Beta-caryophyllene reduces lung inflammation secondary to chronic kidney disease.

BACKGROUND: Chronic kidney disease (CKD) causes millions of deaths and is a known risk factor for the development of other diseases. CKD has been associated with lung injury characterized by neutrophil infiltration and edema. Despite evidence that the lungs are distant organs affected in CKD, there is no treatment to impair lung injury. Cannabinoid receptors type 2 (CB2) activation has been shown to reduce renal dysfunction and organ damage in sepsis. OBJECTIVE: Therefore, this study aimed to evaluate the effects of β-caryophyllene (βC), a CB2 agonist, on CKD-induced pulmonary alterations. M

Safety Analysis of IV Methadone vs. IV Hydromorphone in Post-Procedural Adults.

IV Methadone may be used post-procedurally to manage pain; however, it carries risks of respiratory depression and QTc prolongation. The purpose of this study is to compare safety profiles of multiple doses of intravenous (IV) methadone and IV hydromorphone in post-procedural patients. The primary outcome was incidence of respiratory event, defined as oxygen saturation (O2Sat) less than 90%, respiratory rate less than 8 breaths per minute (bpm), increased supplemental oxygen, or naloxone administration within 24 h of IV methadone or hydromorphone. Secondary outcomes included incidence of QTc p

Examining Obstructive Sleep Apnea Risk and Changes in Oxygen Saturation in an Outpatient Endoscopy Center.

As obesity becomes more prevalent in the United States, the incidence of undiagnosed or untreated obstructive sleep apnea (OSA) and the associated comorbidities present challenges when providing sedation to office-based gastroenterology patients. This study examined changes between pre-post procedural oxygen saturations among patients with moderate to high OSA risk. Patients (N = 316) in the preprocedural area were screened using the STOP-BANG questionnaire prior to receiving anesthesia. The patients' STOP-BANG score (SBS) and pre-post procedural oxygen saturations were obtained and documented

Significance of diffusing capacity of the lungs for carbon monoxide on chronic thromboembolic pulmonary hypertension.

Reduced diffusing capacity of the lungs for carbon monoxide (DLco) reflects microvasculopathy in chronic thromboembolic pulmonary hypertension, yet its clinical value is uncertain. In a Japanese nationwide registry (2018-2023) we studied 1270 patients: 486 formed an event cohort and 299 a treatment cohort who underwent pulmonary endarterectomy or balloon pulmonary angioplasty. Lower baseline DLco was indicative of smaller postprocedural improvements in mean pulmonary artery pressure, pulmonary vascular resistance and cardiac index (all p≤0.023) and a higher risk of clinical events (HR 0.971, p

Intralesional embolization for a hypervascular lesion of the nasal vestibule.

INTRODUCTION: Lobular capillary hemangioma is benign vascular lesion commonly encountered in the head and neck region. Although surgical excision remains the standard treatment, management may be challenging in postoperative settings or cosmetically sensitive areas such as the nasal vestibule. We present a case of a hypervascular nasal vestibular lesion with clinical and radiological features consistent with lobular capillary hemangioma, managed with intralesional foam embolization. CASE SUMMARY: A 40-year-old woman presented with unilateral nasal obstruction and left alar swelling. MRI showe

Severe rheumatic mitral stenosis and ARDS in pregnancy managed with percutaneous mitral commissurotomy and ECMO: a case report and literature review.

BACKGROUND: Rheumatic mitral stenosis (MS) remains a leading cause of cardiovascular complications in pregnancy, particularly in low-resource settings. Hemodynamic changes during pregnancy can precipitate decompensation in previously asymptomatic patients, increasing the risk of maternal and fetal morbidity. Early recognition and management, including percutaneous mitral commissurotomy (PMC) in selected cases, are crucial to optimizing outcomes. CASE PRESENTATION: We present the case of a 32-year-old pregnant woman (G4P3) at 22.6 weeks of gestation who developed severe respiratory distress and

Aspiration Pneumonia After Lumbar Spinal Intervention.

Low back pain is a prevalent complaint among patients, leading to the frequent use of various interventional procedures in pain clinics. While these interventions are generally regarded as safe, complications, particularly in elderly patients, can include serious conditions such as infectious spondylitis or spinal hematoma. We report a case of an elderly patient who developed aspiration pneumonia shortly after undergoing a lumbar spinal intervention. The initial clinical suspicion was infectious spondylitis due to the temporal association with the procedure. However, further evaluation reveale

Comparison of Incidence of Respiratory Depression Between Ciprofol versus Propofol for Procedural Sedation During First-Trimester Surgical Abortion: A Randomized Controlled Trial.

BACKGROUND: Respiratory depression remains a major safety concern during propofol-based sedation for surgical abortion. Ciprofol is a novel intravenous sedative-hypnotic agent with pharmacological properties similar to propofol and a potentially improved safety profile. This randomized controlled trial aimed to compare the incidence of respiratory depression of ciprofol versus propofol, both combined with alfentanil, for procedural sedation in patients undergoing surgical abortion. METHODS: In this single-center, assessor-blinded, randomized controlled trial, 234 patients scheduled for surgic

Histotripsy for neuroendocrine liver metastases: Early single-institution outcomes and safety.

BACKGROUND: Histotripsy is a novel, noninvasive, nonionizing focused ultrasound technology that mechanically destroys tissue via cavitation. Neuroendocrine tumors, which frequently metastasize to the liver and follow an indolent course, may be particularly well-suited for histotripsy, and we report an early experience. METHODS: This retrospective analysis summarizes the first 32 patients consecutively treated with histotripsy in a neuroendocrine practice. Intraoperative targeting and cavitation were performed with the EDISON system. Clinical outcomes, postprocedural imaging, and adverse event

Risk assessment and prevention of postoperative and postprocedural respiratory failure: recommendations from an international expert panel.

Respiratory failure after surgery and other major invasive procedures, defined as the unanticipated need for invasive or noninvasive ventilation, is a life-threatening complication affecting ∼10 million patients annually. At present, there are no guidelines for risk assessment and prevention of postprocedural respiratory failure. We developed expert-informed practice recommendations for the prediction and prevention of postoperative and postprocedural respiratory failure. These recommendations focused on modifiable pre- and post-procedural risk factors and implementation feasibility, using a s

Understanding exacerbation risk in BLVR: A logistic regression approach to complication prediction.

BackgroundChronic obstructive pulmonary disease (COPD) with emphysema is associated with persistent airflow limitation and frequent exacerbations. Bronchoscopic lung volume reduction (BLVR) with endobronchial valves (EBVs) improves lung function and quality of life but carries a risk of postprocedural complications, including acute exacerbations and pneumonia. Predictors of these adverse events remain incompletely defined.PurposeTo identify clinical and inflammatory factors associated with postprocedural exacerbations in patients undergoing BLVR with EBVs, aiming to support individualized risk
