Disseminated Ganciclovir-Resistant Cytomegalovirus in a Nontransplant Pediatric ALL Patient: A Case Report.

BACKGROUND: Cytomegalovirus (CMV) infection is an underrecognized complication in children with acute lymphoblastic leukemia (ALL). While CMV disease is well documented in transplant recipients, its clinical course in nontransplant settings remains poorly defined. Disseminated ganciclovir-resistant CMV infection is a rare complication in nontransplant children with ALL, and its optimal management remains uncertain. OBSERVATIONS: We describe an 8-year-old patient with T-ALL receiving maintenance chemotherapy who developed high levels of CMV viremia without initial end-organ disease and subsequ

Isolation and molecular characterization of West Nile Virus with evidence of vertical transmission in the Coastal region, Kenya.

West Nile virus (WNV) is a mosquito-borne flavivirus of global public health importance, maintained in an enzootic cycle between birds and mosquitoes, with humans and other mammals as incidental hosts. Although WNV has been documented in Kenya, surveillance remains inconsistent, geographically fragmented, and largely dependent on serological methods. This limits understanding of circulating lineages, vector diversity, and transmission dynamics, particularly in coastal regions. This study aimed to address these gaps through systematic arboviral surveillance of mosquito populations across four e

Linker-mediated single-transcript mRNA vaccine expressing virus-like particles of enterovirus D68.

Enterovirus D68 (EV-D68) causes pediatric respiratory illness and is linked to acute flaccid myelitis (AFM), posing a global health threat. We have previously reported that an mRNA vaccine expressing a virus-like particle (VLP) of EV-D68, which co-administers two mRNA-lipid nanoparticles (LNPs) encoding the viral structural protein P1 and viral protease 3CD, conferred protection against infection. Nonetheless, the requirement for two mRNA-LNPs presents challenges in manufacturing and quality control. Here, we investigated the potential of single mRNA constructs encoding both P1 and 3CD, connec

Targeting white matter astrocytes with a Gfap-disruptive Cas9 nuclease benefits a murine model of Alexander disease.

Astrocytes are intriguing therapeutic targets due to their crucial role in initiating and promoting demyelination in the central nervous system. By efficient on-target editing, we can permanently disrupt detrimental genes and restore normal physiological processes. Here, we have optimized an AAV platform to deliver the CRISPR/Cas9 system to white matter astrocytes. A single neonatal intra-CSF administration of an all-in-one AAV-DJ8 vector expressing the Staphylococcus aureus Cas9 enzyme under an astrocyte-specific promoter resulted in effective downregulation of Glial Fibrillary Acidic Protein

A Proof-of-Concept Study for the Efficacy of Dispirotripiperazine PDSTP in a Mouse Model of Herpes Simplex Herpetic Encephalitis.

Herpes simplex encephalitis (HSE) remains a life-threatening disease with high mortality and severe neurological complications, despite acyclovir therapy being limited by delayed diagnosis, drug resistance, and poor blood-brain barrier penetration. New antivirals with different mechanisms of action are urgently needed. We evaluated PDSTP, a novel dispirotripiperazine-based compound targeting host heparan sulfate proteoglycans (HSPGs), in mouse models of HSV-1 encephalitis. This study evaluates the therapeutic and prophylactic potential of PDSTP, an antiviral compound that blocks HSV glycoconju

Severe ADEM-Like Neuroinflammatory Disease and Cerebrovascular Fragility With Recurrent Pseudoaneurysms and Moyamoya in a Familial Germline CBL Mutation: Expanding the Clinical Phenotype.

Heterozygous germline variants in CBL disrupt its function as a negative regulator of the Ras/MAPK pathway, classically predisposing to Juvenile myelomonocytic leukemia (JMML) and moyamoya. We describe two affected siblings carrying a paternally inherited CBL variant (c.1210 T> C, p. Cys404Arg) who exhibited markedly divergent phenotypes, uncovering novel neuroinflammatory and systemic vascular fragility manifestations. The older sibling, a previously healthy 4-year-old girl, presented with acute, rapidly progressive acute disseminated encephalomyelitis (ADEM)-like neuroinflammatory disease po

The AHEPA EEG benchmark: setting the standard for machine learning in dementia diagnosis, a scoping review.

Accurate and reproducible electroencephalography (EEG)-based classification of dementia remains a key challenge in computational neurodiagnostics. The open-access AHEPA dataset has become the most commonly used benchmark for Alzheimer's disease (AD) and Frontotemporal dementia (FTD) classification, yet reported results vary widely due to methodological inconsistencies. This study presents the first systematic and quantitative benchmark review of all published machine learning approaches applied to the AHEPA dataset. Forty-six studies were reviewed and stratified into three validity tiers, with

The International Sports Physiotherapy Competencies and Standards 2025: The SportsComp Project on Higher Education to Improve Competency in Sports Physiotherapy.

Sports Physiotherapy is a specialised area of practice requiring specific competencies and standards to be met. The initial Sports Physiotherapy Competencies and Standards at the international level were first developed in 2005. With the sports physiotherapy landscape evolving, ensuring the currency and an update of the 2005 competencies was important. Through the SportsComp project (Higher Education to Improve Competency in Sports Physiotherapy) funded by an Erasmus+ grant, a consortium of five partners, including the International Sports and Exercise Physiotherapy Assocation, updated these c

Toscana virus encephalitis with reversible self-limiting vasculopathy.

Toscana virus (named after Tuscany, Italy) is an arthropod borne virus transmitted by sandflies. It is common in the Mediterranean region, several southern European countries and some North African countries, but is rare in the UK. Most Toscana virus infections are asymptomatic, but severe infections can have neurological manifestations. Toscana virus encephalitis is an uncommon self-limiting encephalitis with a vasculopathy. We report an elderly man who developed Toscana virus encephalitis and vasculopathy causing multiple ischaemic strokes with evidence of reversible intravascular stenosis,

Genetic Landscape of a Cohort of Children With Varicella Zoster Virus Encephalitis, Cerebellitis, and Stroke.

Varicella zoster virus (VZV) is a neurotropic member of the Herpesviridae family that causes varicella in primary infection and zoster during reactivation but in rare cases can lead to severe neurological complications, such as encephalitis. To identify inborn errors of immunity and unravel pathways involved in VZV central nervous system (CNS) immune responses and pathogenesis, we performed whole exome sequencing on a cohort of 38 children with neurological manifestations, including VZV encephalitis, cerebellitis, or stroke. We identified a total of 46 rare potentially pathogenic variants pred
