The roentgenographic prevalence and anatomic distribution of pleural plaques were studied in the US Navy Asbestos Medical Surveillance Program population (105,064 individuals as of July 17, 1985). "Definite" or "probable" pleural plaques were noted in 4.4 percent of films. These were unilateral in 19.3 percent of roentgenograms with "definite" pleural plaque and 33.9 percent of films with "probable" pleural plaque. In Norway the Directorate of Labour Inspection's pneumoconiosis panel consists of two radiologists. Pneumoconioses are notifiable diseases, and doctors are required by law to notify suspected and confirmed cases to the Directorate of Labour Inspection. Chest films are sent to the Directorate. The pneumoconiosis panel reads and classifies the cases according to ILO-standards. During a 9 months' period, 177 chest films were examined, 162 of which were from persons exposed mainly to asbestos. This report is the result of an analysis of the medical records of 124 patients presenting with a malignant pleural mesothelioma. Information about asbestos exposure was available in 104 of them, which appeared to be positive in 95 (91%). The median duration of exposure was 34 yrs. The median latent period was 41 yrs. Parietal pleural plaque is a well-recognized sign of exposure to asbestos. Visceral pleural involvement is an uncommon manifestation, and calcified visceral pleural plaques are rare. Those reported have been in the minor fissure and inferior major fissure. We describe a unique calcified plaque in the superior major fissure.

We investigated the relationship between number of asbestos bodies and pleural plaques. Using sodium hypochrolite, we examined 400 autopsy lungs and could detect 71 cases whose asbestos bodies were significantly high. We checked pleural plaques on chest x-ray films of these 71 cases and compared the exact plaque at autopsy. By the criteria of Askergren and Szamosi, we classified these into three groups (probable, definite, definite with calcification). Rounded atelectasis is an atelectasis of a peripheral part of the lung due to pleural adhesions and fibrosis causing deformation of the lung and bending of some small bronchi. Increased volume of trapped gas (VTG) was found in 87 asbestos-exposed railroad repair shop workers, divided into three subgroups according to smoking habits. All the examined subjects had pleural plaques. Determination of VTG was used to study function of small airways. Increased VTG was found among asbestos-exposed smokers as well as nonsmokers. In both groups the VTG:TLC ratio was elevated compared with controls. Increased VTG is a sign of small airways dysfunction. In asbestos-exposed subjects, it might be caused by an early peribronchial reaction to inhaled asbestos fibers.
