INTRODUCTION: Gastrointestinal metastasis from lung cancer is uncommon and rarely symptomatic, and small-bowel metastasis causing perforation is rare with a poor prognosis. Obscure gastrointestinal bleeding as the initial manifestation, preceding perforation, is seldom documented. We report such a case requiring emergency surgery.
CASE PRESENTATION: A man in his 70s with a heavy smoking history was incidentally found, during an admission for bowel obstruction, to have left-lung lesions with hilar and subcarinal lymphadenopathy on CT, without abdominal disease. Bronchoscopic biopsy showed aden

Paravalvular leak (PVL) occurs in up to 17% of prosthetic valve replacements, mainly in the mitral position. Surgery is the gold standard but carries high morbidity in patients with a history of previous sternotomies. Transcatheter closure of PVL offers a minimally invasive alternative with a very high success rate. Device migration, while uncommon, can be catastrophic when impinging on mechanical prosthetic valve performance. We report the case of a 37-year-old woman with prior mechanical mitral and aortic valve replacements, complicated by recurrent heart failure due to a mitral PVL resultin

Adrenocortical carcinoma (ACC) represents a very rare (an incidence of 1-2 cases per million) and aggressive malignancy (an overall 5-year survival of 15%). Less than 15-20% of cases involve a single metastatic disease. In this presentation, we documented a most exceptional survival span in a case of ACC, between the female patient's age of 50 and the current age of 66. The subject underwent an open right adrenalectomy three times (at first diagnosis, after recurrence 2 years later, and, most recently, 15 years after the initial diagnosis amid a multi-visceral resection). She initially display

Acute esophageal necrosis (AEN) is a rare and life-threatening condition most commonly associated with systemic hypoperfusion and critical illness. Aortic-related AEN is exceptionally uncommon and may result from direct mediastinal compression and vascular compromise rather than generalized hemodynamic insufficiency. We report the case of a 77-year-old man presenting with a Stanford type B aortic dissection associated with aneurysmal degeneration complicated by rupture. The patient presented with progressive dysphagia, back pain, and hemoptysis. Computed tomography angiography demonstrated a l

Bronchoscopic lung volume reduction (BLVR) is an established treatment for patients with severe chronic obstructive pulmonary disease (COPD), characterized as emphysema and hyperinflation, but current methods of patient selection and lobe identification are limited due to invasiveness, lack of specificity, and poor regional ventilation and perfusion assessments. This study aimed to evaluate lobar changes in ventilation, perfusion, and volume before and after endobronchial valve (EBV) placement using SPECT/CT imaging in patients with severe emphysema, and to assess their association with clinic

BACKGROUND: Long-term durability of degenerative mitral regurgitation (DMR) reduction after transcatheter edge-to-edge repair (TEER) remains limited, and reliable anatomical predictors are still lacking.
METHODS: This study prospectively enrolled patients with symptomatic DMR undergoing TEER across 27 centers in China. All pre-procedural clinical and echocardiographic parameters were collected and analyzed. The primary outcome was MR ≤ 1+ at 3-year follow-up. Logistic regression and receiver operating characteristic (ROC) analyses were performed to identify predictors and optimal cutoff value

BACKGROUND: The increasing indications of transcatheter edge-to-edge repair (TEER) have led to its use in more diverse populations, including patients with complex comorbidities where standard transfemoral access may not be feasible. In these cases, utilizing the right internal jugular vein (IJV) access has been reported as a safe and practical alternative route. Herein, we report the first case of mitral TEER via left IJV access, using PASCAL system.
CASE PRESENTATION: A 50-year-old woman with long-standing chronic kidney disease and two previously failed renal transplants suffered repeated

BACKGROUND: Transcatheter aortic valve replacement (TAVR) has become the standard of care for patients with severe aortic stenosis (AS) who are at elevated surgical risk. Although generally well tolerated, TAVR can be associated with uncommon but potentially serious complications, including acute mitral regurgitation (MR) following valve deployment.
CASE SUMMARY: We present the case of a 94-year-old woman with symptomatic severe AS who developed acute, new-onset severe MR immediately after TAVR. Preprocedural imaging had shown no significant MR. The patient experienced acute hemodynamic compr

BACKGROUND: The coexistence of severe mitral regurgitation (MR) and mild-to-moderate aortic stenosis (AS) presents diagnostic and therapeutic challenges. Limited data exists on outcomes following mitral transcatheter edge-to-edge repair (M-TEER) therapy in this patient population.
AIMS: This study is aimed to evaluate clinical outcomes following M-TEER in patients with mild-to-modearte AS compared with those without aortic stenosis.
METHODS: A single-center retrospective study was conducted on 238 patients who underwent M-TEER therapy between January 2014 and December 2024. Patients with sev

Acute severe mitral regurgitation (MR) can cause cardiogenic shock and fulminant pulmonary edema. Veno-arterial extracorporeal membrane oxygenation (VA-ECMO) may worsen pulmonary congestion by increasing left ventricular afterload. We report a 61-year-old man with acute severe MR and extreme respiratory failure (FiO₂ 1.0: pH 7.144, PaO₂ 48 mmHg, PaCO₂ 101 mmHg) who underwent emergency mitral valve replacement. Because severe pulmonary edema and post-cardiotomy shock compromised separation from cardiopulmonary bypass, an Impella 5.5 was inserted via a direct aortic graft, followed by veno-venou

An adolescent boy with a history of chronic rheumatic heart disease (RHD) and severe mitral regurgitation presented with progressive exertional dyspnoea and palpitations. Echocardiography revealed preserved left ventricular function preoperatively, and the patient underwent mechanical mitral valve replacement (MVR). Postoperative management included anticoagulation and RHD prophylaxis. 4 months later, he developed acute heart failure (New York Heart Association class IV) secondary to atrial fibrillation (AF) with rapid ventricular rate despite a normally functioning prosthetic mitral valve. Adults with repaired tetralogy of Fallot (rTOF) constitute a rapidly growing adult congenital heart disease population with excellent early survival but substantial late morbidity. Chronic pulmonary regurgitation and/or residual right ventricular outflow tract obstruction drive progressive right ventricular dilatation, dysfunction, dyssynchrony, and myocardial fibrosis, predisposing to heart failure and ventricular arrhythmias. This review summarizes contemporary concepts in the surveillance and management of adults with rTOF, focusing on multimodality imaging, pulmonary valve replacement (PVR

BACKGROUND Differentiating traumatic tricuspid regurgitation (TR) from infective endocarditis (IE) is clinically crucial in patients presenting with abnormal tricuspid valve findings after blunt chest trauma. Particularly, papillary muscle rupture caused by trauma can produce mobile valvular structures that closely resemble vegetations, making accurate diagnosis challenging in the acute setting. CASE REPORT A man in his 60s with pulmonary sarcoidosis was admitted after a 5-m fall, sustaining multiple fractures. Preoperative transthoracic echocardiography revealed a 7-mm mobile structure on the

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

OBJECTIVE: Childbirth is a natural process, and according to the World Health Organization, oral intake is recommended for patients at usual risk. However, due to rare cases of pulmonary aspiration (known as Mendelson's syndrome) during general anesthesia and the limited evidence supporting its benefits, this practice remains controversial. This systematic review summarizes evidence on interventions that recommend oral intake during labor compared to fasting, focusing on labor duration and perinatal complications.

This review aims to provide an extensive overview of the literature on the clinical pharmacokinetics of mycophenolate in solid organ transplantation and a briefer summary of current pharmacodynamic information. Strategies are suggested for further optimisation of mycophenolate therapy and areas where additional research is warranted are highlighted. Mycophenolate has gained widespread acceptance as the antimetabolite immunosuppressant of choice in organ transplant regimens. Mycophenolic acid (MPA) is the active drug moiety. Currently, two mycophenolate compounds are available, mycophenolate mo
