Traumatic pneumatoceles in adulthood: A case highlighting the limitations of supine chest radiography in trauma.

Traumatic pneumatoceles are uncommon pulmonary pseudocysts that may occur following blunt thoracic trauma and can be entirely occult on chest radiography, potentially mimicking other cavitary lung pathologies. We present the case of a 52-year-old male transferred to a tertiary trauma center following a motorcycle accident. On presentation, the patient was hypoxic with extensive right-sided chest wall injuries. Initial supine chest radiography demonstrated multiple right-sided rib fractures, patchy airspace opacities, a veiling opacity, and a suspected pneumothorax, raising concern for a large

Non-intubated Video-Assisted Thoracoscopic Pleurodesis Under Paravertebral Block in a Patient With Fibrotic Hypersensitivity Pneumonitis and Chronic Respiratory Failure.

Non-intubated video-assisted thoracoscopic surgery (NIVATS) has increasingly been reported as an alternative to general anesthesia in patients at high risk of complications associated with mechanical ventilation. Fibrotic hypersensitivity pneumonitis (FHP) with chronic respiratory failure may pose an indication for this approach, given the risk of positive-pressure ventilation precipitating acute exacerbation of the underlying interstitial lung disease, even in patients with an otherwise uncomplicated intraoperative course. We report the anesthetic management of a 42-year-old woman with FHP an

The PVL threat: A multicenter, single-cohort retrospective study of Panton-Valentine leukocidin staphylococcal infections in critically ill children.

UNLABELLED: Staphylococcus aureus carrying the Panton-Valentine leukocidin (PVL) genes, in both methicillin-susceptible (MSSA) and methicillin-resistant (MRSA) strains, causes severe infection in previously healthy children. We described critically ill children with PVL-positive infection and explored predictors of prolonged intensive care. In this multicenter, single-cohort retrospective study, we included 44 consecutive children aged 0-16 years with PVL-producing S. aureus infection admitted to a neonatal or pediatric intensive care unit (PICU) at 13 Italian centers between 2022 and 2025. Procedural efficiency, diagnostic yield, and safety of a novel positioning grid combined with a non-coplanar puncture-angle guide for CT-guided percutaneous transthoracic lung biopsy: a retrospective contemporaneous-control study.

BACKGROUND: Computed tomography (CT)-guided percutaneous transthoracic lung biopsy is an important method for obtaining pathological diagnoses of pulmonary lesions. However, conventional manual localization and needle advancement are operator dependent, and technically difficult lesions may require repeated angle adjustments and CT acquisitions. The aim of this study was to evaluate the procedural efficiency, diagnostic performance, and safety of a novel positioning grid combined with a non-coplanar puncture-angle guide. METHODS: This single-center, retrospective, nonrandomized contemporaneou

Nasal intermittent positive pressure ventilation versus nasal continuous positive airway pressure as initial respiratory support during minimally invasive surfactant therapy in preterm infants with respiratory distress syndrome: a retrospective real-world cohort study.

BACKGROUND: Nasal continuous positive airway pressure (NCPAP) and nasal intermittent positive pressure ventilation (NIPPV) are commonly used as initial non-invasive respiratory support in preterm infants with respiratory distress syndrome (RDS). Minimally invasive surfactant therapy (MIST) is increasingly used to reduce exposure to endotracheal intubation, but data on NIPPV compared to NCPAP during MIST remain limited. This study aimed to compare NIPPV and NCPAP as initial respiratory support during MIST in preterm infants with RDS, with the primary outcome of invasive mechanical ventilation w

RibXcar: Safety Fundamentals and Surgical Protocol for Controlled Costal Osteotomy in Body Contouring Surgery: A Prospective Study of 100 Patients.

BACKGROUND: Aesthetic body contouring surgery has incorporated costal modification techniques to improve waist definition. The lack of standardized protocols and prospective safety data has generated controversy. The RibXcar technique proposes a controlled monocortical costal osteotomy as an alternative to rib resection; however, no prospective studies have comprehensively evaluated its safety profile. OBJECTIVES: To describe a standardized surgical protocol for the RibXcar technique and prospectively evaluate its safety, efficacy, impact on pulmonary function, postoperative pain, and patient

Mechanical versus manual bag-valve ventilation during cardiopulmonary resuscitation in adults with cardiac arrest: A systematic review and Bayesian meta-analysis.

BACKGROUND/OBJECTIVE: Optimal ventilation delivery during cardiopulmonary resuscitation (CPR) remains uncertain. Current resuscitation guidelines do not recommend mechanical ventilation (MV) over manual bag-valve ventilation (BV). We evaluated the association between MV use during CPR and clinical outcomes in adults with cardiac arrest compared with manual BV. METHODS: We searched 6 bibliographic databases from inception through May 31, 2026. Included studies compared MV and BV ventilation CPR in adult cardiac arrest patients. Patients receiving MV at any moment during CPR were classified as

Paradoxical reactions during treatment of miliary tuberculosis: a 25-year single-centre case series and literature review.

BACKGROUND: Paradoxical reactions during otherwise effective anti-tuberculosis treatment can resemble treatment failure or drug resistance. Their frequency and clinical spectrum in miliary tuberculosis are incompletely characterised. AIMS: To describe the timing, manifestations, management and outcomes of paradoxical reactions identified in a defined cohort of patients with miliary tuberculosis and to place these events in the context of the published literature. METHODS: We retrospectively reviewed the records of 63 patients with miliary tuberculosis managed at a single centre between 1981

Pneumothorax Following Botulinum Neurotoxin Injection for Trapezius Hypertrophy.

Botulinum neurotoxin (BoNT) injection into the trapezius muscle is increasingly performed for aesthetic contouring. However, iatrogenic pneumothorax has not been previously documented for this cosmetic indication, despite the inherent anatomical risks. We report a 26-year-old woman who developed immediate severe radiating pain during bilateral trapezius BoNT type A (BoNT-A) injections using a 4.8-cm, 25-gauge needle. During the injection, the patient experienced immediate severe radiating pain, followed by pleuritic chest pain and dyspnea within 30 minutes. Computed tomography confirmed a 30%

Birt-Hogg-Dubé syndrome, prevalence of identified cases according to clinical indication in a cohort of 1,084 patients with genetic analysis.

PURPOSE: Birt-Hogg-Dubé syndrome (BHD) is an autosomal dominant disorder caused by pathogenic variants (PVs) in the FLCN gene. This study aimed to characterize the clinical presentation of patients carrying a FLCN pathogenic variation and to identify the most indicative clinical features of the syndrome. METHODS: We analysed clinical and molecular data from 1,084 consecutive probands with a clinical suspicion of BHD. The frequency of FLCN PVs was assessed, based on clinical presentation and family history. RESULTS: Among the 1,084 probands analysed, 29% carried a PV in FLCN. Isolated fibrofo

Hydropneumothorax as a Diagnostic Clue to Pulmonary Tuberculosis With Pleural Involvement: A Case Report.

Hydropneumothorax represents an unusual presentation of pulmonary tuberculosis (TB), posing diagnostic and therapeutic challenges. We report the case of a 36-year-old woman who presented with productive cough, dyspnea, right-sided pleuritic chest pain, and constitutional symptoms. Chest imaging revealed a large right-sided hydropneumothorax with extensive parenchymal and cavitary disease. Sputum smear microscopy was positive for acid-fast bacilli, and molecular testing of both sputum and pleural fluid detected Mycobacterium tuberculosis complex, confirming pulmonary TB with pleural involvement

Obturator hernia misdiagnosed as inguinal incarceration: a case of postoperative small-bowel perforation and subcutaneous fecal emphysema.

INTRODUCTION AND IMPORTANCE: An obturator hernia is a rare condition accounting for less than 1% of abdominal wall hernias. Due to its deep pelvic location and nonspecific clinical presentation, it is frequently misdiagnosed, most often as an inguinal hernia. Delayed or incorrect diagnosis may result in bowel ischemia and perforation. CASE PRESENTATION: We report the case of a 61-year-old man who underwent emergency open surgery at another institution for a presumed strangulated right inguinal hernia with placement of prosthetic mesh. No operative report was available regarding intraoperative
