National Impact of Virtual Residency Interview Format on the Integrated Vascular Surgery Match.

OBJECTIVE: Due to the coronavirus disease 2019 (COVID-19) pandemic, the Association of American Medical Colleges (AAMC) required that residency interviews be conducted virtually, instead of the traditional in-person format. Previous survey studies have indicated that vascular surgery (VS) residency applicants favor remote interviews, noting sufficient exposure to the program, location and residents, with minimal travel-related cost and time burden. However, the overall impact of the virtual interview on objective measures of VS residency match have not been examined. DESIGN: Residency applica

Internet Addiction Disorder and Cybersecurity Risk: A Neurobiological and Behavioral Review.

Internet addiction disorder (IAD) has attracted substantial clinical and scientific interest over the past two decades. Although the condition remains outside formal diagnostic frameworks in the fifth edition of the Diagnostic and Statistical Manual of Mental Disorders, accumulating neuroimaging evidence demonstrates that compulsive Internet use is associated with disruptions in dopaminergic reward circuitry and prefrontal cortical function that closely parallel those observed in substance use disorders. These neurobiological changes, encompassing impaired executive function, elevated impulsiv

Extravasation of TPN into the pleural space, a rare complication of central venous catheter placement: a case report.

BACKGROUND: Complications associated with central venous catheters are well documented in the literature, most of which relate to catheter placement or infections. Nonetheless, on rare occasions, the catheter can erode the arterial wall, allowing medication, fluids, or even total parenteral nutrition to pass into the pleural space. CASE PRESENTATION: We present the case of a 68-year-old male who was critically ill in the intensive care unit and required total parenteral nutrition. A central venous catheter was placed, and the patient's condition improved. While in the intensive care unit, it

Establishing influenza epidemic thresholds and intensity levels in Türkiye using the moving epidemic method and assessing concordance with WHO transmission zones: A sentinel surveillance analysis of ten seasons, 2013-2025.

The Moving Epidemic Method (MEM) is the ECDC/WHO-Europe-endorsed approach for establishing influenza epidemic thresholds and intensity levels, yet no positivity-based MEM analysis exists for Türkiye or any country in the WHO Western Asia transmission zone. Weekly influenza positivity rates from FluNet sentinel surveillance were analysed for ten of twelve seasons (2013/2014-2024/2025, excluding two COVID-19 pandemic seasons) using the mem R package with cross-validated optimized slope parameter, and Türkiye's average epidemic curve and thresholds were compared with the Western Asia (excluding T

Memorialization practices in the context of the COVID-19 pandemic in Brazil: the online wake ritual.

This study aimed to understand the online wake ritual as a virtual Memorialization practice in the context of the COVID-19 pandemic in Brazil. This is an exploratory and descriptive qualitative study, with document analysis as the main study design. A search was conducted on the Google platform, with links published between March 11, 2020 and July 27, 2021, in Portuguese, with "Brazil" as the geographic definition. Among the 4924 links identified, 162 links referred to online news articles published in the context of the pandemic and available for full and free access. Among these, 65 were sel

Mortality From 1918-1919 Spanish Flu Among the American Expeditionary Forces Analyzed Through People-Powered Research.

INTRODUCTION: The Card Register of Burials of Deceased American Soldiers, 1917-1922 holds the burial cards of American servicemen and women who died at sea or overseas during or after World War I (WWI). We aimed to use this online register to study mortality statistics in the American Expeditionary Forces (AEF) during the 3 waves of Spanish flu in 1918 and 1919. However, the register, which includes 104 file units, lacks database query functionality. MATERIALS AND METHODS: Over 6,000 volunteers attached to the Zooniverse platform for people-powered research submitted 4 million transcription t

[The history of nursing in France: between lack of knowledge and the construction of ignorance].

At the start of the Covid-19 crisis in France, nurses were hailed as heroes. Yet this enthusiasm quickly waned, and healthcare workers were once again overlooked. This is not the first time they have been treated as heroes, only to be forgotten. In fact, it is a historical pattern in France. Why is that? This article explores this question, shedding light on some of the factors that cause the nursing profession to be cyclically celebrated, only to be neglected afterwards.

RNA sequencing significantly improves HYDIN diagnosis and defines the effects of common deletions in primary ciliary dyskinesia.

Primary Ciliary Dyskinesia (PCD) is a rare genetic disorder affecting ~1:7500 individuals characterised by recurrent chronic respiratory infections, bronchiectasis, laterality defects, and infertility. While at least 60 PCD-causing genes are reported, approximately 30% of patients remain without a confirmed molecular diagnosis. PCD caused by biallelic HYDIN variants is difficult to diagnose due to a 98% homologous pseudogene (HYDIN2). Additionally, functional ciliary diagnostic testing can be inconclusive due to the lack of discernible HYDIN ciliary ultrastructure defects by electron microscop

Evidence-based design of vaccine hesitancy information resources for health care providers.

BACKGROUND: The University of Florida Health Science Center Libraries (HSCL) sought to address the increasing challenge of vaccine hesitancy during the COVID-19 pandemic by developing evidence-based information resources for health care providers. To do this, HSCL librarians assessed the vaccine hesitancy information needs of local health care providers to design a resource that could be used to support conversations with vaccine hesitant patients at the point of care. CASE PRESENTATION: A team of six librarians, supported by student research assistants, conducted interviews with 20 health ca

Early functional maturation of anti-Spike antibodies predicts efficient SARS-CoV-2 RNA clearance.

INTRODUCTION: Viral RNA clearance following mild SARS-CoV-2 infection varies considerably among individuals, but the early humoral immune features underlying this heterogeneity remain incompletely understood. METHODS: We investigated whether the kinetics and functional maturation of the antibody response were associated with viral RNA clearance in a longitudinal study of 78 healthcare professionals infected with the ancestral Wuhan SARS-CoV-2 strain and followed for up to three years. Participants were stratified according to viral RNA clearance kinetics as nonpersistent (≤21 days) or persist

Construction and Validation of a Predictive Model for Voriconazole Treatment Efficacy in Influenza-Associated Pulmonary Aspergillosis Based on Machine Learning.

PURPOSE: To analyze risk factors affecting voriconazole efficacy in influenza-associated pulmonary aspergillosis (IAPA) and build interpretable machine learning (ML) predictive models. PATIENTS AND METHODS: This retrospective study enrolled voriconazole-treated IAPA patients into a primary retrospective set (Jan 1, 2020-Jul 31, 2025) and a separate temporal external validation set (Aug 1, 2025-Apr 30, 2026). To avoid data leakage, the primary set was stratified by treatment response and randomly split at a 7:3 ratio into training and internal validation set prior to feature screening. Forty-t

"It's Like a Whole Set of Things That We Do": A Qualitative Study of the Roles of Journal Editors-in-Chief.

INTRODUCTION: Journal editors-in-chief (EICs) play a central role in shaping scholarly publishing, yet little is known about how they understand and enact their roles. We explored how medical education journal EICs describe their roles in shaping the publication process. METHODS: We conducted a qualitative study using thematic analysis of interviews with nine medical education journal EICs. The interviews were part of a broader investigation of publication timelines and explored editorial leadership and practices, including experiences during the COVID-19 pandemic. Participants' accounts also

Mixed Pneumonia with Pulmonary Embolism and Multiple Pathogens Detected in Bronchoalveolar Lavage Fluid: A Case Report.

Severe community-acquired pneumonia may involve mixed pathogens and concurrent vascular complications. A 72-year-old man presented with fever, nonproductive cough, vomiting, hypoxemia, markedly elevated inflammatory markers, and an elevated D-dimer concentration. Chest computed tomography showed left lower-lobe consolidation and pleural effusion. Computed tomography pulmonary angiography, obtained because of hypoxemia and suspected pulmonary embolism, showed a filling defect in a right upper-lobe pulmonary artery. The patient received antimicrobial therapy prior to bronchoalveolar lavage (BAL)

Diagnostic yield of endotracheal aspirate and bronchoalveolar lavage in ventilator-associated pneumonia: A single-centre study.

Ventilator-associated pneumonia (VAP) remains a significant cause of morbidity and mortality in critically ill patients. This study aimed to compare the diagnostic accuracy of endotracheal aspirate (ETA) with bronchoalveolar lavage (BAL) culture, considered the reference standard, in identifying the causative pathogens of VAP. This study was conducted as a cross-sectional study at Haji Adam Malik General Hospital, Medan, Indonesia. A total of 95 samples from mechanically ventilated patients were analysed using both ETA and BAL in similar institutions. The results demonstrated that 49 samples w

The bacteriology of fallen leaves (Phylloplane) from council parks and green spaces in Northern Ireland: Clinical significance for at risk people living with cystic fibrosis (CF).

BACKGROUND: People living with cystic fibrosis (CF) may suffer from chronic respiratory infections due to acquired environmental organisms, ultimately leading in increased morbidity and mortality. Consequently, parents of children living with CF frequently ask their CF healthcare professional about sources and potential routes of transmission of environmental organisms to their child. This allows parents to make an informed decision regarding risk assessment/risk management of outdoor/ nature-based activities. Mitigations can then be put in place to help minimise or absolutely avoid exposure o
