Tuberculosis: frequency of unusual radiographic findings.

The changing pattern of the radiologic presentation of tuberculosis was noted in a review of 100 consecutive cases of pulmonary tuberculosis. Active tuberculosis was not suggested as a radiologic diagnosis in 43% of the patients. Of the 100 cases, only 66 had characteristics of reactivation tuberculosis (i.e., minimal upper lobe infiltrate or more advanced cavitary disease). Of the other 34 patients, 18 had less usual presentations, making the radiographic diagnosis difficult. The remaining 16 patients had radiographic patterns quite unusual for tuberculosis, including masslike densities resem

[Some interesting cases of pulmonary echinococcosis].

The authors had the opportunity in the interim of the past fifteen years to treat at the hospital in Zadar 277 patients suffering from hydatid disease of different body organs. The most frequently affected was the liver--56.3%, followed by the lungs--24.9% and 18.8% were other organs. Out of the 69 cases of affected lungs, the authors chose several which they considered to be most interesting. These consisted of: 1. A fistula between the cyst in the liver and the right lung. 2. A syndrome of the middle lung lobe caused by the hydatid cyst. 3. The Cumbo's symptom following a perforated lung cys

Trephine air drill, bronchial brush, and fiberoptic transbronchial lung biopsies in immunosuppressed patients.

Seventy-nine nonthoracotomy lung and bronchial biopsy procedures were performed in 52 immunosuppressed patients: 22 renal transplants, 24 lymphoreticular malignancies, and 6 other disorders. The total diagnostic yield was 74 per cent (23 of 31) of the forcepts transbronchial biopsy procedures, 82 per cent (14 of 17) of the percutaneous trephine lung biopsies, and 28 per cent (9 of 31) of the bronchial brush biopsies. An etiologic diagnosis, including a variety of viral fungal, and parasitic diseases, was obtained in 42 per cent (13 of 31) of the transbronchial biopsy procedures and 65 per cent

Refinements in Chiba needle trans hepatic cholangiography.

The recent wide acceptance of Chiba needle transhepatic cholangiography prompted this elaboration of present techniques, intraprocedure observations, and special interpretive considerations based on a series of 100 consecutive examinations. The established safety margin of this technique now permits up to 15 needle passes to obtain a cholangiogram; thus success rates of duct opacification presently approach 100%. Special phenomena related to the small caliber of the needle include duct filling from "invisible" radicles, perivenous or periductal dissection of contrast, and pseudolesions in an i

Percutaneous transthoracic needle aspiration of the lung. Diagnosing Pneumocystis carinii pneumonitis.

Percutaneous transthoracic needle aspiration was performed on 228 occasions to obtain lung specimens from 202 patients with suspected Pneumocystis carinii pneumonitis. In 121 patients the diagnosis was established by identifying P carinii organisms in lung aspirates. Six patients whose aspirates did not contain P carinii were found to have the organism at autopsy. Findings from toluidine blue O and Gomori methenamine silver nitrate stains were equally satisfactory for detecting P carinii, but the percentage of specific diagnosis was higher when specimens were stained with both. Pneumothorax th

Squamous-cell carcinoma of the pleura.

There are few reports of primary malignant tumours of the pleura other than mesothelioma. Reports of squamous-cell carcinoma of the pleura are especially rare. Epidermoid carcinoma of the pleura may occur in connection with persistent empyema drained by a pleurocutaneous fistula or long-standing extrapleural pneumothorax without fistula.

[Percutaneous implantation of permanent pacemakers (author's transl)].

2 methods for the implantation of permanent pacemakers with percutaneous puncture of the subclavian vein are described. The first one was used in 44 patients with the aid of an introducing catheter-set Desilets-Hoffman consisting of guide-wire, inner Teflon dummy catheter and outer thin-walled sheath; through this a shoulder-less electrode catheter no. 8 could be inserted for permanent pacing. The other technique was applied to 20 patients and used thinner special electrodes no. 6F, which could be introduced directly through a plastic cannula inserted with a puncture-needle. The advantages are

Reactive eosinophilic pleuritis: a lesion to be distinguished from pulmonary eosinophilic granuloma.

Pleural nodules or sheets of histiocytes, admixed with eosinophils, giant cells, and other inflammatory cells are observed frequently in patients with spontaneous pneumothorax. This reaction, designated reactive eosinophillic pleuritis (REP), can closely resemble eosinophilic granuloma. Reactive eosinophilic pleuritis was found in pleural tissue of 22 of 57 patients with spontaneous pneumothorax in whom tissue was available for study. None of these patients had clinical or radiographic evidence of interstitial lung disease. Follow-up of 20 patients with REP ranged from six months to five years

Evacuation of pericardial, anterior mediastinal, and peripleural air collections in neonatal respiratory distress.

Alveolar rupture in the neonate can result in the dissection of air along vascular sheaths and the formation of abnormal collections within the mediastinal, pleural, pericardial, or peripleural spaces or elsewhere. Intercostal tube placement for pneumothorax is frequently required. A method is presented for the placement of Intracath catheters into pericardial, anterior mediastinal, and peripleural collections of air when other adjustments in treatment have not allevaited clinical distress.

Pneumoperitoneum associated with pneumothorax: a surgical dilemma in the post-traumatic patient.

Two patients with pneumoperitoneum associated with post-traumatic pneumothorax are described. The role of artificial respiration in the production of pneumoperitoneum is descussed. This association presents a curious diagnostic dilemma. If the diagnosis of perforated hollow viscus can be eliminated with considerable certainty, then conservative management with careful observation and monitoring may avoid unnecessary surgery, so long as other causes of pneumoperitoneum have been ruled out.

PAO2-PaO2 relationship for the entire therapeutic range of oxygen in acute respiratory failure.

The PAO2-PaO2 relationship was studied for the entire therapeutic range of oxygen in patients and animals in acute respiratory failure. The method is based on the assumption that the steady state values of PaO2 may be obtained as a mean of the two PaO2 values at an identical F1O2, one obtained 6 min after the F1O2 was raised from a lower level, the other obtained 6 min after the F1O2 was reduced from the higher level. We found that the shunts were large in the low F1O2 range (170 mmHg and below), took a minimum value in the moderate PAO2 range (170 to 300 mmHg), and increased again in the high

[The spontaneous, non-traumatic chylothorax. Therapy by means of pleurectomy and decortication].

Two cases of chronic spontaneus chylothorax were successfully treated by small thoracotomy with parietal pleurectomy or decortication after unsuccessful needle aspiration and intercostal tube drainage with suction. In the one case the chylous effusion occurred spontaneously 29 years after extrapleural pneumothorax. The tuberculosis was long cured. In the other, apparently idiopathic case, the chylothorax on the left side disappeared completely after pleurectomy. Six months later a chylous effusion appeared on the right side. Mediastinoscopy then revealed an oatcell carcinoma in lymph nodes wit

A prospective survey on chest malignancies following multiple fluroscopies during artificial pneumothorax therapy for pulmonary tuberculosis.

Patients with pulmonary tuberculosis treated in 4 sanatoria in Niigata prefecture during the period from 1941 to 1961 were followed up. Of 2,756 patients, 1,193 responded and sent us effective information. However, 1,224 did not come into contact because of changed and unknown adress, 326 did not respond, and 13 were omitted because of incomplete answers. Of 1,193 effective responders, 568 had been treated by artificial pneumothorax (the pneumothorax group), and 552 had been treated by the other measures than pneumothorax (the control group). There were 65 deaths in the pneumothorax, group, an

Blast injuries to the lungs: clinical presentation, management and course.

Five patients with blast injuries to the lungs after bomb explosions are reported. In each patient radiological changes were apparent on the initial chest film taken within 4 hours of the explosions. Arterial hypoxaemia was also present. Four patients were actively treated with continuous positive-pressure ventilation, which was adjudged effective therapy. Two patients died, one owing to bilateral pneumothorax which occurred during anaesthesia, and the other owing to overwhelming infection. Hypoxaemia persisted for 4 months in one of the survivors. Lung function tests which were performed on t

Spontaneous pneumothorax: the sharp rib syndrome.

Of 49 patients undergoing operation for benign spontaneous pneumothorax, 28 (57%) were found to have a sharp first or second rib. In a series of 100 patients undergoing thoracotomy for other conditions, only eight (8%) were found to have a sharp rib. The association between sharp ribs, apical scars and bullae, and spontaneous pneumothorax is discussed.

Subpulmonic pneumothorax in patients with chronic obstructive pulmonary disease.

A spontaneous pneumothorax is occasionally seen under the lung on upright views, apparently due to subpulmonic trapping of pleural air by adhesions. This is most likely to occur in patients with chronic obstructive pulmonary disease (COPD), especially if they have extensive parenchymal scarring from previous tuberculosis. The authors describe 4 patients who had subpulmonic pneumothorax, severe COPD, and parenchymal scarring and presented in respiratory failure. None had other clinical symptoms suggesting pneumothorax. Radiologists should be aware of this condition, since it is potentially leth

Roentgenologic diagnosis of interstitial plasma cell pneumonia in infancy.

Sixt-two series of radiographs were analyzed. Interstitial plasma cell pneumonia (IPCP) was divided into six radiographic stages. A schematic description of these stages is given. The correlation between this scheme and the actual radiographs is illustrated with a series of nine antero-posterior and lateral radiographs from one infant. The patterns of the individual stages of the disease are then discussed with the aid of pulmonary radiographs of 12 infants and one 12-month-old child with dysgammaglobulinemia. From a prospective and retrospective analysis of these radiographs, we could delinea

[Spontaneous pneumothorax in women].

Among cases of spontaneous pneumothorax, female cases are extremely rare. They represent only about 10% of observed cases and form a special clinical type. The authors discuss 20 cases out of a total of 261 cases of pneumothorax observed over a period of 7 years. A certain number of facts were noted which distinguished the female forms from the male forms of this disease. Although there is no difference in the symptoms, the disease tends to occur later in women, in the fourth decade of life, and only three of the 20 cases in this series were idiopathic. On the other hand, among the secondary c

Esophageal disruption from blunt and penetrating external trauma.

Eleven patients suffered esophageal perforations from external trauma. This series includes the first report of esophageal perforation that resulted from a cervical flexion-hyperextension injury. The other ten patient had penetrating trauma. Early signs were subtle. Small amounts of mediastinal and cervical air tended to be overlooked or erroneously attributed to other causes, such as associated pneumothorax. Once suspected, the possibility of esophageal disruption was not always pursued with optimum vigor. There was undue reliance on contrast media radiography. There were two patients with fa
