Pericardial Decompression Syndrome After Large-Volume Pericardiocentesis in Systemic Sclerosis.

BACKGROUND: Pericardial decompression syndrome (PDS) is a rare and underrecognized complication of pericardiocentesis, characterized by paradoxical hemodynamic deterioration after relief of pericardial effusion. CASE SUMMARY: A 32-year-old woman with systemic sclerosis presented with chest pain and was found to have a large pericardial effusion with early tamponade physiology. She underwent rapid, large-volume pericardiocentesis. Hours later, she developed acute respiratory failure and cardiac arrest. Repeat echocardiography demonstrated new severe left ventricular systolic dysfunction withou

High-flow nasal-cannula oxygen therapy in intensive-care-unit patients: a prospective multicenter observational cohort study (OHE-REA).

High-flow oxygen therapy (HFOT) has become the most common oxygen-supplementation modality in patients with acute respiratory failure, based chiefly on data from tightly selected patients included in randomized trials. Our objective was to obtain real-life data on HFOT failure. This prospective observational multicenter French study done in 13 intensive care units in 2019–2020 was designed to determine the proportion of patients with failed HFOT, defined as intubation, noninvasive ventilation, standard oxygen therapy with an estimated FiO2>50%. We included 257 patients before prematurely endin

Fatal tracheal perforation following Dabrafenib-Trametinib therapy in BRAF V600E-mutant mixed anaplastic thyroid cancer.

Anaplastic thyroid cancer (ATC), pure or mixed, is the most aggressive form of thyroid cancer that leads to rapid death if left untreated. BRAF V600E is the most frequent pathogenic variant of ATC. Dabrafenib and Trametinib (D+T) have shown promising efficacy and are approved as first-line therapy for unresectable BRAF-mutant ATC. We report a case of fatal aero-digestive perforation in a patient treated with D+T. A 56-year-old man presented with an enlarging cervical mass and airway compromise. BRAF V600E-mutant high-grade papillary thyroid carcinoma with an anaplastic component and tracheal i

Adult Survival in SMA Type 1: A 23-Year Journey With Home Ventilation and Multidisciplinary Support.

Spinal muscular atrophy (SMA) type 1 is a severe autosomal recessive neuromuscular disorder caused by loss-of-function variants in the SMN1 gene, typically leading to death within the first two years without intervention. Long-term survival is uncommon, especially in patients with the most severe genotypes, such as those carrying only one SMN2 copy, and is rarely reported in resource-limited settings. We describe a 23-year-old man from northeastern Brazil with SMA type 1 who has been fully ventilator-dependent since infancy. Early symptoms included hypotonia, weak cry, and paradoxical breathin

Comparative outcomes of closed, percutaneous fixation, and ORIF in nutritional vulnerable adults with calcaneus fractures.

BACKGROUND: Nutritional Vulnerability may increase complications and reintervention after calcaneus fracture care. This study compared 90-day and 2-year outcomes among malnourished adults treated with closed management, percutaneous fixation, or open reduction and internal fixation (ORIF). METHODS: Adults (≥18 years) with calcaneus fracture and laboratory-defined nutritional vulnerability (albumin ≤3.5 g/dL and/or leukocytes ≤1.5 × 10³/µL within 1 year pre-index) were identified. Three independent 1:1 propensity score-matched comparisons were performed (closed vs ORIF, percutaneous vs ORIF, a

The impact of positive airway pressure (PAP) on severe Coronavirus Disease (COVID-19) among the patients with obstructive sleep apnea (OSA).

BACKGROUND: Obstructive sleep apnea (OSA) has been proposed as a risk factor for severe Coronavirus Disease (COVID-19). We hypothesized that treating OSA with positive airway pressure (PAP) decreases the risk of severe COVID-19. To answer this question, the risk of severe COVID-19 was compared between PAP adherent and PAP non-adherent OSA patients. METHODS: This is a retrospective cohort study of patients aged 18-89 with OSA and available data on PAP use who tested positive for SARV-COV-2 before 12/14/2020. The patients were divided into PAP adherent and PAP non-adherent groups based on their

Diaphragm physiological similarity index (DPSI): a de Novo speckle-tracking ultrasound metric for HFNC adjustment in acute respiratory failure.

BACKGROUND: Individualized diaphragm-protective respiratory support remains challenging, particularly during high-flow nasal cannula (HFNC) therapy. We developed the diaphragm physiological similarity index (DPSI) using speckle-tracking ultrasound to quantify diaphragmatic coordination and explore its potential to inform HFNC flow adjustment in acute respiratory failure (ARF). METHODS: In this prospective observational study, 103 participants were enrolled: 45 healthy volunteers, 38 intensive care unit (ICU) patients for reproducibility assessment, and 20 ARF patients undergoing a prespecifie

Urine Metanephrine Concentration Can Early and Accurately Predict Etiology of Acute Respiratory Failure in Critically Ill Patients with Subarachnoid Hemorrhage: A Prospective Single-Center Pilot Study.

Background: Subarachnoid hemorrhage (SAH) can cause remote organ failure through complex systemic reactions. Acute respiratory failure (ARF) in the course of SAH may have a diverse etiology, including cardiogenic origin. The aim of the study was to evaluate the utility of urine metanephrine measurement in identifying the ARF phenotype in patients with SAH. Methods: A prospective single-center study was conducted between January 2022 and February 2023. The study included consecutive adult patients admitted to the Intensive Care Unit (ICU) within 24 h of SAH diagnosis and requiring mechanical ve

[Ventilation strategies in intensive care medicine].

Mechanical ventilation is a life-saving intervention in intensive care, ensuring adequate gas exchange and reducing respiratory workload. However, inappropriate ventilation strategies can cause ventilator-induced lung injury (VILI) through mechanisms such as volutrauma, barotrauma, atelectrauma, and biotrauma, which may lead to systemic inflammation and multiorgan failure. This review summarizes evidence-based approaches for managing acute respiratory failure, focusing on both non-invasive and invasive ventilation strategies.Non-invasive techniques (HFNO, CPAP, NIV) are established for COPD ex

Relationship between red cell distribution width to albumin ratio (RAR) and mortality in ICU patients with sepsis-associated ARDS: a retrospective study based on MIMIC-IV.

BACKGROUND: The red cell distribution width-to-albumin ratio (RAR) is an emerging prognostic indicator. However, its correlation with clinical endpoints in individuals with sepsis-associated acute respiratory distress syndrome (SA-ARDS) remains unclear. This study examined the relationship between RAR and all-cause mortality (ACM) in the intensive care unit (ICU) patients with SA-ARDS using the MIMIC-IV database. The goal was to identify high-risk individuals early on and improve prognostic assessment. METHODS: Analyses were conducted using data from the MIMIC-IV (V3.1) database. Adult patient

Diaphragm neurostimulation mitigates the adverse cardiopulmonary effects of positive pressure ventilation.

BACKGROUND: Positive pressure ventilation has multiple adverse cardiopulmonary effects leading to impaired lung mechanics and hemodynamic performance. Controlled levels of diaphragm activity may mitigate these. The objectives of the study were to assess the effect of varying levels of diaphragm neurostimulation on respiratory mechanics and hemodynamics during positive pressure ventilation and to establish whether these effects depend on the level of applied positive end-expiratory pressure (PEEP). METHODS: Patients enrolled in a clinical trial of diaphragm neurostimulation underwent a nested r

Physiology-guided personalized mechanical ventilation to prevent ventilator-induced lung injury.

Mechanical ventilation is essential for managing acute respiratory failure, yet it carries a significant risk of ventilator-induced lung injury (VILI). Lung-protective ventilation, most notably through the use of low tidal volumes, has improved outcomes in acute respiratory distress syndrome (ARDS), but these conventional strategies do not fully account for the profound heterogeneity of the injured lung or the variability in patient-specific physiology. Although tidal volumes of 4-8 ml/kg predicted body weight (PBW) provide a general reference for limiting strain, truly protective ventilation

Ventilation-perfusion effects of negative-pressure ventilation: insights from an experimental rat model.

BACKGROUND: Mechanical ventilation typically utilizes positive-pressure ventilation (PPV), which fundamentally differs from physiological pressure conditions. In contrast, negative-pressure ventilation (NPV) more closely mimics physiological pressure conditions; however, its impact on ventilation-perfusion matching remains unclear. Therefore, we compared PPV and NPV in terms of their effects on ventilation-perfusion matching and determined the consequences of increasing end-expiratory pressure (EEP). METHODS: Anesthetized rats (n = 9) were ventilated using PPV at a positive EEP of 0, 3, 6, an

Long-term respiratory stability of elderly patients recovering from acute respiratory failure and invasive mechanical ventilation: a retrospective cohort study.

Long-term outcomes after discharge remain a challenge in critically ill elderly patients. We aimed to evaluate the long-term survival without repeat mechanical ventilation (MV) in elderly survivors of acute respiratory failure (ARF). We retrospectively included 1533 elderly patients aged 65 years or older who were admitted with ARF and invasive MV, successfully recovered, were liberated from MV, and were discharged between January 2010 and December 2019. Patients who were MV-dependent at discharge or died in the hospital were excluded. The primary outcome was survival without repeat MV. Second

Identifying the association between post-extubation laryngeal edema and aspiration in acute respiratory failure patients.

BACKGROUND: Following greater than or equal to 48-h intubation, one-third of acute respiratory failure (ARF) patients aspirate. Laryngeal edema has been identified as a potentially modifiable risk factor of post-extubation aspiration. The aim of this case-control study was to characterize the anatomical distribution of post-extubation laryngeal edema and to correlate anatomical laryngeal edema severity with aspiration risk in ARF survivors. The study also assessed whether patient weakness influenced the relationship between laryngeal edema and aspiration risk. METHODS: Flexible endoscopic eva
