Pulmonary Eosinophilia Mimicking Metastatic Lung Lesions on FDG PET-CT: A Diagnostic Pitfall.

Eosinophilic pneumonia (EP) is a rare disease, comprising a group of heterogeneous diseases. The 2 main subtypes are acute eosinophilic pneumonia (AEP) and chronic eosinophilic pneumonia (CEP), both of which are characterized by accumulation of eosinophils in lung tissues, BAL fluid, and metabolically active nodular lesions in FDG PET-CT scan. Here we present a case of 67-year-old woman, diagnosed with carcinoma cervix and staging whole body PET-CT scan suggestive of metabolically active nodular lesions in bilateral lungs with perilesional ground-glass haziness apart from primary lesion in cer

Effectiveness of biologics for reducing occlusive mucus plugs in patients with severe asthma: a systematic review.

Asthma is a heterogeneous disease characterized by chronic airway inflammation and reversible airflow obstruction. Particularly in severe asthma, airway mucus plugs can contribute to substantial and persistent airflow obstruction, despite inhaled corticosteroid and bronchodilator treatment. Consequently, it is important that clinicians assess and treat mucus plugs. Increased mucus production and airway eosinophilia caused by type 2 (T2) inflammation contributes to mucus plug formation and persistence. Several biologics are available to target T2 inflammation in asthma and studies have describe

Genetic predisposition to elevated total immunoglobulin E levels defines a distinct adult-onset-predominant asthma phenotype.

Asthma heterogeneity remains a major barrier in precision medicine. Although elevated total serum immunoglobulin E (IgE) is a hallmark of asthma, even in nonatopic patients, its causal role in asthma pathogenesis is debated. We hypothesized that genetic predisposition to increased IgE defines a distinct asthma endotype. A genome-wide association study of total serum IgE in 1,287 non-asthmatic Japanese adults was used to construct IgE polygenic risk scores (IgE_PRS). Applying IgE_PRS to 745 patients with asthma, we performed cluster analysis incorporating age at onset, total IgE levels, IgE_PRS

Fungal Sensitization and the Manifestation of Asthmatic Features in Chronic Obstructive Pulmonary Disease.

Objective Fungal sensitization is known to be prevalent in chronic obstructive pulmonary disease (COPD) and it is recognized as a risk factor for severe asthma. However, its relationship to the development of asthmatic features in COPD remains unclear. Because the diagnostic criteria for asthma-COPD overlap (ACO) are not standardized, we focused on the specific asthmatic features that form the diagnostic basis of ACO, namely, blood eosinophilia and recurrent wheezing, to clarify how fungal sensitization contributes to their manifestation in COPD. Methods We retrospectively analyzed data from 7

ANCA-negative EGPA with migratory pulmonary masses: A case report of mepolizumab response and literature review.

RATIONALE: Eosinophilic granulomatosis with polyangiitis (EGPA) is a small- to medium-vessel vasculitis commonly associated with asthma and eosinophilia. Misdiagnosis as isolated asthma may delay recognition and treatment. PATIENT CONCERNS: A patient with a long history of asthma presented with persistent respiratory symptoms and a pulmonary mass initially presumed to be asthma-related pathology. DIAGNOSIS: Clinical evaluation, laboratory findings of eosinophilia, and imaging supported the diagnosis of EGPA rather than isolated asthma. INTERVENTIONS: The patient received glucocorticoid ther

Subclinical pulmonary abnormalities on chest CT in patients with eosinophilic chronic rhinosinusitis.

BACKGROUND: Chronic rhinosinusitis (CRS) often coexists with lower respiratory tract diseases, and such cases tend to be more refractory. However, few studies have specifically investigated chest computed tomography (CT) findings in patients with CRS. This study analyzed chest CT findings in patients with CRS and investigated their associations with CRS phenotypes and clinical characteristics. METHODS: We retrospectively analyzed 278 patients with CRS who underwent preoperative chest CT prior to endoscopic sinus surgery. Patients were stratified based on the presence or absence of abnormal ch

Outcomes following the discontinuation of biologic therapy in patients with severe asthma.

BACKGROUND: Biologic therapies are pivotal in managing severe asthma. Despite their efficacy, some patients discontinue biologics, with varying outcomes. Predictors of successful versus unsuccessful discontinuation remain poorly defined. This study aimed to identify clinical factors associated with post-discontinuation outcomes in a real-world practice. METHODS: This retrospective cohort included adults with severe asthma who had received biologics for at least 12 months and subsequently discontinued therapy for a minimum of three consecutive months. We assessed the effects of baseline blood

Rare presentation of Pre-B acute lymphoblastic leukemia with severe eosinophilia, bicytopenia, and cardiac mass in a 4-year-old boy: a case report.

INTRODUCTION: Acute lymphoblastic leukemia (ALL) is the most common childhood malignancy and most often presents with an increase in the number of blasts in the peripheral blood and bone marrow. Although ALL typically presents with circulating blasts, atypical findings such as hypereosinophilia may obscure diagnosis and delay appropriate management. Severe eosinophilia in patients with ALL is a very rare phenomenon and is usually associated with specific genetic abnormalities or myeloid malignancies. The combination of severe eosinophilia, bicytopenia, and cardiac mass is unusual and challengi

Novel Diagnostic Approaches for Eosinophilic Lung Diseases.

Eosinophilic lung diseases (ELDs) represent a heterogeneous group of airway and parenchymal disorders unified by eosinophilic inflammation but distinguished by diverse clinical features, mechanisms of persistence, and variable therapeutic responses. Traditional diagnostic tools, including blood eosinophil counts, bronchoalveolar lavage, sputum cytology, and exhaled nitric oxide, predict the eosinophilic/T2 burden of the disease but often fail to distinguish IL-5-dependent from IL-5-independent pathways, overlook compartment-specific inflammation, and inadequately predict or monitor response to

Case Report: Angioimmunoblastic T-cell lymphoma initially diagnosed as eosinophilic granulomatosis with polyangiitis.

BACKGROUND: Eosinophilic granulomatosis with polyangiitis (EGPA) and angioimmunoblastic T-cell lymphoma (AITL) are distinct entities that can present with overlapping clinical features, posing a significant diagnostic challenge. This case highlights a critical diagnostic pitfall where AITL was initially misdiagnosed as EGPA. CASE PRESENTATION: A 55-year-old man presented in 2021 with recurrent wheezing, dyspnea, chronic sinusitis, peripheral eosinophilia (7.35 × 10⁹/L), and skin rash, leading to an initial diagnosis of asthma and later EGPA. Despite treatment with systemic corticosteroids, om

ANCA-associated vasculitis following sotatercept initiation in a patient with heritable pulmonary arterial hypertension and previously silent eosinophilic granulomatosis with polyangiitis: a case report.

BACKGROUND: Pulmonary arterial hypertension (PAH) is a progressive disease characterized by increased pulmonary vascular resistance and right ventricular failure. Sotatercept, a novel activin receptor ligand trap, has demonstrated promising haemodynamic benefits in PAH treatment. Eosinophilic granulomatosis with polyangiitis (EGPA) is an extremely rare, Th2-driven, small-vessel vasculitis, and its overlap with PAH is scarcely reported. CASE SUMMARY: We report the case of a woman with heritable PAH initially stabilized with oral therapy. In 2024, after further clinical decline, subcutaneous tr

Eosinophilic Bronchiolitis Diagnosed by a Transbronchial Lung Cryobiopsy and Successfully Treated with Biologic Agents: A Report of Two Cases and Literature Review.

Eosinophilic bronchiolitis (EB) is a rare airway disease characterized by eosinophilic bronchiolar inflammation. We herein report two cases of EB diagnosed by a transbronchial lung cryobiopsy (TBLC), a less invasive diagnostic procedure than a surgical biopsy. Two patients with a history of refractory asthma responded to treatment with oral corticosteroids and biologic agents. To our knowledge, these are among the first reported cases of EB that were successfully managed with dupilumab and benralizumab. These cases underscore the diagnostic utility of TBLCs and suggest the potential use of bio

Levofloxacin-Induced Eosinophilic Pneumonia with Systemic Manifestations: A Case Report and Literature Review.

Drug-induced eosinophilic pneumonia (D-EP) is a rare adverse drug reaction that can rapidly progress to life-threatening respiratory failure without the timely withdrawal of the offending agent. We herein present the case of a 73-year-old man who developed diffuse erythema, dyspnea, and renal impairment 22 days after the initiation of levofloxacin. Laboratory evaluation revealed marked eosinophilia, and chest CT revealed diffuse ground-glass opacities. His respiratory status deteriorated, thus necessitating noninvasive positive pressure ventilation. Discontinuation of levofloxacin and prompt c

Functional respiratory imaging reveals early structural and functional airway responses to benralizumab in severe eosinophilic asthma.

BACKGROUND: Benralizumab improves outcomes in severe eosinophilic asthma (SEA). We aimed to evaluate early effects of benralizumab on airway pathophysiology using functional respiratory imaging (FRI). METHODS: In this prospective, single-arm study, 20 adults with SEA underwent CT at baseline, 4 and 12 weeks. Coprimary endpoints were changes in specific airway volume (siVaw) and resistance (siRaw) at functional residual capacity (FRC) and total lung capacity (TLC) at 12 weeks. Imaging outcomes included mucus score/volume and air-trapping. RESULTS: At week 12, siVaw increased at FRC by 0.16 mL

Clinical, molecular and microbial characterisation of the eosinophilic endotype of bronchiectasis: data from the EMBARC-BRIDGE study.

OBJECTIVES: Eosinophilic bronchiectasis is defined by a blood eosinophil count (BEC) ≥300 cells/µL, but blood eosinophils imperfectly reflect airway eosinophilic inflammation. Here, we investigated the relationship between eosinophilic airway inflammation, blood eosinophils and clinical severity in bronchiectasis and explored the phenotype associated with eosinophilic bronchiectasis. METHODS: Sputum from 180 patients with stable CT-confirmed bronchiectasis was utilised to investigate airway levels of eosinophil proteins (eosinophil peroxidase (EPX), eosinophil derived-neurotoxin (EDN), eosino

Inflammatory phenotype drives different immunosuppressive response in COPD exacerbations.

BACKGROUND: COPD is heterogeneous, with immunosuppression contributing to immune dysfunction. We hypothesized that the immunosuppressive response varies by inflammatory phenotype during both stable disease (SCOPD) and acute exacerbations (AECOPD). METHODS: COPD patients were monitored during AECOPD hospitalization and subsequent clinical stability. Serum key pro- and anti-inflammatory molecules were measured. Circulating Th17 and regulatory T (Treg) cells, early-myeloid-derived suppressor cells (e-MDSC), monocytic-MDSC (M-MDSC), and plasmacytoid dendritic cells (pDCs) were analyzed by flow cyt
