One hundred and eight-five cases of environmentally-caused malignant pleural mesothelioma were analysed for clinical and radiological pictures. The first group consisted of 122 subjects from villages with asbestos deposits. The second group consisted of 63 patients from the Cappadocia region where there are no asbestos deposits but where zeolite occurs. This presentation is the result of the survey of eight industries with asbestosis risk in the Barcelona area (two of fibro-cement, three of auto brakes, two of textiles, and one of insulation materials). Of 1,472 workers, 271 or 18.5% have asbestosis. Pleural involvement in asbestosis is twice as common as that of the lung, 15.7% versus 8.5%. This study was designed to test the hypothesis that subjects with many asbestos bodies in their lungs at autopsy would also have asbestos bodies in various other organs. All patients newly diagnosed as bronchial carcinoma were studied during a 5-year period. There were 212 male and 61 female patients. The incidence of pleural mesothelioma (PM), chronic fibrosing pleurisy (CFP) and calcified pleural plaques (CPP) has been studied in Anatolia (Turkey) in relation with environmental pollution by mineral fibers: 1) In central Anatolia, where asbestos deposits have been described, the frequency of CFP and CPP was in the range 2-25%; several cases of PM have been encountered. 2) One area (Cappadoce) is free of asbestos, but contains another kind of mineral fiber: zeolite. Bilateral pleural thickening in a relatively asymptomatic patient without evidence of pulmonary disease is frequently a manifestation of asbestos dust exposure, particularly when it is localized or in the form of a noncalcified pleural plaque. Bilateral pleural thickening will be discovered with increasing frequency if specifically searched for on every radiograph. This radiographic finding is suggested as a more realistic indication of asbestos dust exposure, particularly when correlated with a complete occupational history.

All cases of pleural plaques discovered at mass X-ray screening in Uppsala County during 1970-1976 were collected. Case histories, including the history of exposure to asbestos and smoking habits, were taken. A total of 508 cases, 492 males and 16 females, were found. Eighty per cent were smokers, which was more than expected, and 80% confirmed exposure to asbestos. In an agricultural town in Burgenland (Austria) we found an increased prevalence of pleural plaques. These calcifying thickenings of the pleura are related to minimal asbestos exposure such as is mesothelioma, but they cannot be regarded as a precancerosis. The increased occurrence of pleural plaques in this town of nearly 3500 inhabitants (in which during 1916 to 1945 asbestos was mined) we first found at the chest x-ray archives of a pulmologic hospital, then by mass radiography and blind comparison with control groups. The 575 inhabitants of the remote Anatolian village of Karain suffered 11 deaths from pleural mesothelioma in 1975/76 and there were five cases of fibrosing pleurisy. In the previous five years there had been 25 cases of mesothelioma. Calcified pleural plaques were common on survey radiography. Asbestos does not occur in the local soil or rock, nor is it handled in the village, but a few fibres were found in the water. Fibres were also found in the pleural tissue of two of five cases examined. Inhabitants of the neighbouring villages are free of mesothelioma.

Regional lung function was measured using radioactive xenon-133 in a group of normal subjects and in two groups of asbestos workers. When compared with the normal group, patients with pulmonary asbestosis showed impaired ventilation of the lower zones. Subjects with calcified pleural plaques without radiological evidence of lung parenchymal fibrosis did not show this abnormality.

In 198 randomized autopsies of adults, hyaline pleural plagues were found in 49 men and 17 women; bilateral in 47 cases, unilateral in 19. No correlation between previous pr present pulmonary disease and hyaline plaques was found. Asbestos bodies were found in nine men and five women; nine out of these 14 cases with asbestos bodies also had hyaline pleural plaques.
