Protocol for robotic-assisted bronchoscopy versus electromagnetic navigation bronchoscopy for the diagnosis of peripheral pulmonary nodules: a randomized trial (ARTICULAtE study).

BACKGROUND: Electromagnetic navigation bronchoscopy (ENB) and shape-sensing robotic-assisted bronchoscopy (ssRAB) are the two dominant bronchoscopic technologies for the diagnosis of peripheral pulmonary nodules (PPNs). However, data directly comparing ENB and ssRAB are limited. We designed a randomized controlled trial to compare the diagnostic performance of these two technologies. The primary objective is to evaluate if ssRAB is superior to ENB for the diagnosis of PPNs. METHODS: It is a multicenter, open-label, superiority randomized controlled trial. Patients with PPNs suspicious for lun

Risk assessment of spinal surgery in chronic kidney disease and dialysis patients: a systematic review and meta-analysis of over 5 million cases.

The purpose of this study was to conduct a systematic review and meta-analysis of the outcomes of spinal surgery in patients with chronic kidney disease (CKD), including those undergoing dialysis. A comprehensive literature search was conducted following PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-analyses) guidelines. Statistical analyses were performed using Review Manager software, utilizing mean differences (MD), odds ratios (OR), and random effects models to account for heterogeneity. Heterogeneity was assessed using the I2 statistic. The primary outcomes were operat

Obstructive sleep apnea in adult ambulatory anesthesia: navigating guidelines and evidence for safe home discharge.

PURPOSE: Ambulatory anesthesia for surgical and interventional procedures is increasingly common. In this Continuing Professional Development module, we present a postoperative risk prediction and care model framework for patients with obstructive sleep apnea (OSA) undergoing ambulatory anesthesia. PRINCIPAL FINDINGS: Ensuring that patients with OSA are safe to be discharged home on the same day as their procedure requires careful consideration of multiple relevant risk factors. Preoperative patient assessment for the prediction of postprocedural complications is based on OSA status and sever

Implementation of a Hybrid Cardiac Rehabilitation and Symptom Scoring System in Patients with Inappropriate or Postural Sinus Tachycardia Referred for Sinus Node Sparing Hybrid Ablation.

Background/Objectives: Patients with inappropriate sinus tachycardia (IST) and postural orthostatic tachycardia syndrome (POTS) exhibit complex clinical profiles due to autonomic dysfunction. While sinus node sparing (SNS) hybrid ablation is emerging as a promising therapy, there are no established guidelines worldwide for post-procedure patient management and care is mainly based on telemonitoring. In contrast, our hybrid cardiac rehabilitation (HCR) program integrates inpatient care and home-based telerehabilitation. We aim to evaluate the implementation of the HCR program, patient acceptanc

Impact of renal function on the efficacy of low-voltage area ablation after pulmonary vein isolation: a sub-analysis of the SUPPRESS-AF trial.

AIMS: The SUPPRESS-AF trial showed that pulmonary vein isolation (PVI) plus low-voltage area (LVA) ablation may reduce atrial fibrillation (AF) recurrence in some subgroups. Renal dysfunction is a cause of LVAs due to atrial cardiomyopathy and is also a risk factor for AF recurrence after catheter ablation. The aim of this study was to investigate the efficacy of LVA ablation after PVI stratified by renal function. METHODS AND RESULTS: This study was a sub-analysis of the SUPPRESS-AF trial, a multicentre, prospective, randomized, open-label trial. A total of 341 consecutive patients who under

SARS-CoV-2 infection dynamics in a MHCI-mismatched lung transplant recipient.

A 48-year-old patient underwent lung transplantation because of severe COVID-19, which aggravated his underlying interstitial lung disease, despite the presence of detectable SARS-CoV-2. Subsequently, the graft is re-infected early in the post-procedural phase, leading to viral persistence for more than five months. By analyzing viral evolution and effector immune response within the transplanted organ, we observe three main findings. First, virus evolution differs in the transplanted organ compared to that in the upper respiratory tract and is affected by monoclonal SARS-CoV-2-specific antibo

Anesthetic Management of a Patient With Symptomatic May-Thurner Syndrome: A Case Report.

May-Thurner syndrome (MTS) is characterized by the compression of the left common iliac vein by the overlying right common iliac artery, which can lead to venous insufficiency, obstruction, and an increased risk of iliofemoral deep vein thrombosis (DVT) and pulmonary embolism (PE). We report the perioperative anesthetic management of a 38-year-old female with symptomatic MTS who underwent a total laparoscopic hysterectomy, bilateral salpingectomy, and lysis of adhesions for abnormal uterine bleeding. The patient had a history of persistent left lower extremity symptoms despite prior left commo

Trends in MitraClip Placements and Predictors of 90-Day Heart Failure Rehospitalization: A Nationwide Analysis.

Background: Chronic mitral regurgitation (MR) is categorized into primary and secondary MR (SMR). While primary MR arises from structural abnormalities of the mitral valve apparatus, SMR is a consequence of cardiac remodeling, typically due to heart failure or atrial fibrillation. Management strategies differ significantly, with primary MR requiring direct valvular intervention and SMR necessitating a comprehensive approach incorporating guideline-directed medical therapy (GDMT), revascularization, and resynchronization strategies. The MitraClip, a transcatheter edge-to-edge repair (TEER) devi
