An Unusual Case of Anterior Displacement of the Vagus Nerve during Total Thyroidectomy: A Surgical Dilemma.

Staphylococcus aureus is a major cause of hospital-acquired infections and infections in kidney dialysis patients. Over 90% of these infections are due to types 5 and 8. Nabi, under a CRADA agreement with the US National Institutes of Health, is developing a bivalent vaccine against S. aureus types 5 and 8 [Nabi-StaphVAX, StaphVAX]. The vaccine contains type 5 and type 8 S. aureus capsular polysaccharides conjugated to a protein carrier (recombinant exoprotein A, a genetically detoxified form of Pseudomonas aeruginosa exotoxin A). Nabi changed its name to Nabi Biopharmaceuticals on 5 March 200

Emergency laparoscopic splenectomy for a grade-V splenic injury following blunt abdominal trauma: A case report.

INTRODUCTION AND IMPORTANCE: The utilization of laparoscopy in severe splenic injuries following blunt abdominal trauma is limited because of difficulties encountered due to ongoing bleeding, chances of missed injury, and lack of experience. CASE PRESENTATION: We report a young male with polytrauma (head injury, haemothorax, femur fracture, Grade V splenic injury) who underwent a successful laparoscopic splenectomy on day 3 following the failure of conservative management of grade V blunt splenic injury involving the hilum. CLINICAL DISCUSSION: The utility of laparoscopy in the management of

Efficacy of slow negative pleural suction in thoracic trauma patients undergoing tube thoracostomy-A randomised clinical trial.

INTRODUCTION: Thoracic injuries are prevalent in polytrauma patients, with road traffic accidents being a major cause. In India alone, over 400,000 people were injured in such accidents in 2022. Rib fractures, haemothorax, and pneumothorax are common chest injuries, often managed with tube thoracostomy. While standard procedures for chest tube placement are established, consensus on post-insertion management, particularly regarding negative pleural suction, is lacking. Research on this topic mostly pertains to planned thoracotomies rather than trauma cases. This study seeks to compare outcomes

Video-assisted thoracoscopic surgery repair of an idiopathic internal mammary artery aneurysm.

A discussion of an internal mammary artery aneurysm is a rare finding in the literature. This condition can cause serious complications, including haemothorax and pneumo-haemothorax, and can lead to haemorrhagic shock; it can also be fatal. These effects can be explained by the rate of flow of the blood in the internal mammary artery, that is, 150 ml/minute, which leads to a blood loss of one litre in minutes. As reported in the literature, it has many different clinical presentations. Because an internal mammary artery aneurysm occurs so rarely, agreement regarding the best way to manage its

Thoracic splenosis: Precision medicine can prevent thoracic surgery.

Thoracic splenosis is a rare condition referring to the auto-transplantation of splenic tissue into the thoracic cavity following splenic trauma. We present a case of thoracic splenosis in a 62-year-old man who at the age of 17 suffered three gunshot wounds to the posterior thorax and abdomen, requiring a splenectomy and intercostal catheter insertion. In 2007, he underwent a thoracotomy and biopsy of a left sided pulmonary mass which was complicated by a haemothorax requiring an emergent return to theatre and rib resection to achieve haemostasis. The biopsy revealed granulation tissue. In 202

4D computed tomography assessment of ruptured triple-lumen type B aortic dissection: a case report.

BACKGROUND: Few studies have investigated the effect of the intimal morphology of type B aortic dissection (TBAD) on the blood flow after rupture. We report a case of a 30-year-old male with complicated TBAD, who underwent assessment with 4D computed tomography (4D-CT). CASE SUMMARY: Patient presented with chest tightness for 14 days, a heart rate of 67 b.p.m., regular rhythm, and 2 years of hypertension. Precisely 14 days prior, he had been diagnosed with aortic dissection by ultrasound at another hospital. 4D-CT showed thoracoabdominal aortic dissection (Stanford type B), left haemothorax,

Unusual cause of domestic blast injury to the chest: Case report.

INTRODUCTION AND IMPORTANCE: Domestic blast chest injuries are because of suicide and accidents. Explosion injuries (trauma) are common in conflicts or military settings. Terrorists attacks result in injuries among the civilian population. A case of a 52-year-old African man who had an oxygen canister/grinder explode on him whilst working with it is reported. This case confirms that the mortality rate of patients with chest blast injuries who reach the hospital is low at less than 5 %. CASE PRESENTATION: A 52-year-old man suffered a blast injury from an exploded oxygen canister in July 2018.

Obstructive shock due to a spontaneous haemothorax caused by primary lung cancer.

We describe the case of a 67-year-old man with shock and hypoxemia. Chest X-ray showed bilateral lung mass shadows and left pleural effusion with a mediastinal shift, suggesting malignancy. Physical examination and point-of-care ultrasound findings did not suggest obstructive or cardiac shock, but the patient had prolonged shock refractory to fluid and blood transfusion therapy. We inserted a drain into the left thoracic cavity, which enabled the patient to recover from shock. We diagnosed the patient with obstructive and hypovolemic shock due to spontaneous haemothorax caused by primary lung

Valproic acid-induced eosinophilic pleural effusion: An uncommon occurrence.

A 43-year-old male using valproic acid (VA) for 2 years for seizure disorder presented with right-sided moderate pleural effusion. Pleural fluid analysis revealed exudative effusion with 42% eosinophils. There was no evidence of haemothorax, pneumothorax, malignancy, and parasitic infections. Suspecting a drug-related event, VA was discontinued. The patient showed clinical improvement with resolution of pleural effusion on chest radiograph three weeks later. VA is a popular drug used for variety of disorders like seizures, migraines, and schizophrenia. There is a paucity of literature on VA-in

Switch to a bicarbonate-based Impella CP™ purge solution in a patient with haemothorax: a case report.

BACKGROUND: Percutaneous mechanical circulatory support (pMCS) by an Impella™ device implies the initiation of systemic anticoagulation to prevent systemic thrombotic complications and a purge fluid to prevent device blockage. Traditionally, unfractionated heparin (UFH) was used for both. In April 2022, the use of bicarbonate-based purge solution (BBPS) as an alternative to UFH in dextrose solution was approved by the Food and Drug Administration in case of contraindications for UFH. CASE SUMMARY: We present the case of a 73-year-old female that was admitted to the cardiac intensive care unit

Sternal fractures and thoracic injury: an analysis of 288 sternal fractures attending a major trauma centre.

INTRODUCTION: Sternal fractures (SF) are uncommon injuries usually associated with a significant mechanism of injury. Concomitant injury is likely, and a risk of mortality is substantial. AIM: Our aim in this study was to identify the risk factors for mortality in patients who had sustained sternal fractures. METHODS: We conducted a single centre retrospective review of the trust's Trauma Audit and Research Network Database, from May 2014 to July 2021. Our inclusion criteria were any patients who had sustained a sternal fracture. The regions of injury were defined using the Abbreviated Injur

17 years' experience of surgical management of thoracic outlet syndrome at a district general hospital.

INTRODUCTION: Thoracic outlet syndrome (TOS) is caused by compression of the neurovascular structures passing through the thoracic inlet. It is categorised into three subtypes: neurogenic TOS (NTOS), venous TOS (VTOS) and arterial TOS (ATOS). This study evaluates the outcomes of patients who underwent first rib resection (FRR) for TOS during a period of 17 years at a single district general hospital. METHODS: Retrospective review of patient notes of individuals treated with FRR from August 2004 to August 2021. RESULTS: A total of 62 FRRs were performed on 51 individual patients. Indications

[Chest Tube in Thoracic Trauma - Recommendations of the Interdisciplinary Thoracic Trauma Task Group of the German Society for Thoracic Surgery (DGT) and the German Trauma Society (DGU)].

For unstable patients with chest trauma, the chest tube is the method of choice for the treatment of a relevant pneumothorax or haemothorax. In the case of a tension pneumothorax, needle decompression with a cannula of at least 5 cm length should be performed, directly followed by the insertion of a chest tube. The evaluation of the patient should be performed primarily with a clinical examination, a chest X-ray and sonography, but the gold standard of diagnostic testing is computed tomography (CT).A small-bore chest tube (e.g. 14 French) should be used in stable patients, while unstable patie

Massive haemothorax from percutaneous nephrolithotomy requiring video-assisted thoracoscopic surgery: A case report.

INTRODUCTION: Massive haemothorax can occur following percutaneous nephrolithotomy (PCNL), which is a significant adverse event and a life-threatening condition. PRESENTATION OF CASE: A 65-year-old male who presented with a full right staghorn stone was treated with PCNL. Two days later, he developed massive haemothorax and conservative management with intercostal drainage failed. The patient successfully underwent video-assisted thoracoscopic surgical decortication (VATSD). DISCUSSION: PCNL is the mainstay procedure for complex renal stones. Because it is aggressive, it can also have seriou

[Pacemaker infection in fragile patients].

Complications associated with cardiac implantable electric devices (CIED) are manifold. They include lead dislocation, twiddler's syndrome, device malfunction, haematoma formation and infection. Infections can be divided into acute, subacute and late infections. Both the time of onset and the route of infection play a crucial role. The consequences of a CIED infection are devastating. The most modern treatment methods include the removal of all implanted implants. If complete removal is not followed in the event of infection, there is a high rate of infection recurrence. Open thoracic surgery

Spontaneous Non-Traumatic Mediastinal Hematoma in a Patient on Imatinib Therapy for a Gastrointestinal Stromal Tumor (GIST).

Mediastinal hematomas are thoracic complications often resulting from direct trauma or aortic dissections. Spontaneous non-traumatic mediastinal hematomas are rare. We present a case of spontaneous non-traumatic mediastinal hematoma in a patient on Imatinib therapy for a gastrointestinal stromal tumor (GIST). A 67-year-old female presented to the ER with the chief complaint of constant sharp right shoulder pain that progressed to her chest. The patient was not on any anticoagulants and had not complained of shortness of breath. Under suspicion of a pulmonary embolism, a CT chest scan was perfo

The Safety and Efficacy of Ultrasound-Accelerated Catheter-Directed Thrombolysis in Patients with Intermediate-High-Risk Pulmonary Embolism: Bo-NE-Experience.

Ultrasound-accelerated thrombolysis (USAT) is an advanced interventional therapy for patients with intermediate-high-risk pulmonary embolism (PE) who deteriorated on anticoagulation or for high-risk patients for whom systemic thrombolysis is contraindicated. The aim of this study is to investigate the safety and efficacy of this therapy with a focus on the improvement of vital signs and laboratory parameters. Seventy-nine patients with intermediate-high-risk PE were treated with USAT from August 2020 to November 2022. The therapy significantly decreased the mean RV/LV ratio from 1.2 ± 0.22 to

Thoracic aortic dissection presenting with bilateral haemothoraces: a potentially missed diagnosis.

Thoracic aortic dissection is a life-threatening diagnosis, which should not be missed. We present a case in which a patient who presented to the emergency department with chest pain was assessed and referred for admission for treatment of pneumonia, due to misinterpretation of a chest radiograph finding. The patient was re-reviewed and subsequently underwent further investigation, which confirmed aortic dissection. She underwent emergency thoracic endovascular aortic graft repair with stent graft insertion. This case demonstrates haemothorax as an uncommon complication of aortic dissection.
