A Case of Exuberant Type 2 Pneumocyte Hyperplasia in a Young Man with Recurrent Spontaneous Pneumothorax: A Diagnostic Pitfall.

INTRODUCTION: Exuberant type 2 pneumocyte hyperplasia is a rare histopathological pattern of reactive epithelial change associated with spontaneous pneumothorax, in which preoperative clinical findings and radiological images usually show only nonspecific bullae and blebs. However, it is difficult to differentiate this entity from other conditions, especially pulmonary epithelial tumors, creating significant diagnostic pitfalls in postoperative histopathological evaluation. CASE PRESENTATION: A 28-year-old man presented with recurrent left-sided spontaneous pneumothorax. Preoperative chest CT

Association between fentanyl dose and willingness to undergo repeat bronchoscopy in a standardized EBUS-GS-TBB setting.

BACKGROUND: Discomfort experienced during bronchoscopy may limit patients' willingness to undergo repeat procedures. Although guidelines recommend benzodiazepine-opioid sedation, evidence regarding optimal opioid dosing remains limited. We evaluated whether the total fentanyl dose was associated with willingness to undergo repeat bronchoscopy in a standardized endobronchial ultrasonography with guide-sheath transbronchial biopsy (EBUS-GS-TBB) setting. METHODS: We conducted a retrospective single-center study of consecutive patients who underwent radial endobronchial ultrasonography with guide

Impact of Artificial Intelligence Assistance on Diagnosing Traumatic Pneumothorax: A Comparison of Specialist and Non-Specialist Emergency Physicians.

INTRODUCTION: Supine chest radiography is routinely used in trauma care; however, its sensitivity is limited in pneumothorax detection. Although artificial intelligence (AI) has recently been introduced as a diagnostic support tool for chest radiograph interpretation, its use in trauma radiographs and its interaction with physician experience remain unclear. We aimed to evaluate the diagnostic performance of AI-assisted image interpretation in traumatic pneumothorax detection among emergency medicine specialists and non-specialists. METHODS: In this retrospective single-center study, 34 supin

Experience With More Than 1,000 Cuffed Tunneled Hemodialysis Catheter Insertions Over Four Years at a Single Center.

INTRODUCTION: Cuffed tunneled hemodialysis catheters are an important vascular access option for patients with end-stage renal disease (ESRD) when arteriovenous fistula (AVF) creation is not feasible or when urgent initiation of hemodialysis is required. However, clinical outcomes may vary according to the catheter insertion site. OBJECTIVE: To compare procedural safety, biochemical outcomes, and six-month clinical outcomes among different tunneled hemodialysis catheter insertion sites. METHODS: This retrospective observational study included 1,050 consecutive patients who underwent tunneled

Exploring Inter-Rater Agreement in Classification of Radiological Features of Tension in Adults With Primary Spontaneous Pneumothorax.

OBJECTIVE: To explore agreement between emergency physicians in classifying chest x-rays (CXR) of patients with primary spontaneous pneumothorax (PSP) as showing the presence/absence of so-called radiological tension features. METHODS: Observational study. Five emergency physicians blinded to patient characteristics, radiologist report, patient outcome and other reviewers' classification assessed each of 80 PSP CXR and classified them as showing radiological tension features or not. Analysis using Krippendorff's Alpha (K-Alpha). RESULTS: Inter-rater agreement was 0.524 (95% CI: 0.400-0.623)-

Cabozantinib-related pneumothorax in responsive patients with renal cell carcinoma: a case report.

BACKGROUND: Cabozantinib is a multi-kinase inhibitor that has been shown to be effective in the treatment of unresectable or metastatic renal cell carcinoma and unresectable hepatocellular carcinoma that worsened after anticancer drug therapy. Tyrosine kinase inhibitors (TKIs) are known to suppress tumor angiogenesis by inhibiting vascular endothelial growth factor (VEGF). Furthermore, cabozantinib possesses inhibitory effects against AXL and mesenchymal-epithelial transition (MET) not seen with other TKIs. Consequently, cabozantinib may exhibit stronger anti-angiogenic activity than other vas

Prophylactic versus selective use of surfactant for preventing morbidity and mortality in preterm infants at risk of respiratory distress syndrome.

BACKGROUND: Respiratory distress syndrome (RDS), or hyaline membrane disease, is a common condition in preterm infants (< 37 weeks' gestation) and a leading cause of neonatal morbidity and mortality. The risk is highest in extremely preterm (< 28 weeks) and very preterm (28 to < 31 weeks) infants due to immature lung and cardiovascular development. RDS results from a deficiency or dysfunction of pulmonary surfactant, which lines the alveoli to reduce surface tension, prevent atelectasis, and protect the lungs. Surfactant is primarily composed of dipalmitoylphosphatidylcholine (DPPC), other pho

Fatal Presumed Re-expansion Pulmonary Edema Following Suction-Assisted Drainage of a Complete Spontaneous Pneumothorax in a Patient With Advanced Emphysema: A Case Report.

Re-expansion pulmonary edema (RPE) is a rare but potentially life-threatening complication of pleural decompression. Although most reported cases are self-limiting, severe presentations may rapidly progress to respiratory failure and death. We report a fatal case of presumed unilateral RPE following drainage of a complete spontaneous pneumothorax in a patient with advanced emphysema. A 51-year-old man with chronic obstructive pulmonary disease presented to the emergency department with progressive dyspnea of approximately one week's duration. On admission, he was ambulatory, hemodynamically st

Fatal Rapidly Progressive Interstitial Lung Disease in Anti-Melanoma Differentiation-Associated Gene 5 (MDA5) Dermatomyositis in a Brazilian Patient: A Case Report.

Anti-melanoma differentiation-associated gene 5 (anti-MDA5) dermatomyositis (DM) is a subtype of idiopathic inflammatory myopathy. It is characterized by prominent cutaneous manifestations, minimal muscle involvement, and a strong association with interstitial lung disease (ILD), particularly rapidly progressive ILD (RP-ILD), a life-threatening complication with high early mortality. Although RP-ILD is less frequently reported in Western populations than in Asian cohorts, severe presentations may still occur. We report the case of a previously healthy 35-year-old woman who presented with typic

Successful Management of a Large Pediatric Traumatic Pneumothorax Using a Thoracic Vent Device After Blunt Chest Injury: A Case Report.

Traumatic pneumothorax is a common complication of blunt thoracic trauma and is traditionally managed with tube thoracostomy, particularly in larger or symptomatic cases requiring pleural decompression. Thoracic vent devices, such as the UreSil Thora-Vent, represent a less invasive pleural drainage option for the management of spontaneous or iatrogenic pneumothorax. However, literature regarding their use in traumatic pneumothorax, particularly in pediatric patients, remains limited. We present a case of a 13-year-old girl who developed a large left-sided traumatic pneumothorax after blunt tho

Diagnostic performance with sensitivity/specificity optimized computer-aided detection to detect common abnormalities in chest radiography.

PURPOSE: To evaluate how two fixed computer-aided detection (CAD) operating modes-sensitivity-optimized (SE-CAD) and specificity-optimized (SP-CAD)-affect radiologists' diagnostic performance in chest radiography (CXR). Previous studies have focused on the performance of standalone CAD at different threshold settings. However, in clinical practice, CAD is commonly used as a decision-support tool for radiologists. This study aimed to evaluate the impact of different CAD threshold settings on radiologists' diagnostic performance. MATERIALS AND METHODS: This retrospective single-center diagnosti

Robotic vs. bedside staplers in robotic lung resection: a real-world cohort study.

Comparative studies of stapler strategy within robotic lung resection remain limited, particularly beyond lobectomy and with contemporary staplers. This study evaluated clinical and economic outcomes associated with robotic versus bedside staplers in robotic lung resections. We conducted a retrospective cohort study using the Premier Healthcare Database (2019-2023), including adults who underwent robotic lobectomy or sublobar resection. Patients were classified into robotic stapler and bedside stapler groups. Overlap weighting (OW), a propensity score weighting method targeting the average tre

Combined Regional Analgesia for Complex Unilateral Clavicular and Thoracic Trauma Undergoing Clavicle Fixation: A Case Report.

Patients with simultaneous clavicular and ipsilateral thoracic trauma, including multiple rib fractures and traumatic pneumothorax, present a complex perioperative analgesic challenge. Effective pain control must be balanced against the preservation of respiratory function while minimizing opioid-related adverse effects. Individual regional anesthesia techniques for clavicle surgery and rib fracture analgesia are well established; however, selecting an appropriate multimodal analgesic strategy for patients with combined injuries requires careful clinical judgment. We report the case of a 51-ye

Diabetes mellitus is associated with short-term postoperative outcomes in patients undergoing vats lobectomy for lung cancer: a 7-year retrospective analysis.

BACKGROUND: Diabetes mellitus (DM) is a prevalent metabolic disorder and a well-recognized perioperative risk factor across non-cardiac surgical populations. However, its correlative association with short-term postoperative complications after video-assisted thoracoscopic surgery (VATS) lobectomy remains insufficiently characterized. This study explores the relationship between DM and postoperative outcomes using a large national inpatient database. METHODS: A retrospective cohort study was conducted based on the 2016-2022 Nationwide Inpatient Sample. Adult patients undergoing VATS lobectomy

Ruxolitinib-Associated Improvement in Post-transplant Air-Leak Syndrome Complicating Steroid-Dependent Organizing Pneumonia: A Case Report.

Air-leak syndrome (ALS), including pneumothorax, pneumomediastinum, and subcutaneous emphysema, is a rare but life-threatening complication after allogeneic hematopoietic stem cell transplantation (HSCT). However, no standard treatment has yet been established. Here, we report the case of a 60-year-old man with acute myeloid leukemia who underwent allogeneic HSCT. He developed chronic graft-versus-host disease (GVHD) and cryptogenic organizing pneumonia (COP) on day 126 after transplantation. Although initial corticosteroid therapy improved the COP, the relapse occurred during tapering, and re
