Case Report: Catamenial pneumothorax: clinical characteristics, diagnosis, and management - a report of two cases with a review of the literature.

Pneumothorax is a pathological condition in which gas enters the pleural cavity due to trauma, surgery, positive-pressure ventilation, thoracentesis, or other causes, leading to compression and collapse of the lung tissue. It exhibits a male predominance and can occur in all age groups. Catamenial pneumothorax, also referred to as thoracic endometriosis syndrome-associated pneumothorax, is a rare subtype. It is characterized by spontaneous and recurrent pneumothorax occurring in women of reproductive age during the perimenstrual period. Clinicians often fail to associate pneumothorax with the

Thoracoscopic Management of Recurrent Pneumothorax in an Adult Patient with Cri du Chat Syndrome and Severe Scoliosis: A Case Report.

INTRODUCTION: Cri du chat syndrome (CdCS) is a rare genetic disorder that can be associated with scoliosis and marked thoracic deformity. Although scoliosis has been reported to be associated with an increased incidence of pneumothorax, reports of pneumothorax surgery in patients with severe scoliosis associated with CdCS are extremely rare. Herein, we report a case of recurrent pneumothorax in an adult patient with CdCS associated with severe scoliosis who was successfully treated by thoracoscopic surgery. CASE PRESENTATION: A 41-year-old man with CdCS presented with severe scoliosis, intell

Bedside flexible bronchoscopy in the neonatal intensive care unit: Current evidence, clinical applications and future directions.

BACKGROUND: Lung ultrasound and targeted echocardiography let the neonatal team examine lungs and heart at the cot side, but neither shows the airway. Flexible bronchoscopy does, and it is increasingly performed inside the neonatal intensive care unit (NICU). METHODS: Narrative review of published series of flexible bronchoscopy in infants cared for in neonatal units, supported by pediatric intensive care studies and society standards. Certainty of evidence was not graded. RESULTS: Twelve neonatal series (652 infants) were identified; all but one were single-center and most were retrospectiv

An Alternative Approach to Adult One-Lung Ventilation Using a Standard Single-Lumen Endotracheal Tube.

Endobronchial intubation with a double-lumen tube (DLT) is a frequently used method for achieving one-lung ventilation (OLV) in adults, together with simultaneous contralateral lung deflation during intrathoracic surgical procedures. Alternative methods of providing OLV include the bronchial blocker (BB), which comes in several different models (Univent {Columbia, MD: Ambu Inc.}, Arndt blocker {Bloomington, IN: Cook Medical}, Cohen Flex-Tip blocker {Bloomington, IN: Cook Medical}, EZ-Blocker {Morrisville, NC: Teleflex Incorporated}). This case report presents another cost-effective alternative

Spontaneous Pneumothorax During Stereotactic Body Radiotherapy for Pulmonary Metastasis From Osteosarcoma: A Case Report and Literature Review.

Pulmonary metastases represent the predominant pattern of distant disease in osteosarcoma and are associated with a spectrum of thoracic complications. Among these, spontaneous pneumothorax is an infrequent but clinically important event. Here, we present the case of a young man who developed spontaneous pneumothorax during the course of stereotactic body radiotherapy (SBRT) for a subpleural pulmonary metastasis delivered using the Deep Inspiratory Breath Hold (DIBH) technique. The pneumothorax was detected on routine image guidance after the fourth fraction of treatment. Despite this unexpect

Safety and diagnostic yield of transbronchial lung cryobiopsy in patients with interstitial lung disease and severe pulmonary function impairment.

BACKGROUND: Transbronchial lung cryobiopsy (TBLC) has become an established diagnostic modality in interstitial lung disease (ILD). However, current guidelines frequently exclude patients with severely impaired lung function due to safety concerns. This study aimed to evaluate the safety and diagnostic yield of TBLC in patients with advanced pulmonary function impairment. METHODS: We conducted a retrospective analysis of consecutive patients with suspected ILD who underwent TBLC for diagnostic evaluation at Thoraxklinik Heidelberg, University Hospital Heidelberg, between November 2017 and Dec

Comparative outcomes of central venous access via open jugular and ultrasound-guided subclavian approaches in low-weight pediatric patients: a study with secondary outcomes.

BACKGROUND: The necessity of this study stems from the lack of comparative evidence on the outcomes of central venous catheterization via the open internal jugular and ultrasound-guided subclavian approaches in low-weight pediatric patients. Its innovation lies in the simultaneous evaluation of both primary and secondary outcomes of these two techniques in this vulnerable population. METHODS: This prospective randomized clinical trial at Mardani Azar Children's Hospital (winter 2022-2023) included 92 pediatric patients under five kilograms weight, randomly assigned to either open internal jug

AI-augmented cardiopulmonary point-of-care ultrasound: a narrative review for pulmonary and critical care clinicians.

Point-of-care ultrasound (POCUS) is an increasingly utilised tool in pulmonary and critical care medicine, yet its effectiveness remains limited by operator dependency and the required cognitive demands of real-time image acquisition and interpretation. Artificial intelligence (AI) has emerged as a promising approach to augment POCUS by supporting image acquisition, interpretation and quantitative analysis. Given the rapid evolution of this field, clinicians must understand both the capabilities and limitations of AI-enabled POCUS. However, prior reviews have largely catalogued applications wi

A modified cryobiopsy specimen retrieval method using a thin bronchoscope and small guide sheath for the diagnosis of peripheral pulmonary lesions.

BACKGROUND: Retrieval of 1.1-mm ultrathin cryobiopsy specimens through a large guide sheath (GS) enables specimen collection without removal of the therapeutic bronchoscope and GS; however, this approach remains limited to selected patients because therapeutic bronchoscopes have restricted access to the peripheral bronchi. To expand the applicability of cryobiopsy to more peripheral pulmonary lesions (PPLs), we applied a modified cryobiopsy specimen retrieval approach, termed the ultrathin CRYObiopsy specimen with Guide sheath en bloc RetrievAl THrough a bronchoscoPe channel (CRYO-GRATHP) meth

Traumatic pneumatoceles in adulthood: A case highlighting the limitations of supine chest radiography in trauma.

Traumatic pneumatoceles are uncommon pulmonary pseudocysts that may occur following blunt thoracic trauma and can be entirely occult on chest radiography, potentially mimicking other cavitary lung pathologies. We present the case of a 52-year-old male transferred to a tertiary trauma center following a motorcycle accident. On presentation, the patient was hypoxic with extensive right-sided chest wall injuries. Initial supine chest radiography demonstrated multiple right-sided rib fractures, patchy airspace opacities, a veiling opacity, and a suspected pneumothorax, raising concern for a large

Non-intubated Video-Assisted Thoracoscopic Pleurodesis Under Paravertebral Block in a Patient With Fibrotic Hypersensitivity Pneumonitis and Chronic Respiratory Failure.

Non-intubated video-assisted thoracoscopic surgery (NIVATS) has increasingly been reported as an alternative to general anesthesia in patients at high risk of complications associated with mechanical ventilation. Fibrotic hypersensitivity pneumonitis (FHP) with chronic respiratory failure may pose an indication for this approach, given the risk of positive-pressure ventilation precipitating acute exacerbation of the underlying interstitial lung disease, even in patients with an otherwise uncomplicated intraoperative course. We report the anesthetic management of a 42-year-old woman with FHP an

Hydropneumothorax as a Diagnostic Clue to Pulmonary Tuberculosis With Pleural Involvement: A Case Report.

Hydropneumothorax represents an unusual presentation of pulmonary tuberculosis (TB), posing diagnostic and therapeutic challenges. We report the case of a 36-year-old woman who presented with productive cough, dyspnea, right-sided pleuritic chest pain, and constitutional symptoms. Chest imaging revealed a large right-sided hydropneumothorax with extensive parenchymal and cavitary disease. Sputum smear microscopy was positive for acid-fast bacilli, and molecular testing of both sputum and pleural fluid detected Mycobacterium tuberculosis complex, confirming pulmonary TB with pleural involvement

Traumatic High Cervical Spine Injury Presenting as Cardiac Arrest: A Case Report.

Traumatic high cervical spinal cord injury is a potentially devastating condition that may cause profound cardiovascular and respiratory instability through disruption of descending sympathetic pathways. In severe cases, autonomic dysfunction may contribute to marked hypotension, bradycardia, and cardiac arrest. We report the case of a 47-year-old man who sustained blunt trauma of incompletely characterized mechanism in a workplace accident and was found in cardiac arrest by a prehospital emergency medical team. Advanced life support was initiated, and return of spontaneous circulation was ach

Acupuncture-Induced Pneumothorax Following Thoracic Acupuncture in a Patient With Ehlers-Danlos Syndrome: A Case Report.

Acupuncture is widely used as an adjunctive therapy for chronic pain and headache disorders and is generally considered a safe procedure when performed by trained practitioners. However, pneumothorax remains one of its most serious, albeit uncommon, complications. Patients with connective tissue disorders, such as Ehlers-Danlos syndrome (EDS), may theoretically be more susceptible to procedure-related complications because of underlying tissue fragility, although reports describing acupuncture-induced pneumothorax in this population are exceptionally rare. We present the case of a young woman

Oscillation and Lung Expansion Therapy as Rescue Airway Clearance in Critically Ill Adults: A Case Series.

INTRODUCTION: Retained airway secretions and lobar collapse are common in hospitalized adults with advanced lung disease. Conventional airway clearance depends on respiratory effort, requiring both an adequate inspiratory volume and sufficient expiratory airflow, and on cough strength, and therefore fails most often in the patients who need it most. Oscillation and lung expansion (OLE) therapy delivers continuous positive expiratory pressure (CPEP), continuous high-frequency oscillation (CHFO), and aerosolized medication in a single 10-min treatment, but reported experience is largely confined

Radiological Confirmation of Enteral Feeding Tube Placement Before Initiating Enteral Nutrition: A Review.

Radiological confirmation of enteral feeding tube placement is widely regarded as the gold standard for verifying the position of blind-inserted feeding tubes before initiating enteral nutrition. Misplacement of feeding tubes into the respiratory tract or incorrect positioning within the gastrointestinal tract can lead to severe complications, including aspiration pneumonia, pneumothorax, pulmonary injury, and death. Although bedside methods such as pH testing are useful for ongoing monitoring, radiographic imaging remains the most accurate technique for confirming initial tube placement. This

Description of Endobronchial Ultrasound-Guided Transbronchial Mediastinal Cryobiopsy for Mediastinal Lesions: A Real-World Study.

Endobronchial ultrasound-guided transbronchial mediastinal cryobiopsy (EBUS-TBNC) enables the acquisition of larger and higher-quality tissue samples than conventional endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA); however, evidence regarding its use and outcomes in Latin America remains limited. This retrospective observational study describes the clinical and procedural characteristics, the frequency of diagnosis obtained by histopathology, and the safety of patients subjected to EBUS-TBNC between June 2023 and December 2025. Tension Gastrothorax in a Child With Undiagnosed Congenital Diaphragmatic Hernia: A Case Report and Proposed Emergency Management Algorithm.

BACKGROUND: Tension gastrothorax is a rare but life-threatening pediatric emergency that most commonly results from herniation of the stomach through an unrecognized congenital diaphragmatic defect, followed by progressive intrathoracic gastric distension and compression of intrathoracic structures. Because its clinical presentation often mimics tension pneumothorax, delayed diagnosis, or inappropriate management may rapidly lead to obstructive shock, respiratory failure, and cardiac arrest. Prompt recognition and timely intervention are therefore essential to prevent fatal outcomes. CASE PRE
