The Cases of Acute Necrotizing Encephalopathy Due to Human Herpes Virus-6.

BACKGROUND: Human herpesvirus-6 (HHV-6) is a neurotropic virus that typically remains latent in immunocompetent individuals but may cause severe neurologic complications, including encephalitis, particularly in immunocompromised hosts. Rarely, HHV-6 encephalitis can also occur in immunocompetent children, often presenting with features of acute necrotizing encephalopathy. CASES: We report 2 cases of previously healthy toddlers who presented with fever, seizures and altered mental status. Neuroimaging revealed bilateral thalamic involvement consistent with acute necrotizing encephalopathy. HHV

Severe herpes simplex virus encephalitis complicated by atypical Takotsubo cardiomyopathy with left ventricular systolic dysfunction.

A previously healthy woman in her 30s presented with high-grade fever, photophobia and fluctuating consciousness, and was diagnosed with herpes simplex virus type 1\ encephalitis. Her condition deteriorated, requiring intensive care support for type 1 respiratory failure. During her intensive care unit stay, she was found to have significant left ventricular systolic dysfunction with basal-mid akinesia and apical sparing on echocardiography. Cardiac MRI demonstrated features consistent with stress-induced (Takotsubo) cardiomyopathy. She made a complete neurological and cardiac recovery with su

Reversible bilateral striatal lesions in NPSLE.

A 17-year-old female with systemic lupus erythematosus developed acute neuropsychiatric symptoms despite improvement in systemic disease activity. Brain MRI revealed bilateral striatal and thalamic hyperintensities on FLAIR, which progressed transiently and subsequently regressed following high-dose corticosteroids, intravenous cyclophosphamide, and plasma exchange. The clinical and radiological features were consistent with neuropsychiatric systemic lupus erythematosus. This case highlights that NPSLE can newly emerge during apparent systemic remission and emphasizes the importance of timely

Dynamic chromatin accessibility and transcriptional landscapes of porcine kidney cells during pseudorabies virus infection.

BACKGROUND: Pseudorabies virus (PRV) is a major swine pathogen that causes substantial economic losses. The dynamic remodeling of host cell chromatin plays a pivotal role during viral infections. However, the epigenetic mechanisms underlying PRV-host interactions remain unclear. METHODS: This study integrates ATAC-seq and RNA-seq to investigate the dynamic changes in host chromatin accessibility and gene transcription during PRV infection. The accessible chromatin regions were analyzed for enrichment in genomic features and transcription factor binding motifs. RNA-seq data were used to identi

The association between early pregnancy infection with SARS-CoV-2 and fetal birth defects: a prospective study.

This multicenter prospective cohort study investigated the association between maternal SARS-CoV-2 infection in early pregnancy and birth defects. Although placental damage and fetal distress are known risks associated with SARS-CoV-2 infection during pregnancy, evidence regarding the impact of first-trimester infection on congenital anomalies remains limited. In this study, we prospectively enrolled 1,127 first-trimester pregnant women from three hospitals in China (535 SARS-CoV-2-infected, 592 non-infected as controls). The clinical characteristics, symptoms and treatment of COVID-19, and co

Layer-Specific Astrocyte Morphological Responses in the CA3 Hippocampus Region During Piry Virus-Induced Encephalitis.

Astrocytes from distinct hippocampal layers exhibit region-specific morphological traits, which may be influenced by their local microenvironment. During viral encephalitis, these cells undergo dynamic changes that can reflect layer-specific vulnerability. In this study, we characterized whether astrocytes from different CA3 hippocampal layers display distinct morphological responses to Piry virus-induced encephalitis. Adult female Swiss mice were intranasally inoculated with the Piry virus and sacrificed at 20- or 40-days post-infection (dpi). GFAP+ astrocytes from the Stratum lacunosum-molec

RNA Polymerase III Suppresses Varicella Zoster Virus in Human Neurons.

INTRODUCTION: Varicella zoster virus (VZV) is a ubiquitous human alphaherpesvirus, latently present in the majority of the population. Knowledge on the molecular mechanisms underlying VZV reactivation from peripheral ganglia is incomplete, as is our understanding of why only a small number of individuals develop life-threatening complications following reactivation. Previously, we have reported a pathogenic mutation in the POLR3F subunit of the cytosolic DNA sensor RNA polymerase III in 2 monozygotic twins diagnosed with recurring encephalitis and vasculitis, caused by VZV reactivation. SHFL Post-Transcriptionally Restricts Coxsackievirus A16 In Vitro and In Vivo.

Coxsackievirus A16 (CVA16), a major etiological agent of hand, foot, and mouth disease, is increasingly contributing to neurological complications, with no vaccines or virus-specific antivirals currently available. To identify CVA16-restricting host factors, we investigated the role of the interferon-stimulated gene shiftless (SHFL), previously implicated in the control of other RNA viruses. Using CRISPR-Cas 9, we generated SHFL knockout rhabdomyosarcoma cells and assessed viral replication, cytopathic effects, and replication stage dynamics. We evaluated disease progression and tissue injury

Rationale for investigating the use of anifrolumab in neuropsychiatric systemic lupus erythematosus: a combined narrative and case-based systematic literature review.

BACKGROUND: Neuropsychiatric (NP) involvement represents one of the major challenges in Systemic Lupus Erythematosus (SLE), often requiring individualized therapeutic strategies. While anifrolumab inhibits the type I interferon receptor 1 (IFNAR1) and is approved for the treatment of moderate-to-severe SLE, randomized controlled trials have not evaluated its efficacy in NPSLE. METHODS: We examined the pathophysiological rationale for inhibiting IFN-α using anifrolumab in NPSLE. To supplement this, we report an original case of NPSLE successfully treated with anifrolumab, along with similar ca

Spillover of pseudorabies virus variants to humans: an urgent call for pseudorabies eradication in domestic pigs.

Pseudorabies virus, an α-herpesvirus historically considered a veterinary pathogen restricted to pigs, has now emerged as a zoonotic agent capable of causing severe and often fatal infections in humans. Since 2017, novel pseudorabies virus variants in China have been shown to be able to breach species barriers, leading to 31 laboratory-confirmed human cases of encephalitis and endophthalmitis with high mortality and disability rates by 2023. In this Personal View, we synthesise current evidence on the virological evolution, cross-species transmission dynamics, and clinical consequences of pseu

Tunneling nanotubes induced by pseudorabies virus facilitate viral transmission in neuronal cells to evade the immune system.

BACKGROUND: Pseudorabies virus (PRV), an alphaherpesvirus endemic in swine, has recently emerged as a causative agent of fatal human encephalitis, yet its neuropathogenic mechanisms remain largely unexplored. Tunneling nanotubes (TNTs) are actin-rich intercellular membrane extensions that have been found to mediate intercellular communication and facilitate the cell-to-cell transmisssion of viruses. METHODS: Immunofluorescence analysis, confocal imaging, time-lapse photography, in situ sectioning transmission electron microscopy, and scanning electron microscopy were employed to characterize t
