Thoracic endometriosis syndrome diagnosed by dry thoracoscopy: Novel diagnostic method for a rare disease.

Endometriosis is an ectopic implantation of uterine tissue and can affect the thoracic cavity, resulting in Thoracic Endometriosis Syndrome (TES). TES is rare and presents with catamenial pneumothorax, haemothorax, and hemoptysis, often coinciding with menstruation. We report a case of a 46-year-old female with a decade-long history of hemoptysis and periodic chest pain associated with her menstrual cycle. High-resolution CT scans revealed persistent pneumothorax and pleural nodules. A dry thoracoscopy was performed, and biopsies confirmed the diagnosis of TES through histopathology and immuno

Right haemothorax secondary to pulmonary vein laceration following left-sided pacemaker implantation: a case report.

BACKGROUND: Lead perforation is a rare but potentially fatal complication of cardiac device implantation. Atrial lead perforations, though less common than ventricular ones, can have severe consequences, especially in elderly patients with underlying structural heart disease. CASE SUMMARY: An 83-year-old male with amyloid and valvular cardiomyopathy underwent dual-chamber pacemaker implantation for third-degree atrioventricular block following transcatheter aortic valve implantation. The active-fixation atrial lead was positioned on the right atrial free wall. Six hours post-implantation, the

Longitudinal outcomes following international multicentre experience with robotic aortic valve replacement†.

OBJECTIVES: In an effort to maintain the technical aspects of traditional prosthetic surgical aortic valve replacement (AVR) while reducing invasiveness and facilitate options for concomitant operations, transaxillary lateral mini-thoracotomy endoscopic robotic-assisted aortic valve replacement (RAVR) has been introduced. The present data highlight the contemporary international collaborative experience. METHODS: All consecutive patients undergoing standardized RAVR across 10 international sites (1/2020-7/2024) were evaluated using a central database with 1 year follow-up. RESULTS: A total o

Cardiothoracic injuries and mechanical cardiopulmonary resuscitation - A forensic autopsy convenience control study on 436 cases.

This study documents the types and frequencies of injuries related to chest compressions during resuscitation attempts in a cohort of 436 non-traumatic, forensic autopsy cases from Eastern Denmark. We hypothesized that there would be a difference in types and frequencies of injuries seen after mechanical cardiopulmonary resuscitation (mCPR) compared to manual basic life support (BLS). We included all non-traumatic deaths referred for a forensic autopsy in eastern Denmark in the period 2015-2017, for a total of 436 cases (females, n = 146; males, n = 290), of which 75 cases had mCPR performed.

Reliability of Pain Thresholds and Exercise-Induced Hypoalgesia Following 45-Degree Roman Chair Exercise.

BACKGROUND: Acute exercise may induce a reduction in pain sensitivity, termed exercise-induced hypoalgesia (EIH). The 45-degree Roman Chair (low back extension) is a dynamic resistance exercise that may be beneficial for low back pain as it allows for direct overload of the spinal flexors and extensors. However, the ability of this exercise to reliably produce EIH has not been examined. PURPOSE: The purposes of this study were to determine the reliability of heat pain threshold (HPT) and pressure pain threshold (PPT) measures, examine local and systemic EIH effects after a 45-degree Roman cha

Stillbirth Caused by Vitamin A Deficiency in Captive African Lion Cubs (Panthera leo).

OBJECTIVE: This study investigates the pathology of two stillborn lion cubs presented to the Laboratory of Pathology by analysing their necropsy findings and concluding with possible diagnoses. METHODS: At first, a systematic necropsy was performed, and then, the samples of various tissues were collected and divided into three parts for bacterial culture, histopathology studies and molecular analysis. The conventional polymerase chain reaction (PCR) was conducted to detect common foetal infectious agents associated with stillbirth in carnivores, such as canine adenovirus-2, canine distemper v

Life-Threatening Tension Haemopneumothorax After Removal of a Misplaced Dialysis Catheter: A Novel Complication and Its Management.

Central venous catheters are frequently required for temporary vascular access in patients with inaccessible peripheral venous access and in patients requiring haemodialysis. Although generally safe, catheter insertion without ultrasound guidance can lead to severe complications. This case report discusses a patient who presented with symptoms of a misplaced dialysis catheter and developed a life-threatening tension haemopneumothorax during its removal, in association with prior haemothorax. We believe that tension haemopneumothorax associated with haemothorax resulting from a misplaced dialys

Delayed chest wall abscess after blunt trauma chest leading to multi-organ failure: A case report.

Blunt Trauma Chest (BTC) is a common cause of morbidity and mortality in trauma patients. Although rib fractures, haemothorax, pneumothorax and lung contusions are often found in tandem in BTC, delayed necrotising soft tissue infection (NSTI) without any penetrating injuries or overt puncture wounds have rarely been reported. We report a 61-year-old patient with uncontrolled diabetes, admitted after a motor vehicle collision with head, chest and lower limb injuries and discharged after 9 days of stay in the hospital. He returned with NSTI of the chest wall around a week later with a fulminant

Guidelines For Adult Pleural Fluid Effusion In Critical Care.

OBJECTIVE: Chest drainage is a very common procedure in critical care. It is performed by practitioners from various specialties in a wide range of clinical settings. Liquid pleural effusions are the most frequent indication for chest drainage. However, practices regarding chest tube insertion, monitoring, and removal vary considerably between institutions. The objective of these recommendations is to provide guidance for the optimal management of pleural effusion (excluding purulent pleurisy, haemothorax, and malignant pleural effusion) in adult critical care patients. DEVELOPMENT: Fifteen e

Predictive factors of early mortality in patients with type A aortic dissection: association between pulmonary artery adventitial hematoma and type A aortic dissection.

OBJECTIVES: Pulmonary artery adventitial hematoma (PAAH) is a potential complication of aortic dissection (AD) that has been shown to have a deleterious effect on prognosis. However, the specific relationship between PAAH secondary to the rupture of type A AD with early mortality has not been fully characterised. We aimed to evaluate the relationship betweeen PAAH and the short-term prognosis of patients with this condition, and to determine how PAAH impacts the prognosis of patients with type A AD compared with other complications of type A AD. METHODS: We retrospectively studied 344 patient

Extended Focused Assessment with Sonography for Trauma in the Emergency Department: A Comprehensive Review.

The Extended Focused Assessment with Sonography for Trauma (eFAST) plays a crucial role in the emergency department (ED) by providing rapid and non-invasive diagnostic information in trauma patients. It is a diagnostic-free fluid detection technique that uses sonography to assess trauma in different anatomical windows of the chest and abdomen and has been accepted in multiple studies as the initial diagnostic tool for torso injuries in blunt abdominal trauma. By promptly identifying potentially life-threatening injuries, such as haemoperitoneum, haemothorax, and cardiac tamponade, eFAST facili

Haemothorax as an Unusual Presentation of Osteochondroma: A case report and brief literature review.

Exostosis, or osteochondroma, is an irregular bone growth commonly occurring near the femur, scapula, humerus and ribs. It can be a single isolated lesion or multiple lesions as in hereditary multiple exostoses (HME) which is a genetic autosomal dominant disorder. Although often asymptomatic, complications such as haemothorax are rare and have serious implications if not promptly recognised. We report a 15-year-old male patient who presented to a tertiary care hospital in Muscat, Oman, in 2023 who presented with progressive chest pain and dyspnoea. The patient had a family history of HME. Vide

Occupational horizontal pushing inducing massive haemothorax: an unusual case report.

We report an unusual case of a 55-year-old male machine operator who developed a massive unilateral haemothorax and ipsilateral rib fracture following occupational horizontal pushing of a cold-rolled steel coil. Notably, this occurred in the absence of direct chest trauma, coagulopathy or underlying anatomical abnormalities. The patient fully recovered after timely tube thoracostomy and hospital monitoring. This case highlights the potential for severe occupational injuries resulting from horizontal pushing activities, which are often overlooked and underreported in injury statistics. To our k

Extended Use of Central Veno-Arterial Extracorporeal Membrane Oxygenation in Lung Transplantation for Patients with Pulmonary Arterial Hypertension.

OBJECTIVES: Lung transplantation (LTx) for patients with pulmonary arterial hypertension (PAH) is associated with high postoperative morbidity and mortality. Extracorporeal membrane oxygenation (ECMO) supports patients' haemodynamics during and after LTx. However, the optimal ECMO strategy, especially for patients with PAH, is debated. Here, we report our unique strategy for patients with PAH, using postoperative central veno-arterial (VA)-ECMO combined with delayed chest closure. METHODS: This was a retrospective single-centre study of consecutive bilateral lung transplantations for adult pa

Left atrial appendage tear due to blunt trauma to the chest.

This case report presents the successful surgical management of a 34-year-old male who had blunt chest trauma resulting in a left atrial appendage tear after a traffic accident. He arrived at the emergency department 30 minutes post-injury in a gasping state, showing signs of hypovolemic shock, tachycardia (132 bpm), hypotension (blood pressure 88/48 mm Hg) and decreased breath sounds on the left side. The initial management included fluid resuscitation and the insertion of an intercostal drain, which drained 1.2 litres of blood. After the initial fluid resuscitation, haemodynamic stability wa

Video-assisted thoracoscopic surgery in a high-volume urban trauma centre.

BACKGROUND: Tube thoracostomy (TT) is the standard treatment for haemothorax, but 5-30% of cases may result in retained haemothorax. Video-assisted thoracoscopy surgery (VATS) is a recognised treatment for retained haemothorax, although its timing and feasibility can be challenging in resource-limited settings with restricted theatre access. The objective was to evaluate and describe our experience with VATS at Charlotte Maxeke Johannesburg Academic Hospital (CMJAH), South Africa. METHODS: This was a retrospective study over seven years (1 January 2017 - 31 December 2023). All adult trauma pa

Predicting the Need for Tube Thoracostomy in Blunt Trauma Patients With Occult Pneumothorax: Observation Versus Intervention.

OBJECTIVES: Occult pneumothorax is increasingly diagnosed in trauma patients due to widespread use of computed tomography (CT), yet its optimal management remains controversial. This study aimed to identify clinical and radiological predictors of deterioration requiring tube thoracostomy and to develop a predictive model to guide management decisions. METHODS: In this retrospective single-centre study, 166 patients with blunt trauma-associated occult pneumothorax were analyzed. Perioperative Veno-Venous Extracorporeal Membrane Oxygenation in Non-Elective Thoracic Surgery: A Propensity-Matched Bicentric Comparison With Medical Indication.

OBJECTIVES: Perioperative veno-venous extracorporeal membrane oxygenation (VV-ECMO) can be used during non-elective thoracic surgery to manage hypoxaemia and facilitate complex surgical procedures. However, outcome data in this setting remain limited, particularly regarding bleeding and thrombotic risks. We aimed to evaluate whether perioperative VV-ECMO during non-elective thoracic surgery is associated with increased 90-day all-cause mortality compared with VV-ECMO used for medical indications, using a propensity score-matched analysis. Secondary outcomes included major bleeding and thrombot
