Birt-Hogg-Dubé syndrome: From molecular mechanisms to emerging systemic therapeutic strategies.

Birt-Hogg-Dubé (BHD) syndrome is a rare autosomal dominant disorder caused by germline mutations in the FLCN gene. It is characterized by pulmonary cysts with recurrent pneumothorax, renal tumors, and cutaneous lesions, with marked phenotypic variability. This review summarizes current evidence on the molecular basis and clinical manifestations of BHD, highlighting the role of folliculin in cellular metabolism through the AMPK-mTOR signaling pathway and related mechanisms involved in tumorigenesis. From a clinical perspective, diagnosis is often challenging and relies on combined genetic and p

Birt-Hogg-Dubé Syndrome-A Rare Cause of Recurrent Pneumothoraces in a Midshipman.

Birt-Hogg-Dubé Syndrome (BHD) is a rare autosomal dominant disorder caused by mutations in the FLCN gene, leading to tumor formation in the skin and kidneys, as well as cystic lung disease predisposing to spontaneous pneumothoraces. These pneumothoraces often recur despite surgical intervention. We present a case of a 20-year-old male of East Asian descent with no significant medical history who developed recurrent spontaneous pneumothoraces despite undergoing video-assisted thoracoscopic surgery with pleurodesis. After undergoing the procedure, he was noted to have a recurrent pneumothorax at

Development of a Novel, Low-Power, Ultrasound Algorithm for the Detection of Pneumothorax Using a Large Animal Model.

INTRODUCTION: Tension pneumothorax is the third leading cause of potentially survivable death in the prehospital, combat setting. Identification of the presence of a pneumothorax before tension physiology develops remains challenging in this setting. We conducted an early developmental pilot study to determine if unprocessed raw radio frequency (RF) data from a single-crystal ultrasound array could fill this gap. MATERIALS AND METHODS: We prospectively enrolled sus scrofa models as part of a medical education training program with intentional induction of pneumothorax. We obtained thoracic im

Age > 50 years and PaO₂ ≤ 90 mmHg are the two dominant predictive factors for secondary spontaneous pneumothorax in male patients: an observational study.

BACKGROUND: Current understanding indicates that primary spontaneous pneumothorax (PSP) typically occurs in younger individuals, whereas secondary spontaneous pneumothorax (SSP) is more common in older patients. However, the specific age distribution patterns distinguishing these two types of spontaneous pneumothorax (SP) remain poorly characterized. Furthermore, while a low partial pressure of oxygen (PaO₂) is a recognized clinical feature of pneumothorax, limited research has explored whether lower PaO₂ levels are specifically indicative of underlying lung disease in patients with SSP. Role of the general medical registrar in performing out-of-hours pleural procedures: a UK national survey of trainee and trainer perspectives.

BACKGROUND: UK General Internal Medicine (GIM) registrars are traditionally expected to be able to perform pleural procedures out-of-hours (OOH) if required. Pleural exposure for GIM registrars has declined, with procedures increasingly performed by specialists in-hours. Pleural procedures can cause significant harm. We explored the role of the GIM registrar in performing OOH pleural procedures, with particular focus on competence and confidence. METHODS: An anonymous survey was distributed to GIM registrars and respiratory consultants across the UK. RESULTS: Responses were received from 391

Accuracy of Needle Chest Decompression Site Selection in Simulated High Stress Environments Among Air Force Healthcare Professionals: A Randomized Controlled Trial.

INTRODUCTION: Tension pneumothorax (tPTX) is a life-threatening but treatable condition in combat environments, where survivability depends on rapid and accurate needle chest decompression (NCD). However, prior research shows a notable gap between clinical knowledge and hands-on procedural skills, with NCD site selection accuracy as low as 26.1% among military medics. This study's purpose was to evaluate the overall accuracy of NCD site selection among active duty military medical professionals in a novel, simulated high-stress combat environment. This study aimed to determine if environmental

Outcomes of Pleural Sealant Application in Pneumothorax Surgery: A Comparative Analysis.

BACKGROUND: Recurrence after video-assisted thoracoscopic surgery (VATS) bullectomy for primary spontaneous pneumothorax (PSP) remains a major clinical concern. Pleural reinforcement has been proposed to reduce recurrence; however, comparative evidence between fibrin sealant (FS) patches and polyglycolic acid (PGA) sheets is limited. METHODS: We retrospectively reviewed 188 patients with PSP who underwent VATS bullectomy with pleural reinforcement between 2015 and 2025 (FS group, n=42; PGA group, n=146). Perioperative outcomes and recurrence-free survival (RFS) were compared. Baseline imbalan

Ultrasound elastography for pneumothorax diagnosis: beyond the elasto-lung point.

OBJETIVES: Ultrasound elastography is a promising non-invasive imaging technology that quantifies tissue elasticity. Only 2 studies have investigated ultrasound elastography for pneumothorax, both in strain elastography, reporting positive results and describing the novel elasto-lung point sign to aid in identifying the lung point. The objectives of this study are to confirm the existence of the elasto-lung point using 2-dimensional shear-wave elastography (2D-SWE), and to evaluate the utility of elastography in pneumothorax cases lacking a detectable lung point. METHODS: Prospective single-c

Pneumomediastinum and surgical emphysema following EBUS-TBNA with a 22-gauge needle in a patient with hypersensitivity pneumonitis.

Endobronchial ultrasound with transbronchial needle aspiration (EBUS-TBNA) is a commonly performed safe diagnostic procedure, though complications such as pneumomediastinum and surgical emphysema are rare. In this report, we describe the case of a male in his early 50s diagnosed with hypersensitivity pneumonitis who developed these complications following an EBUS-TBNA and fibreoptic bronchoscopy procedure. The patient's condition was promptly recognised and managed conservatively, leading to a rapid recovery within 24 hours. This case emphasises the need for clinical vigilance, even with routi
