Rheumatoid arthritis-associated interstitial lung disease presenting before articular manifestations: A case report.

Rheumatoid arthritis-associated interstitial lung disease (RA-ILD) may precede overt articular manifestations, posing significant diagnostic and therapeutic challenges. The usual interstitial pneumonia (UIP) pattern is the most common radiologic phenotype and is associated with a poorer prognosis and a progressive fibrosing course.

Temporal Trends in Six Major Cancers: Implications for Lifestyle-Based Cancer Prevention and Early Detection Across the Life Course.

BACKGROUND/OBJECTIVES: Cancer remains a serious public health challenge in the United States. Six major cancers (female breast, colorectal, liver, lung/bronchus, pancreas, and prostate) account for a substantial proportion of the national cancer burden. Understanding the changes in the incidence of these cancers over time can provide insights for prevention and control. This study examined temporal trends in the cancer incidence according to age at onset and stage at diagnosis. METHODS: This population-based study analyzed data from the United States Cancer Statistics database for 2001-2023,

Late cardiopulmonary morbidity in patients with Hodgkin lymphoma: a retrospective nationwide registry study.

Hodgkin lymphoma (HL) is a B-cell neoplasm with a cure rate of 90%. Previous data on late morbidity in patients with HL cover the era of older treatment modalities. This real-world study consisted of 2912 patients diagnosed with HL between 2000 and 2019 in Finland and aimed to clarify the incidence of late-onset cardiopulmonary morbidity compared to matched controls. Patient data were collected from four nationwide Finnish healthcare registries. We reported a higher occurrence of coronary artery disease in patients aged < 50 years at diagnosis than in controls (RR 3.80 (2.16-6.74), p < 0001).

SMARCA4-deficient undifferentiated thoracic tumors with rare nasal cavity and gluteal metastases: a case report and literature review.

BACKGROUND: SMARCA4-deficient undifferentiated thoracic tumor (SMARCA4-UT) is an extremely aggressive malignancy that primarily affects the lung and mediastinum. It predominantly occurs in males with a heavy smoking history, and the associated survival typically does not exceed 1 year. Currently, no standard treatment regimen has been established for this disease, and a limited response to conventional chemotherapy has been noted in SMARCA4-UT. Some studies have suggested that multimodal management combining immunotherapy with chemotherapy and local consolidation therapy may offer a certain su

Late-Diagnosed Cystic Fibrosis with Co-Occurring CFTR 3849+10kbC>T and 5T Variants: A Case Report.

Background: The deep-intronic variant 3849+10kbC>T preserves residual CFTR function and is recurrently associated with pancreatic sufficiency, normal or borderline sweat chloride and diagnosis beyond early childhood. A co-occurring poly-T 5T allele adds a second layer of uncertainty: its penetrance depends on the TG repeat carried in cis, and its contribution to the phenotype can be weighed only once its phase relative to the second variant has been established. Case Presentation: A boy with recurrent respiratory infections from the age of three was diagnosed with cystic fibrosis at 12 years,

D-Dimer in Viral Respiratory Infections: Established Evidence in COVID-19 and Emerging Evidence in Other Respiratory Viruses.

Viral respiratory infections remain a major cause of global morbidity and mortality, with severe disease increasingly recognized to involve profound interactions between inflammation, endothelial dysfunction, coagulation activation, and immunothrombosis. D-dimer, a fibrin degradation product and established biomarker of fibrin turnover, has gained considerable attention because of its strong association with adverse outcomes in coronavirus disease 2019 (COVID-19). This narrative review compares the evidence for D-dimer in COVID-19 with that available for other respiratory viruses and deliberat

Tracking eco-evolutionary patterns among oropharyngeal pathobiome members in a pediatric cystic fibrosis cohort.

UNLABELLED: Polymicrobial airway colonization is common in individuals with cystic fibrosis (CF). Pathogen communities typically follow an ecological succession in which early colonizers, such as Haemophilus influenzae and Staphylococcus aureus, are later joined by Pseudomonas aeruginosa. Although adaptation to the lung environment is well described for single pathogens in adult people with CF, much less is known about the role of pathogen interactions and how changes in individual pathogens influence community dynamics at the early stages of disease. To address these questions, we combined ge

Fully automated diaphragm segmentation on routine 2D-cine and 3D thoracic MRI: multireader validation.

OBJECTIVE: The diaphragm is central to respiratory mechanics and may be impaired in various diseases. Parametric magnetic resonance imaging (MRI) assessment is limited by time-consuming manual segmentation. We developed and validated fully automated diaphragm segmentation on two-dimensional (2D)-cine and three-dimensional (3D) thoracic MRI. MATERIALS AND METHODS: In this single-center study, eligible adults without known diaphragmatic disorders underwent 2D-cine MRI, 3D MRI, or both. Two U-Net models (2D/3D) were trained using expert manual contours as ground truth. Performance was evaluated

Post-extubation negative pressure pulmonary edema following acute upper airway obstruction in a young athlete: a case report.

BACKGROUND: Negative pressure pulmonary edema (NPPE) is a non-cardiogenic hemodynamic pulmonary edema resulting from significant inspiratory effort against obstructed airways. Although rare, it is a recognized cause of postoperative respiratory distress and can mimic differential diagnoses. Patients most predisposed to this condition are muscular or athletic individuals, due to their ability to generate significant negative intrathoracic pressure. CASE PRESENTATION: An 18-year-old male patient underwent a laparoscopic appendectomy under general anesthesia with tracheal intubation. Two minutes

Elevated platelet-to-lymphocyte ratio is associated with stroke-associated pneumonia in acute ischemic stroke: a retrospective cohort study.

BACKGROUND: Stroke-associated pneumonia (SAP) is a common and serious complication of acute ischemic stroke (AIS) that substantially worsens clinical outcomes. The platelet-to-lymphocyte ratio (PLR), an accessible composite index derived from routine complete blood counts, may reflect post-stroke immune imbalance. However, its role in predicting SAP in AIS patients remains unexplored. METHODS: This retrospective cohort study included 1,745 patients with acute ischemic stroke (AIS) after systematic screening of 2,150 patients. After the original screening, 1,766 eligible patients remained; 21

[Subclavian steal syndrome: a case report].

An 80-year-old female presented to the emergency department with dizziness, generalized weakness, and exertional dyspnea. Physical examination revealed a significant inter-arm systolic blood pressure discrepancy. Computed tomography angiography (CTA) of the chest demonstrated high-grade stenosis at the origin of the left subclavian artery, correlating with both the clinical symptoms and the hemodynamic findings. This case underscores the diagnostic utility of CTA in ensuring a prompt and precise evaluation of subclavian artery stenosis in patients presenting with inter-arm blood pressure diffe
