Calcified Pleural Dissemination and Malignant Pleural Effusion in Dedifferentiated Chondrosarcoma: A Case Report with an Institutional Review.

Dedifferentiated chondrosarcoma (DDCS) rarely presents with pleural dissemination or malignant pleural effusion. A 62-year-old man developed massive left pleural effusion with diffuse calcified pleural thickening and nodular lesions. Pleural fluid cell block immunohistochemistry showed cytokeratin AE1/AE3 negativity and SATB2 positivity, suggesting a mesenchymal tumor. Pelvic imaging suggested a chondroid matrix, while a biopsy revealed a high-grade osteoid-producing component with an IDH1 p.R132C mutation. Integrated pleural fluid, imaging, histopathological, and molecular findings supported

Intrapleural TPA vs Normal Saline in Pediatric Empyema: A Comparative Study.

OBJECTIVE: To determine whether intrapleural tissue plasminogen activator (TPA) reduces hospital length of stay (LOS) vs intrapleural normal saline in children with complicated parapneumonic effusion or empyema. METHODS: A retrospective cohort study at a tertiary pediatric referral hospital in Iran, comparing children aged 1 month to 18 years treated during TPA unavailability (2012-2015; intrapleural saline, 40 mL, 1-hour dwell, daily ×3 days) with those treated after TPA became available (2015-2018; TPA 4 mg in 40 mL saline, same protocol). The primary outcome was hospital LOS, and the secon

Multiple Cold Abscesses Without Respiratory Symptoms Revealing Multifocal Tuberculosis: A Case Report.

Soft-tissue tuberculosis is an uncommon extrapulmonary manifestation and may be difficult to recognize when respiratory symptoms are absent. We report a 33-year-old, HIV-negative Sri Lankan woman without diabetes or known pharmacological immunosuppression who presented with three months of progressively enlarging, minimally tender superficial collections over the left wrist, right knee, and right ankle, accompanied by anorexia and unquantified weight loss. The swellings had minimal local inflammatory signs. She was anemic and hypoalbuminemic, with elevated inflammatory markers. Separate draina

Non-traumatic acute mediastinitis secondary to acute phlegmonous esophagitis: a rare case report.

Acute mediastinitis secondary to acute phlegmonous esophagitis (APE) is an exceptionally rare, life-threatening condition with high morbidity. We report a 75-year-old diabetic female with recent severe pneumonia who presented with pleuritic chest pain, dyspnea, and elevated inflammatory markers. Computed tomography pulmonary angiography excluded embolism but revealed diffuse long-segment esophageal wall thickening with mediastinal fat stranding, aortic encasement, and pleural effusion, features consistent with APE and secondary non-traumatic mediastinitis without overt perforation. Given clini

Lymphomas Diagnosed Primarily on Serous Effusion Cytology: Cytology Rises to the Challenge!

BACKGROUND: Lymphomas involving serous effusions as the initial clinical presentation are uncommon and pose significant diagnostic challenges because of their overlapping cytomorphologic features with reactive lymphocyte-rich effusions. Early diagnosis is crucial, particularly when tissue biopsy is difficult or inconclusive. AIM: This study aimed to evaluate the cytomorphologic spectrum of previously undiagnosed lymphoproliferative disorders presenting in serous effusions and to assess the complementary role of immunocytochemistry (ICC) and flow cytometry (FCM) in their diagnosis and subtypin

[Mechanisms and Clinical Translation Advances of Physical Targeted Therapy
for Malignant Pleural Effusion].

Malignant pleural effusion (MPE) is a common severe complication of advanced lung cancer and significantly worsens prognosis. Physical targeted therapies, represented by piezocatalytic therapy, sonodynamic therapy, photodynamic therapy, hyperthermic intrathoracic chemotherapy, radiofrequency ablation and thermosensitive in situ hydrogel drug delivery system, induce reactive oxygen species generation via light, ultrasound, or piezocatalytic energy, or achieve tumor killing through local hyperthermia and temperature-controlled drug release, offering new strategies for MPE. However, existing revi

Extravasation of TPN into the pleural space, a rare complication of central venous catheter placement: a case report.

BACKGROUND: Complications associated with central venous catheters are well documented in the literature, most of which relate to catheter placement or infections. Nonetheless, on rare occasions, the catheter can erode the arterial wall, allowing medication, fluids, or even total parenteral nutrition to pass into the pleural space. CASE PRESENTATION: We present the case of a 68-year-old male who was critically ill in the intensive care unit and required total parenteral nutrition. A central venous catheter was placed, and the patient's condition improved. While in the intensive care unit, it

Mixed Pneumonia with Pulmonary Embolism and Multiple Pathogens Detected in Bronchoalveolar Lavage Fluid: A Case Report.

Severe community-acquired pneumonia may involve mixed pathogens and concurrent vascular complications. A 72-year-old man presented with fever, nonproductive cough, vomiting, hypoxemia, markedly elevated inflammatory markers, and an elevated D-dimer concentration. Chest computed tomography showed left lower-lobe consolidation and pleural effusion. Computed tomography pulmonary angiography, obtained because of hypoxemia and suspected pulmonary embolism, showed a filling defect in a right upper-lobe pulmonary artery. The patient received antimicrobial therapy prior to bronchoalveolar lavage (BAL)

Comparing a straight-to-biopsy versus pleural aspiration-first approach in malignant and tuberculous pleural effusions.

BackgroundPleural aspiration has limited diagnostic yield in malignant (MPE) and tuberculous pleural effusions (TPE), more so when requiring molecular profiling in MPE and due to the paucibacillary nature of TPE. In our institution, patients with suspected MPE or TPE are offered a straight-to-biopsy approach with pleural biopsy as the initial diagnostic procedure, or a pleural aspiration-first approach, through shared decision-making.ObjectivesTo compare clinical outcomes between a straight-to-biopsy strategy and a pleural aspiration-first approach in patients diagnosed with MPE or TPE.DesignW

A case of recurrent pleural effusion: tuberculous pleurisy or other disease? A case report.

BACKGROUND: POEMS syndrome (polyneuropathy, organomegaly, endocrinopathy, M-protein, skin changes) is a paraneoplastic syndrome caused by a plasma cell proliferative disorder. Although POEMS syndrome can cause polyserous effusions, the report of pleural effusion as the main manifestation is not common. Here, we describe a case of POEMS syndrome misdiagnosed as tuberculous pleurisy due to massive pleural effusion. CASE PRESENTATION: A 70-year-old man was admitted with chest tightness and was diagnosed with tuberculous pleurisy. Despite receiving anti-tuberculosis treatment, patient exhibited i

Transdiaphragmatic rupture of a calcified hepatic hydatid cyst into the bronchial tree: CT findings of a bronchobiliary fistula.

Transdiaphragmatic rupture of hepatic hydatid cysts into the bronchial tree is a rare but severe complication that may mimic pulmonary infection. We report the case of a 61-year-old man who presented with fever, dyspnea, and right thoracic pain, with initial CT findings suggestive of pneumonia and pleural effusion. Despite antibiotic therapy and surgical drainage, clinical deterioration persisted. Bronchoscopy revealed biliary secretions, and thoracoabdominal CT demonstrated a calcified hepatic hydatid cyst communicating with the right lower lobe bronchial tree through a diaphragmatic defect,

A Clinician-Configurable FAHP-Based Ensemble Framework for Multi-Label Chest X-Ray Classification.

Background/Objectives: Deep learning-based chest X-ray (CXR) analysis has shown strong diagnostic performance; however, conventional ensemble methods typically rely on equal-weight aggregation and do not account for disease-specific model behavior or clinician-defined priorities. Methods: We developed a clinician-configurable ensemble framework based on the fuzzy analytic hierarchy process (FAHP) for multi-label CXR classification. The proposed method employs a two-stage design consisting of disease-specific model weighting using multi-criteria decision analysis and utility-guided seed aggrega

Diagnostic Performance of Different AI Tools for Chest Radiography: A CT-Anchored Per-Abnormality Analysis in a Tertiary Referral Center.

Background/Objectives: Artificial intelligence (AI) increasingly supports chest radiograph interpretation, but per-abnormality comparisons of commercial tools using CT-anchored reference standards remain limited. We compared two commercial AI tools using a CT-anchored, CXR-targeted reference framework. Methods: We retrospectively reviewed chest radiographs obtained between 30 October 2023 and 10 October 2024. Each radiograph was paired with CT performed within 14 days or within 2 days for rapidly progressive findings. Seven abnormalities across five analysis categories were evaluated: fracture

Bacteroides fragilis Empyema Secondary to Ruptured Pyogenic Liver Abscess: A Rare Cause of Septic Shock.

Pyogenic liver abscess is an uncommon but potentially life-threatening intra-abdominal infection associated with significant morbidity and mortality. Although hepatic abscesses most commonly present with systemic infection or intra-abdominal complications, transdiaphragmatic spread into the pleural cavity resulting in empyema is rare. Bacteroides fragilis, an anaerobic gram-negative bacillus that forms part of the normal gastrointestinal flora, is a well-recognized pathogen in polymicrobial intra-abdominal infections but is infrequently reported as a primary pathogen in empyema originating fro

Pulmonary epithelioid hemangioendothelioma with pleural dissemination: a case report of prolonged diagnostic delay and the role of surgical biopsy.

BACKGROUND: Pulmonary epithelioid hemangioendothelioma (PEH) is a rare vascular tumor with low incidence and nonspecific clinical and radiological features, frequently leading to misdiagnosis and delayed diagnosis. CASE PRESENTATION: We report a 32-year-old man with bilateral pulmonary nodules that were followed for 6 years before the development of a new left lower lobe mass, pleuritic chest pain, dyspnea, and massive pleural effusion in 2025. Invasive Infections Caused by Streptococcus pseudoporcinus: A Two-Case Series and Review of the Literature.

Streptococcus pseudoporcinus (S. pseudoporcinus) is a beta-hemolytic, Gram-positive, catalase-negative, nonmotile coccus that primarily colonizes the female genitourinary tract. Although it has historically been considered a colonizing organism, it has increasingly been associated with invasive infections. Its identification may be challenging because of its phenotypic similarity to other beta-hemolytic streptococci. We report two invasive infections caused by S. pseudoporcinus. The first occurred in a 59-year-old woman with cirrhosis who developed spontaneous bacterial peritonitis in the sett
