Follicular lymphoma (FL) rarely presents with central nervous system involvement at the time of the diagnosis. We describe the case of a 44-year-old man who presented with meningoencephalitis and subsequently developed status epilepticus due to diffuse leptomeningeal dissemination of grade 1 FL. Cerebrospinal fluid (CSF) cytology yielded negative results, but flow cytometry detected monoclonal B-cell proliferation, leading to a diagnosis. Rapid neurological recovery was achieved after immunochemotherapy. This case underscores the importance of CSF flow cytometry in mononuclear cell-predominant

Neuropsychiatric systemic lupus erythematosus (NPSLE) remains challenging to diagnose because of heterogeneous clinical presentations, nonspecific findings, and the absence of definitive biomarkers. Artificial intelligence (AI) methods have been increasingly explored using neuroimaging and other biologically informative data to support identification of neuropsychiatric involvement in systemic lupus erythematosus (SLE). However, the reported performance and methodological robustness of these approaches have not been systematically characterized. To perform an exploratory meta-analysis describi

The purpose of this paper is to describe a challenging case of a Varicella Zoster Virus (VZV) meningoencephalitis-vasculopathy in an immunocompromised patient with severe Rheumatoid Arthritis (RA), emphasizing on diagnostic tools. Hereby, we present an elderly immunocompromised patient with RA that developed meningoencephalitis and Central Nervous System (CNS) vasculitis, only two months after discontinuation of her RA medication due to a VZV rash. The clinical, imaging and laboratory investigation that was performed led to a diagnostic dilemma, as it could argue in favor both of VZV infection

A small proportion of small cell lung cancer (SCLC) presents as paraneoplastic limbic encephalitis (PLE). The most implicated antibody is anti-Hu, but others such as anti-ampiphysin, anti-CRMP5 and anti-SOX1 have been frequently detected. Autoimmune encephalitis mediated by antibodies against α-amino-3-hydroxy-5-methyl-4-isoxazolepropionic acid receptor (AMPAR) is a rare but important neurological complication of SCLC. The incidence of SCLC presenting with anti-AMPAR antibody PLE is extremely low. Our case illustrates a rare presentation of SCLC with an intention to highlight the importance of

Dengue encephalitis is an uncommon neurological complication of dengue virus infection. In immunocompromised patients, such as solid organ transplant recipients, the clinical presentation may be atypical and nonspecific, posing significant diagnostic and therapeutic challenges. We report a clinical case and use it as a framework to discuss key considerations for suspicion of dengue encephalitis. Topics include optimal timing for clinical suspicion, essential differential diagnoses, and the most appropriate diagnostic strategies. Emphasis is placed on the correct interpretation of cerebrospinal

INTRODUCTION: Vancomycin is an antimicrobial agent for treating central nervous system (CNS) infections caused by Gram-positive bacteria. Due to practical and ethical reasons, it is difficult to evaluate vancomycin exposure in cerebrospinal fluid (CSF) and its relationship with therapeutic outcomes. Therefore, alternative methodologies are required. We developed a physiologically based pharmacokinetic (PBPK) model to characterize vancomycin exposure in plasma and CSF in patients with ventriculitis enrolled in a therapeutic drug monitoring program.
METHODOLOGY: PBPK modeling and simulation wer

Long COVID (LC) is a multisystem, post-infectious conditions diagnosed ≥3 months after acute SARS-CoV-2 infection and marked by relapsing, persistent, or progressive symptoms, especially fatigue, post-exertional symptom exacerbation and neuropsychiatric syndromes. We synthesized evidence suggesting that LC arises from intersecting pathways including viral persistence, intestinal dysbiosis and barrier compromise with microbial translocation, innate immune activation with neutrophil extracellular traps (NET) and thromboinflammation, and immune dysregulation with features of exhaustion and autoim

Zika virus (ZIKV) is an emerging arbovirus belonging to the Flaviviridae family and Orthoflavivirus genus, with a pronounced tropism for the central nervous system (CNS), where it induces neuroinflammation and neuronal death. ZIKV is known to exploit host cellular mechanisms, including the activation of survival pathways such as the PI3K/AKT signalling cascade, to evade apoptosis and enhance its replication. However, the role of the PI3Kγ isoform in ZIKV-induced neuroinflammation has not been previously explored, and this study aimed to investigate PI3Kγ in ZIKV pathogenesis. Primary neuronal

Neuropsychiatric systemic lupus erythematosus (NPSLE) manifesting as Guillain-Barré syndrome (GBS) is exceptionally rare, with only 28 documented cases in medical literature, and current biological therapies show limited efficacy. We report a 34-year-old woman who presented with progressive quadriparesis, bilateral ptosis, and facial palsy developing over one month. Laboratory investigations revealed marked B-cell dysregulation with elevated immunoglobulin G (24.30 g/L, normal 7-16), positive antinuclear antibodies (1:320), anti-double-stranded DNA antibodies (128 IU/ml, normal <20), anti-ribo

Acute respiratory infections (ARIs) are among the most common infectious diseases encountered in clinical practice, with approximately 70-80 % of cases attributed to viral pathogens. Among these, seasonal influenza A virus (IAV) and severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) persistently pose public health threats due to their high genetic variability, strong transmissibility and significant pathogenicity. Recently, accumulating evidence has suggested that these viruses can not only cause typical respiratory symptoms but also affect the central nervous system (CNS) through ne

Oropouche fever is a re-emerging global viral threat caused by infection with Oropouche virus (OROV). While disease is generally self-limiting, historical and recent reports of neurologic involvement highlight the importance of understanding the neuropathogenesis of OROV. In this study, we characterize viral replication kinetics in neurons, microglia, and astrocytes derived from immortalized, primary, and induced pluripotent stem cell-derived cells, which are all permissive to infection with the prototypic OROV BeAn19991. We demonstrate cell-type dependent replication kinetics with both histor

INTRODUCTION: Autoimmune encephalitis (AE) is a spectrum of autoimmune disorders manifesting by epilepsy, loss of consciousness, and neuropsychiatric symptoms. Anti-recoverin antibodies are intracellular antibodies and a well-known marker of paraneoplastic retinopathy. However, they have rarely been associated with neurological and psychiatric diseases with or without cancer. To date, there have been only a few identified cases of anti-recoverin antibody-associated limbic AE.

Pseudorabies virus (PRV) is an important swine pathogen causing severe economic losses worldwide. Nasal mucosa serves as the initial entry site for PRV, highlighting the importance of nasal mucosal immunity in limiting infection. Here, we investigated the immune responses and protection against PRV for the intranasal vaccination of glycoprotein B (gB) subunit vaccine adjuvanted with poly(I:C). In piglets, immunohistochemistry showed rapid uptake of the gB by the nasal mucosa within 2 h, supporting subsequent immune activation. Upon challenge with the PRV variant ZJ01, intranasal gB vaccination

BackgroundNeuropsychiatric systemic lupus erythematosus (NP-SLE) in children is uncommon but often severe, with a wide spectrum of neurologic presentations and sometimes atypical or initially normal neuroimaging. Among reported manifestations, acute cerebellitis is rare, and simultaneous thalamic involvement has scarcely been described. Differentiating inflammatory NP-SLE from mimics such as posterior reversible encephalopathy syndrome (PRES), infection, or ischemia is essential, since management strategies and prognosis differ substantially.CaseWe describe a 13-year-old female with newly diag

INTRODUCTION: Pseudorabies virus (PRV), a significant pathogen of swine, causes substantial economic losses, and even poses an emergent public health concern due to its zoonotic potential. The continuous evolution of PRV has undermined the effectiveness of current vaccines and antiviral drugs. Consequently, there is an urgent need to develop novel strategies to curb its spread.
METHODS: The inhibitory effect of garcinone C on PRV replication was assessed in vivo and in vitro. To determine the stage of antiviral action, treatments were administered at different time points: pre-treatment, co-t
