The radiographic changes in 200 patients with known or suspected asbestos exposure were reviewed. Pleural plaques or thickening, with or without calcification, occurred solely or predominantly on the left side in 90, on the right side in 32, and equally bilaterally in 44. Asymmetry is common in asbestos-related disease, and apparently unilateral findings should prompt a search for more subtle changes elsewhere.

Among 1,577 persons with asbestos exposure followed up from 3 to 30 years, 113 had thoracic surgical procedures for asbestos-related disorders. Twenty-six individuals suspected of having asbestosis with atypical features underwent open-lung biopsy; a different disease was revealed in 14. Most of the 29 patients with mesothelioma had a small thoracotomy for diagnosis only; chemotherapy in half of them proved entirely ineffective. Experience with 23 patients with bronchogenic carcinoma did not differ from that in persons not exposed to asbestos. Problems of causal relationship are discussed. Mos

A four year follow up of the ventilatory function in former asbestos cement workers has been performed to determine whether any further decrease occurred after cessation of exposure. Seventy five of 125 subjects were eligible for re-examination and were compared with local referents. None showed signs of asbestosis but 32% had pleural plaques at the renewed examination. Cumulative asbestos exposure calculated as fibre x years had been estimated individually in the original examination. After adjustment for age, height, tracheal area, and smoking category the FVC and FEV1 for all exposed subjec

A cross-sectional prevalence study of 377 railroad workers was carried out. The purpose was to investigate the prevalence of asbestos-related disease. We compared conventional with more recently adopted criteria for defining abnormal lung function. We compared the test results of those who performed reproducibly on spirometry with those who did not. Medical and occupational histories, flow-volume loops, and posteroanterior and lateral chest radiographs were obtained. Single-breath carbon monoxide diffusing capacity (DLCO) was measured, and chest auscultation was performed. Related to time sinc

28 patients presenting with proven pleuro-pulmonary pathology due to asbestos underwent conventional radiography and also computed tomography (TDM). It is evident that TDM was more sensitive in detecting the presence of pleural plaques which were either hidden or invisible on standard radiographs. False parietal images were often eliminated. With a mesothelioma it can guide the biopsy approach and by visualising contra-lateral plaques that the origin is asbestos. It should enable irregular plaques to be monitored more easily.

Asbestos and other mineral fibers are well known to cause pleural changes. These changes are of various types, the most common being pleural plaques, which are slowly growing and of little clinical importance. Other benign changes include asbestos pleurisies, rounded atelectases, and other pleuropulmonary changes.

This paper reviews the radiographic, clinical, pathologic, and epidemiological features of pleural and pulmonary parenchymal disorders which have been related to asbestos exposure. In particular, the following are discussed: (1) pleural plaques--radiographic detection by plain films and computed tomography, normal and abnormal densities which may mimic plaques, the 1980 ILO U/C classification, recent epidemiological data on plaques including their relationship to carcinoma and mesothelioma; (2) diffuse pleural thickening; (3) benign asbestos pleural effusions; (4) mesothelioma--emphasizing rec

The effect on lung function of exposure in an asbestos cement plant was investigated in male workers, employed more than 10 years. The exposed group was selected to exclude subjects with pneumoconioses and ex-smokers and consisted of 77 smokers and 48 never-smokers. Referents were chosen from plants where asbestos had not been in use and restricted to those without X-ray signs of chest disease. The primary type of asbestos used was chrysotile and the general dust level in the two decades 1950-1970 seems to have been around 10 mg/m3 while the asbestos fiber concentration averaged 2 fibers/ml. An investigation was made to correlate post-mortem findings of pleural plaques (PP) with the smoking habits and occupational history of asbestos exposure of 898 adults from the general population. A significant association between PP and smoking was found in middle-aged and elderly men (p less than 0.001). When occupational history was considered, such an association was present in both exposed (p less than 0.01) and non-exposed (p less than 0.05) men aged over 50 years. No association was observed in non-exposed women, while in those exposed, the absolute number of cases was too small to eval

Marine engineers undergoing routine annual chest roentgenography showed an unusual prevalence of pleural abnormalities including plaques suggestive of past asbestos exposure. A pilot survey, and a subsequent comprehensive study of the films of more than 5,000 men, showed an overall prevalence of 12% with pleural abnormality (typical calcification or plaque, or diffuse thickening). Prevalence of films classifiable for pneumoconiotic small opacities was negligible--1.2% in the pilot study. Prevalences of pleural abnormality were significantly higher among men with longer union membership, after

From 1970 to 1980, mortality from cancer of the lung and stomach was analyzed in a town with asbestos deposits and in a town with asbestos processing. The populations of the entire country, the province, the district, and of all communities of the same size and agricultural index were used as references. In the town with asbestos contamination of air and water from natural tremolite deposits and an endemic occurrence of pleural plaques, no increased risk for lung or stomach cancer was found. In the town with asbestos cement production lung cancer rates were lower in males and higher in females

Thirty patients with known asbestos dust exposure were studied because of uncertainty as to whether or not the pleural changes observed on the radiographs were due to plaques or subpleural fat. The CT scans confirmed that the changes were due to subpleural fat in 14 cases (48%). Characteristic subpleural fat shadows on radiographs and CT scans are described, and the importance of differentiating fat from plaques for medico-legal reasons is emphasized.

To study the clinical usefulness of computed tomography (CT) scanning of the thorax in asbestos related pleuropulmonary disease, 127 long term asbestos workers of the mines and mills in the Eastern Townships of Québec were examined. The CT scan was compared with the standard posteroanterior (PA) chest film and the four view films using the ILO grading system for profusion of disease. Six lung areas and six pleural sites were studied. On the basis of the usual diagnostic criteria, 41% of the workers had asbestosis. For profusion of parenchymal disease, there was an excellent correlation (r = 0.

Thirty patients (27 men and 3 women) with an average industrial asbestos exposure of 19 years have been examined. The results of conventional radiography and CT have been analysed and compared with pulmonary function studies. The advantages of CT depend on a better demonstration of the pleura and lung periphery. In 24% of cases plaques were diagnosed by chest x-rays which could, however, be excluded by computed tomography. Correct prediction of restricted ventilation was possible in 84% patients by means of CT. On the other hand, this prediction by means of conventional radiographs could be ma

This account reviews the clinical and pathological features of asbestosis, with a brief comment on the incidence of asbestosis in workers exposed occupationally to Western Australian crocidolite at Wittenoom Gorge. In keeping with common usage, the term "asbestosis" is restricted to pulmonary parenchymal fibrosis--with or without associated visceral pleural fibrosis--related to inhalation of asbestos fibres, thereby excluding parietal pleural fibrous plaques. The mechanisms underlying the development of the interstitial fibrosis are incompletely understood, but they appear to involve complex i

An investigation was made to correlate autopsy and roentgenographic findings of pleural plaques with occupational exposure to asbestos and occurrence of respiratory tract tumors. Of the 434 autopsies performed over a 2 1/2 year period, 25 (5.8 percent) had pleural plaques but no gross evidence of parenchymal fibrosis. Review of the posterior-anterior chest roentgenograms using the International Labor Office criteria for classification of pneumoconiosis (1980) revealed that only seven of the 25 cases had detectable pleural thickening or calcification, which demonstrates the poor sensitivity of

The lung concentration of coated and uncoated mineral fibres of more than 10 micron in length was determined by light microscopy in 57 autopsied subjects (23 out of which professionally exposed to asbestos) with pleural plaques but no other asbestos-related diseases. Comparisons were made with 57 controls matched for sex, age and smoking habits, who did not show any asbestos-associated diseases or history of previous occupational exposure. Subjects with pleural plaques had a median concentration of coated fibres consistently greater than that observed in those without. However, no substantial

Oblique views of the chest under scopic control and computed tomography have been obtained in 7 patients with a known history of exposure to asbestos fibers, and evidence of pleural plaques on the postero-anterior view of the chest. The distribution of the radiological findings is similar with both the techniques; however, CT is more sensitive in detecting small lesions at the margino-costal anterior and posterior level, both calcific or not. CT seems also more sensitive in detecting parenchymal lesions. The role of CT in this pathology is briefly discussed.
