Shape-Sensing Robotic-Assisted Bronchoscopy vs. Electromagnetic Robotic-Assisted Bronchoscopy-A Comparative Cohort Study.

Introduction: Lung cancer is a leading cause of cancer-related deaths globally, with approximately 1.5 million new peripheral pulmonary lesions (PPLs) detected annually in the United States. Robotic-assisted bronchoscopy (RAB) has emerged as a promising technology, with two platforms initially approved, the Monarch platform (Auris Health Inc, Redwood City, CA, USA) and the Ion Endoluminal System (Intuitive Surgical, Sunnyvale, CA, USA), offering improved stability and distal airway visualization. As RAB adoption increases, there is a critical need for comparative effectiveness data of these sy

Artificial Intelligence Could Predict Chest Tube Drainage Necessity for Spontaneous Pneumothorax.

Artificial intelligence (AI) is increasingly utilized in the medical field, primarily for diagnostic purposes. Although AI has demonstrated efficacy in pneumothorax detection using chest X-rays (CXR), it has yet to be applied for decision-making regarding subsequent treatment. This study aims to develop and evaluate an AI-based system capable of predicting the necessity of chest tube drainage (CTD) in spontaneous pneumothorax patients based on CXR and clinical data. A two-stage AI model was developed: (1) segmentation and quantification of pneumothorax size from CXR using deep learning and (2)

An Uncommon Cause of Massive Hemothorax: Pulmonary Arteriovenous Malformation in the Right Middle Lobe.

Pulmonary arteriovenous malformations (PAVMs) are rare vascular anomalies that can lead to severe complications when ruptured. We report the case of a 65-year-old active smoker who presented to the emergency department with acute pleuritic chest pain, severe dyspnea, and hypoxemia. Chest CT revealed a large PAVM in the right middle lobe, associated with massive hemothorax and a small pneumothorax. The patient had no history of hereditary hemorrhagic telangiectasia. Initial management included pleural drainage and endovascular embolization with a 16 mm Amplatzer™ device. Due to persistent respi

Pediatric lung ultrasound: Tips and tricks for better scanning and interpretation.

In recent decades lung ultrasound (LUS) has emerged as an invaluable bedside imaging tool for the evaluation of respiratory conditions such as pneumonia, bronchiolitis, pneumothorax, and pleural effusion in pediatric age, thanks to the thin chest wall and absence of rib ossification. Despite its growing application, optimizing technique and interpretation remains crucial for reliable diagnostic performance. The present educational paper provides practical tips and tricks to enhance the accuracy, efficiency, and clinical value of LUS in pediatric patients, focusing on probe selection, patient p

The vanishing lesion: A procedural clue to pneumothorax during endobronchial ultrasound.

Endobronchial ultrasound (EBUS) has become an essential tool in lung cancer staging and the diagnosis of centrally located pulmonary lesions. Vigilance during EBUS-guided biopsy is crucial. The disappearance of a previously visualised lesion should raise suspicion of a developing pneumothorax. We present our experience with an intra-procedural pneumothorax following EBUS-guided biopsy and discuss the procedural considerations relevant to its prompt recognition and management.

Case Report: CT-guided biopsy of a mediastinal mass in the visceral compartment.

BACKGROUND: The pathological diagnosis of mediastinal lesions is crucial for precision oncology. While endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is the preferred minimally invasive method for visceral mediastinal lesions, its diagnostic yield can be limited for high-risk lesions surrounded by great vessels. This case demonstrates that under such complex anatomical constraints, a meticulously planned CT-guided percutaneous biopsy serves as a safe and effective alternative. CASE PRESENTATION: A middle-aged male patient was highly suspected of having lung cance

Biological alternatives to chemical pleurodesis: Evaluation of autologous blood, platelet-rich plasma and platelet-rich fibrin in an experimental rat model.

BACKGROUND: Pleurodesis is a widely used technique in thoracic surgery aimed at preventing the recurrence of pleural effusion and pneumothorax. Although chemical sclerosing agents such as talc are effective, their use is often limited by significant adverse effects, prompting interest in safer and more biocompatible alternatives. This experimental study evaluates the pleurodesis-inducing potential of three autologous biological agents-autologous blood (AB), platelet-rich plasma (PRP), and injectable platelet-rich fibrin (i-PRF)-in a rat model. METHODS: Twenty-eight adult Wistar albino rats we
