Analytical and clinical performance evaluation of the STANDARD™ M10 MTB/NTM assay for the simultaneous detection of Mycobacterium tuberculosis and nontuberculous mycobacteria.

The global surge in nontuberculous mycobacteria (NTM) infections, which clinically mimic Mycobacterium tuberculosis (MTB) infections, underscores the need for rapid differential diagnosis. This study evaluated the analytical and clinical performance of the STANDARD™ M10 MTB/NTM assay, a fully automated, cartridge-based, sample-to-result real-time PCR system. Analytical performance was assessed using contrived artificial sputum samples to evaluate inclusivity (3 MTB-complex, 12 NTM species), cross-reactivity (65 respiratory pathogens), and interference (33 substances). The limit of detection (L

The north-south divide in English mortality, 1981-2024: a longitudinal population study.

BACKGROUND: The north-south mortality divide in England, UK, has persisted for several decades. Previous longitudinal analyses up to 2016 have highlighted a widening mortality gap among those aged 25-44 years since the mid-1990s. We aimed to update mortality trends in England and evaluate how trends differed by age and sex up to 2024. METHODS: We did a segmented longitudinal population study of the north-south mortality divide in England (1981-2024). Using registered deaths and mid-year population estimates by 5-year age group and sex, we calculated standardised mortality rates and estimated

Effectiveness of respiratory syncytial virus immunisation in practice.

Respiratory syncytial virus (RSV) affects all ages, with infants and older adults most affected by severe disease. The RSV prevention landscape includes 39 candidates in clinical development, alongside setbacks such as the discontinuation of a promising paediatric live-attenuated vaccine candidate. Several products have been approved, including a maternal vaccine, infant monoclonal antibodies, and vaccines for older adults and adults at high risk. Due to high cost, implementation of these products remains limited to higher-income settings. Real-world evidence confirms high effectiveness, with

Safety outcomes of semaglutide by chronic kidney disease severity in the FLOW trial.

INTRODUCTION: People with type 2 diabetes (T2D) and chronic kidney disease (CKD) often experience medication intolerance and side effects that worsen with lower estimated glomerular filtration rate (eGFR). This prespecified secondary analysis assessed the safety of semaglutide by baseline eGFR. METHODS: FLOW (NCT03819153) was a double-blind, randomized, placebo-controlled trial testing the efficacy of subcutaneous semaglutide 1.0 mg once-weekly on a primary kidney outcome (50% or more eGFR reduction, kidney failure, or death due to kidney or cardiovascular causes) in participants with T2D and

A multi-objective sequence engineering framework for rational genetic vaccine design.

Genetic vaccine performance is determined not only by the delivery platforms and antigen choice but also by the coding sequence that influences antigen expression, innate immune sensing, transcript stability and sequence-associated risks. However, the design principles governing these sequence-level effects remain insufficiently defined. Thus, we consider vaccine coding-sequence design as a multi-objective engineering problem that requires simultaneous consideration of multiple, potentially competing biological properties. We present a multi-parameter computational framework for analysing SARS

Delayed introduction, transmission variation and susceptible dynamics explain spatial asynchrony during Republic of Korea's large pertussis outbreak.

Infectious disease outbreaks typically exhibit a large-scale spatial synchrony, reflecting coupling through shared environmental forcing and human mobility. Exceptions to this theoretical expectation have been reported, but the conditions under which synchrony breaks down remain poorly understood. Republic of Korea's 2024-2025 pertussis outbreak provides a striking example, during which epidemic trajectories diverged substantially throughout the country: notably, the decline in spatial synchrony with distance was more pronounced than that reported in many previous outbreaks. Integrating high-r

Investigation of the effect of simulated gastrointestinal digestive fluids on the infectivity of influenza A virus H1N1.

The recent emergence of highly pathogenic avian influenza (HPAI) in dairy cattle has raised concerns about potential non-respiratory exposure routes including ingestion of contaminated raw milk. In this study, we assessed the survival of influenza A virus (IAV) in milk during simulated human digestion and estimated infection risk using quantitative microbial risk assessment (QMRA). Whole milk artificially inoculated with IAV A(H1N1) was exposed to human saliva, fed-state simulated gastric fluids (pH 3-6) containing pepsin, and fed-state simulated intestinal fluids containing pancreatin, in the

A case of recurrent pleural effusion: tuberculous pleurisy or other disease? A case report.

BACKGROUND: POEMS syndrome (polyneuropathy, organomegaly, endocrinopathy, M-protein, skin changes) is a paraneoplastic syndrome caused by a plasma cell proliferative disorder. Although POEMS syndrome can cause polyserous effusions, the report of pleural effusion as the main manifestation is not common. Here, we describe a case of POEMS syndrome misdiagnosed as tuberculous pleurisy due to massive pleural effusion. CASE PRESENTATION: A 70-year-old man was admitted with chest tightness and was diagnosed with tuberculous pleurisy. Despite receiving anti-tuberculosis treatment, patient exhibited i

Clinical factors associated with CT-confirmed pneumonia in febrile patients: a retrospective study addressing verification bias with inverse probability weighting.

BACKGROUND: Identifying pneumonia among febrile patients is clinically important, particularly in real-world settings where chest computed tomography (CT) is not performed uniformly. We aimed to identify clinical factors associated with CT-confirmed pneumonia in febrile patients and to examine the robustness of these associations after accounting for potential verification bias due to non-random CT completion. METHODS: This retrospective observational study included 902 febrile patients from Huadong Hospital between December 2022 and May 2023. The primary analysis was restricted to patients w

Changes in the microbial spectrum and antimicrobial resistance of bloodstream infections in intensive care units before and during the COVID-19 pandemic.

BACKGROUND: Bloodstream infections (BSIs) are major infectious conditions associated with high morbidity and mortality, particularly in intensive care units (ICUs). In critically ill patients, early and appropriate empirical antimicrobial therapy plays an important role in preventing adverse clinical outcomes. However, the increasing burden of antimicrobial resistance necessitates that empirical treatment strategies be continuously reassessed based on up-to-date local resistance data. This study aimed to compare the distribution of bacterial pathogens and antimicrobial resistance profiles of B

Inhalable epitope vaccine 1022-SpaA V3 establishes nasal mucosal fortress against zoonotic Streptococcus suis.

Streptococcus suis serotype 2 (SS2), a zoonotic pathogen, can breach the respiratory barriers and cause systemic infections in both humans and swine. Current vaccines against SS2 are mainly formulated as an injectable product and primarily induce serum IgG antibodies. However, there is an urgent need for inhalable vaccines that induce the mucosal immune response. Leveraging immunoinformatics, we designed a multi-epitope vaccine targeting conserved epitopes from two virulence factors of SS2, SSU05-1022 (IgA1 protease) and SpaA (pilus-associated protein). Four cytotoxic T-lymphocyte (CTL), seven

Diagnosing and treating influenza in the emergency department: an evidence-based update.

Influenza is a common emergency department presentation that can cause serious complications, particularly in young children, older adults, pregnant and postpartum patients, and patients with underlying conditions. Because its clinical features overlap with those of other respiratory illnesses, emergency clinicians must determine when testing will inform care, which patients need prompt antiviral treatment, and who can be safely discharged. This review presents evidence-based recommendations for the evaluation and management of influenza in the emergency department, with attention to diagnosti

Acceptability and uptake of COVID-19 vaccines among pregnant and lactating women seeking services at a tertiary public hospital in Kampala, Uganda.

BACKGROUND: Globally, several vaccines, including COVID-19 vaccines, have been routinely recommended during pregnancy and lactation. However, data on COVID-19 vaccine acceptability and uptake among pregnant and lactating women are limited in Sub-Saharan Africa. While COVID-19 is no longer a public health emergency it is important to take stock of lessons learnt to prepare for future health threats including those that disproportionately affect pregnant/lactating women. We aimed to assess acceptability and uptake of COVID-19 vaccines among pregnant and lactating women in Uganda. METHODS: This
