Gallbladder ascariasis is an uncommon manifestation of ascaris lumbricoides infection, typically seen in the endemic regions. Even rarer is the concurrent presence of Löffler's syndrome. We report the two cases - the first, a 92-year-old male, presented with typical biliary colic and was diagnosed with gallbladder ascariasis. The second, a 38-year-old female, presented with respiratory symptoms and was found to have ground-glass opacities on high-resolution computed tomography and peripheral eosinophilia, later associated with gallbladder ascariasis on imaging. Both cases were managed conserva

BACKGROUND: Mild and persistent peripheral eosinophilia is a common laboratory finding, often of unclear significance. However, it may represent an early sign of a hypereosinophilic disorder, such as Hypereosinophilic syndrome (HES) or Eosinophilic granulomatosis with polyangiitis (EGPA). These entities share overlapping clinical and laboratory features, rendering the differential diagnosis challenging especially at initial presentation.
OBJECTIVES: A 19-year-old female began her clinical course at 15 years of age with asthma-like respiratory symptoms and received conventional and biologic th

BACKGROUND: Benralizumab, a monoclonal antibody, targets eosinophils by blocking an interleukin-5 receptor, improving severe allergic asthma. Severe eosinophilic asthma affects about 10% of asthma patients and often requires high-dose corticosteroids, which are linked to adrenal insufficiency and poor outcomes. This study will perform a detailed review and meta-analysis to evaluate the effectiveness and safety of benralizumab in eosinophilic asthma.
METHOD: The analysis included relevant RCTs identified through systematic searches of PubMed, EMBASE, and the Cochrane Central Register of Contro

Asthma and chronic obstructive pulmonary disease (COPD) are the most prevalent chronic inflammatory airway diseases worldwide, significantly affecting patients' quality of life and mortality and imposing a substantial socioeconomic burden. Increasing insights into their pathophysiology have led to the recognition of both conditions as complex syndromes encompassing distinct pheno-endotypes. Asthma patients can be broadly stratified into type 2 (T2)-high and T2-low phenotypes based on biomarker expression and clinical features. More than half of asthma patients exhibit a T2-high phenotype, whic

BACKGROUND: Cystic fibrosis-asthmaoverlap syndrome (CFAOS) has high incidence in people with CF (pwCF) yet remains poorly defined, hindering both diagnosis and treatment. Asthmatic type2 (T2) inflammation (eosinophilia, elevated IgE, and increased IL-4/IL-5/IL-13), identifies an endo type within the CF community with accelerated disease progression and increased mortality. However, diagnostic criteria for asthma in the general population are less useful inpwCF due to overlapping phenotypes generated by chronic inflammation and structural lung disease. Therapeutically, the lack of standardized

Acute eosinophilic pneumonia (AEP) is a type of interstitial lung disease characterized by eosinophilic infiltration of the lung parenchyma and can present as acute hypoxemic respiratory failure. Certain medications, including carbamazepine, have been rarely implicated in eosinophilic pulmonary syndromes and delayed systemic hypersensitivity reactions with pulmonary involvement. In this case, we report a 50-year-old man who developed acute eosinophilic pneumonia (AEP) characterized by diffuse bilateral pulmonary infiltrates with small pleural effusions on imaging following the initiation of ca

Eosinophilic granulomatosis with polyangiitis is a rare small-vessel vasculitis that may affect different organs, leading to a highly variable and often challenging initial clinical presentation. We report the case of a 67-year-old man presenting with acute non-exertional dyspnoea and chest tightness. He had a history of adult-onset asthma, chronic rhinosinusitis and peripheral eosinophilia. Imaging revealed bilateral diaphragmatic elevation, and needle electromyography confirmed diaphragmatic paralysis. His medical history, combined with the presence of an eosinophilic exudative pleural effus

BACKGROUND: Acute eosinophilic pneumonia (AEP) is a rare disorder characterized by febrile illness, diffuse pulmonary infiltrates, and pulmonary eosinophilia. Diagnosis is challenging, especially in elderly patients, where atypical presentations can mimic community-acquired pneumonia (CAP) or even lung malignancy. Here we report a case of AEP in an elderly male with multiple comorbidities, highlighting the diagnostic dilemma and the critical role of a multidisciplinary team (MDT) approach.

Nowadays, the management of chronic obstructive airway diseases is shifting from traditional diagnostic labels toward a precision medicine approach based on "treatable traits." Among these, blood eosinophilia is emerging as a biomarker of type 2 (T2) inflammation across asthma and chronic obstructive airway diseases (COPD), even if its role as diagnostic tool and therapeutic target remains debated. In this manuscript we presented a clinical case of a 60-year-old male, former mild smoker (5 p/y), initially diagnosed with severe COPD with persistent airflow limitation, frequent severe exacerbati

UNLABELLED: Chronic eosinophilic pneumonia (CEP) is a rare interstitial lung disease. Although CEP has been partially characterized in adults, pediatric data remain scarce, which often leads to diagnostic delays and management uncertainty. This retrospective cohort study analyzed demographic, clinical, laboratory, imaging, bronchoalveolar lavage fluid (BALF), and treatment data from children with CEP between 2014 and 2025. Patients were stratified into 'Cured' (complete resolution) and 'Protracted' (persistent symptoms or imaging abnormalities > 3 months) groups for exploratory comparison. Thi

Eosinophilic pneumonia secondary to drug exposure is a recognized but infrequent complication associated with a variety of medications, including oncological therapies. Among immunomodulatory drugs, pulmonary toxicity has been described for thalidomide and lenalidomide but remains a rare complication.Pomalidomide is a third-generation immunomodulatory agent used in relapsed or refractory multiple myeloma. It has been authorized in Europe since 2013. To date, only a limited number of cases of pomalidomide-induced lung injury, and even fewer cases of eosinophilic pneumonia, have been documented

RATIONALE: Severe eosinophilic asthma (SEA) is driven by type 2 (T2) inflammation, characterised by dysregulated cytokine release and aberrant expression of adhesion molecules involved in immune cell trafficking and activation. Despite the established role of intercellular adhesion molecules (ICAMs) and L-selectin (CD62L) in these processes, their dysregulation in SEA and their potential remodulation in response to biologic therapy remain unclear. To investigate the expression of adhesion molecules (ICAM-1, ICAM-3, CD62L) on T-cell subsets in SEA, their modulation by IL-25 and IL-33, and the im

Exogenous lipoid pneumonia is an uncommon inflammatory lung disease caused by the aspiration or inhalation of lipid-containing substances. Clinical presentation is highly variable, ranging from asymptomatic disease to progressive respiratory failure, and the condition is often underrecognized in clinical practice. Diagnosis can be challenging because of nonspecific symptoms and imaging findings. We report a case of a 38-year-old woman with suspected exogenous lipoid pneumonia associated with exposure to a commercially available bug spray containing plant-based oils. Chronic eosinophilic pneumonia (CEP) is a rare, idiopathic condition characterized by pulmonary eosinophilic infiltration and peripheral opacities, typically responsive to corticosteroids. Tracheobronchial involvement is uncommon. We report a 44-year-old male patient presenting with clinical picture consistent with CEP with marked eosinophilia of 2.7 × 109/L. Bronchoscopy showed diffuse endobronchial nodules, which were resolved after steroid treatment. Biopsy confirmed eosinophilic infiltration. The diagnosis of CEP with endobronchial involvement was made. The patient showed rapid clinical an

BACKGROUND AND OBJECTIVE: Chronic obstructive pulmonary disease (COPD) is marked by persistent airflow limitation. Blood eosinophilia has emerged as a potential biomarker for predicting the risk of exacerbations as well as response to inhaled corticosteroids (ICS). However, its impact on lung function in stable COPD remains uncertain. This study aimed to assess the prevalence of blood eosinophilia in stable COPD patients and explore its relationship with lung function, exercise capacity and exacerbation history.
METHODS: A total of 60 stable COPD patients were included in this cross-sectional

BACKGROUND: Transient increases in blood eosinophils may accompany dupilumab treatment, but these increases are rarely associated with clinical symptoms, and clinical significance in children needs further assessment.
OBJECTIVE: We sought to evaluate long-term efficacy and safety of dupilumab in children 6 to 11 years old with type 2 asthma who completed the VOYAGE study and enrolled in the open-label extension study EXCURSION and experienced an early increase in blood eosinophils.
METHODS: This post hoc analysis assessed annualized severe exacerbation rates, change from baseline in pre-bron

BACKGROUND: Eosinophilic granulomatosis with polyangiitis (EGPA) is a rare anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitis characterized by eosinophilic inflammation and necrotizing vasculitis. Benralizumab, an anti-IL-5Rα monoclonal antibody that directly targets eosinophils, was approved for EGPA in China in December 2025. We report an early real-world experience of benralizumab combined with corticosteroids in a patient with severe ANCA-negative EGPA.

BACKGROUND: Drug reaction with eosinophilia and systemic symptoms (DRESS) is a severe T-cell-mediated drug hypersensitivity reaction associated with an increased risk of multiple drug hypersensitivity (MDH). Identifying culprit drugs is challenging, particularly in polymedicated patients, and the immunologic mechanisms underlying MDH are poorly understood.
OBJECTIVE: Evaluate whether cytokine secretion patterns of drug-specific T-cell responses reveal MDH in patients with nonimmediate drug hypersensitivity reactions (NIDHRs).
METHODS: In this multicenter cross-sectional study, we examined 20

BACKGROUND: Lung cancer-associated pulmonary embolism (PE) is an acute and fatal clinical syndrome. Kimura disease is defined as a rare, chronic inflammatory disease, primarily characterized by swollen lymph nodes in the head and neck, elevated levels of eosinophils and immunoglobulin (Ig) E. PE can also occur in patients with Kimura disease due to eosinophilia. We herein reported a case of lung cancer-associated PE mimicking Kimura disease.
CASE PRESENTATION: A 36-year-old Han Chinese male was admitted complaining of repeated cough, hemoptysis, and chest pain, and his recurrent PE with pulmo

Severe eosinophilic asthma remains a major therapeutic challenge despite advances in biologic therapies targeting type 2 inflammation. Depemokimab (Exdensur®), a novel ultra-long-acting anti-interleukin-5 (IL-5) monoclonal antibody, introduces a new treatment paradigm through sustained eosinophil suppression and twice-yearly administration. Engineered with enhanced IL-5 binding affinity and fragment crystallizable (Fc) modifications that prolong systemic persistence, depemokimab achieves durable pharmacodynamic activity and extended dosing intervals. Phase III SWIFT-1 and SWIFT-2 trials demons
