Predictors of pleurodesis success after combined outpatient thoracoscopic talc poudrage and tunneled indwelling pleural catheter placement.

BACKGROUND: Combined ambulatory thoracoscopic talc poudrage and tunneled indwelling pleural catheter (TIPC) placement may facilitate early pleurodesis while preserving outpatient management for malignant pleural effusion (MPE). Predictors of pleurodesis success after this combined approach remain incompletely defined. In this study, we aimed to identify the predictors of successful pleurodesis after this combined approach among patients with expandable and non-expandable lung subgroups. METHODS: We performed a retrospective cohort study of 100 patients who underwent outpatient thoracoscopic t

Computed Tomographic Features Associated with Pulmonary Melioidosis versus Pulmonary Tuberculosis in Diabetic Patients: A Retrospective Comparative Cohort Study.

Pulmonary melioidosis is frequently mistaken for pulmonary tuberculosis because of overlapping clinical and radiological features, often delaying appropriate treatment. We conducted a retrospective comparative cohort study at a tertiary care center in southern India between January 2018 and August 2025 to compare chest computed tomography (CT) features of microbiologically confirmed melioidosis and tuberculosis in diabetic adults who underwent CT within 30 days of symptom onset. A total of 151 patients were included: 46 with melioidosis and 105 with tuberculosis. Cavitation (n = 81/105 [77.1%]

Case Report: Dasatinib-associated chylothorax and occult Bartonella henselae infection in pediatric leukemia: a case report and systematic review.

Dasatinib is a second-generation tyrosine kinase inhibitor (TKI) used in Philadelphia chromosome-positive (Ph+) acute lymphoblastic leukemia (ALL), and chylothorax is one of its rare but serious lymphatic complications. We encountered a 14-year-old boy with Ph + acute lymphoblastic leukemia (ALL) who developed bilateral chylothorax while on dasatinib, with concurrent B. henselae infection (cat-scratch disease) confirmed in the pleural fluid. Prompted by this index case, we performed a systematic review of the literature to explore the potential association between dasatinib-associated chylotho

Perinatal Outcomes of Pleuro-Amniotic Shunting in Isolated Pleural Effusions: Systematic Review and Meta-analysis.

INTRODUCTION: Pleuro-amniotic shunting has been proposed for fetal pleural effusion, although its safety and efficacy remain uncertain. Therefore, we aimed to evaluate the perinatal outcomes of the fetal therapy for isolated pleural effusion through a systematic review and single-arm meta-analysis. METHODS: PubMed/MEDLINE, Embase, Scopus, and Cochrane Central were searched for studies reporting outcomes of fetuses undergoing pleuro-amniotic shunting for isolated pleural effusion. The primary outcomes were overall survival and resolution of fetal hydrops. Single-arm random-effects meta-analyse

Diagnostic Accuracy of Pleural Fluid Biomarkers and Nucleic Acid Amplification Tests in Tuberculous Pleural Effusion: An Umbrella Review.

INTRODUCTION/OBJECTIVE: Tuberculous Pleural Effusion (TPE) is a common but diagnostically challenging form of extrapulmonary tuberculosis. Many overlapping systematic reviews have evaluated pleural-fluid biomarkers and Nucleic Acid Amplification Tests (NAATs). This umbrella review aimed to synthesise and compare the diagnostic accuracy of pleural-fluid tests for TPE within a single appraised framework. METHODS: We conducted an umbrella review (JBI methodology; PRISMA 2020 reporting) of 20 systematic reviews with meta-analysis, appraised them using AMSTAR 2, quantified primary-study overlap wi

Complex Cardiac Tumor with Concomitant Pericardial Effusion and Hemodynamic Compromise in a Septuagenarian: Case Report of Anesthetic and Surgical Considerations.

Primary cardiac tumors are rare, and cardiac lymphomas are particularly uncommon. Their presentation can mimic benign tumors, but perioperative management is highly challenging due to risks of tumor-related obstruction or invasion of surrounding cardiac structures. We present a 76-year-old woman with rapidly progressing dyspnea, found to have large biatrial masses, significant pericardial and pleural effusions, and evidence of tamponade. The patient underwent successful surgical resection of bilateral atrial tumors after careful anesthetic preparation and intraoperative monitoring. Histology r

Reversible osimertinib-associated heart failure mimicking cancer recurrence in EGFR-mutated non-small cell lung cancer after pneumonectomy: A case report.

Osimertinib is a third-generation epidermal growth factor receptor (EGFR) tyrosine kinase inhibitor widely used in patients with EGFR mutation-positive non-small cell lung cancer (NSCLC). Although generally well tolerated, clinically notable cardiovascular toxicities, including heart failure, have been recognized. The present study describes the case of a 77-year-old man who developed symptomatic heart failure 5 months after initiating osimertinib treatment for postoperative recurrence of NSCLC. The patient presented with dyspnea and right-sided pleural effusion, which was initially suspected

Pleural Mesothelioma With Unusual Squamous Differentiation: A Clinicopathological Case Report With Molecular Insights From Whole-Exome Sequencing.

Pleural mesothelioma is an aggressive tumor with a poor prognosis and limited therapeutic options. While histological heterogeneity is common, true squamous differentiation is extremely rare and diagnostically challenging. We report an 81-year-old man with asbestos exposure who presented with recurrent right-sided pleural effusion. Thoracoscopic biopsies revealed two distinct tumor components. The first expressed mesothelial immunohistochemical markers, whereas the second showed a squamous profile. Both components demonstrated loss of BAP1 expression and homozygous deletion of CDKN2A, supporti

A phase 2 study of tipifarnib in children and young adults with tumors harboring HRAS genomic alterations: NCI-COG Pediatric MATCH trial (APEC1621M).

BACKGROUND: APEC1621 arm M, a subprotocol of the NCI-COG Pediatric MATCH clinical trial, is a phase 2 study evaluating tipifarnib, a farnesyl transferase inhibitor, in patients with relapsed or refractory solid tumors harboring HRAS mutations at codons G12, G13, Q61, or A146 (NCT04284774). METHODS: Patients identified by the MATCH screening protocol and enrolled to the arm M subprotocol received tipifarnib at 350 mg/m2/dose (maximum 600 mg/dose) orally twice daily on an intermittent dosing schedule (days 1 to 7 and 15 to 21, every 28 days) until disease progression or intolerable toxicity. Cytomorphology of Hematolymphoid Neoplasms in Body Cavity Effusions: Diagnostic Value and Prognostic Implications- A case Series.

BACKGROUND & OBJECTIVES: Involvement of the pleural and peritoneal cavities by hematolymphoid malignancies is relatively infrequent compared to epithelial malignancies, though reported rates vary widely depending on institutional and demographic factors. When present, they may or may not be the first manifestation of disease; however, they usually indicate an advanced stage. Cytological recognition is often challenging due to morphological overlap with reactive processes, yet it remains a valuable, minimally invasive diagnostic tool. This study describes the cytomorphological spectrum, diagnos

Evaluation of Delphi high-resolution computed tomography phenotype classification for treatment-naïve patients with pulmonary sarcoidosis.

BACKGROUND: The recently developed Delphi consensus provides a simplified high-resolution computed tomography (HRCT) phenotype classification system for pulmonary sarcoidosis. This study aimed to investigate the clinical value of HRCT phenotype classification from the Delphi consensus study on pulmonary sarcoidosis in treatment‑naïve patients and to identify potential refinements for classifying early‑stage sarcoidosis. METHODS: This retrospective single-center study included 278 treatment‑naïve patients with pulmonary sarcoidosis. HRCT signs were scored, and clinical data were recorded for a

Pulmonary tuberculosis occurring during the diagnosis and treatment of sarcoidosis complicated by chylothorax: A case report.

Sarcoidosis is a systemic granulomatous disorder, and chylothorax is a rare but severe pleural complication of this disease and exhibits a prominent diagnostic overlap with tuberculosis. The present study reported the case of a 48-year-old woman who was admitted in May 2018 with chronic cough and chest tightness. After hospitalization for 17 days, comprehensive examinations excluded tuberculosis and malignancy, and the patient was finally diagnosed with sarcoidosis complicated with pleural effusion. Treatment with prednisone combined with therapeutic thoracentesis was initiated to relieve symp

Hydropneumothorax as a Diagnostic Clue to Pulmonary Tuberculosis With Pleural Involvement: A Case Report.

Hydropneumothorax represents an unusual presentation of pulmonary tuberculosis (TB), posing diagnostic and therapeutic challenges. We report the case of a 36-year-old woman who presented with productive cough, dyspnea, right-sided pleuritic chest pain, and constitutional symptoms. Chest imaging revealed a large right-sided hydropneumothorax with extensive parenchymal and cavitary disease. Sputum smear microscopy was positive for acid-fast bacilli, and molecular testing of both sputum and pleural fluid detected Mycobacterium tuberculosis complex, confirming pulmonary TB with pleural involvement

Successful repair of a penetrating right atrial gunshot injury via right posterolateral thoracotomy in a resource-limited hospital: a case report.

BACKGROUND: Penetrating cardiac injuries are uncommon but frequently fatal, particularly after gunshot wounds. Diagnosis may be challenging when pleural bleeding predominates, and pericardial fluid is minimal because blood may decompress through a pericardial defect into the thoracic cavity. CASE PRESENTATION: A 25-year-old man sustained a penetrating chest gunshot wound during wartime and presented to a resource-limited hospital in hemorrhagic shock with a massive right hemothorax. Right tube thoracostomy immediately drained approximately 900 mL of blood, but he remained hemodynamically unst

Echocardiographic changes in hyperthyroid cats: comparison of subclinical disease and congestive heart failure.

BACKGROUND: Hyperthyroidism induces a wide spectrum of cardiovascular changes in cats, yet the factors distinguishing subclinical cardiac involvement from congestive heart failure (CHF) remain poorly defined. Previous studies have focused primarily on structural changes, with limited evaluation of atrial function or right-sided involvement. OBJECTIVES: To compare echocardiographic variables between hyperthyroid cats with subclinical cardiac disease and those presenting in CHF, with emphasis on left atrial remodelling, left atrial function, and left ventricular dimensional changes, and right-s

Sacituzumab govitecan combined with anlotinib in metastatic triple negative breast cancer patient with repeated pleural effusion: a case report.

Metastatic triple-negative breast cancer (mTNBC) is an aggressive malignancy with limited targeted therapeutic alternatives and dismal long-term prognosis. Sacituzumab govitecan (SG) is a Trop-2-directed antibody-drug conjugate (ADC) conjugated to the topoisomerase I inhibitor SN-38. SG has demonstrated robust anti-tumor activity across multiple clinical trials for mTNBC. We present a hypothesis-generating case report of a 50-year-old female patient with stage III triple-negative breast cancer (TNBC) who received curative-intent surgery followed by adjuvant multimodal therapy. Fourteen months
