Children and young people assessed by ambulance clinicians in a sparsely populated circumpolar region: non-conveyance and subsequent healthcare contacts in Northern Sweden.

Ambulance services in sparsely populated circumpolar regions operate over large distances with limited access to alternative care, potentially expanding their role beyond transport to assessment, decision-making, and healthcare navigation. In such settings, conveyance decisions involving children and young people (CYP) may be complex for ambulance clinicians. This retrospective study examined factors associated with non-conveyance and subsequent healthcare contacts among CYP assessed by ambulance clinicians in northern Sweden and compared characteristics of conveyed and non-conveyed patients.

The impact of breathing route on pulmonary and sensory responses to ozone exposure during exercise in adults.

Ozone (O3) is a photochemical air pollutant known to reduce lung function and induce respiratory symptoms. This study aimed to determine whether breathing route influences O3-induced lung function changes during exercise. Thirteen recreationally active participants (6 females, V̇O2peak = 45.8 ± 5.3 mL/kg/min), with no history of cardiovascular or pulmonary disease or exercise-induced bronchoconstriction, completed a double-blind crossover study with four experimental visits. Participants performed 30 min of low-intensity cycling (90% of power at the first ventilatory threshold) while breathing

Relationship between body weight and daytime sleepiness in patients with untreated OSA: The role of hypoventilation due to obesity.

PURPOSE: To evaluate the relationship between body weight and daytime sleepiness (DS) in patients with obstructive sleep apnoea (OSA). METHODS: We prospectively evaluated 353 untreated patients with OSA, categorised by body mass index (BMI) as normal weight, overweight, grade 1, grade 2, or grade 3 obesity. Patients with OSA and obesity hypoventilation syndrome (OHS) were analysed as a separate group. Demographic and anthropometric data, nocturnal polygraphy parameters, and DS (by the Epworth Sleepiness Scale-ESS) were collected. Excessive DS was defined as ESS > 10. RESULTS: Patients were cla

A proposed uniform 4-point Likert scoring framework for the Functional Airway Evaluation Screening Tool-15: A scoring standardization study for pediatric sleep-disordered breathing screening.

CONTEXT: The FAirEST-15 is a comprehensive 15-item clinical screening tool for pediatric sleep-disordered breathing (SDB). Although its domain structure has been previously established, the existing non-uniform scoring system-comprising both 3-point and 4-point items-limits score comparability and reduces statistical sensitivity. AIM: To standardize the FAirEST-15 by converting all clinical items into a uniform 4-point Likert scale based on graded severity. METHODS: A methodological refinement study was conducted using an expert consensus approach. Each of the 15 items was redefined to incor

Daytime sleepiness in patients with obstructive sleep apnea and associated comorbidities.

INTRODUCTION: Obstructive sleep apnea (OSA) is characterized by recurrent upper-airway collapse during sleep, leading to ineffective respiratory efforts, intermittent hypoxia, and sleep fragmentation. Patients with OSA often have comorbid conditions. Excessive daytime sleepiness (EDS), defined as an inability to remain awake during the day, is common among patients with OSA; however, its perception may vary with comorbidities that affect autonomic and neuroendocrine regulation. AREAS COVERED: We reviewed studies examining the prevalence and clinical impact of EDS in patients with OSA and its

Clinical characteristics and factors associated with in-hospital mortality in patients with Stenotrophomonas maltophilia infection: a single-center retrospective cohort study.

BACKGROUND: Stenotrophomonas maltophilia is an intrinsically multidrug-resistant opportunistic pathogen that causes clinically significant infections, particularly in immunocompromised and critically ill patients. The independent contributions of prior antimicrobial exposure and early active therapy to mortality remain uncertain. This study evaluated clinical characteristics, antimicrobial exposure patterns, empirical treatment appropriateness, and mortality-associated factors in patients with clinically confirmed S. maltophilia infection. METHODS: This single-center retrospective cohort stud

Facilitators and barriers to exercise adherence in patients with COPD: A qualitative study guided by the fear-avoidance model.

BackgroundExercise adherence is central to pulmonary rehabilitation in chronic obstructive pulmonary disease (COPD), yet sustained adherence remains difficult. Fear of exertional symptoms, particularly dyspnoea, may promote exercise avoidance. This study explored perceived facilitators and barriers to exercise adherence using the fear-avoidance model.MethodsFace-to-face semi-structured interviews were conducted with 18 patients with COPD recruited from respiratory inpatient wards. Data were analysed using qualitative content analysis, with the fear-avoidance model used as a sensitising framewo

Identifying clinical subtypes in acute dyspnoea patients admitted to the emergency department: a secondary model-based clustering of prospective cohorts.

INTRODUCTION: Acute dyspnoea is a frequent reason for emergency department (ED) presentation, with heterogeneous causes and prognosis often insufficiently captured by traditional diagnostic categories. We applied model-based clustering (i.e. latent class analysis [LCA]) to identify distinct subtypes of acute dyspnoea and assess their prognostic relevance. METHODS: We analysed two prospective ED acute dyspnoea adult cohorts (LEDA and BASEL V). LCA was performed in the LEDA derivation cohort using nine admission clinical/biological variables. A simplified decision tree was derived in the LEDA c

Roles of sex and SP-A genetic variants in modulating multiorgan injuries post viral infection.

BACKGROUND: Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection can lead to acute lung injury (ALI)/acute respiratory distress syndrome (ARDS), and multi-organ dysfunction (MOD). Human surfactant protein A (SP-A), a key component of innate immunity, exhibits genetic polymorphisms that may influence host responses to viral infection. Human patient studies have shown that biological sex has been associated with differences in COVID-19 severity. However, the combined effects of SP-A variants and sex on SARS-CoV-2-induced organ injury remain poorly understood. METHODS: We utili

Breathe, Eat, Talk: Three Essential Ingredients to Quality-of-Life Outcomes in Movement Disorders.

BACKGROUND: Breathing, eating, and talking (BET) impairments are common yet frequently underrecognized features of movement disorders. Deficits in respiration, swallowing, voice, and speech may emerge early in the course of a disease, adversely affecting safety, participation, and quality of life. Routine neurological examinations may not adequately capture their impact. OBJECTIVE: To characterize the clinical importance of BET dysfunction across several movement disorders by comparing neurological and speech-language pathology (SLP) approaches to assessment and recommendations for improved e

Who Really Benefits from CPAP? Disease Severity, Response Quality, and Long-Term Survival in Obstructive Sleep Apnea.

Obstructive sleep apnea (OSA) is associated with increased cardiovascular, respiratory, and all-cause mortality, yet the long-term survival impact of continuous positive airway pressure (CPAP) remains contested, and treatment is usually analysed as a binary exposure rather than by the quality of the response achieved. In a single-centre cohort of 4368 adults referred for polysomnography and followed for 8-20 years (prespecified subgroup with apnea-hypopnea index [AHI] ≥ 15, n = 2304), we applied cause-specific Cox and Fine-Gray competing-risks models, together with machine-learning classifiers

Immune-endothelial-coagulation crosstalk as a driver of multi-organ dysfunction in severe viral pneumonia.

Viral burden or pathogen identity alone cannot adequately explain the progression of severe viral pneumonia from a compartmentalized respiratory infection to acute respiratory distress syndrome, multi-organ failure, and death. Maladaptive immunity, endothelial damage, and coagulation dysregulation are all functionally integrated in a host-driven pathological mechanism that mediates disease escalation. Systemic microvascular damage and pulmonary inflammation are linked by immune-endothelial-coagulation interaction. This review investigates the ways in which immunothrombosis and microcirculatory

A Portable Neck-Surface Piezoelectric Sensor for Evaluating Subclinical Carotid Atherosclerosis via Snoring Vibratory Analysis: An Exploratory Dual-Modality Study.

Obstructive sleep apnea syndrome (OSAS) is heavily implicated in subclinical cardiovascular disease; however, traditional polysomnographic metrics fail to capture the localized mechanical trauma exerted on the carotid artery. To address this methodological gap, this study evaluated exploratory associations between frequency-domain snoring characteristics and right carotid artery alterations in 50 patients with OSAS. Snoring was quantified using a dual-modality bioelectronic approach: an ambient microphone captured airborne snoring sound energy (SSE), while a portable neck-surface piezoelectric

CD63hi macrophages define a conserved terminal inflammatory state driving lung immunopathology in severe COVID-19.

BACKGROUND: Severe COVID-19 is characterized by profound lung immune dysregulation, yet the pathogenic states and regulatory mechanisms of monocyte-derived macrophages (MDMs) remain poorly defined due to substantial phenotypic heterogeneity across studies. METHODOLOGY: We integrated multiple single-cell RNA sequencing datasets from COVID-19 lung tissues and bronchoalveolar lavage fluid (BALF) samples to systematically characterize macrophage heterogeneity and differentiation dynamics across anatomical compartments. A Wearable Respiratory Monitor for Home-Based Screening and Stratification of Obstructive Sleep Apnea: A Pilot Study in Participants with Intermediate-to-High STOP-Bang Scores.

Access to in-laboratory polysomnography (PSG) is restricted by prolonged waiting lists, and first-night effects can distort typical sleep architecture. This study evaluates the PneumoWave biosensor as a scalable, unobtrusive alternative for longitudinal single-channel home sleep apnea monitoring. Over three nights in an uncontrolled home environment, the biosensor's respiratory rate agreement was evaluated against smartwatch-derived respiratory rate estimates, whilst apnea/hypopnea detection was compared with concurrent pulse oximetry. PneumoWave and smartwatch devices demonstrated good correl
