Anatomical Mechanisms Underlying Clinically Reported Complications of the Infraclavicular Brachial Plexus Block: A Narrative Review.

Background: The infraclavicular brachial plexus block is a widely used regional anesthesia technique for surgery of the distal upper limb. Although generally considered safe-particularly with ultrasound guidance-a range of vascular, neurological, respiratory, and anesthetic-related complications continues to be reported. Understanding how anatomic factors can influence the occurrence of these events is essential for improving procedural safety. Objective: This narrative review aims to correlate clinically reported complications of the infraclavicular block with underlying anatomical mechanisms

Pulmonary Hemorrhage and Pneumothorax Risk During CT-Guided Lung Biopsy for Suspected Lung Cancer.

OBJECTIVES: The aim of this study was to evaluate the association between pulmonary hemorrhage and PTX following CT-guided lung biopsy for suspected or confirmed lung malignancy and to assess whether lesion depth modifies this relationship. METHODS: This retrospective single-center study included 118 consecutive patients undergoing CT-guided lung biopsy for oncologic indications between 2020 and 2025 (66 men, 52 women; median age 69 years). Immediate post-biopsy CT was assessed for PTX and focal pulmonary hemorrhage. Multivariable logistic regression identified predictors of PTX, including pu

Fentanyl-Induced Chest Wall Rigidity Causing Recurrent Ventilator Dyssynchrony in the Intensive Care Unit: A Case Report.

BACKGROUND: Fentanyl-induced chest wall rigidity represents an important and often overlooked cause of ventilator dyssynchrony in mechanically ventilated patients. Its clinical presentation overlaps with bronchospasm and ventilator malfunction which delays diagnosis and appropriate management. CASE PRESENTATION: A 75-year-old male, mechanically ventilated with pneumonia and asthma exacerbation developed ventilator desynchrony, characterized by sudden-onset hypercarbia, decreased lung compliance, and episodic breath-holding. Escalation of bronchodilators, corticosteroids, and sedation failed t

Conservative Management of Bilateral Spontaneous Pneumothorax.

Bilateral simultaneous spontaneous pneumothorax is a rare occurrence that generally requires prompt intervention. We report a case of simultaneous bilateral spontaneous pneumothorax. As the patient had minimal symptoms and normal vital signs, he was conservatively managed. Follow-up imaging showed resolution of the pneumothorax with no evidence of underlying lung abnormality or abnormal pleural communications. This rare case highlights the feasibility and safety of adopting a conservative approach in the management of bilateral spontaneous pneumothorax.

Using point-of-care lung ultrasound to detect post-thoracentesis complications: the hydro-point case report.

INTRODUCTION: Pneumothorax is a common complication of medical procedures that often occurs after thoracentesis despite the use of ultrasound guidance. Point-of-care lung ultrasound (POCUS) is a rapid and efficient diagnostic tool for pneumothorax. CASE PRESENTATION: A 78-year-old man with severe ischemic cardiomyopathy was admitted with acute pulmonary edema and hypotension. He was managed with diuretics and inotropes. Due to severe left pleural effusion, a diagnostic left thoracentesis was performed, and the pleural fluid analysis revealed it was transudative. The follow-up chest X-ray reve

Bronchoscopic endobronchial valves.

Bronchoscopic endobronchial valves (EBVs) have emerged as a minimally invasive alternative to surgical lung volume reduction for patients with severe emphysema. Emphysema, a progressive lung disease associated with COPD, leads to hyperinflation of the affected lung and reduces respiratory efficiency. EBVs function by limiting airflow to diseased lung segments, causing atelectasis of the emphysematous lung, and allowing healthier lung regions to expand, thereby improving overall pulmonary function. Two primary EBV systems, Zephyr and Spiration valves, have demonstrated efficacy in clinical tria

Thoracostomy and thoracotomy for emergency management of pediatric pleural space pathologies.

While indications and techniques for thoracostomy are well studied in adults, pediatric-specific evidence remains limited for emergency department-based thoracostomy and especially thoracotomy. The ability to perform urgent interventions for life-threatening pleural space pathologies is essential for all emergency clinicians. This issue reviews the clinical indications for thoracostomy and thoracotomy in pediatric patients, outlines procedural considerations for these techniques, and discusses potential complications in the emergency department. Advancements in the field are also highlighted,

Unilateral orbital surgical emphysema following laparoscopic hiatal hernia repair and Nissen fundoplication: a case report.

Subcutaneous emphysema (SE) is a recognized but rare complication of laparoscopic surgery, and orbital involvement is particularly unusual. We report a rare case of unilateral orbital SE following laparoscopic hiatal hernia repair with Nissen fundoplication. A 49-year-old hypertensive woman with gastroesophageal reflux disease (GERD) underwent elective laparoscopic hiatal hernia repair. Intraoperatively, she developed sudden-onset left orbital emphysema. Imaging revealed mild left pneumothorax and bilateral pleural effusions. Orbital swelling was identified intraoperatively, prompting multidis

Development and validation of a nomogram for predicting localization-related complications after CT-guided soft hook-wire placement for solitary pulmonary nodules.

BACKGROUND: Although the soft hook-wire has been designed to be less traumatic than rigid devices, localization-related complications are still at risk to perioperative safety. This study aimed to find out independent risk factors for these complications in solitary pulmonary nodules patients (SPNs) and create a new nomogram for exact risk stratification. METHODS: Retrospectively analyzed 244 consecutive patients who underwent CT-guided soft hook-wire localization and subsequently had video-assisted thoracoscopic surgery for SPNs in our department from January 2025 to November 2025. Effects of age, sex and body anthropometry on needle thoracostomy in a Singapore paediatric cohort: a chest CT study.

INTRODUCTION: Tension pneumothorax in children, although infrequently encountered, requires management with a high level of confidence and skill from the attending physician. Despite this, recommendations on location and needle length for needle thoracostomy (NT) in paediatric patients are not well-established. We therefore aimed to (1) identify how age, sex and body anthropometry affect chest wall thickness (CWT) at common NT landmarks and (2) determine the adequacy of needle lengths used. METHODS: A retrospective review was undertaken of chest CT scans performed on children aged 0-17 years

Video-assisted thoracoscopic blebotomy for spontaneous pneumothorax treatment using the no-knife endoscopic stapler.

BACKGROUND: A standard treatment for pneumothorax due to bleb rupture is thoracoscopic blebectomy, often combined with pleurodesis to reduce recurrence risk. However, pleurodesis can cause significant postoperative pain. This study evaluates the efficacy of the no-knife endoscopic stapler blebotomy without pleurodesis using video-assisted thoracoscopic surgery (VATS) for spontaneous pneumothorax. METHODS: We conducted a retrospective chart review of patients treated with no-knife endoscopic stapler blebotomy for spontaneous pneumothorax at UCSF Benioff Children’s Hospital Oakland from 2020 to

Artificial intelligence models for point-of-care ultrasound diagnostics in dogs.

INTRODUCTION: Point-of-care ultrasound (POCUS) for the purpose of Focused Assessment with Sonography for Trauma (FAST) is an essential diagnostic tool for triage in canine patients, but its accuracy is highly operator-dependent. Artificial intelligence (AI) offers a potential solution for improving diagnostic capability by providing real-time, automated interpretation of ultrasound images, particularly in resource-limited or pre-hospital settings. This study evaluated the feasibility and diagnostic performance of deep learning models for detecting life-threatening effusions and pneumothorax (P

Iatrogenic risk of tube thoracostomy: A retrospective trauma database analysis.

OBJECTIVES: Traumatic hemothorax, pneumothorax, and hemopneumothorax are life-threatening conditions which are often treated with thoracostomy. Prior research has identified thoracostomy as an independent risk factor for both mortality and increased hospital length of stay (LOS). Given the coexistence of increased mortality and LOS, the objective of this study was to examine whether this mortality risk is driven by iatrogenic factors such as in-hospital complications. METHODS: This retrospective cohort study used data from the 2019 American College of Surgeons-Trauma Quality Program database.
