The incidence, risk factors, and outcomes of acute kidney injury after minor lower-limb amputations.

BACKGROUND AND AIMS: Minor lower-limb amputations are limb and potentially life-saving procedures. However, they are associated with serious adverse events, including acute kidney injury (AKI). MATERIALS AND METHODS: We conducted a single-center retrospective study to determine the incidence of AKI after these procedures, identify risk factors, and assess impact on patient survival. RESULTS: We included 201 patients. AKI occurred in 18.9% using AKIN criteria, and 24.9% using KDIGO criteria. Only 1 patient required temporary dialysis. Patients with AKI were older (73.0 ± 10.4 vs. 68.5 ± 11.8

Diagnostic yield and safety of Dual-functIon Semi-rigid thoraCoscOpy Versus rigid thoracoscopy for the diagnosis of plEuRal disease (DISCOVER-2): protocol for a randomised controlled trial.

INTRODUCTION: Rigid thoracoscopy is an established diagnostic instrument for pleural diseases; however, it is limited by challenges related to manoeuvrability, patient comfort and operator learning curve. A novel dual-function semi-rigid thoracoscope with associated semi-rigid forceps has been engineered to integrate the diagnostic efficacy of rigid biopsy forceps with the ergonomic advantages of a semi-rigid system. The Dual-functIon Semi-rigid thoraCoscOpy Versus rigid thoracoscopy for the diagnosis of plEuRal disease trial was designed to evaluate whether this device achieves a diagnostic y

Persistent critical illness in acute hypoxic respiratory failure: A retrospective cohort study.

INTRODUCTION: Persistent critical illness (PerCI) describes patients with prolonged intensive care unit (ICU) dependence after their acute phase of illness and affects 5-20% of the general ICU population. PerCI has been studied in the general ICU cohort, but prevalence and outcomes remain unclear among mechanically ventilated patients with acute hypoxic respiratory failure (AHRF), a high-risk cohort that is more prone to ICU-associated complications. The objective of this study is to quantify PerCI prevalence, identify risk factors for PerCI development, and describe one-year mortality in this

Platypnea-Orthodeoxia Syndrome Caused by a Patent Foramen Ovale and Interatrial Septal Aneurysm: A Case Report.

Platypnea-orthodeoxia syndrome (POS) is a rare condition characterized by positional hypoxemia, often due to right-to-left shunting across a patent foramen ovale (PFO) despite normal right-sided pressures. Early recognition is essential as symptoms may be subtle and imaging is often unrevealing. A 78-year-old female patient with a known PFO presented with one week of lower abdominal pain and mild exertional dyspnea without chest pain. She was profoundly hypoxic to 76% on room air, with improvement when supine and worsening when upright. Initial labs were unremarkable, and arterial blood gas re

Amputation and reamputation for dry gangrene of both lower extremities in with chronic kidney disease patients with calciphylaxis accompanied by multidrug-resistant bacterial infections: A case report and literature analysis.

RATIONALE: Nephritis can further develop into chronic kidney disease (CKD). CKD with calciphylaxis is mainly manifested as necrotic ulcers of terminal tissues or even gangrene in the skin and distal limbs. Patients with CKD have a high risk of developing multidrug-resistant bacterial infections. PATIENT CONCERNS: Our patient developed dry gangrene of both feet for 1 month, accompanied by complications, such as anemia, hypoproteinemia, and pulmonary infection. The patient had a history of CKD for 30 years, calciphylaxis for 5 years, and multidrug-resistant bacterial infection for 1 month. DIA

Kidney and blood pressure sequelae after pediatric extracorporeal membrane oxygenation: a secondary analysis of the Kidney Injury During Membrane Oxygenation pediatric ECMO cohort.

BACKGROUND: Acute kidney injury (AKI) is common in children treated with extracorporeal membrane oxygenation (ECMO) and is associated with increased morbidity and mortality. AKI may also increase the risk of long-term chronic kidney disease (CKD) and persistent kidney dysfunction. Hospital discharge kidney dysfunction (HDKD) or the need for antihypertensive medication may further be associated with long-term CKD risk. This study aims to determine, in children treated with ECMO, the prevalence of HDKD and the use of antihypertensive medication at hospital discharge, as well as their association

[Rapid Deterioration of Preeclampsia and Suspected Monogenic Hypertension - A Case Report].

Despite all the knowledge reached regarding preeclampsia, including preventive strategies, predictive algorithms, diagnosis and treatment, we still encounter cases that rapidly deteriorate while jeopardizing the patient and her fetus. It is known that hypertension constitutes a considerable portion of pregnancy complications and is associated with significant maternal and perinatal morbidity and mortality. A continuous global increase in its prevalence is noted, mainly due to lifestyle changes, a tendency toward obesity, and delayed childbearing to a more advanced age. We present a case of rap

Follow-up of neonates who received ECMO: how are they doing?

UNLABELLED: Extracorporeal membrane oxygenation (ECMO) has been used since 1975 to provide temporary cardiorespiratory support in neonates, children, and adults with severe cardiopulmonary conditions unresponsive to conventional therapy. Initially focused on neonatal respiratory failure, ECMO indications have expanded to include older children with cardiac failure or post-cardiopulmonary arrest. Epidemiological data indicate a decline in neonatal respiratory ECMO cases over the recent decades, largely due to advances in conventional therapies. Conversely, neonatal cardiac ECMO has shown a more

Short-term corticosteroid therapy in aspiration pneumonitis complicated by acute respiratory distress syndrome: A case report.

RATIONALE: Aspiration of gastric contents (Mendelson syndrome) can cause chemical pneumonitis and quickly progress to acute respiratory distress syndrome (ARDS). Evidence for corticosteroids in this setting is limited, and guidelines are unclear. We describe a postpartum case of aspiration pneumonitis evolving to ARDS that improved after a short course of methylprednisolone. PATIENT CONCERNS: A 27-year-old woman developed severe dyspnea, chest tightness, and hypoxemia after a cesarean section performed under general anesthesia. DIAGNOSES: On arrival at Cho Ray Hospital, she was tachypneic wi

Combined treatment of deep and superficial venous reflux accelerates ulcer healing and improves symptoms: A real-world retrospective study.

BACKGROUND: The optimal surgical strategy for treating chronic venous disease with concomitant deep and superficial reflux but without iliac/proximal obstruction remains unclear. This study aimed to compare clinical symptoms, health-related quality-of-life, and ulcer outcomes between superficial venous surgery alone and combined deep-superficial surgery (performed concurrently or in stages). METHODS: In this single-center retrospective cohort, 229 patients were grouped by treatment and received superficial-only (n = 138), concurrent deep + superficial (n = 49), or staged deep + superficial (n

Severe Occupational Hypersensitivity Pneumonitis: A Case Series of Four Patients Requiring Lung Transplantation.

Hypersensitivity pneumonitis (HP) is an immune-mediated interstitial lung disease triggered by repeated inhalation of organic or chemical antigens. Occupational exposures account for approximately 19% of all cases. Early diagnosis, identification of the responsible antigen(s), and immediate avoidance of exposure are crucial to prevent irreversible pulmonary fibrosis. However, HP often remains unrecognized or is misclassified as another respiratory disorder such as asthma, chronic obstructive pulmonary disease (COPD), or idiopathic pulmonary fibrosis. As a result, the causal link between sympto

Skin laceration during closed reduction of distal radius fracture in the elderly: an underappreciated complication.

BACKGROUND: Distal radius fractures are common osteoporotic injuries in elderly patients. Dermatoporosis, an age-related cutaneous insufficiency often associated with corticosteroid use and malnutrition, is frequently overlooked in these cases despite its clinical relevance during fracture management. MATERIALS AND METHODS: A 72-year-old underweight woman with chronic obstructive pulmonary disease and long-term corticosteroid use presented with an extra-articular Colles-type distal radius fracture after a trivial fall. Closed reduction under hematoma block was attempted. During disimpaction,

Late-onset sequelae after right middle and lower lobectomy: a case report.

BACKGROUND: Surgical resection remains a cornerstone of curative treatment in lung cancer, but delayed complications are under-recognised. CASE PRESENTATION: We describe a 67-year-old East Asian man who developed an unusual sequence of symptoms months after right middle and lower lobectomy: a posture-dependent triad of palpitations, throat tightness, and nasal congestion; paradoxical weight loss despite hyperphagia; and later neuropsychiatric disturbance associated with chronic respiratory insufficiency. Investigations excluded tumour recurrence and metabolic disorders. CONCLUSIONS: These ob
