BACKGROUND: Herpes simplex virus encephalitis (HSE) is a rare but life-threatening condition. While the disease has been well characterized in Western countries, large-scale epidemiological data from Japan are lacking.
METHODS: Using the Ministry of Health, Labour and Welfare (MHLW) Diagnosis Procedure Combination (DPC) Study Group database, we identified patients hospitalized with HSE between July 2010 and March 2023. Patients were identified using International Classification of Diseases, 10th Revision codes. Nationwide incidence was estimated by multiplying the observed case counts by the

Emerging infections represent a major challenge for public health due to the urgent need to develop efficient clinical protocols in a scenario with few available scientific studies. The experience with Zika virus (ZIKV) shares important points with the COVID-19 pandemic, since both viruses are associated with community transmission, limited diagnostics, significant perinatal neurological risks, prognostic uncertainty, and lack of effective therapies. Zika virus and SARS-CoV-2 infect cells from the central nervous system causing neuroinflammation and neurodegeneration. Moreover, these neurotrop

BACKGROUND: Post-infectious inflammatory response syndrome (PIIRS) is recognized as a cause of neurologic deterioration in previously healthy patients with cryptococcal meningoencephalitis (CM). However, data on non-human immunodeficiency virus (HIV), immunosuppressed patients remain limited.
METHODS: Between July 2018 and April 2025, 13 non-HIV immunosuppressed patients with CM who subsequently developed PIIRS were included. Clinical features, Karnofsky performance scores, cerebrospinal fluid (CSF) parameters, and magnetic resonance imaging (MRI) findings were compared at PIIRS diagnosis and

PURPOSE: Herpes simplex virus 1 encephalitis (HSE) is the most common sporadic infectious encephalitis in Western countries, with a 70% mortality rate and only 9% of survivors free from neurological sequelae. While definitive diagnosis relies on cerebrospinal fluid testing, magnetic resonance imaging (MRI) plays a crucial role in identifying typical acute patterns and features. However, the imaging evolution of encephalitic lesions is not well understood. We aimed to identify and evaluate the prevalence and progression of cortical lesions, as well as the recurrence of these patterns in HSE and

Parvovirus B19 is a non-enveloped DNA virus of the Parvoviridae family, which usually causes erythema infectiosum in children. Although rare in adults, this virus can cause severe complications, including encephalitis. We report a case of a 50-year-old male patient, previously healthy, who was admitted to the hospital with high fever, headache, nausea, vomiting, and rapidly progressing to impaired consciousness. The patient was intubated and placed on mechanical ventilation. A lumbar puncture was performed, which showed mild leukocytosis with normal protein and glucose levels, and negative cul

BACKGROUND: The virulence and replication of PRV are not only regulated by the virulence genes gE or gI, but also related to host circRNA, miRNA, and lncRNA. Circular RNAs (circRNAs) fulfill a diverse array of biological functions. However, the functions of circRNAs in PRV infections by attenuated and virulent strains are not yet fully elucidated. In this study, the circRNA expression profiles of the PRV virulent strain FA and attenuated strain FB were established and analyzed by high-throughput sequencing technology. A ceRNA network was established utilizing differentially expressed circRNAs

Micropterus salmoides is an economically significant freshwater fish that faces serious threats from various pathogens. Micropterus salmoides rhabdovirus (MSRV) is a pathogenic virus that primarily infects juvenile fish, leading to severe disease. Although some studies have reported the transcriptome sequencing of immune-related tissues in Micropterus salmoides after MSRV infection, studies focusing on the central nervous system remain scarce. In this study, largemouth bass juvenile fish were infected by intraperitoneal injection of MSRV. Severe external clinical sign, together with high virus

BACKGROUND: Ventriculitis in patients with complex, multiloculated hydrocephalus and a history of multiple shunt revisions represents a formidable challenge. Conventional external ventricular drain (EVD) systems are frequently limited by obstruction from purulent material, requiring repeated manual irrigation, which compromises infection control, intracranial pressure (ICP) stability, and superinfection. Novel technologies integrating continuous, controlled irrigation with simultaneous drainage offer a potential solution for refractory cases.

BACKGROUND: Acute encephalitis caused by human parvovirus B19 (PVB19) is an uncommon clinical manifestation observed in both immunocompetent and immunocompromised individuals. Angioimmunoblastic T-cell lymphoma (AITL) is a distinct subtype of lymphoma characterized by aberrant immune dysregulation, predisposing patients to opportunistic infections even in the absence of chemotherapy.
CASE PRESENTATION: A 66-year-old woman with newly diagnosed, treatment-naïve AITL presented with acute confusion, dysarthria, and severe anemia. Cerebrospinal fluid (CSF) analysis and neuroimaging findings were n

A 67-year-old woman undergoing chemoradiotherapy for breast cancer had fever and impaired consciousness. Brain MRI showed abnormal signals in the bilateral frontal lobes. The patient was diagnosed with a brain abscess. The abnormal signal in the right frontal lobe decreased in size with antibiotic treatment, but the abnormal signal in the left frontal lobe worsened. Cytomegalovirus (CMV) was detected in the cerebrospinal fluid. Following administration of ganciclovir, the abnormal signal in the left frontal lobe decreased in size and consciousness improved. Based on the clinical course, we dia

Acyclovir (ACV) is the first-line agent for varicella zoster virus (VZV) encephalitis. Acute kidney injury (AKI) remains a clinical concern. We report the case of a sarcopenic obese woman with VZV encephalitis. She developed AKI 2 days after receiving intravenous ACV based on her actual body weight. After temporarily discontinuing ACV and confirming recovery of the renal function, ACV was restarted at a dose based on her ideal body weight. The patient recovered without any further complications. This case underscores the importance of dose adjustment of water-soluble medications, such as ACV,

The gut-brain axis (GBA) is a complex two-way communication system that links the gastrointestinal tract and the central nervous system (CNS) through neural, immune, hormonal, and microbial pathways. The microbiota-gut-brain axis (MGBA), a more specific concept, focuses on how gut microorganisms, including bacteria, viruses, and other microbes, modulate this communication and influence neurological health. This comprehensive review examines the intricate mechanisms through which gut microorganisms modulate neural function and contribute to neurological health and disease pathogenesis. The gut

This study investigated the molecular features of Aujeszky's disease in a hunting dog from southern Italy. After consuming meat from infected wild boars, a 5-year-old male dog developed intense itching and died within 48 hours. Pseudorabies was suspected and necropsy was performed at the Experimental Zooprophylactic Institute of Apulia and Basilicata. SuHV-1 DNA was detected in the brain tissue, and the virus was isolated using Vero cells. Histological features and the analysis of genome sequencing revealed a non-suppurative meningoencephalitis caused by Aujeszky's disease. The yielded genome

We describe a woman in her 30s who presented with progressive cognitive decline and reflex focal seizures triggered by specific auditory stimuli. Serum autoimmune testing demonstrated markedly elevated anti-glutamic acid decarboxylase 65 (GAD65) antibody titres (>2000 IU/mL). A paraneoplastic work-up revealed acute myeloid leukaemia (AML). After comprehensive neurological and haematological evaluation, she was diagnosed with paraneoplastic anti-GAD65-mediated autoimmune epilepsy, consistent with limbic encephalitis. This case is notable as it highlights a rare association between AML and paran

Bacterial meningoencephalitis (BME) in cattle is responsible for economic losses due to mortality, neurological impairment, and decreased productivity. The use of cerebrospinal fluid (CSF) bacterial culture-the gold standard for antemortem bacterial detection-is limited by its lengthy processing times and potential false negatives. With this study we wanted to determine whether 16S rRNA gene real-time quantitative PCR (16S rRNA qPCR) of DNA extracted from CSF could be used to detect BME in cattle. We applied a modified low-biomass microbial investigation protocol to extract CSF DNA from 15 hea

BACKGROUND: Although external ventricular drains (EVDs) are crucial in neurosurgical care, they carry a high risk of healthcare-associated ventriculitis, with reported infection rates. Numerous interventions were thought to aid in infection control, but there is still an ongoing increase in infection incidence. Recently, bundled interventions with standardized protocols, antimicrobial-impregnated catheters, tunneling techniques, chlorhexidine dressings, and limited antibiotic prophylaxis have shown an interesting efficacy in shunt-related infection control.
METHODS: A systematic review was co

While the EV-A71 vaccine is available, its real-world effectiveness against severe neurological complications such as encephalitis and its influence on circulating enterovirus serotypes require further clarification. This single-center, retrospective study analyzed 7823 children hospitalized with enterovirus-associated hand, foot, and mouth disease or herpangina between 2016 and 2023. Among them, 414 children had received EV-A71 vaccination (376 fully, 38 partially) and 7409 were unvaccinated. Encephalitis, defined by clinical symptoms, neuroimaging evidence, and CSF pleocytosis (>5 WBC/mm3),

Paraneoplastic neurological syndromes (PNS) are typically associated with malignant neoplasms. The anti-Yo antibody is classically linked to paraneoplastic cerebellar degeneration; its coexistence with limbic encephalitis is extremely rare. We report the first case of anti-Yo–positive limbic encephalitis associated with a benign thyroid–parathyroid adenoma. A 59-year-old male presented with epileptic seizures, left upper limb weakness, dysarthria, and confusion. Serum anti-Yo antibody was positive. Cranial MRI revealed diffusion restriction in the cerebellar hemispheres and middle cerebellar p

RATIONALE: The diagnosis of viral encephalitis by conventional and biochemical results of lumbar puncture of cerebrospinal fluid (CSF) is common in neurology, but the diagnosis of specific pathogens has benefited from the improvement of CSF diagnosis technology. Second-generation sequencing allows for more sensitive and accurate pathogen detection. A case of suid herpesvirus encephalitis was reported here.
PATIENT CONCERNS: A 35-year-old male experienced severe right head pain and intermittent right upper limb convulsions, occurring approximately 10 times daily since January 7, 2019. He was s
