Significance of diffusing capacity of the lungs for carbon monoxide on chronic thromboembolic pulmonary hypertension.

Reduced diffusing capacity of the lungs for carbon monoxide (DLco) reflects microvasculopathy in chronic thromboembolic pulmonary hypertension, yet its clinical value is uncertain. In a Japanese nationwide registry (2018-2023) we studied 1270 patients: 486 formed an event cohort and 299 a treatment cohort who underwent pulmonary endarterectomy or balloon pulmonary angioplasty. Lower baseline DLco was indicative of smaller postprocedural improvements in mean pulmonary artery pressure, pulmonary vascular resistance and cardiac index (all p≤0.023) and a higher risk of clinical events (HR 0.971, p

Aspiration Pneumonia After Lumbar Spinal Intervention.

Low back pain is a prevalent complaint among patients, leading to the frequent use of various interventional procedures in pain clinics. While these interventions are generally regarded as safe, complications, particularly in elderly patients, can include serious conditions such as infectious spondylitis or spinal hematoma. We report a case of an elderly patient who developed aspiration pneumonia shortly after undergoing a lumbar spinal intervention. The initial clinical suspicion was infectious spondylitis due to the temporal association with the procedure. However, further evaluation reveale

Histotripsy for neuroendocrine liver metastases: Early single-institution outcomes and safety.

BACKGROUND: Histotripsy is a novel, noninvasive, nonionizing focused ultrasound technology that mechanically destroys tissue via cavitation. Neuroendocrine tumors, which frequently metastasize to the liver and follow an indolent course, may be particularly well-suited for histotripsy, and we report an early experience. METHODS: This retrospective analysis summarizes the first 32 patients consecutively treated with histotripsy in a neuroendocrine practice. Intraoperative targeting and cavitation were performed with the EDISON system. Clinical outcomes, postprocedural imaging, and adverse event

Understanding exacerbation risk in BLVR: A logistic regression approach to complication prediction.

BackgroundChronic obstructive pulmonary disease (COPD) with emphysema is associated with persistent airflow limitation and frequent exacerbations. Bronchoscopic lung volume reduction (BLVR) with endobronchial valves (EBVs) improves lung function and quality of life but carries a risk of postprocedural complications, including acute exacerbations and pneumonia. Predictors of these adverse events remain incompletely defined.PurposeTo identify clinical and inflammatory factors associated with postprocedural exacerbations in patients undergoing BLVR with EBVs, aiming to support individualized risk

Diagnostic yield and safety of Dual-functIon Semi-rigid thoraCoscOpy Versus rigid thoracoscopy for the diagnosis of plEuRal disease (DISCOVER-2): protocol for a randomised controlled trial.

INTRODUCTION: Rigid thoracoscopy is an established diagnostic instrument for pleural diseases; however, it is limited by challenges related to manoeuvrability, patient comfort and operator learning curve. A novel dual-function semi-rigid thoracoscope with associated semi-rigid forceps has been engineered to integrate the diagnostic efficacy of rigid biopsy forceps with the ergonomic advantages of a semi-rigid system. The Dual-functIon Semi-rigid thoraCoscOpy Versus rigid thoracoscopy for the diagnosis of plEuRal disease trial was designed to evaluate whether this device achieves a diagnostic y

Platypnea-Orthodeoxia Syndrome Caused by a Patent Foramen Ovale and Interatrial Septal Aneurysm: A Case Report.

Platypnea-orthodeoxia syndrome (POS) is a rare condition characterized by positional hypoxemia, often due to right-to-left shunting across a patent foramen ovale (PFO) despite normal right-sided pressures. Early recognition is essential as symptoms may be subtle and imaging is often unrevealing. A 78-year-old female patient with a known PFO presented with one week of lower abdominal pain and mild exertional dyspnea without chest pain. She was profoundly hypoxic to 76% on room air, with improvement when supine and worsening when upright. Initial labs were unremarkable, and arterial blood gas re

External validation of the CAPAD score to predict the occurrence of clinically significant pneumothorax after CT-guided percutaneous transthoracic lung biopsy.

BACKGROUND: The most common complication preventing outpatient management after percutaneous CT-guided lung biopsy (PTLB) is the occurrence of pneumothorax requiring drainage. We previously developed a pragmatic score to predict the risk of clinically relevant pneumothorax after PTLB (hereafter called the CAPAD score), to guide patient management. OBJECTIVES: To externally validate the CAPAD score in an independent cohort, with a specific focus on its ability to rule out pneumothorax requiring drainage after PTLB. METHOD: We retrospectively analysed a pre-existing institutional database of c

Regional ventilation-perfusion changes after endobronchial valve therapy assessed by single-photon emission computed tomography in adults with severe COPD.

Bronchoscopic lung volume reduction (BLVR) is an established treatment for patients with severe chronic obstructive pulmonary disease (COPD), characterized as emphysema and hyperinflation, but current methods of patient selection and lobe identification are limited due to invasiveness, lack of specificity, and poor regional ventilation and perfusion assessments. This study aimed to evaluate lobar changes in ventilation, perfusion, and volume before and after endobronchial valve (EBV) placement using SPECT/CT imaging in patients with severe emphysema, and to assess their association with clinic

Incidence and outcomes for post-bronchoscopic lung volume reduction (BLVR) pneumothoraces stratified by target lobe treated.

UNLABELLED: Bronchoscopic lung volume reduction (BLVR) is a minimally invasive procedure used to improve shortness of breath in some patients with hyperinflation secondary to emphysema. Treatment with BLVR involves placement of small, one-way endobronchial valves (EBV) into airways to induce lobar atelectasis. Patients treated with BLVR show an improvement in forced expiratory volume at 1 s (FEV1), 6-min walk distance (6MWD), and an improvement in St. George's Respiratory Questionnaire (SGRQ) scores. The most common complication following BLVR is pneumothorax. The incidence of a pneumothorax i

Protocol for robotic-assisted bronchoscopy versus electromagnetic navigation bronchoscopy for the diagnosis of peripheral pulmonary nodules: a randomized trial (ARTICULAtE study).

BACKGROUND: Electromagnetic navigation bronchoscopy (ENB) and shape-sensing robotic-assisted bronchoscopy (ssRAB) are the two dominant bronchoscopic technologies for the diagnosis of peripheral pulmonary nodules (PPNs). However, data directly comparing ENB and ssRAB are limited. We designed a randomized controlled trial to compare the diagnostic performance of these two technologies. The primary objective is to evaluate if ssRAB is superior to ENB for the diagnosis of PPNs. METHODS: It is a multicenter, open-label, superiority randomized controlled trial. Patients with PPNs suspicious for lun

Implementation of a Hybrid Cardiac Rehabilitation and Symptom Scoring System in Patients with Inappropriate or Postural Sinus Tachycardia Referred for Sinus Node Sparing Hybrid Ablation.

Background/Objectives: Patients with inappropriate sinus tachycardia (IST) and postural orthostatic tachycardia syndrome (POTS) exhibit complex clinical profiles due to autonomic dysfunction. While sinus node sparing (SNS) hybrid ablation is emerging as a promising therapy, there are no established guidelines worldwide for post-procedure patient management and care is mainly based on telemonitoring. In contrast, our hybrid cardiac rehabilitation (HCR) program integrates inpatient care and home-based telerehabilitation. We aim to evaluate the implementation of the HCR program, patient acceptanc

SARS-CoV-2 infection dynamics in a MHCI-mismatched lung transplant recipient.

A 48-year-old patient underwent lung transplantation because of severe COVID-19, which aggravated his underlying interstitial lung disease, despite the presence of detectable SARS-CoV-2. Subsequently, the graft is re-infected early in the post-procedural phase, leading to viral persistence for more than five months. By analyzing viral evolution and effector immune response within the transplanted organ, we observe three main findings. First, virus evolution differs in the transplanted organ compared to that in the upper respiratory tract and is affected by monoclonal SARS-CoV-2-specific antibo

Intraoperative and postprocedural respiratory disorders admissions profile: An ecological longitudinal study in England and Wales.

Postoperative respiratory complications are widespread and associated with several healthcare systems and financial burdens. Studying admissions due to these complications may aid in developing preventive measures and approaches. However, such studies are lacking in England and Wales. Therefore, this study aims to study the admissions related to intraoperative and postprocedural respiratory disorders (IPPRDs). This was an ecological study using publicly available data extracted from the Hospital Episode Statistics database in England. The diagnostic code (J95) was used to identify all hospital

Discrepant Results of Post-valve CT Analysis and Pulmonary Function Test in Patients Undergoing Bronchoscopic Lung Volume Reduction.

Introduction Emphysema is a debilitating form of chronic obstructive pulmonary disease (COPD) that causes lung hyperinflation and air trapping, leading to reduced quality of life. Bronchoscopic lung volume reduction (BLVR) with endobronchial valves (EBVs) offers a minimally invasive treatment option by collapsing diseased lung segments to improve respiratory mechanics. While both CT and pulmonary function testing (PFT) are used to assess outcomes, it remains unclear whether post-procedural volume changes measured by CT correlate with functional improvements seen on PFT. Comparison of cryobiopsy and forceps biopsy for the diagnosis of mediastinal lesions: A randomised clinical trial.

INTRODUCTION: Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is the standard approach for lung cancer staging. However, its diagnostic utility for other mediastinal diseases might be hampered by the limited tissue retrieved. Recent evidence suggests the novel sampling strategies of forceps biopsy and cryobiopsy as auxiliary techniques to EBUS-TBNA, considering their capacity for larger diagnostic samples. METHODS: This study determined the added value of forceps biopsy and cryobiopsy for the diagnosis of mediastinal diseases. Consecutive patients with mediastinal

An adjudication algorithm for respiratory-related hospitalisation in idiopathic pulmonary fibrosis.

BACKGROUND: There is no standard definition of respiratory-related hospitalisation, a common end-point in idiopathic pulmonary fibrosis (IPF) clinical trials. As diverse aetiologies and complicating comorbidities can present similarly, external adjudication is sometimes employed to achieve standardisation of these events. METHODS: An algorithm for respiratory-related hospitalisation was developed through a literature review of IPF clinical trials with respiratory-related hospitalisation as an end-point. Experts reviewed the algorithm until a consensus was reached. The algorithm was validated
