Lymph Node Dissection and Postoperative Complications After Lung Cancer Resection.

IMPORTANCE: Current lung cancer guidelines recommend lymph node sampling (LNS) with the 3 + 1 rule, which recommends station-based sampling with at least 3 N2 (mediastinal) and 1 N1 (hilar) nodal stations. Although there is oncologic benefit to rigorous LNS, potential negative impacts on surgical outcomes have not been well investigated. As adoption of the 3 + 1 rule increases given its formalization as guideline-concordant care, associations with adverse outcomes are crucial to characterize. OBJECTIVE: To evaluate whether satisfying the 3 + 1 rule is associated with increased postoperative c

A New Concept of Costal Cartilage Harvest in the Reduction of Postoperative Pain at Donor Site.

BACKGROUND: Autologous costal cartilage was preferred in Chinese rhinoplasty. However the severe postoperative pain at the donor site was a common morbidity, which hindered patient's daily activities. OBJECTIVE: To evaluate postoperative pain at the donor site after harvesting the 6th costal cartilage by preserving interchondral joints (ICJ) with its adjacent 5th and 7th costal cartilages. METHODS: Forty patients underwent the 6th costal cartilage harvest for rhinoplasty, with the preservation of ICJ with its adjacent 5th and 7th costal cartilages in 20 patients (PICJ group), and non-preserv

An Unusual Cause of Post-cesarean Intraperitoneal Hemorrhage: Inadvertent Intra-abdominal Placement of a Femoral Venous Catheter.

Severe abruptio placentae is a major obstetric emergency that may require aggressive resuscitation and central venous catheterization. Femoral venous access is often selected in this context because of its accessibility and absence of pneumothorax risk; however, inadvertent intraperitoneal malposition of the catheter is an exceptionally rare and potentially life-threatening complication. We report the case of a 25-year-old woman at 36 weeks of gestation, admitted for abruptio placentae with intrauterine fetal death and hemorrhagic shock, who underwent emergency cesarean section and was transfe

Surgical technique of uniportal VATS for pneumothorax: the lateral and subxiphoid approaches.

Uniportal video-assisted thoracoscopic surgery (VATS) is considered the optimal access method for pneumothorax surgery. Currently, uniportal VATS is used worldwide because it requires an incision in only one intercostal space, which simplifies postoperative pain control, due to reduced acute pain, and the incidence of chronic pain is also potentially lower. It is essential to tailor the surgical technique to each individual case, ensure reliable closure of air leaks, and minimize the risk of recurrence. This article introduces the practical aspects of uniportal VATS for pneumothorax, focusing

Micro-incision uniportal video-assisted thoracoscopic surgery for primary spontaneous pneumothorax in an adolescent: a technical case report.

BACKGROUND: Primary spontaneous pneumothorax (PSP) commonly affects adolescents and young adults, with recurrence reaching 30%. While uniportal video-assisted thoracoscopic surgery (U-VATS) has improved cosmetic outcomes and reduced access trauma, micro-incision access (<2 cm) introduces significant technical constraints, particularly regarding triangulation and instrument crowding. This report describes the successful management of an adolescent PSP case using a 1.8-cm U-VATS bullectomy, specifically addressing the technical challenges of ultra-small access through targeted ergonomic refineme

Surgical strategies for spontaneous pneumothorax: a narrative review.

BACKGROUND AND OBJECTIVE: Spontaneous pneumothorax encompasses both primary spontaneous pneumothorax (PSP) and secondary spontaneous pneumothorax (SSP), which differ in etiology and recurrence risk. Surgical management aims to control air leaks and prevent recurrence while minimizing invasiveness. This narrative review synthesizes current strategies, adjunctive methods, and special scenarios, and outlines future perspectives. METHODS: A narrative search of PubMed and major thoracic surgery journals was conducted for English-language publications published between January 1991 and January 2026

Non-intubated video-assisted thoracic surgery with an integrated postoperative strategy for secondary spontaneous pneumothorax in high-risk patients.

BACKGROUND: Non-intubated video-assisted thoracic surgery (VATS) is an option for the treatment of secondary spontaneous pneumothorax (SSP) in high-risk patients. Although persistent air leakage remains a major postoperative challenge, the efficacy of subsequent treatment to arrest such leakage has not been fully clarified. This study aimed to describe the surgical procedures and postoperative management of non-intubated VATS and to evaluate treatment outcomes. METHODS: A single-center retrospective study was conducted on patients who underwent non-intubated VATS for intractable SSP between 2

Severe Respiratory Syncytial Virus Bronchiolitis Complicated by Bilateral Massive Pleural Effusions in a Previously Healthy Infant: A Case Report.

Respiratory syncytial virus (RSV) bronchiolitis is one of the leading causes of hospitalization in infants, typically presenting with mild-to-moderate respiratory distress. Severe complications such as apnea, respiratory failure, and pneumothorax are known, but massive pleural effusions remain an exceptionally rare manifestation. We report a case of an eight-week-old previously healthy Saudi female infant presenting with severe RSV bronchiolitis. The patient developed progressive respiratory failure requiring mechanical ventilation, re-intubation, and circulatory support. Notably, she develope

Novel dolphin morbillivirus epizootic in stranded striped dolphins in the western Mediterranean Sea, 2025.

Dolphin morbillivirus (DMV) epizootics have recurrently occurred in the western Mediterranean Sea since the first reported outbreak in 1990-1992. In August-November 2025, 5 stranded striped dolphins Stenella coeruleoalba were diagnosed with systemic DMV infection in the Spanish Mediterranean Sea. Dolphins were thin, and 4 of them had typical viral lesions of bronchiolointerstitial pneumonia, lymphoid depletion, and encephalitis. RT-PCR for DMV was positive in at least 2 organs in all dolphins, and viral antigen was detected by immunohistochemistry in 4 dolphins. Four dolphins died due to morbi

Point-of-Care Ultrasound Education and Use in Conflict Settings: Lessons From the War in Ukraine.

Introduction Point-of-care ultrasound (POCUS) has a well-established role in acute medical care and is particularly valuable in conflict zones where access to advanced imaging is limited. To aid Ukrainian healthcare providers, POCUS courses were implemented, and doctors received education and ultrasound probes to use on the frontlines. The objectives of this study were to evaluate the use of POCUS by novice users in conflict-related medical environments, using the war in Ukraine as a case study. We sought to identify how and where medical personnel use ultrasound after an introductory POCUS co

International evidence-based recommendations for point-of-care lung ultrasound : 2025 focused update of the 2012 recommendations.

PURPOSE: Since the publication of the previous consensus document on point-of-care lung ultrasound (PoCLUS) in 2012, new evidence has emerged. This consensus aims to update current recommendations by focusing on the clinical applications of PoCLUS as a standalone tool, while acknowledging that this focused approach represents a necessary preliminary step toward its effective integration with other organ-specific ultrasound examinations and complementary diagnostic modalities. METHODS: A Delphi-based consensus process was conducted under the supervision of a Steering Committee (five voting mem

Birt-Hogg-Dubé syndrome: a hidden genetic cause of recurrent pneumothorax in a non-smoker.

Birt-Hogg-Dubé (BHD) syndrome is a rare autosomal dominant disorder characterised by pulmonary cysts, recurrent spontaneous pneumothorax, cutaneous lesions and an increased risk of renal tumours. Pulmonary manifestations often precede other features, leading to delayed recognition. We report the case of a non-smoking male in his early 60s who presented with recurrent left-sided spontaneous pneumothorax and exertional dyspnoea. Cutaneous examination revealed multiple acrochordons over the neck. Chest imaging demonstrated pneumothorax, and high-resolution CT of the thorax showed a left upper lob

Experience in Molecular Genetic Diagnostics of Birt-Hogg-Dubé Syndrome: Characteristics of Identified Mutations and Evolution of the Methodological Approach.

Birt-Hogg-Dubé syndrome (BHDS) is a hereditary cancer syndrome caused by pathogenic variants in the FLCN gene. BHDS is characterized by clinical heterogeneity and similarities with other non-hereditary diseases, which can complicate diagnosis. The aim of our study was to analyze FLCN variants in Russian patients and select the optimal diagnostic approach. We studied 121 unrelated patients suspected for BHDS and 29 of their relatives. Germline variants were analyzed using Sanger sequencing and Multiplex Ligation-dependent Probe Amplification (MLPA). Variant annotation was performed according to

Sex-specific epidemiological and clinical differences in spontaneous pneumothorax: A cross-sectional cohort study in Japan.

BACKGROUND: Pneumothorax is less common in females than in males, presenting with distinct features, including catamenial pneumothorax (CP) and lymphangioleiomyomatosis (LAM). However, its clinical characteristics in female patients remain poorly understood. We elucidated the clinical characteristics of pneumothorax in female patients and compared them with those of male patients. METHODS: This nationwide observational study of 1747 patients with spontaneous pneumothorax was conducted in Japan using a large inpatient database. We characterized spontaneous pneumothorax in female patients and e

Navigating Anesthetic Management in the Setting of Uterine Rupture and Spinal Fracture: A Case Report.

Intrapartum trauma with spinal instability may require delivery by cesarean section (CS). While regional anesthesia is typically preferred in CS, general anesthesia may be necessary in emergent presentations that require rapid induction of anesthesia. We report a case wherein a patient presented with uterine rupture and spinal instability following a motor vehicle accident and received regional anesthesia prior to CS. A 27-year-old multiparous female at 37 + 6 weeks of gestation presented to the Emergency Department after a high-speed motor vehicle accident with significant abdominal pain and

Clinical application of liquid biopsy in the diagnosis and treatment of lung cancer: taking ctDNA for example-a narrative review.

BACKGROUND AND OBJECTIVE: Lung cancer is one of the malignant tumors with the highest incidence and mortality rates. Most patients are diagnosed at advanced stages, thus missing the golden period for diagnosis and treatment. Tissue biopsy is the most commonly used method for diagnosing lung cancer, but it is an invasive procedure. Patients with poor physical conditions may be unable to tolerate it, and it may even cause complications such as infection, pneumothorax and hemorrhage. On the other hand, repeated sampling is difficult to perform, which makes dynamic monitoring of tumor changes impo
