Hyponatremia and hyperkalemia associated with chylous pleural and peritoneal effusion in a cat.

A 6-year-old, castrated male shorthair was evaluated for chylous effusion in the thorax and abdomen. An underlying disease process was not determined. The cat developed hyponatremia and hyperkalemia, which was attributed to the mechanical drainage of pleural fluid and to the decreased urinary excretion of potassium.

[Acute kidney failure and ascites in Burkitt's lymphoma of the stomach].

A case of 25 old man with acute renal failure as the result of uric acid nephropathy in Burkitt's lymphoma of the stomach and ascites, probably chylous, is presented. After starting chemotherapy a regression of stomach tumor and the chylous effusion was observed. Unfortunately, after a short period of remission the regression was observed with tumor lysis syndrome. The reason of death was bleeding from digesting tract.

Right ventricular cardiomyopathy accompanied by protein-losing enteropathy and chylous effusion.

Severe right-side heart failure developed in a 47-year-old Japanese woman who suffered from hypoalbuminemia and a massive right side chylous pleural effusion. She had been diagnosed as having protein-losing enteropathy with right ventricular cardiomyopathy. Autopsy showed congenital anomalies of the lymph ducts and abnormal deposition of fibrous and fatty tissue in the right ventricular myocardium. The clinical and pathological findings are consistent with the nonarrythmogenic form of the arrythmogenic right ventricular dysplasia.

[Chylothorax as the initial manifestation of malignant pleural mesothelioma--a case report].

A case of chylothorax associated with diffuse malignant pleural mesothelioma in a 53-year-old woman was reported. Chest radiography in a mass examination revealed right pleural effusion. A thoracentesis yielded fluid with characteristics consistent with chyle. In spite of our suggestion that thoracoscopy for further examination was necessary, the patient and her husband had refused the operation for months. Chest CT scanning revealed a nodule behind the xiphoid process. Lymphoscintigraphy suggested an obstruction of the right parasternal lymphatic vessel. Repeated thoracentesis did not yield a

Chylothorax.

Chylothorax or chylous pleural effusion occurs when chyle accumulates in the pleural space usually secondary to disruption of thoracic lymphatics. Chyle is a milky, white, opalescent fluid that is formed when long-chain triglycerides in the diet are transformed into chylomicrons and very-low-density lipoproteins, which are then secreted into intestinal lacteals. These lymphatic channels coalesce to form the thoracic duct, which transports chyle and ultimately drains it into the left subclavian vein. Any injury to the duct (or its major tributaries) as it courses through the thoracic cavity can

Lymphoscintigraphy demonstrating thoracic duct injury in an infant with hypoplastic left heart syndrome.

Chylous effusion is an infrequent complication of cardiothoracic surgery and a less frequent complication of central line placement. The authors describe a novel application of lymphoscintigraphy showing thoracic duct injury in an infant after surgery for hypoplastic left heart syndrome.

Chylothorax: a novel therapy.

OBJECTIVE: To report a novel therapy for chylothorax. DESIGN: Case report. PATIENT: Severely ill neonate with congestive heart failure and high surgical risk. INTERVENTIONS: Standard therapy for chylous effusion with the addition of pressure control ventilation and positive end-expiratory pressure (high mean airway pressure). RESULTS: Rapid resolution of effusion with elimination of excessive fluid needs and correction of coagulation profile. CONCLUSION: Increasing mean airway pressure by pressure control ventilation and positive end-expiratory pressure should be considered as a therapeut

Pulmonary lymphangiomyomatosis (LAM) developing chylothorax.

We describe a case of pulmonary lymphangiomyomatosis (LAM) with chylothorax that developed in a 46-year-old Japanese woman. This patient exhibited clinical symptoms of dyspnea and chest X-ray showed right pleural effusion. Thoracocentesis demonstrated chylous effusion. Chest computed tomography (CT) scan revealed multiple cystic lesions. Subsequent thoracoscopy revealed the chylorrhea from swelled vessels on the diaphragm. The clinical diagnosis, based on histological examinations with biopsy specimens obtained by thoracoscopy, was pulmonary LAM. Although the hormone therapy was not effective,

Video-assisted thoracic surgery for the management of pleural and pericardial effusion in Behçet's syndrome.

A 28-year-old man, who presented with pleural and pericardial chylous effusion secondary to superior vena cava syndrome, was diagnosed with Behcet's syndrome. A pericardial window was created by video-assisted thoracic surgery to relieve cardiac tamponade, and this technique also enabled lung biopsy, pleural abrasion, and drainage of the pleural effusion. We report this case because of its rarity. To the best of our knowledge, the literature contains no other report of the use of video-assisted thoracic surgery for creating a pericardial window for the treatment of cardiac tamponade in a case

Chylous ascites: diagnosis, causes and treatment.

Chylous ascites is a rare form of ascites and generally associated with a poor outcome since it is often secondary to neoplasms. Its true incidence is not well established in the general medico-surgical population. Any source of lymph vessels obstruction or leakage can potentially cause chylous effusions in the peritoneal or retroperitoneal cavities. Any type of cancer and lymph node involvement may be associated with this uncommon type of ascites. Traumatic, and mainly surgical, vessels leakage is the second most common source of chylous effusions. Other even more rare underlying conditions h

Lung transplantation for lymphangioleiomyomatosis: role of imaging in the assessment of complications related to the underlying disease.

PURPOSE: To identify the complications and imaging findings related to lymphangioleiomyomatosis (LAM) after lung transplantation. MATERIALS AND METHODS: The authors retrospectively reviewed the clinical histories and imaging studies of 13 patients from five major medical centers who underwent unilateral (n = 8) or bilateral (n = 5) lung transplantation for LAM between 1991 and 1997. Complications related to LAM, both before and after transplantation, were recorded. RESULTS: The following LAM-related complications were found during and after transplantation: excessive pleural adhesions (n = 4

Use of rutin for medical management of idiopathic chylothorax in four cats.

Four cats with idiopathic chylothorax were given rutin. Three of the 4 cats had clinical improvement, and complete resolution of chylous effusion was evident in 2 cats. Idiopathic chylothorax in cats is a condition that may be difficult to resolve with medical or surgical treatment. Currently, thoracic duct ligation is considered the preferred technique, but expense and a guarded prognosis for resolution may make this procedure undesirable to cat owners. Rutin administration may offer an alternative to traditional treatments or may be useful as an adjunctive treatment.

Pulmonary lymphangioleiomyomatosis. A study of 69 patients. Groupe d'Etudes et de Recherche sur les Maladies "Orphelines" Pulmonaires (GERM"O"P).

Pulmonary lymphangioleiomyomatosis (LAM) is a rare disorder of unknown cause characterized by peribronchial, perivascular, and perilymphatic proliferation of abnormal smooth muscle cells leading to cystic lesions. The hypothesis of hormonal dependence and the effectiveness of hormonal therapy have not yet been demonstrated conclusively, and the prevalence of extrathoracic manifestations and the survival of patients with LAM are somewhat contradictory. A multicentric retrospective study was conducted in an attempt to describe better the initial features, the diagnostic procedures, the associate

Medical thoracoscopy in the diagnosis of unexplained pleural effusion.

Approximately 20% of pleural effusions remain without an established aetiology after evaluation. Thoracoscopy has a very high sensitivity for the diagnosis of both benign and malignant diseases and greatly increases the diagnostic yield for pleural effusion. We sought to evaluate the diagnostic yield and safety of medical thoracoscopy at this institution. The records of all patients undergoing medical thoracoscopy for the evaluation of undiagnosed pleural effusion between 1990 and 1996 were reviewed. The procedure was performed under local anaesthesia with sedation using a Stortz rigid thoraco

[Chylothorax after pressure pneumothorax].

Chylothorax is a pleural effusion caused by leakage of chyle into the pleural cavity. The commonest causes of pleural effusion are trauma to the ductus thoracicus or neoplasms in the mediastinum. The trauma can be iatrogenic or accidental. Chylothorax caused by tension pneumothorax has not previously been reported. We describe a case where this etiology is the most likely explanation of the chylous effusion in our patient.

Primary idiopathic chylopericardium--a case report and review of the literature.

Primary idiopathic chylopericardium is a rare entity with obscure etiology. In recent years, a few reports have been published where the lymphatic leak and fistula were identified by combined lymphangiography and computed tomography or intraoperative thoracic ductogram. Here we report a case occurring in a previously healthy 51 year old woman, to our knowledge the first case described in Scandinavia. She presented with cough and dyspnea of about 1 year duration. Chest X-ray demonstrated enlargement of the cardiac silhouette. Echocardiography revealed pericardial effusion and at puncture perica

Chylothorax complicating a thoracic aortic aneurysm.

Chylothorax is an uncommon condition, usually associated with advanced malignant disease or trauma to the thoracic duct. Idiopathic chylothorax and other diverse causes have been anecdotally reported. We describe a rare case of chylothorax, developing as a result of damage to the thoracic duct by a thoracic aortic aneurysm. Tetracycline pleurodesis was successful in controlling the chylous effusion.

Accessory lung lobe torsion and chylothorax in an Afghan hound.

Torsion of the accessory lung lobe is extremely rare and has been reported only in one human. Accessory lung lobe torsion (LLT) and chylothorax occurred in a 3.5-year-old, male Afghan hound. An exploratory thoracotomy was used to remove the torsed lung lobe. When medical management of the chylous effusion failed, thoracic duct ligation was performed, resulting in resolution of the effusion. The dog remains clinically healthy 29 months postoperatively.
