Branched Endovascular Aneurysm Repair (BEVAR) to Rescue Failed Complex EVAR (C-EVAR): Technical Challenges and Outcomes in a 12-Case Series.

Background: Complex EVAR is a well-established option for treating complex aortic pathologies. However, depending on the type of it, long-term effectiveness is often compromised. For example, chimney EVAR is related to type IA endoleaks related to the gutter and proximal neck degeneration, late failures after fenestrated or branched EVAR are rare. Although redo-endovascular procedures are recommended for failed repairs, the use of branched endoprostheses (BEVAR) to address failed Complex EVAR (C-EVAR) cases is rarely documented. This study aims to evaluate the technical feasibility and 30-day

Ruptured right coronary artery bypass graft aneurysm causing haemothorax.

A woman in her 80s with a background of a coronary artery bypass graft (CABG) presented with chest pain. Initial investigations revealed an elevated D-dimer, normocytic anaemia and right-sided pleural effusion. CT pulmonary angiogram ruled out a pulmonary embolism but revealed an incidental cyst-like rounded lesion next to the ascending aorta. A triphasic CT of the chest confirmed this to be an aneurysm arising from the right coronary artery bypass vein graft, measuring 5.4 cm. After the multidisciplinary team meeting discussion, the likely diagnosis was thought to be a ruptured aneurysm, lead

Fatal Injuries Caused by Falling Trees: Correlation Between Postmortem Computed Tomography and Forensic Autopsy Findings in Eight Cases.

Falling trees and large branches can result in severe blunt trauma; however, detailed correlations between postmortem computed tomography (PMCT) and forensic autopsy findings in such deaths remain limited. We retrospectively analysed eight consecutive deaths occurring between 2012 and 2025 in which impact or compression by a tree trunk or large branch constituted the primary traumatic event and in which both PMCT and complete forensic autopsy were performed. Thin-slice PMCT of the head and trunk was obtained using standard brain, soft-tissue, lung, and bone windows, supplemented by a gas-optim

External validation of the CAPAD score to predict the occurrence of clinically significant pneumothorax after CT-guided percutaneous transthoracic lung biopsy.

BACKGROUND: The most common complication preventing outpatient management after percutaneous CT-guided lung biopsy (PTLB) is the occurrence of pneumothorax requiring drainage. We previously developed a pragmatic score to predict the risk of clinically relevant pneumothorax after PTLB (hereafter called the CAPAD score), to guide patient management. OBJECTIVES: To externally validate the CAPAD score in an independent cohort, with a specific focus on its ability to rule out pneumothorax requiring drainage after PTLB. METHOD: We retrospectively analysed a pre-existing institutional database of c

Suspected Mast Cell Activation Mimicking Heart Failure Exacerbation and Abdominal-Wall Cellulitis in a Resource-Limited Setting: A Case Report.

INTRODUCTION: Mast cell activation syndrome (MCAS) is a mediator-driven disorder characterized by episodic, multisystem symptoms that can resemble common cardiopulmonary and infectious conditions. Diagnostic confirmation often relies on demonstration of mast cell mediator release (eg, event-related serum tryptase rise) and clinical response to anti-mediator therapy; however, access to specialized testing may be limited in low-resource settings. CASE PRESENTATION: A 75-year-old woman with recently diagnosed heart failure with reduced ejection fraction (HFrEF) presented with seven days of diffu

Evaluation of the safety and effectiveness of an accelerated treatment protocol for traumatic haemothorax and pneumothorax: a prospective, multicentre observational study in South Africa.

BACKGROUND: Penetrating pneumothorax and haemothorax are common injuries seen in South African trauma centres. Hospitals in the Western Cape Province of South Africa utilize a protocol for the expedited management of patients with chest injuries requiring intercostal drains (ICD) within emergency department observation units, with emphasis on early mobilization and pulmonary rehabilitation. The study objective was to evaluate the safety and effectiveness of the Western Cape ICD protocol for unilateral traumatic pneumothorax, haemothorax, or pneumo-haemothorax. METHODS: This was a prospective,

Shot gun injuries: Fatal massive haemothorax following traumatic haemoperitoneum and trans-diaphragmatic blood migration.

Shotgun injuries provide a unique challenge to both forensic pathology and acute clinical care due to their complex internal ballistic behaviour and variable injury patterns. This report describes a case of a 33-year-old man who initially presented with apparent haemodynamic compensation following a mid-range abdominal shotgun injury; subsequently, he developed rapid respiratory deterioration and died. Post-mortem examination revealed a massive left-sided haemothorax caused not by direct thoracic injury, but by trans-diaphragmatic migration of blood from the abdominal cavity through multiple p

Massive Haemothorax Following CT-Guided Lung Biopsy Revealing Previously Undiagnosed Acquired Haemophilia A.

CT-guided lung biopsy is a commonly performed procedure with recognised complications, including pneumothorax and pulmonary haemorrhage. Massive haemothorax following biopsy is rare and may indicate an underlying coagulopathy. We present the case of a 75-year-old man who developed a life-threatening haemothorax following a CT-guided biopsy of a left lower lobe lung lesion. Despite initial haemodynamic stability, the patient's clinical course required transfusion, pulmonary artery embolisation, chest drainage, and video-assisted thoracoscopic surgery (VATS). The development of spontaneous iliop

Balancing Thrombosis and Bleeding: A Case of Massive Haemothorax Complicating Anticoagulation for Pulmonary Embolism.

Massive haemothorax is a life-threatening condition most commonly associated with thoracic trauma, malignancy, or iatrogenic injury. Anticoagulation, while essential for thromboembolic disease management, may precipitate or exacerbate occult bleeding complications. We report the case of a 69-year-old woman who presented with pleuritic chest pain, productive cough, and progressive dyspnoea following a recent fall and long-haul travel. CT pulmonary angiography confirmed segmental and subsegmental pulmonary emboli with bilateral consolidation, and therapeutic low-molecular-weight heparin was init

Cough-induced chest wall trauma with spontaneous intercostal lung herniation complicated by haemothorax.

We report a rare case of cough-induced chest wall trauma with spontaneous intercostal lung herniation complicated by haemothorax. A man in his early 70s with chronic obstructive pulmonary disease, asthma and prior thoracic trauma presented with chest pain. Initial investigations focused on a cardiac cause due to elevated troponin levels. During admission, he developed extensive ecchymosis over the flanks, abdomen and back, prompting CT imaging of the abdomen that included the lung bases. This revealed new intercostal lung herniation that was not present on CT pulmonary angiography performed 4

Fatal accidental wooden branch impalement mimicking homicidal sharp-force injury.

BACKGROUND: Penetrating neck injuries are usually caused by sharp-edged weapons, and their morphology often raises suspicion of homicidal sharp-force trauma. Accidental impalement by a rigid natural object, such as a wooden branch, is rare but may closely simulate a stab injury, complicating the determination of the manner of death. CASE PRESENTATION: A 30-year-old man was brought dead to the emergency department following a road traffic accident. External examination revealed a solitary spindle-shaped wound over the left supraclavicular region, with sharply split, gaping margins resembling a

Unveiling the rare: biliary tract ascariasis complicated by liver abscess and haemothorax.

Ascariasis, caused by Ascaris lumbricoides, is a common parasitic infection in developing countries, whereas biliary ascariasis is an uncommon manifestation that may present as cholangitis, biliary obstruction, or cholecystitis. We report the first documented case of biliary ascariasis in a 43-year-old Saudi man presenting with right upper quadrant pain and initially diagnosed with acute cholecystitis. Imaging revealed tubular filling defects within the biliary tree consistent with ascariasis, complicated by a liver abscess. The patient was treated with albendazole, followed by percutaneous dr

TEVAR for Nontraumatic Ruptured Type B Aortic Dissections: A European Multicentre Study.

OBJECTIVE: Ruptured acute type B aortic dissection (TBAD) is rare, carries high mortality risk, and presents major challenges with limited supporting evidence. This study evaluated early and mid-term outcomes after thoracic endovascular aortic repair (TEVAR) for nontraumatic ruptured TBAD. METHODS: This retrospective multicentre study included patients undergoing TEVAR for nontraumatic ruptured TBAD at 6 vascular centres (2010-2023). Rupture was defined as haemothorax (30-45 HU) on preoperative computed tomography with or without aortic wall disruption or active extravasation. Outcomes were 3

Chylous Collection in the Pacemaker Pocket Masquerading as Infection.

BACKGROUND: Although a largely safe procedure, a variety of complications may arise from pacemaker implantation. One rare complication involves chylous collections forming at the pacemaker pocket. CASE SUMMARY: A 58-year-old woman with myotonic dystrophy and a dual-chamber pacemaker presented with chronic swelling over her pacemaker site. Imaging revealed a large fluid collection. Needle aspiration yielded 600 mL of milky, odorless fluid with a triglyceride level of 403 mg/dL, consistent with a chylous effusion. Lymphangiography and lymphoscintigraphy suggested impaired thoracic duct drainage

Effective Thoracic Duct Ligation at T9 for Postoperative Chylothorax: A Case Report.

Postoperative chylothorax after lung resection and mediastinal lymph node dissection is a rare complication that may be difficult to manage when conservative treatment is ineffective, and the thoracic duct or leak point cannot be identified intraoperatively. Chylothorax is thought to result from thoracic duct injury associated with mediastinal lymph node dissection. Although surgical thoracic duct ligation is recommended in refractory cases, identification of the leakage site or thoracic duct itself is often challenging, particularly in patients with intrathoracic adhesions or in facilities wh

Diagnostic Dilemma of Milky Yellow Pleural Effusion.

BACKGROUND: Chylothorax, a common complication post-lung cancer surgery, lacks standardized laboratory diagnostic criteria. Misdiagnosis remains prevalent due to variable pleural fluid appearance. METHODS: We analyzed pleural fluid via biochemical tests, lipid profiling, and Sudan III staining. Ratios of effusion-to-serum triglycerides (P/S TG > 1) and cholesterol (P/S CHOL < 1) were applied to confirm chylous effusion. RESULTS: The patient's pleural fluid exhibited milky yellow appearance, elevated TG (5.46 mmol/L), P/S TG ratio (3.62), and positive Sudan III staining. P/S CHOL ratio (0.43)
