Doppler assessment of changes in right-sided cardiac hemodynamics after pulmonary thromboendarterectomy.

It is not known whether Doppler echocardiography can accurately follow changes in right-sided cardiac hemodynamics after a therapeutic intervention in patients with pulmonary artery (PA) hypertension. Therefore, Doppler measurements of the maximal velocity of the tricuspid regurgitant jet and the acceleration time of the PA velocity profile were obtained in 28 patients before and after pulmonary thromboendarterectomy for chronic thromboembolic PA hypertension. Doppler values were compared with hemodynamic variables obtained at cardiac catheterization. Postoperatively, decreases in mean PA pres

Use of thymic grafts or thymic factors to augment immunologic recovery after bone marrow transplantation: brief report with 2 to 12 years' follow-up.

Thymus tissue implants, thymic epithelial cells obtained from third party donors sharing one HLA-A and -B locus with the recipient, or the thymic hormones thymosin fraction 5 and thymopentin were given to recipients of HLA-identical sibling bone marrow to prevent chronic graft-versus-host disease (GVHD) and accelerate immunologic reconstitution. The clinical courses of 17 patients receiving thymus tissue and 18 patients receiving thymic hormones were reported initially 5 years ago and showed no difference in the incidence of chronic GVHD or immunologic recovery from those of concurrent or hist

[Medical-work capacity examination of patients with chronic empyema of the pleura after pulmonectomy].

Treatment and analysis of immediate results were described in 269 patients with chronic empyema of the pleura as well as their long-term results in 207 of them. Under consideration were main criteria of the examination of labor capacity in such patients. It was shown that the establishment of the phase of "compensation at the limit" (IIa degree of pulmonary insufficiency) and subcompensation (IIb degree of pulmonary insufficiency) allowed more exact evaluation of the degree of compensatory processes of respiratory organs in patients after operation and the degree of loss of their working capac

Etiology of pure tricuspid regurgitation.

Of multiple etiologies for pure tricuspid regurgitation, the causes may be divided into those associated with anatomically abnormal and anatomically normal tricuspid valves. Conditions associated with anatomically abnormal valves include rheumatic, floppy, Ebstein's anomaly, carcinoid, infective endocarditis, papillary muscle dysfunction, and other rarer causes such as radiation, hypereosinophilic syndrome, and endomyocardial fibrosis. Conditions associated with anatomically normal tricuspid valves include multiple causes of cor pulmonale (chronic obstructive pulmonary disease, primary pulmona

Use of profound hypothermia and circulatory arrest in complex intra-cardiac repair in adults.

Profound hypothermia and circulatory arrest is a well worked out technique for total repair of congenital defects in infants. Recently, it has been popularized for the repair of aneurysms of the transverse aortic arch. We have applied this technique of profound hypothermia and circulatory arrest in three other adult patients in whom conventional techniques would not allow safe and adequate complete repair of acquired intra-cardiac defects. The first patient, a 76-year-old female, had a large chronic ascending aortic aneurysm involving the aortic valve, as well as the innominate and left common

Diagnostic and surgical considerations in the treatment of thoracoabdominal and suprarenal aortic aneurysms.

Thoracoabdominal and suprarenal aortic aneurysms may occur more frequently than originally thought. A review of our operative experience with aortic aneurysm during a 4 1/2 year period disclosed 17 (8 percent) aneurysms of the thoracoabdominal and suprarenal aorta. Acute rupture in five patients (29 percent) emphasizes the possible need for urgent treatment precluding transfer to a major university referral center. Nine patients (53 percent) had previously been operated on for aortic aneurysm, illustrating the systemic nature of aortic aneurysm disease and the need for lifelong follow-up. All

Bullectomy for giant bullae in emphysema.

The results of changes in dyspnea and pulmonary function are reported in 27 emphysematous patients followed up for about 10 years after removal of giant bullae, which occupied at least 50% of a hemithorax. In 10 patients bilateral bullectomy was done. The spirographic improvement depends on the type of bulla. Resection of bullae at open communication with the bronchial tree resulted predominantly in improvement of forced expiratory volume as a percentage of vital capacity, whereas after closed bullae were resected, the increase in vital capacity was most apparent. Dyspnea lessened in all patie

Mitral regurgitation: relationship of noninvasive descriptors of right and left ventricular performance to clinical and hemodynamic findings and to prognosis in medically and surgically treated patients.

To determine objective predictors of survival, 53 patients with chronic, hemodynamically severe mitral regurgitation underwent rest and exercise radionuclide cineangiography, echocardiography, treadmill exercise testing, and ambulatory electrocardiographic monitoring before prospective (average 30 month) follow-up. At entry, symptom status correlated best with radionuclide-based right ventricular ejection fraction (RVEF) and left atrial size, while treadmill exercise tolerance correlated best with RVEF during exercise (r = .48, p less than .005). Correspondingly, in 23 patients who underwent c

Pulmonary calcinosis after renal transplantation in pediatric patients.

Pulmonary calcinosis is a recognized complication of renal failure. The resulting pulmonary compromise may be severe or even fatal. The potential contribution of hypercalcemia, hyperphosphatemia, and increased calcium-phosphorus product to the development of pulmonary calcinosis has been controversial. We describe four patients (ages 2 1/4 to 18 years) who had severe pulmonary calcinosis and respiratory failure within three to five days after renal transplantation. Initial clinical and roentgenographic findings suggested noncardiogenic pulmonary edema. Marked pulmonary hypertension was present

Juxtarenal infrarenal abdominal aortic aneurysm. Special diagnostic and therapeutic considerations.

Juxtarenal infrarenal abdominal aortic aneurysms are defined as those aneurysms that involve the infrarenal abdominal aorta adjacent to or including the lower margin of renal artery origins. The misinterpretation of findings at exploratory operation or special studies may suggest renal artery involvement and result in abandonment of operation and/or referral to distant centers, thus delaying treatment. This report is concerned with 101 patients with a median age of 68 who had such aneurysms, all referred with a diagnosis of renal or visceral arterial involvement either after exploratory operat

Femoral and iliofemoral thrombectomy to prevent chronic venous insufficiency. Follow-up of 18 patients.

Eighteen patients with femoral and iliofemoral venous thrombosis were treated surgically. Five of the patients had a moderate degree of venous congestion and were classified as having phlegmasia alba dolens and 13 patients had phlegmasia cerulea dolens. The mean age was 39 years, range 18-60 years; 6 were men and 12 women. Thrombectomy was performed with a Fogarty venous thrombectomy catheter. In all cases the thrombosis was verified by phlebography. Pre- and postoperative phlebography was used in all cases to show the patency of the femoral and iliofemoral segment. There was no operative pulm

[Determinants of long-term prognosis following prosthetic mitral valve replacement].

UNLABELLED: Data of 235 patients with chronic mitral valve disease (NYHA class III or IV) were analyzed retrospectively. Mitral valve replacement was performed in all patients between 1974 and 1983, 46 patients underwent additional tricuspid valve repair during the same operation. Preoperatively, right and left heart catheterization as well as coronary angiography were carried out in all patients. Operative mortality was 8%. For all patients 5 year survival rate was 76% and 10 year survival rate was 72%. Patients with pure mitral insufficiency had a poorer long-term prognosis than patients wit

Bilateral congenital eventration of the diaphragms: successful surgical management.

Bilateral congenital eventration of the diaphragms is a relatively rare occurrence, associated with an almost uniform mortality. We recently have had experience with three patients who underwent successful treatment of this anomaly. Each patient presented as a newborn with severe respiratory insufficiency. One had had a sibling who died shortly after birth with a congenital diaphragmatic hernia. One infant had associated respiratory distress syndrome. PA and lateral chest roentgenograms demonstrated the anomaly in each patient. In two, the use of ultrasound clearly demonstrated the presence of

Surgical management of chronic pulmonary embolism.

Recurrent pulmonary emboli ultimately may produce respiratory insufficiency, severe hypoxemia, and progressive pulmonary hypertension. In many patients this syndrome is silent in its initial phases, and when thrombophlebitis is present it is often unresponsive to anticoagulant therapy. Unless pulmonary embolectomy is undertaken, most of these patients characteristically succumb with severe respiratory insufficiency. Twenty-five patients with this syndrome have been evaluated at the Duke University Medical Center, and 14 were selected for elective pulmonary embolectomy for relief of severe and

Bronchiolitis obliterans after bone marrow transplantation.

Bronchiolitis obliterans occurred in the setting of chronic graft-versus-host disease 1 year after allogeneic bone marrow transplantation for chronic myelogenous leukemia. The severe obstructive pulmonary disease followed an episode of interstitial pneumonitis. The etiology and possible relationship to graft-versus-host disease of this rare pulmonary lesion following bone marrow transplantation are discussed.

The effect of O-(beta-hydroxyethyl)-rutosides on central haemodynamics during and after aortocoronary bypass surgery.

Clinical use of O-(beta-hydroxyethyl)-rutosides (HR) has been mainly in chronic venous insufficiency, but to some extent also in arterial insufficiency. We investigated the effect of HR on the central circulation in a group of patients undergoing aortocoronary bypass surgery. Measurements were made during operation and on the first postoperative day, both before and after intravenous injection of 15 ml 10% HR (1.5 g). None of the obtained or calculated values for cardiac output, central blood pressures, coronary blood flow, heart rate, stroke volume and systemic, pulmonary and regional myocard

Mitral valve replacement in the elderly.

The results of elective mitral valve replacement between January 1978 and June 1982 in 35 patients aged over 70 are reported. The early mortality was 22,8% and late mortality 17,1%, after a mean follow-up of 25,1 months. Patient-related risk factors included chronic mitral stenosis, often after a previous closed mitral valvotomy, increased cardiothoracic ratio, pulmonary arterial hypertension, functional tricuspid incompetence and a raised left ventricular end-diastolic pressure.

Mitral valve regurgitation.

The mitral apparatus is a complex structure composed of several components, each of which can be affected by a variety of diseases, resulting in mitral regurgitation. The physiologic consequences of mitral regurgitation include reduced forward stroke volume; increased left atrial volume and pressure; and reduced resistance to left ventricular ejection. The latter explains why indices of systolic left ventricular function (ejection fraction) are often increased early in the course of mitral regurgitation. With the insidious development of mitral regurgitation, the left atrium dilates to accommo

[Role of surgery in the treatment of chronic aortic insufficiency].

The long-term prognostic factors which determine the indications for surgery are studied on the basis of a series of 198 cases operated for chronic aortic incompetence, in terms of their influence on the actuarial long-term mortality rate due to heart disease and irreversible myocardial dysfunction. The mortality in the first month after surgery is 5 per cent. The factors which significantly modify the long-term outcome of operated patients are clinical (cardiomegaly, degree of preoperative heart failure, ventricular extrasystole, functional grade) and haemodynamic (mean pulmonary artery press
