Prediction models for refractory Mycoplasma pneumoniae pneumonia and corticosteroid treatment escalation in hospitalized children: a retrospective cohort study.

BACKGROUND: Mycoplasma pneumoniae carries a clinically important risk of a refractory course or the need for intensified corticosteroid therapy. No admission-day prediction tool has been validated to identify children at risk for either outcome. METHODS: Children admitted to a single tertiary centre with a diagnosis of MPP between January 2022 and June 2025 were enrolled in a retrospective cohort study. Two penalised logistic regression models were constructed from routine admission-day clinical and laboratory variables: one targeting RMPP occurrence across the full eligible cohort (Cohort A)

Rapid, Recurrent Unilateral Pleural Effusion Presenting as Cryptococcus Infection in a Non-immunocompromised Host.

Cryptococcus neoformans infection presenting as a rapidly reaccumulating unilateral pleural effusion in a non-immunocompromised host is a rare presentation of pulmonary cryptococcal disease. We present a rare case of recurrent unilateral pleural effusion as the initial presentation of cryptococcal disease in an immunocompetent patient. An 85-year-old male with a past medical history of hypertension, hyperlipidemia, diabetes mellitus, chronic kidney disease stage IIIb, and coronary artery disease s/p stent placement in the second obtuse marginal branch of the circumflex artery presented to the

Median Effective Concentration of Ropivacaine for Ultrasound-Guided T11 Thoracic Paravertebral Block in Patients with End-Stage Renal Disease: A Dixon Up-and-Down Dose-Finding Study.

BACKGROUND: Patients with end-stage renal disease (ESRD) may exhibit altered pharmacokinetic and pharmacodynamic responses to local anesthetics. But the ropivacaine concentration-response profile for ultrasound-guided T11 thoracic paravertebral block (TPVB) in this population remains undefined. This study aimed to determine the median effective concentration (EC50) of ropivacaine for TPVB in ESRD patients undergoing peritoneal dialysis catheterization (PDC) and evaluate early postoperative recovery. METHODS: In this prospective dose-finding study, patients aged 18-75 years with ESRD and ASA p

Colchicine for Prevention of Postoperative Atrial Fibrillation After Coronary Artery Bypass Grafting: Mechanisms, Evidence, and Clinical Implications.

Cardiovascular disease represents the foremost cause of mortality globally. Annually, there are approximately 720,000 new cases of myocardial infarction and 335,000 recurrent events. Coronary artery bypass grafting (CABG) continues to be a primary intervention for multivessel coronary artery disease, using venous or arterial conduits to circumvent obstructed coronary vessels. In the United States, approximately 200,000 CABG procedures are conducted each year. Compared with percutaneous coronary intervention, CABG is favored in patients with advanced atherosclerosis, diabetes, and complex coron

Clinical utility of lung ultrasound in differentiating and monitoring complicated pediatric pneumonia: a prospective diagnostic accuracy study.

BACKGROUND: Lung ultrasound (LUS) is an established diagnostic tool for pediatric pneumonia, yet its utility as radiation-free alternative to computed tomography (CT) for monitoring disease progression and differentiating complicated phenotypes, specifically necrotizing pneumonia (NP) and parapneumonic effusion (PPE), has not been prospectively validated in a single comparative cohort. This study aimed to evaluate the diagnostic and longitudinal performance of LUS, including a quantitative LUS score, in children with community-acquired pneumonia (CAP), CT-confirmed NP, and PPE, and to evaluate

Recurrent rupture of a pancreatic mucinous cystic neoplasm with suspected main pancreatic duct communication, presenting with a mediastinal pancreatic pseudocyst, pancreatic pleural effusion, and pericardial effusion.

Spontaneous rupture of pancreatic mucinous cystic neoplasm (MCN) is rare, particularly outside pregnancy, and mediastinal extension has not been reported. We report a 34-year-old woman with a pancreatic tail MCN demonstrating continuity with the main pancreatic duct on computed tomography (CT), suggesting ductal communication. The neoplasm ruptured dorsally, and cyst fluid extended through the aortic hiatus, resulting in a mediastinal pancreatic pseudocyst, pancreatic pleural effusion, and pericardial effusion. Pleural fluid amylase was 1541 IU/L. Endoscopic ultrasonography showed a cyst-in-cy

A Case of a Cardiac Re-Rupture Caused by the Removal of Pericardial Patches Late after a Sutureless Repair for a Cardiac Rupture.

INTRODUCTION: Surgical treatment is the only reliable therapeutic option for a left ventricular free wall rupture (LVFWR). There are 2 major surgical techniques, namely, sutureless repair and sutured repair; a ventricular pseudoaneurysm is a late postoperative complication. We herein report a case of a re-ruptured left ventricular pseudoaneurysm after a sutureless bovine pericardial patch repair. CASE PRESENTATION: The patient was a 79-year-old woman who underwent a sutureless pericardial patch repair for a blow-out-type left ventricular rupture following an anterior myocardial infarction 10

Plasmablastic Richter Transformation of Chronic Lymphocytic Leukemia With Aberrant Epithelial Marker Expression Presenting as a Pleural Effusion: A Diagnostic Challenge.

Richter transformation (RT) is a well-known complication of chronic lymphocytic leukemia (CLL), and it is usually characterized by high-grade transformation to diffuse large B-cell lymphoma or, less commonly, to classic Hodgkin lymphoma. Rare reports describe RT presenting as plasmablastic lymphoma or myeloma. In this scenario, complete clinicopathological evaluation is critical to correctly classify the atypical transformation, and demonstration of a clonal relationship with precursor CLL is essential to confirm RT. Herein, we present a case of CLL with plasmablastic RT diagnosed on pleural e

Two Cases of Right-Sided Pleural Effusion as the Initial Manifestation of Severe Ovarian Hyperstimulation Syndrome.

INTRODUCTION: Severe ovarian hyperstimulation syndrome (OHSS) occurs less frequently due to increased knowledge of potential risk factors and implementation of preventive strategies. Right-sided pleural effusion as a presentation of severe OHSS is rare, and the symptoms can be nonspecific, which may delay diagnosis. However, early recognition is essential, as the condition can deteriorate rapidly and patients often need hospitalisation and supportive treatment. CASE PRESENTATION: We describe two patients with right-sided pleural effusion as the initial manifestation of severe OHSS following c

Environmental Asbestos Exposure as a Possible Pathway to Early-Onset Pleural Mesothelioma: A Case From Afghanistan and Pakistan to London.

Malignant pleural mesothelioma (MPM) is strongly associated with occupational asbestos exposure and typically presents decades after exposure, in older men. The association of MPM with environmental exposure and genetic susceptibility is less recognised. We report a case of a 40-year-old man presenting in London with dyspnoea, weight loss and a persistent right-sided pleural effusion following hospitalisation for pneumonia. Imaging showed circumferential nodular thickening of the right pleura with lymph node involvement, and histopathology was in keeping with epithelioid-type MPM. With no know

Pulmonary Vein Stenosis Following Radiofrequency Catheter Ablation Presenting With Massive Pleural Effusion: A Case Report.

We report a rare case of pulmonary vein stenosis (PVS) presenting with massive pleural effusion 5 months after radiofrequency catheter ablation (RFCA) for atrial fibrillation. A 58-year-old woman developed persistent fever, cough, and left lower lobe consolidation with a large pleural effusion. Initial antibiotic therapy was ineffective. Three-dimensional computed tomography (3D-CT) revealed severe stenosis of the left inferior pulmonary vein. Following balloon angioplasty, both pulmonary consolidation and pleural effusion resolved. Because PVS may present with nonspecific findings and delayed

Multisystem imaging findings as diagnostic red herrings in Wilson's disease.

A previously well man in his 20s presented with fever, cough, new-onset jaundice and a right-sided pleural effusion. Initial investigations demonstrated marked hepatic synthetic dysfunction. Subsequent imaging demonstrated cirrhotic liver morphology with splenomegaly and ascites. Additional findings of gallbladder wall oedema and focal colonic wall thickening broadened the differentials but following specialist radiology review, these abnormalities were recognised as manifestations of portal hypertension rather than separate disease processes.Further investigations confirmed Wilson's disease i

Oscillation and Lung Expansion Therapy as Rescue Airway Clearance in Critically Ill Adults: A Case Series.

INTRODUCTION: Retained airway secretions and lobar collapse are common in hospitalized adults with advanced lung disease. Conventional airway clearance depends on respiratory effort, requiring both an adequate inspiratory volume and sufficient expiratory airflow, and on cough strength, and therefore fails most often in the patients who need it most. Oscillation and lung expansion (OLE) therapy delivers continuous positive expiratory pressure (CPEP), continuous high-frequency oscillation (CHFO), and aerosolized medication in a single 10-min treatment, but reported experience is largely confined

Unilateral Pulmonary Artery Lesion in an Immunocompromised Patient.

We report a 62-year-old man with myelodysplastic syndrome on immunosuppressive therapy who presented with right lung consolidation and neutropenia. Despite broad antimicrobial therapy for presumed pneumonia, negative microbiologic studies including bronchoalveolar lavage, and a nondiagnostic transbronchial biopsy, serial computed tomography (CT) scans showed progressive right-sided consolidation, pleural effusion and CT signs of pulmonary hypertension. A routine contrast-enhanced CT revealed a filling defect in the right pulmonary artery (PA). Dedicated CT pulmonary angiography confirmed persi

Radiological Confirmation of Enteral Feeding Tube Placement Before Initiating Enteral Nutrition: A Review.

Radiological confirmation of enteral feeding tube placement is widely regarded as the gold standard for verifying the position of blind-inserted feeding tubes before initiating enteral nutrition. Misplacement of feeding tubes into the respiratory tract or incorrect positioning within the gastrointestinal tract can lead to severe complications, including aspiration pneumonia, pneumothorax, pulmonary injury, and death. Although bedside methods such as pH testing are useful for ongoing monitoring, radiographic imaging remains the most accurate technique for confirming initial tube placement. This

The consistency of reporting adverse events associated with dry needling: a narrative review.

Dry needling interventions are associated with disabling and potentially life-threatening events. Presently, undesired treatment effects are not reported consistently across the literature and as such, the degree of risk to and impact on patients cannot be clearly articulated. To systematically summarise available literature and capture trends in the reporting of undesired treatment effects occurring during and after dry needling, we searched Scopus, PubMed, Cochrane, CINAHL, Sports Medicine and Education Index and Web of Science. In total, 130 adverse events were reported synthesized into 31

Clinical Characteristics and Outcomes of Patients with Lymphangioleiomyomatosis.

BACKGROUND: Lymphangioleiomyomatosis (LAM) is a rare, progressive cystic lung disease that affects women of reproductive age. Although registries from Western countries have provided key insights, comprehensive nationwide data from Asian countries are scarce. This study aimed to characterise the clinical, functional and outcome features of Korean patients with LAM from a nationwide multicentre registry. METHODS: We analyzed data from patients with LAM enrolled in the Korean Nationwide Rare Interstitial Lung Disease Registry in 2024. Data on clinical and functional characteristics were collect

Chylothorax Secondary to Behçet's Disease: A Case Report.

Chylothorax is an uncommon cause of pleural effusion and is most frequently associated with trauma or malignancy. Its occurrence in Behçet's disease (BD) is exceptionally rare and is usually related to extensive central venous thrombosis causing obstruction of the thoracic duct. We report the case of a 35-year-old woman with a five-year history of BD who presented with progressive dyspnea, dry cough, and mild hemoptysis. Chest computed tomography (CT) revealed extensive thrombosis of the superior vena cava extending to both brachiocephalic veins and the right subclavian vein, associated with l
