Obstructive jaundice induced by postoperative intra-cystic infection with polycystic liver disease: a rare case report and literature review.

INTRODUCTION AND IMPORTANCE: Polycystic liver disease (PLD) is a rare genetic disease characterized by progressive development of cholangiocyte-derived fluid-filled hepatic cysts, and obstructive jaundice is a rare complication of PLD. CASE PRESENTATION: A 72-year-old man presented with obstructive jaundice and bile-like pleural effusion following an initial laparoscopic fenestration and drainage of hepatic cysts. Upon admission to our hospital, imaging revealed an enlarged cystic lesion in segment S7 and some small cystic lesions around the liver with intrahepatic bile duct dilation. Further

Pancreaticopleural fistula originated from a tiny pancreatic pseudocyst: a case report and literature review.

INTRODUCTION AND IMPORTANCE: Pancreatic pseudocysts (PPCs) cause two-thirds of pancreatic cystic diseases; pancreaticopleural fistula (PPF), a rare (0.4% in pancreatic diseases) life-threatening complication, occurs when mediastinal pseudocysts (60% from PPCs) rupture into the thorax, with 4.5% risk in PPC patients (highest in middle-aged males with alcohol-related chronic pancreatitis). Its nonspecific symptoms (dyspnea, mild/absent abdominal pain) delay diagnosis. CASE PRESENTATION: A 44-year-old male with a 3-year recurrent acute pancreatitis and hyperlipidemia was admitted for sudden dysp

On Demographic Group Fairness Guarantees in Deep Learning.

We present a theoretical framework analyzing the relationship between data distributions and fairness guarantees in deep learning. Our work establishes novel bounds that explicitly account for data distribution heterogeneity across demographic groups, while introducing a formal analysis framework that minimizes expected loss differences across these groups. Moreover, we derive bounds for fairness errors and convergence rates, characterizing how distributional differences between groups affect the fundamental trade-off between fairness and accuracy. Through extensive experiments on diverse data

A new diagnostic prediction model for infective endocarditis based on the 2023 duke-international society for cardiovascular infectious disease criteria: a multicenter observational study.

BACKGROUND: Infective endocarditis (IE) remains a diagnostically challenging disease with diverse symptoms. Whether IE is suspected frequently depends on clinical judgement. A reliable model to estimate the likelihood of IE at presentation across clinical departments is needed; however, no such model is currently available. We aimed to develop such a diagnostic prediction model using objective and reproducible variables and to validate its performance. METHODS: We included inpatients aged ≥ 20 years who had either a diagnosis code for IE or for undiagnosed fever during hospitalization. The mod

Esophageal Perforation With Right-Sided Hemothorax in a Patient With Suspected Variceal Bleeding: A Diagnostic Challenge.

Esophageal perforation (Boerhaave syndrome) is a rare but life-threatening condition that may mimic other causes of upper gastrointestinal bleeding, making early diagnosis challenging. We report a 76-year-old woman with disseminated breast cancer who presented with massive hematemesis and profound hemodynamic instability, initially suspected to represent variceal bleeding. Emergency endoscopy achieved hemostasis at the gastroesophageal junction but did not identify a perforation. During anesthesia, point-of-care ultrasound revealed a large right-sided pleural effusion, prompting CT imaging. CT

Development and validation of a predictive model for pleural effusion control following talc pleurodesis in malignant pleural effusion.

BACKGROUND: Talc slurry pleurodesis is widely used for malignant pleural effusion (MPE); however, its success rate ranges from 50 to 75%, and reliable predictors are lacking. This study aimed to identify predictors of pleural effusion control failure following pleurodesis and develop a short-term predictive scoring system. METHODS: We retrospectively analyzed 170 patients with MPE who underwent talc pleurodesis at a single center between 2014 and 2024. The cohort was divided into training (n = 136) and validation (n = 34) cohorts. Logistic regression was used to identify independent predictor

Development and multicenter validation of a predictive model for malignant pleural effusion recurrence.

Early prediction of malignant pleural effusion (MPE) recurrence within 3 months is essential for optimizing management in lung cancer patients. This study developed and validated a machine learning model to estimate the 3-month recurrence risk of MPE in patients with newly diagnosed lung cancer. Using data from 221 patients for model training and 237 from two external validation cohorts, the Elastic Net model-based solely on four routine clinical features (treatment regimen, alanine aminotransferase, total pleural effusion volume, and tumor diameter)-achieved excellent performance, with areas

Organ-metastatic landscape delineates overall and site-specific immune-related adverse events of PD-L1 blockade in advanced NSCLC.

It remains unknown whether metastases to different organs confer varying susceptibility to immune-related adverse events (irAEs) within a specific tumor type. We conducted a retrospective study included 708 advanced patients with non-small cell lung cancer (NSCLC) who received atezolizumab in two randomized controlled trials (OAK and POPLAR). Overall, irAEs were less frequent in patients with bone (OR = 0.52, p = 0.039) and pleural effusion metastases (OR = 0.65, p = 0.039), while more frequent in those with brain metastasis (OR = 1.96, p = 0.023). Cumulative irAEs risk differed significantly

Metastatic Papillary Thyroid Carcinoma in Pleural Effusion: Diagnostic Challenges in Effusion Cytology.

Metastatic papillary thyroid carcinoma (PTC) presenting in serous effusion cytology is rare and usually reflects advanced disease with an adverse prognosis. Accurate identification in effusion specimens is diagnostically challenging because of overlapping cytomorphological features with other papillary malignancies. We report a case of a 74-year-old woman with a prior history of PTC who presented with bilateral pleural effusion. Cytological examination of the pleural fluid revealed papillary clusters of tumour cells, psammoma bodies and frequent intranuclear cytoplasmic inclusions. Cell block

XIAOSHUI formula inhibits malignant pleural effusion by targeting the STC1/p65/CXCL5 axis to reprogram tumor-associated macrophages.

BACKGROUND: Malignant pleural effusion (MPE) is a common and debilitating complication in advanced lung carcinoma, driven by an immunosuppressive tumor microenvironment (TME), where tumor-associated macrophages (TAMs) play a pivotal role. This specialized milieu not only facilitates neoplastic proliferation but also contributes to immune system dysfunction. Preliminary evidence suggests that Xiaoshui Formula (XSF) could inhibit MPE by modulating TAM polarization, a key mechanism underlying T cell dysfunction and compromised tumor surveillance capabilities. PURPOSE: This study seeks to elucida

Cascading inertial microfluidics for high-throughput, multi-scale enrichment of tumor cells and intact clusters towards enhanced malignancy diagnosis.

The presence and abundance of malignant tumor cells (MTCs), particularly cell clusters (MTCCs) in clinical effusion samples serve as established correlative indicators for aggressive malignancy, elevated metastatic potential, and adverse clinical prognosis. While tumor cell-based liquid biopsy enables the detection of single MTCs, but clinical utility is limited by cellular damage and attrition of MTCCs. To overcome these limitations, we developed a cascaded inertial microfluidic device for high-throughput and multi-scale enrichment of both MTCs and intact MTCCs through two-stage inertial sort

Lung ultrasound versus lung auscultation to detect pulmonary congestion in patients with advanced heart failure before discharge.

BACKGROUND: Residual lung congestion is common in patients discharged after an acute heart failure (HF) hospitalization and represents a risk for HF rehospitalization. The aim of this study was to compare the diagnostic accuracy of B-lines on lung ultrasound and lung auscultation to detect residual congestion. We hypothesized that lung ultrasound would be more sensitive than physical examination. METHODS: In this observational study of consecutive chronic HF patients discharged after an acute HF exacerbation, auscultation performed by two cardiologists and ultrasound examination performed by

Analysis of pleural fluid biomarkers for diagnosis of malignant pleural effusion and molecular characterization of lung cancer: a review.

PURPOSE OF REVIEW: Malignant pleural effusion (MPE) is a common complication of advanced lung cancer and often represents the first sign of malignancy. Conventional diagnostic methods, including cytology and pleural biopsy, have limited sensitivity. This review provides a timely assessment of emerging pleural fluid biomarkers and their utility in differentiating malignant from benign effusions, as well as their role in guiding molecular characterization of lung cancer to improve diagnostic accuracy and therapeutic decision-making. RECENT FINDINGS: Recent studies highlight the diagnostic and m

Immunodynamic changes in a mouse model of malignant pleural effusion.

BACKGROUND: Malignant pleural effusion (MPE), a common complication of advanced cancers, is associated with poor prognosis and reduced quality of life. Although host–tumor interactions are known to drive MPE development, the associated immune dynamics during disease progression remain unclear. Using a Lewis lung carcinoma-induced MPE model in C57BL/6JNidfc mice, we systematically evaluated general parameters and immune cell changes at two-day intervals throughout disease progression. RESULTS: The day of Lewis lung carcinoma cell injection into the pleural space was designated as day 0. By day

Fatal Disease Progression Driven by Acquired MET Amplification After EGFR-TKI Therapy in EGFR- and RBM10-Mutant Lung Adenocarcinoma.

Although mesenchymal-epithelial transition proto-oncogene (MET) amplification is a common resistance mechanism in targeted therapy for lung cancer, rapid disease progression associated with this resistance mechanism in patients with epidermal growth factor receptor (EGFR) mutation has scarcely been reported. Herein, we report a fatal case of lung adenocarcinoma that rapidly progressed after failure of EGFR-tyrosine kinase inhibitor treatment following the emergence of MET amplification. A 62-year-old man was diagnosed with metastatic lung adenocarcinoma containing mutations in EGFR exon L858R

Red blood cell distribution width-to-albumin ratio and postoperative pleural effusion risk after gastrectomy for esophagogastric junction adenocarcinoma: a retrospective study.

BACKGROUND: Postoperative pleural effusions (PPE) are common complications following gastrectomy, particularly in patients diagnosed with Siewert type II/III esophagogastric cancer. The red blood cell distribution width-to-albumin ratio (RAR) is a marker indicative of systemic inflammation and nutritional status. This study investigates the association between preoperative RAR and the incidence of PPE in patients with Siewert type II/III adenocarcinoma of the esophagogastric junction (AEG) who have undergone gastrectomy. METHODS: A retrospective analysis was performed on data from 299 patient
