35 mobilized patients received continuous parenteral nutrition consisting of an all-in-one solution mixed together in this department right before use over a total period of 1155 days (mean per patient 33 days). The primary complication rate was: second puncture in 14%, pneumothorax in 11% and haemothorax in 3% of cases. Secondary complications: catheter-related sepsis was documented in 8.5%. Metabolic complications were not seen. The all-in-one solution is a satisfactory nutrient for long-term, low-risk administration by the route, suitable even in the case of mobilized patients, without the

The diagnosis of life threatening situations due to blunt chest trauma, the priority of treatment, and therapeutical consequences are discussed. Rib fractures, pulmonary contusions could be treated conservatively. Tension pneumothorax and haemothorax need immediate decompression by intercostal tube drainage. The necessity for prompt exploratory surgery should always be considered in patients with massive haemothorax, tracheobronchial disruption and traumatic rupture of the diaphragm. Rupture or perforation of the oesophagus, traumatic aneurysms of the thoracic aorta, large diaphragmatic hernia

The authors recommend a simple procedure, efficient in cases with thoracic flaps, called voletopexia. The thoracic flaps are severe complications of thoracic trauma, accompanied by paradoxal respiration, mediastinal pendullation, severe respiratory failure, frequently in association with subcutaneous and/or mediastinal emphysema, parietal hematomas, haemothorax or haemopneumothorax, pulmonary tissue ruptures, or ruptures of the large vessels. The method proposed consists in a fixation of the mobile flap to the intact ribs above and under it by one or two knits with double thread, in a perpendi

In a developing country with inadequate clinical facilities a conservative method of management of a major clinical problem is often the only rational approach. This policy was adopted in the management of 145 patients with chest trauma in a teaching hospital in Nigeria. Automobile accidents were the cause of the thoracic injuries in 73.1% of the patients; 71.7% of the patients were managed as in-patients. The management of the patients was essentially aimed at correction of hypovolaemia, tube drainage of pleural collections, and relief of pain by intercostal nerve block. Major operative proce

A report is given about 360 percutaneous transthoracic lung biopsies in diffuse affections of the lung framework. By means of a good cooperation between skilled lung physician and experienced lung pathologist it was able to get a definitive diagnosis in 88.3% of the cases. The expense concerning this diagnostic procedure is small. As for complications there occurred pneumothoraces (15.8%), lung haemorrhage (5.3%) and haemothorax (1 case).

Central venous catheterisation via the supraclavicular and subclavicular route was performed in 61 and 44 cases respectively at the department's intensive therapy unit. A statistical assessment was made of the modalities of application, complications, time of indwelling, and reasons for removal. A large number of more serious complications (pneumothorax, haemothorax, arterial puncture) were noted with the supraclavicular technique, whereas subclavian catheterisation permitted longer residence times and was rarely responsible for serious disturbances, even though there were more instances of po

Thirty six cases of traumatic rupture of the thoracic aorta (TRA) were diagnosed during the hours following the accident responsible. Rupture was situated at the aortic isthmus in 32 cases, the ascending aortic in 2 cases, the arch of the aorta in 1 case and the descending sub-isthmic aorta in 1 case. Ten patients had no rib fractures. In 16 patients not undergoing surgery before 20th hour after the trauma, 10 died of secondary rupture. The course of a TRA is thus unpredictable and it is of fundamental importance to make the diagnosis and undertake appropriate surgical treatment immediately. T

The pulmonary artery wedge pressure was increased because of the development of a haemothorax. After drainage of the haemothorax, the pulmonary artery wedge pressure was found to be less than that required to maintain an effective cardiac output and a rapid infusion of blood was required.

Pneumothorax and haemothorax constitute serious complications of a blunt chest trauma. During 1970-1978 64 persons with pneumothorax and 109 cases of haemothorax were admitted to and treated in the Surgical Unit of the University Hospital, Göttingen. In nearly every case fracture of several ribs had also occurred. The method of choice for treating cases of pneumothorax was by Bülau drainage; surgical closure of the air leakage was rarely necessary. Conservative measures are indicated only if the pneumothorax is narrow or confined to the apical region. It is worth mentioning that in a large num

263 persons with chest injuries were treated over a period of 16 years. 47 had performating chest injuries, in the remaining cases it was a blunt trauma. Prompt recognition of a life-endangering situation (cardiac tamponade, tension pneumothorax, mediastinal emphysema, massive haemothorax) is essential; radiological and laboratory diagnostic methods play a secondary role. The most effective emergency treatment is intercostal continuous suction; in many cases it is the only one needed. A haemothorax must be evacuated completely because of the risk of complications and fibrin formation. Thoracot

The massive blood loss that accompanies penetrating chest wounds involving the heart or intrathoracic vessels generally leaves no option but to act immediately. Blunt chest trauma in combination with other injuries makes particularly stringent demands on diagnostic and therapeutic skills. Most injuries to the intrathoracic aorta are in the form of intiially closed tears of the descending part; but rupture is an ever-present threat. The symptoms are: enlargement of the mediastinum in the chest roentgenogram, differences in blood pressure between the upper and lower half of the body and haemotho

During 1968-1977 86 persons with chest trauma were treated at the Chest Clinic Heckeshorn. Until 1973 only a few cases of, mostly slight, chest injuries were admitted and treatment was mainly concerned with pneumothorax and haemothorax induced by fracture of ribs. Since 1974, however, cases of chest trauma have markedly increased in both number and seriousness of the injury. During the past years 50 patients with chest injuries were treated. They included 6 cases of stab wounds, 4 cases of bullet wounds and one case each of damage to the trachea and bronchial rupture respectively. Thoracotomy

A statistical analysis of the case material at the Intensive Care Unit, Freiburg, for the years 1975 and 1976 established that 40% and 39% respectively of patients with multiple injuries had also suffered a chest trauma and that the latter was the direct cause of respiratory insufficiency in 61% (1975) and 57% (1976) of patients in need of controlled respiration, i.e. respiratory insufficiency dominated the clinical and pathophysiological picture. The causes were: restricted respiratory movements due to pain, compression of the lungs or pathological changes in the injured lung, and they affect

A 72-years old man was severely injured when a lorry rolled back and pinned him down, causing contusion of the chest, fractures of ribs 3-10 on the right and haemothorax. Treatment of the chest injuries was by drainage and by positive end-expiratory pressure ventilation because of the development of severe pneumonia with wet lung. Persistent renal insufficiency, a gastro-intestinal haemorrhage and diabetes also required treatment. The patient developed septic endomyocarditis as a late complication, possibly attributable to the central venous catheter. All pulmonary and extrapulmonary injuries

Our experience indicates a continued rising incidence of gunshot wounds of the chest in the United States. During the past 4 1/2 years, 250 consecutive cases were treated at the King-Drew Medical Center in Los Angeles. Ninety per cent presented with a haemothorax or haemopneumothorax. Pneumothorax alone was present in only 3 per cent of cases. Twenty per cent had associated intra-abdominal injuries, involving mostly the diaphragm, liver, spleen and the gastrointestinal tract. Eighty per cent were treated with tube thoracostomy and among these there were 2 deaths, neither being related to the c

Patients suffering from uncomplicated pneumothorax, haemothorax or haemopneumothorax of clinical significance are treated with fluid replacement, intercostal under-water drainage, antibiotics and routine physiotherapy. The commencement of routine physiotherapy may be delayed 8-60 hrs depending on the time the patient is admitted and arrangement of 'out-of-hours' physiotherapy services, but immediate physiotherapy produces superior results from every point of view.

Haemothorax, haemopneumothorax, and pneumothorax were the most common complications in 85 patients with penetrating stab injuries of the chest. These complications were amenable to conservative treatment by aspiration or drainage of the pleural space. Immediate operation was indicated in 30 cases. Indications for surgery were haemorrhage from a major systemic or pulmonary vessel or the heart, cardiac tamponade, diaphragmatic penetration, oesophageal and bronchial tears, and sucking chest wounds. The need for immediate operation was clinically obvious at presentation in most cases. It should ha

Percutaneous, transhepatic catheterisation of the portal vein in order to occlude the gastric coronary vein was carried out in six patients with bleeding oesophageal varices. Bleeding stopped in three patients, but in the other three ceased only temporarily. Complications were one pleural effusion and one haemothorax. At autopsy, marginal portal vein thrombosis was found in two cases. The indication for this procedure is the presence of bleeding from oesophageal varices which cannot be controlled by conservative means in a patient unfit for operation. It provides an opportunity for carrying ou
