Investigating the Outcomes of Advanced Neonatal Resuscitation in Preterm Neonates: A Retrospective Cohort Study in Iran.

BACKGROUND AND AIMS: Given the higher need for advanced resuscitation methods at birth in premature neonates, it is crucial to identify the associated consequences. Due to a lack of data in this area, we aimed to investigate the association of advanced neonatal resuscitation at birth with the short-term prognosis of preterm neonates. METHODS: This retrospective cohort study examined premature neonates with gestational age < 33 weeks born at Vali-e-Asr Hospital, Tehran, Iran, during the years 2020-2021. Advanced resuscitation included endotracheal intubation, chest compression, or epinephrine

Efficacy of saline tract sealing after CT-Guided lung biopsy: a systematic review and meta-analysis.

INTRODUCTION: CT-guided percutaneous transthoracic lung biopsy is a high-value procedure with risks such as pneumothorax and chest tube placement. Several needle-tract sealing techniques have been proposed to reduce post-biopsy pneumothorax, although evidence regarding saline tract sealing remains limited. This study updated a meta-analysis comparing normal saline tract sealing to usual care for pneumothorax and chest tube placement in guided lung biopsies. METHODS: Embase, Scopus, PubMed, and Cochrane databases were systematically searched for studies comparing tract instillation with normal

A machine learning model for predicting pneumothorax risk after computed tomography-guided percutaneous transthoracic needle biopsy: A two-hospital retrospective study.

Pneumothorax is a common complication after computed tomography-guided percutaneous transthoracic needle biopsy (PTNB), but individualized prediction tools based on routinely available clinical variables remain limited. This two-hospital retrospective study included 768 patients who underwent CT-guided PTNB between January 2024 and August 2025. The pooled dataset was randomly divided into a training cohort and an internal test cohort at a 7:3 ratio. Twelve predictors were selected using LASSO regression. Seven machine-learning models were developed and evaluated using discrimination, calibrati

Ventilator-Associated Pneumomediastinum With Secondary Pneumothorax in Myasthenic Crisis.

Myasthenia gravis (MG) is an autoimmune disorder of the neuromuscular junction that can progress to myasthenic crisis, a life-threatening condition requiring mechanical ventilation. While ventilatory support is essential, it carries the risk of barotrauma, including pneumomediastinum and pneumothorax. We present the case of a 58-year-old female with a history of MG who developed acute hypercapnic respiratory failure requiring intubation and mechanical ventilation. Her clinical course was complicated by pneumomediastinum with associated subcutaneous emphysema and a secondary pneumothorax identi

Bilateral Apical Congenital Pulmonary Airway Malformation Presenting With Spontaneous Pneumothorax in an Adolescent: A Case Report.

Congenital pulmonary airway malformation (CPAM), previously termed congenital cystic adenomatoid malformation (CCAM), is a rare developmental anomaly of the lower respiratory tract that is most commonly diagnosed prenatally or during early infancy. Presentation in adolescence is uncommon, particularly when associated with bilateral lesions and pneumothorax. We report the case of a 14-year-old female with no relevant past medical history who presented with sudden-onset progressive dyspnea and was found to have an approximately 50% right-sided pneumothorax. Contrast-enhanced chest computed tomog

WABIP/AABIP statement on therapeutic indications of medical thoracoscopy for benign pleural disease.

Medical thoracoscopy (MT) is a minimally invasive procedure with a well-established role in the diagnosis and management of malignant pleural disease; however, its therapeutic applications in benign pleural conditions remain variably adopted and inconsistently defined. In response to increasing clinical utilisation and emerging evidence, the World Association for Bronchology and Interventional Pulmonology (WABIP) and the American Association for Bronchology and Interventional Pulmonology (AABIP) convened a multidisciplinary, international expert panel to develop a consensus statement examining

Perioperative Management Strategies for Fracture Surgery in Patients With Traumatic Pneumothorax: A Retrospective Case Series.

INTRODUCTION: When patients with traumatic pneumothorax require fracture surgery, clinicians must decide how to manage the pneumothorax perioperatively. Options include chest tube placement for the pneumothorax itself, prophylactic drainage before surgery, proceeding with general anesthesia and positive pressure ventilation (PPV) without drainage, using alternative anesthetic techniques to avoid PPV, or delaying surgery until the pneumothorax improves. Despite the clinical relevance of this scenario, few studies have described how these strategies are selected in actual perioperative practice.

Evaluation of Nurse-Led Sono-Triage in Identifying the Under-Triage Rate Among Yellow-Triage Patients During Mass Casualty Incidents at a Level 1 Trauma Center in India.

BACKGROUND: Triage is critical during mass casualty incidents (MCIs). Under-triage during MCIs using existing triage tools may lead to errors in making critical treatment decisions. The point-of-care ultrasound has been used during MCIs to diagnose hemoperitoneum, pneumothorax, long bone fractures, cardiac tamponade, and inferior vena cava (IVC), with good sensitivity and specificity. OBJECTIVE: We explored the impact of nurse-led sono-triage in identifying the under-triage rate among yellow triage patients and inter-rater agreement among nurse performers during MCIs. METHODS: This was a ret

Atypical Spontaneous Pneumothorax in an Older Obese Female: A Case Report.

Spontaneous pneumothorax results from the accumulation of air in the pleural space, leading to partial or complete collapse of the affected lung. Primary spontaneous pneumothorax (PSP) classically occurs in young, tall, thin males without clinically apparent underlying lung disease, whereas secondary spontaneous pneumothorax (SSP) occurs in association with structural pulmonary pathology. Tobacco use is a well-established risk factor for spontaneous pneumothorax and may contribute to structural changes that persist after smoking cessation. We present an atypical case of spontaneous pneumothora

An attention-infused deep convolutional paradigm for multi-label classification of thoracic pathologies in chest radiographs.

This research proposes an empirical benchmarking study of an attention-infused deep convolutional framework for multi-label thoracic pathology classification in chest radiographs, designed to emulate how radiologists selectively focus on suspicious regions. Existing CNN models process entire images uniformly, often missing fine-grained or subtle abnormalities that require localized visual emphasis. In spite of the progress of deep convolutional neural networks (CNNs) in automated CXR analysis, traditional architectures do not sufficiently localize fine-grained pathological information, especia

Rethinking Radical Mastectomy Anesthesia: Combined Erector Spinae Plane and Pectoral Nerve Block Type II Blocks in Bullous Chronic Obstructive Pulmonary Disease Without Positive-Pressure Ventilation.

Breast surgery with axillary dissection is traditionally performed under general anesthesia with positive-pressure ventilation. However, in patients with bullous chronic obstructive pulmonary disease (COPD), mechanical ventilation may increase the risk of barotrauma and pneumothorax. We report the case of a 75-year-old male with moderate COPD and a large pulmonary bulla who underwent modified radical mastectomy with axillary dissection using combined erector spinae plane and pectoral nerve block type II, associated with sedation and preservation of spontaneous ventilation. The procedure was co

Boerhaave Syndrome With Concomitant Duodenal Perforation: A Rare Case Complicated by Mediastinitis and Empyema.

Boerhaave syndrome (spontaneous esophageal rupture) remains a surgical emergency with high mortality, particularly when complicated by mediastinitis, pleural empyema, and septic shock. Concomitant intra-abdominal perforation significantly increases morbidity. We report a case of a 61-year-old male patient presenting with hemodynamic instability due to distal esophageal rupture and duodenal perforation. Imaging revealed bilateral pneumothorax, pneumomediastinum, pneumoperitoneum, and extensive pleural effusions. The patient underwent emergent exploratory laparotomy with esophageal repair over T

Recurrent Primary Spontaneous Pneumothorax in an 18-Year-Old Man: A Case Report and Review of Considerations for Prophylactic Contralateral Pleurodesis.

Primary spontaneous pneumothorax (PSP) is defined as the accumulation of air in the pleural space originating from within the lung parenchyma without preceding trauma, which can lead to a stable or tension pneumothorax. Clinical suspicion is key for diagnosis, with chest radiography serving as the primary diagnostic tool. Management ranges from needle aspiration to tube thoracostomy with consideration of surgical intervention for recurrent cases. We present an 18-year-old African-American man who experienced three episodes of radiography-confirmed PSP occurring over a seven-month period. This

Single-Encounter Augmented Reality-Guided Localization for Resection of Suspected Early-Stage Lung Cancer: A Randomized Clinical Trial.

IMPORTANCE: Preoperative localization is often required to achieve successful sublobar resection with adequate margins for computed tomography (CT)-detected pulmonary nodules suspicious for early-stage lung cancer. Conventional CT-guided localization involves a multiple-encounter workflow that may cause pain, radiation exposure, and complications. OBJECTIVE: To determine whether a single-encounter augmented reality (AR)-guided percutaneous localization strategy is noninferior to standard multiple-encounter CT-guided localization for achieving successful sublobar resection. Pre-hospital and emergency department point-of-care ultrasound in cardiovascular emergencies: a scoping review.

BACKGROUND: Point-of-care ultrasound (POCUS) is integral to emergency department (ED) and pre-hospital medicine, yet evidence comparing its diagnostic and operational performance across environments remains fragmented, especially in cardiovascular emergencies. This review aimed to evaluate and compare the diagnostic performance, workflow impact, and clinical influence of POCUS for six major cardiovascular emergencies across pre-hospital and ED settings, while examining the effects of operator training, device technology, and governance structures. METHODS: A structured scoping review search w

Traumatic Uterine Rupture in a Nonpregnant Patient: A Case Report.

Traumatic uterine rupture is a rare and easily overlooked condition. Most cases of traumatic uterine rupture have been reported to occur in pregnancy. Here, we reported the case of a 14-year-old nonpregnant girl who presented to the emergency department with hemorrhagic shock due to multiple traumatic injuries after falling from a height during a suicide attempt. Initial resuscitation stabilized the patient's vital signs. A whole-body computed tomography (CT) scan revealed bilateral pneumothorax, multiple rib fractures, a grade III renal injury, a liver injury, and an unstable pelvic fracture.

EVALI and Beyond: An Update on the Spectrum of Vaping-Associated Lung Pathology.

E-cigarette or vaping-associated lung injury (EVALI) was initially recognized as a public health problem in 2019, but pathologic changes from vaping (if any) outside the context of EVALI remain unknown. We aimed to further characterize the pathology of EVALI and vaping-associated changes in otherwise healthy patients without EVALI, including those presenting with spontaneous pneumothorax. Archives were searched for lung specimens from these patient groups (2019 to 2025). Previously reported cases were excluded. Clinical history and pathology slides were reviewed. Sixty-three patients (43 men;
