Clinical application of confocal laser endomicroscopy in the diagnosis of malignant pleural effusion.

OBJECTIVE: Probe-based confocal laser endomicroscopy (pCLE) is an innovative in vivo microscopic imaging technique that enables real-time visualization of tissue cytology during endoscopic examinations. This study aimed to evaluate the clinical utility of pCLE in diagnosing MPE. METHODS: A total of 41 patients with pleural effusion (PE) who underwent pCLE examination were enrolled in this prospective study. The diagnostic performance of pCLE was assessed by calculating sensitivity (SEN), specificity (SPE), positive predictive value (PPV), negative predictive value (NPV), and accuracy (ACC), u

Trastuzumab deruxtecan (DS-8201) plus bevacizumab for HER2-low metastatic breast cancer with malignant pleural effusion: a case report and literature review.

INTRODUCTION: HER2-low breast cancer has emerged as a distinct molecular subtype with unique biological features and therapeutic significance. Trastuzumab deruxtecan, an antibody-drug conjugate, has shown promising efficacy in both HER2-high and HER2-low disease. CASE DESCRIPTION: We report the case of a 69-year-old woman with a 17-year history of breast cancer, initially diagnosed in 2007 with invasive ductal carcinoma and lymph node metastasis. After multiple lines of systemic therapy, the disease progressed with dynamic reduction of HER2 expression from 3+ to 1+, ultimately confirming HER2

First report of Castellaniella spp. infection in dogs and the genomic evidence of a novel species.

AIMS: This study reports the first documented cases of Castellaniella spp. infection in dogs, describing associated clinical and pathological findings and characterizing a novel species within this genus. METHODS AND RESULTS: Pleural effusions from two dogs presenting with acute respiratory distress and systemic illness were evaluated via cytology, bacterial culture, and next-generation sequencing. Both cases exhibited neutrophilic-macrophagic inflammation with intracellular Gram-negative rods, primarily within macrophages. Bacterial culture failed to identify the organisms. Metagenomic analy

Organizing Pneumonia in a Patient with Pulmonary Transthyretin Amyloidosis.

An 86-year-old man who was a retired tunnel construction worker with pleural plaques and chronic pleural effusion was referred for an evaluation of newly developed pulmonary infiltrates. A transbronchial lung biopsy revealed organizing pneumonia with transthyretin amyloid deposition in the alveolar septa. Genetic testing confirmed wild-type transthyretin amyloidosis. Corticosteroid therapy led to partial resolution of the pulmonary infiltrates. Few cases of pulmonary transthyretin amyloidosis associated with organizing pneumonia have been reported. This case highlights the potential involvemen

MTPC integration for improved diagnosis of pleural effusion.

INTRODUCTION: Pleural effusion is clinically common with diverse etiologies, and differentiating benign from malignant cases is critical for treatment planning and prognosis assessment. Traditional single diagnostic methods have inherent limitations, leading to diagnostic challenges. This study aimed to develop a multi-modal diagnostic panel (MTPC) to improve the accuracy and efficiency of initial pleural effusion diagnosis. METHODS: A total of 369 patients (264 with malignant pleural effusion and 105 with benign pleural effusion) were enrolled retrospectively. The MTPC panel integrated four

Mammary malignant peripheral nerve sheath tumor in a 93-year-old male: case report.

Breast sarcomas are rare malignancies, accounting for <1% of all breast malignancies and <5% of all soft tissue sarcomas. Malignant peripheral nerve sheath tumors (MPNSTs) are aggressive soft tissue sarcomas that arise from peripheral nerves. Their occurrence in the breast is exceedingly rare. This is the fourth published case of mammary MPNST in a male patient. We present a case of a 93-year-old male with a three-year history of a progressively enlarging and painful right breast mass. Examination revealed a 20 × 30 cm immobile, fungating, and ulcerating mass with purulent discharge. Histopath

One in One Million-A Case of Pleural Disease.

This case report presents an instance of pleural epithelioid hemangioendothelioma (EHE), a vascular tumour with an incidence of less than 1% among vascular tumors. The patient, a 43-year-old man, presented with a right-sided pleural effusion, longstanding neck and shoulder pain, and worsening pleuritic chest pain. Initial imaging revealed a left infra-clavicular soft tissue mass, pleural thickening, and pulmonary nodules suggestive of metastases. Despite inconclusive initial biopsies, immunohistochemistry and an international pathology review confirmed EHE, characterized by CAMTA1 expression a

Enhanced recovery after thoracic surgery: Clinical outcomes and imaging-based evaluation of a multimodal rehabilitation strategy.

Enhanced recovery after surgery (ERAS) protocols are increasingly adopted in thoracic surgery to optimize recovery, but evidence integrating surgical outcomes with imaging-based postoperative recovery assessment remains limited, particularly for video-assisted thoracoscopic surgery. This single-center retrospective cohort study included patients undergoing elective video-assisted thoracoscopic surgery, divided into an ERAS group and a conventional care group. Outcomes analyzed included clinical indices (postoperative complications, chest tube duration, hospital stay), numerical rating scale pa

Biliopleural Fistula Following Percutaneous Transhepatic Drainage: A Rare Cause of Bilious Pleural Effusion.

Biliopleural fistula is a rare but serious complication of hepatobiliary interventions. It may present with atypical respiratory symptoms and requires a high index of suspicion for diagnosis. We report the case of a 79‑year‑old woman with choledocholithiasis complicated by failed endoscopic retrograde cholangiopancreatography (ERCP), post‑ERCP pancreatitis, and cholangitis. Following percutaneous transhepatic drainage (PTD) with external biliary catheter placement, she developed a loculated right‑sided pleural effusion. Diagnostic thoracentesis revealed bilious pleural fluid, and magnetic reso

A novel approach in the identification of microRNAs in malignant pleural effusion for lung cancer diagnosis.

INTRODUCTION: Pleural effusion, an atypical accumulation of fluid in the pleural space, has been identified as a potential indicator of several diseases, including lung cancer. The presence of biomarkers in malignant pleural effusion has been a subject of investigation; however, the expression of microRNAs has received limited attention. The objective of this study is to present a narrative review of the current scientific literature regarding the presence of microRNAs in malignant pleural effusion and their association as new biomarkers in the diagnosis of lung cancer. METHOD: A comprehensiv

Emerging immunotherapeutic strategies for malignant serous effusions: current evidence and future directions.

The invasion of serous cavities by malignant tumors, leading to implantation metastasis, is a hallmark of advanced cancer, causing pathological buildup of fluid (malignant effusions) containing cancerous cells, which indicates advanced disease and poor prognosis. The primary cancers involved include lung, breast, gastric, and ovarian cancers, affecting a large patient population with a high incidence rate. Because effective treatment options are limited in clinical practice, patients with malignant effusion, based on the severity, typically survive only 3 to 15 months. Traditional therapies, s

Clinical Factors Influencing Spontaneous Pleurodesis Success in Malignant Pleural Effusion in Patients With Indwelling Pleural Catheter.

Background Indwelling pleural catheters (IPCs) provide effective palliation for malignant pleural effusion (MPE) and may induce spontaneous pleurodesis (SP). However, risk factors causing transudative effusion, such as hypoalbuminemia, congestive cardiac failure (CCF), and renal failure (RF), may reduce pleurodesis success. Evidence on the impact of these comorbidities in MPE patients undergoing IPC placement remains limited. Objective To evaluate the impact of hypoalbuminemia, CCF and RF on successful SP in MPE. Methods A retrospective single-centre study was conducted in a tertiary care hosp

Pleural effusion within 24 h of admission predicts severe hypertriglyceridemia-induced acute pancreatitis and organ failure: retrospective cohort of 296 patients.

BACKGROUND: Early risk-stratification in hypertriglyceridemia-induced acute pancreatitis (HTG-AP) is challenging within the first 24 h of admission. We assessed whether pleural effusion (PE) detected on early imaging serves as an early predictor for the development of severe HTG-AP (HTG-SAP) and organ failure (OF), compared with BISAP, MCTSI, and C-reactive protein (CRP). METHODS: A retrospective study included 296 HTG-AP patients at Xuanwu Hospital from August 2013 to February 2024. PE presence within 24 h was abstracted from chest CT; outcomes were HTG-SAP and OF. Diagnostic performance was
