Do pleural plaques produce symptoms? A brief report.

The occurrence of symptoms due to pleural plaques was studied by means of a questionnaire administered to asbestos-exposed workers participating in a health-screening program. One hundred thirty subjects who were found to have pleural plaques were compared with 1,103 control subjects who had no plaques and showed no changes on x-ray examination. No difference in occurrence of thoracic pain was found between the two groups. Dyspnea was more common among patients with pleural plaques, who also tended to have lower lung function values. There was a good correlation between reported dyspnea and lu

Surgical treatment of asbestos-related disease of the chest.

Thoracic surgeons are asked to evaluate various diseases related to asbestos, including benign pleural plaques, malignant mesothelioma, and lung cancer. The benign localized mesothelioma is usually considered in the differential diagnosis of pleural tumors, but it is not related to asbestos exposure. Benign pleural plaques can be diagnosed by history and radiologic studies, and surgery offers no therapeutic benefit. Diffuse malignant mesothelioma is currently an incurable tumor, but pleurectomy can afford some palliation. Extrapleural pneumonectomy can be accomplished with an acceptable mortal

Natural killer cell activity in asbestos workers. Interactive effects of smoking and asbestos exposure.

Both the numbers and function of natural killer (NK) cells in 60 were evaluated in asbestos cement workers grouped by smoking history and chest roentgenogram findings (ILO profusion scores less than 1/0 or greater than or equal to 1/0, or isolated pleural plaques). Worker and control subjects who smoked had smoking histories of less than 27 pack-years, a level of smoking lower than that previously determined to adversely affect NK function. Asbestos workers who did not smoke had percentages and total numbers of NK cells and NK function not different from that of nonsmoker control subjects. Wor

Asbestos-related focal lung masses: manifestations on conventional and high-resolution CT scans.

In 260 asbestos-exposed individuals evaluated by means of computed tomography (CT), 43 unsuspected pulmonary masses were found in 27 individuals. The masses included fissural pleural plaques (n = 10), dense fibrotic bands (n = 3), round atelectasis (n = 11), carcinomas (n = 3), and other presumed benign masses (n = 16). The most helpful features in the diagnosis of rounded atelectasis with CT were (a) contiguity to areas of diffuse pleural thickening, (b) a lentiform or wedge-shaped outline, (c) evidence of volume loss in the adjacent lung, and (d) a characteristic "comet tail" of vessels and

Phenotypes of peripheral blood lymphoid cells in patients with asbestos-related pleural lesions.

Asbestos-related parietal pleural plaques develop slowly and are of little clinical significance. Other asbestos-related pleural reactions, for example acute exudative pleurisy and progressive pleural fibrosis, are of clinical importance. The pathogenesis of these reactions is unknown, but one hypothesis is that immunological disturbances are involved. To investigate this hypothesis a phenotypic characterization of lymphoid cells was performed in the peripheral blood of 45 patients with asbestos-related pleural lesions; 20 with pleural plaques (PP), 15 with diffuse pleural fibrosis (DPF), and

The radiographic pleural abnormalities in asbestos exposure: relationship to physiologic abnormalities.

The effects of asbestos-induced benign pleural conditions on pulmonary function have been controversial since this subject was first studied in the mid-1960s. Firm conclusions have been difficult to reach because of (1) the difficulty of taking into account asbestos exposure, which may have effects on pulmonary function other than those mediated through pleural lesions, (2) the disagreement over the type and extent of radiographic pleural abnormalities, (3) the imprecision in measuring pulmonary function, and (4) the numerous potential confounding factors of reduced pulmonary function, such as

Between- and within-reader variability in the assessment of pleural abnormality using the ILO 1980 international classification of pneumoconioses.

Although there are published data concerning reader variability in the assessment of parenchymal pneumoconiotic changes using the ILO Classification of Radiographs, nothing has been published on reader variability with regard to pleural abnormalities. Therefore, in the context of an epidemiologic study, we assessed between- and within-reader variability in the reading of chest radiographs for pleural abnormality using the ILO 1980 International Classification of Radiographs of Pneumoconioses. Chest radiographs of 182 insulation workers interspersed with 24 subjects without documented exposure

Asbestos-related pleural plaques and lung function.

The present study examines the association between asbestos-related pleural plaques and lung function in a group of workers with occupational exposure to asbestos. Exposure, smoking, and respiratory histories, chest radiographs, flow-volume loops, and single breath DLCOs were obtained on 383 railroad workers. A score based on the ILO-1980 classification system was used to quantify the extent of plaquelike thickening. In order to eliminate potential confounders, we excluded from final analysis subjects with diffuse pleural thickening (n = 10) or small irregular opacities classified as profusion

Issues and controversies in the plain-film diagnosis of asbestos-related disorders in the chest.

This article reviews the chest radiographic manifestations of asbestos exposure. While the chest radiograph (CXR) is a highly valuable tool in the evaluation of asbestos-related disease, there are ongoing controversies regarding the sensitivity and specificity of the plain film in diagnosing asbestos-related disorders. Autopsy series indicate that at least 60% of pleural plaques may be overlooked. Conversely, such series indicate that up to 20% of plaques are falsely diagnosed. The significance of visceral pleural thickening and the definition and positive predictive value of diffuse pleural t

Environmental pleural plaques in residents of a Quebec chrysotile mining town.

We report four cases of pleural plaques found at autopsy in individuals who resided in or near the chrysotile mining town of Thetford Mines, Quebec, and who had never been employed in the chrysotile mining and milling industry. Three of these patients were farmers, and one was a road construction worker. Lung asbestos content of these cases was compared with that of a group of nine persons living in the same vicinity who did not have pleural plaques. The plaque group was found to have an equal chrysotile content but about a fourfold elevation in median tremolite content, a statistically signif

Lung function and respiratory symptoms in subjects with asbestos-related disorders: a cross-sectional study.

The prevalence of respiratory symptoms and lung function impairment was studied in a sample of men from a population screening of asbestos-related disorders. When the rates were adjusted for age and smoking habits, 83 subjects with lung fibrosis had an increased prevalence of respiratory symptoms, in particular, phlegm when coughing and breathlessness grades 1-3. Among 200 subjects under 70 years of age who had pleural plaques only, a statistically significant increase was observed in the prevalence of breathlessness grade 1 compared to an external reference population. Among 98 asbestos-expos

Screening of autopsy populations for previous occupational exposure to asbestos.

A screening procedure to select, in autopsy populations, subjects having a major likelihood of previous occupational exposure to asbestos is described. To test our necropsy population we searched for pleural plaques (PPs); the optical count of both lung asbestos bodies (ABs) and uncoated mineral fibers (UMFs) at least 10 microns in length was recorded. In the adult autopsy population studied in the Turin area, the predictive value given by a positive test for large-size PPs (mostly bilateral) was about 55%. This level of probability did not rise in relation to the AB counts, whereas an increas

[Pleural plaques and an asbestos environment in Northern Corsica].

As opposed to North-West Corsica, North-East Corsica is rich in asbestos outcrops and a source of environmental pollution as can be seen in the airborne concentration of chrysotile and above all tremolite. We have composed the frequency of asbestos pleural plaques occurring in subjects in the two regions starting with a retrospective analysis of the radiological records of 1,721 patients born in Northern Corsica and having no occupational exposure to asbestos. The frequency is 3.8% in the North-East and 1.1% in the North-West (p less than 5%). The relative East-West risk, which is 3.3%, shows

Cancer morbidity in Swedish shipyard workers 1978-1983.

The cancer morbidity in 3787 shipyard workers was studied between 1978 and 1983. In these shipyards the use of asbestos was abandoned in 1972. The overall cancer morbidity was found to be similar to that of the male population of the same city, but there were four cases of mesothelioma. There were 11 cases of lung cancer, as opposed to 9.8 expected cases. Men with both heavy and long exposure to asbestos had no increased risk of lung cancer. The occurrence of pleural plaques was not associated with the risk of developing cancer.

High incidence of malignant pleural mesothelioma in neighbouring villages of Northwestern Greece.

Between 1981 and 1985 seven patients from three villages of the Metsovo area in Northwestern Greece (population 5000) developed malignant pleural mesothelioma (MPM). The diagnosis was made with pleural biopsy and pleural fluid cytology. Six of these patients have died 18-24 months after the first symptoms (usually dyspnea on exertion) and 1 is still alive after 24 months. Seven MPMs in 5,000 in five years is about 280 times the expected incidence of 1/1,000,000/year. In the same area, endemic pleural calcifications linked to nonoccupational asbestos exposure have recently been reported, but no

Estimating asbestos exposure: a comparison of methods.

The association between pleural plaques and asbestos exposure was investigated in a cross-sectional study of 951 male shipyard workers. Asbestos exposure was estimated by the men themselves and also by an expert panel whose judgment was based only on occupational title. The analysis was restricted to men who had had at least 20 years of exposure and who had not changed jobs during their period of employment in the shipyards. The results showed that there was a much closer correlation between the occurrence of pleural plaques and the men's own estimates of exposure than between the occurrence a

Association of Metsovo lung and pleural mesothelioma with exposure to tremolite-containing whitewash.

Pleural thickening, bilateral pleural hyalinised plaques, and restrictive lung function are found among inhabitants of four small villages in northwestern Greece. Transbronchial biopsy samples from patients with disease contained tremolite fibres. Malignant pleural mesothelioma has now been reported in these villages and accounts for approximately 1% of the total mortality from 1981 to 1985. The principal whitewash once used in this area is predominantly asbestiform tremolite. The fibre is identical in every respect to fibres found in the lung tissues of people with Metsovo lung. The membrane

Asbestos induced diffuse pleural fibrosis: pathology and mineralogy.

Lungs from seven cases of diffuse pleural fibrosis with known asbestos exposure were studied to determine the gross and microscopic pathological features and relate these to the analysed mineral fibre content of the lung. All seven individuals had had substantial exposure, ranging from two to 25 years, and chronic chest problems and at necropsy all cases met the criteria for compensatable disease. Macroscopically, all had extensive visceral pleural fibrosis and extensive areas of adhesions, and four also had discrete parietal pleural plaques. The histological features were similar in all the c
