BACKGROUND: Anomalies in chyle flow, obstructive, functional or malformative, are responsible for leakage of chyle into the peritoneal, pericardial, or pleural spaces. Although rare, chronic chylous effusions can have disastrous consequences. Current treatments do not provide permanent and satisfactory solutions. Lymphovenous anastomosis can be a suitable treatment in this context; however, it needs to be fully evaluated regarding its efficacy, indications, and means of achievements. The objective of the study is to assess the efficacy of lymphovenous anastomosis to treat nontraumatic chylous

The diagnosis and management of chylothorax present significant challenges due to the diverse clinical entities that can result in a chylous effusion. A thorough understanding of the underlying disease processes is essential for effective treatment. Although various treatment algorithms have been proposed, the limited availability of high-quality trials precludes a definitive recommendation of any single approach. This article aims to review the current literature on the evaluation and management of chylothorax.

Magnetic resonance lymphangiography (MRL) has revolutionized the evaluation of lymphatic system disorders in pediatric patients, offering unparalleled details without the risks of ionizing radiation. This imaging modality is pivotal for diagnosing congenital lymphatic anomalies, chylothorax, lymphedema, and vascular malformations, which require precise characterization for optimal management. Pediatric radiologists must understand MRL's technical nuances, including contrast protocols, imaging sequences, and postprocessing techniques, as well as its role in guiding therapeutic interventions. Chylopericardium (CP), defined as the accumulation of chyle in the pericardial cavity, is a rare condition, especially in the absence of an identifiable secondary cause. Primary idiopathic chylopericardium (PIC) is even more uncommon, with limited cases reported in the literature. We report the case of a 43-year-old South Asian woman who presented with palpitations and fatigue. Echocardiography revealed a large pericardial effusion with signs of cardiac tamponade, necessitating emergent pericardiocentesis. The pericardial fluid aspirated was pinkish and turbid, which turned to milky white afte

Third-space endoscopy (TSE) has emerged as an effective treatment modality for various gastrointestinal motility diseases and gastrointestinal tumors. TSE is based on the concept of working in the submucosa using a mucosal flap valve technique, which is the underlying premise for all TSE procedures; thus, some complications are shared across the spectrum of TSE procedures. Despite the high safety profiles of most TSE procedures, studies have reported various adverse events, including insufflation-related complications, bleeding, perforation, and infection. Although the occurrence rate of those

Patients with body cavity effusions are often seen in veterinary practice. However, the underlying causes of effusion formation are numerous and to draw conclusions about the underlying pathogenesis, further laboratory diagnostic work-up is essential. An initial classification of the pathomechanism (transudate versus exudate) may already be achieved using simple tools that are generally available in every practice (e. g. refractometer, hematology device/counting chamber, microscope). The effusion analysis includes macroscopic assessment, evaluation of the protein content, determination of the nu

INTRODUCTION: Chylopericardium is a rare but serious complication after thoracic surgery, caused by injury to thoracic duct tributaries during procedures like lymph node dissection. It leads to chylous fluid in the pericardial cavity. Prompt diagnosis is vital to prevent cardiac tamponade, malnutrition, and immunosuppression due to disrupted lymphatic drainage.
CASE PRESENTATION: A 74-year-old man presented with neck discomfort, and a CT scan incidentally revealed a part-solid ground-glass nodule (GGN) in the right upper lung lobe, raising suspicion of primary lung cancer. Thoracoscopic lobec

BACKGROUND: Chylothorax is a rare but serious condition that can lead to significant morbidity if not effectively managed. While conservative measures and transthoracic duct ligation remain first-line approaches, these can fail in the setting of altered thoracic anatomy, bilateral effusions, or previous thoracic interventions. In such cases, transabdominal ligation of the cisterna chyli offers a direct route to the main lymphatic outflow tract. This study analyzes the results in patients with refractory chylothorax who underwent cisterna chyli ligation through laparotomy. METHODS: In this retr

In human medicine, the 2025 International Society for the Study of Vascular Anomalies (ISSVA) classification distinguishes localized lymphatic malformations from systemic disorders referred to as complex lymphatic anomalies (CLAs), including generalized lymphatic anomaly (GLA) and central conducting lymphatic anomaly (CCLA). GLA is characterized by multifocal proliferation of dilated lymphatic channels infiltrating soft tissues and visceral organs, whereas CCLA involves structural or functional abnormalities of the central lymphatic pathways such as the thoracic duct commonly resulting in chyl

INTRODUCTION: Chylothorax is defined by the accumulation of chyle in the pleural space, characterized by triglyceride levels exceeding 110 milligrams per deciliter. The clinical presentation of chylothorax varies depending on its acuity and underlying etiology. Mediastinal lymphangiomas are extremely rare and benign lesions that can affect both infants and adults. They may occur independently or in association with other lymphatic disorders and can lead to complications such as chylothorax.

BACKGROUND: Kaposi sarcoma (KS) is one of the most common tumors in patients with AIDS, while the occurrence of chylous effusions in KS patients is rare and often indicates a poor prognosis. This report present a case of successfully treated AIDS-associated KS complicated by both chylothorax and chylous ascites, along with a review of the relevant literature.
CASE PRESENTATION: A 31-year-old male patient, who was recently diagnosed with HIV infection and oral KS at our clinic center, returned two weeks post-discharge. He reported significant enlargement of the oral tumor, increased abdominal

BACKGROUND: Chylothoraces result from traumatic or non-traumatic insult to the thoracic duct, allowing for lymph to accumulate in the chest. Patients present with increasing dyspnea and fatigue, and the diagnosis is made via chest X-ray, computed tomography (CT), and comparative analysis of the pleural fluid and serum. Management largely entails diet modification and drainage with or without adjunct medications, reserving pleurodesis, percutaneous duct embolization, or thoracic duct ligation for recalcitrant cases.

Klippel-Trenaunay syndrome is a rare congenital malformation predominantly affecting lower limb. In most cases, it is characterized by a classic triad of cutaneous capillary malformation (port-wine stain), lymphatic and venous abnormalities, in association with variable soft tissue and bone overgrowths. We describe a 48-year-old male presenting on the genitalia several whitish vesicles discharging a milky fluid compatible with chyle. Extensive radiology workup revealed pelvic megalymphatic malformations. Pelvic lymphatic ligations and bleomycin sclerotherapy only allowed a partial improvement.

Lymphangioleiomyomatosis is a rare pulmonary disease affecting women of childbearing age. Whilst chylothorax is a well-recognized complication of the condition, management strategies aren't well-defined, have low success rates and are often only available at tertiary or specialist centres. We describe a case of a young woman referred to pleural clinic with a chylous effusion found to be secondary to lymphangioleiomyomatosis. Initial medical management was unsuccessful and recurrent drainages caused significant complications. Remission was ultimately achieved with a combination of mTOR inhibito

Chylothorax is a rare condition characterized by the accumulation of chyle in the pleural space. While it accounts for a small percentage of pleural effusions, chylothorax can lead to significant morbidity and mortality. This article provides a comprehensive overview of chylothorax, covering its relevant anatomy, aetiology, pathophysiology, clinical features, diagnosis, and management. Injury or disruption to the thoracic duct (which is responsible for chyle transport) leads to the development of chylothorax. This may result from trauma, such as iatrogenic injury during surgery, or non-traumat

Veterinary radiology & ultrasound : the official journal of the American College of Veterinary Radiology and the International Veterinary Radiology Association2024eng
A 12-year-old domestic shorthair cat was presented with relapsing noncardiogenic chylothorax. CT-lymphangiography of the thorax confirmed bilateral pleural effusion without evidence of an underlying intra-thoracic origin. Abdominal CT-lymphangiography revealed a 2.6 cm diameter splenic mass surrounded by chylous effusion actively collecting during ultrasonographic assessment. Following splenectomy, histopathological analysis revealed that the splenic mass exhibited characteristics indicative of splenic angiosarcoma. This case report highlights the utility of advanced thoracic and abdominal ima

Chylous joint effusion is a rare condition characterized by the presence of a milky, viscous synovial fluid with abnormal lipid concentrations. The thorax is the most common site of involvement. Only a handful of cases have been reported in the field of orthopedic surgery and even fewer have been reported involving uncommon locations such as the knee. Treatment of chylous joint effusion may require surgical intervention along with the use of somatostatin or octreotide and a low-fat diet. We present herein a case of post-traumatic chylous effusion in the wrist treated with surgical incision and

Chylopericardium is a rare entity in veterinary medicine. In this report we document the development of chylopericardium in two dogs undergoing chronic hemodialysis. An 11-year-old female spayed Labrador retriever (Case 1) presented with acute coughing and lethargy 2 months following initial dialysis catheter placement and initiation of dialysis therapy for severe azotemia. Echocardiography demonstrated severe pericardial effusion and cardiac tamponade. Pericardial fluid analysis was consistent with chylous effusion. The dog underwent a subtotal pericardiectomy with thoracic duct ligation, and

Chylothorax is a rare condition that results from thoracic duct disruption with malignant and nonmalignant etiologies manifesting as a pleural effusion. Typically, chylothorax in the setting of cirrhosis is associated with the migration of chylous ascites. We present the case of a 64-year-old male with prior liver transplant who presented with new-onset transudative chylothorax without chylous ascites who responded to transjugular intrahepatic portosystemic shunt revision, diuresis, and serial thoracentesis.
