to emphasize the significance of the problem and suggest potential research directions, the authors present four potentially most impo

Acute interstitial pneumonitis associated with daratumumab treatment.

Daratumumab is a monoclonal antibody targeting CD38 and is widely used in the treatment of multiple myeloma. While generally well tolerated, pulmonary toxicity is a rare and under-recognised adverse effect. We present the case of an elderly woman with multiple myeloma on maintenance daratumumab therapy who developed subacute hypoxic respiratory failure. Imaging revealed new-onset diffuse bilateral ground-glass opacities. Bronchoscopy with lavage was non-diagnostic. Extensive infectious and rheumatologic workup was unrevealing. The patient improved significantly with corticosteroids and discont

TMP-SMX-associated acute respiratory distress syndrome with DAIDE pathology in a Korean patient lacking HLA-B*07:02 and C*07:02: A case report.

RATIONALE: Trimethoprim-sulfamethoxazole (TMP-SMX)-induced acute respiratory distress syndrome (ARDS) is a rare but potentially fatal adverse drug reaction, most commonly reported in young, otherwise healthy individuals. Although recent studies from the United States have identified a strong association with human leukocyte antigen (HLA)-B07:02 and C07:02 alleles, no cases have been reported in East Asian populations. PATIENT CONCERNS: A 37-year-old Korean male with rheumatoid arthritis-associated interstitial lung disease developed rapidly progressive dyspnea and hypoxemia after receiving TM

Noninfectious Severe Pulmonary Complications in Immunocompromised Critically Ill Patients.

Noninfectious pulmonary complications are a significant cause of morbidity and mortality in immunocompromised patients, particularly in those undergoing hematopoietic stem cell transplantation, solid organ transplantation, chemotherapy, or immunotherapy. These syndromes often mimic infections, leading to delayed diagnosis and inappropriate treatment. Acute complications include peri-engraftment respiratory distress syndrome, diffuse alveolar hemorrhage, drug-induced lung injury, immune checkpoint inhibitor-related pneumonitis, and radiation pneumonitis, while late or chronic complications, suc

CT findings of lung injury during breast cancer treatment.

While breast cancer treatment outcomes have improved significantly through multidisciplinary approaches including surgery, chemotherapy, and radiation therapy, the incidence of non-neoplastic pulmonary complications has also increased. Accurate interpretation of chest imaging is essential for managing these adverse events. This review outlines the major radiological findings of pulmonary injury during breast cancer treatment, focusing on two primary categories: drug-associated interstitial lung disease (DILD) and radiotherapy-associated lung injury (RLI). Regarding DILD, its clinical features,

Rifampicin-associated intravascular haemolysis causing acute kidney injury.

Drug-induced acute kidney injury (AKI) is a rare adverse effect of rifampicin, and is mostly related to acute tubular necrosis and acute interstitial nephritis. We report a sputum- positive, isoniazid mono-resistant, pulmonary tuberculosis patient who had a history of anti-tuberculous therapy (ATT) intake 30 years ago. The patient developed AKI requiring dialysis when he restarted the ATT recently. A renal biopsy was consistent with pigment-cast nephropathy secondary to rifampicin-induced intravascular haemolysis. Rifampicin was stopped, and the patient underwent a total of four dialysis sessi

Exploring the impact of ADIPOQ gene variants on plasma adiponectin levels and oxidative stress in type 2 diabetes mellitus and diabetic nephropathy in South Indian population: a case-control study.

BACKGROUND AND OBJECTIVE: Warfarin is commonly involved in drug-related hospital admissions. The drug is most commonly prescribed to elderly patients in whom polypharmacy is common and, when administered in combination with other drugs such as NSAIDs, aspirin (acetylsalicylic acid) or macrolide antibacterials, is associated with increased bleeding risk. The aim of this study was to investigate the prevalence of prescriptions that might give rise to clinically relevant drug-drug interactions in a warfarinized population. METHODS: Primary care prescribing of warfarin and potentially interacting

Identification of kynurenine and quinolinic acid as promising serum biomarkers for drug-induced interstitial lung diseases.

BACKGROUND: Drug-induced interstitial lung disease (DILD) is a lung injury caused by various types of drugs and is a serious problem in both clinical practice and drug development. Clinical management of the condition would be improved if there were DILD-specific biomarkers available; this study aimed to meet that need. METHODS: Biomarker candidates were identified by non-targeted metabolomics focusing on hydrophilic molecules, and further validated by targeted approaches using the serum of acute DILD patients, DILD recovery patients, DILD-tolerant patients, patients with other related lung d

Acute hypoxic respiratory failure due to Lenalidomide-induced interstitial pneumonitis in a patient with multiple myeloma.

BACKGROUND: Patients with multiple myeloma are immunosuppressed due to both the disease itself and immunosuppressive therapies. Thus, when presenting with respiratory failure and pulmonary opacities, pneumonia must be considered. However, while rare, immunomodulating medications used in the treatment of multiple myeloma can also cause potentially life-threatening respiratory failure, a distinction which has important treatment implications. CASE PRESENTATION: An 80-year-old male with recently diagnosed multiple myeloma undergoing treatment with lenalidomide and daratumumab presented with acut

Serum stratifin measurement is useful for evaluating disease severity and outcomes in patients with acute exacerbation of interstitial lung disease: a retrospective study.

BACKGROUND: Serum levels of stratifin (SFN), a member of the 14-3-3 protein family, increase in patients with drug-induced lung injury associated with diffuse alveolar damage. Therefore, we hypothesised that SFN levels would be higher in those experiencing acute exacerbation of interstitial lung disease (AE-ILD). A secondary analysis was also planned to determine whether SFN levels could discriminate survival in those with AE. METHODS: Thirty-two patients with clinically stable ILD (CS-ILD) and 22 patients with AE-ILD were examined to assess whether high serum SFN levels were associated with
