Physiological effects and optimisation of nasal assist-control ventilation for patients with chronic obstructive pulmonary disease in respiratory failure.

BACKGROUND: A study was undertaken to investigate the effects of non-invasive assist-control ventilation (ACV) by nasal mask on respiratory physiological parameters and comfort in acute on chronic respiratory failure (ACRF). METHODS: Fifteen patients with chronic obstructive pulmonary disease (COPD) were prospectively and randomly assigned to two non-invasive ventilation (NIV) sequences in spontaneous breathing (SB) and ACV mode. ACV settings were always optimised and therefore subsequently adjusted according to patient's tolerance and air leaks. RESULTS: ACV significantly decreased all the

Options for long-term ventilatory support.

Long-term mechanical ventilator support for patients with chronic respiratory failure is becoming more common. This article reviews the common causes for chronic ventilator dependence, and offers an approach to weaning these patients from the ventilator. In addition, the details for preparing these patients for prolonged mechanical ventilation outside of the acute-care hospital setting are discussed. Appropriate education of the patient's caregivers is key to the success of long-term ventilatory support outside of the acute-care hospital.

Long-term efficiency of home nasal mask ventilation in patients with diffuse bronchiectasis and severe chronic respiratory failure: a case-control study.

The aim of this study was to evaluate long-term efficacy and tolerance of nasal mask ventilation (NMV) in a comparative case-control study. Fourteen patients with diffuse bronchiectasis and severe chronic respiratory failure (CRF), treated by long-term oxygen-therapy (LTO) and NMV, were case matched with 14 patients with diffuse bronchiectasis and severe CRF treated with only LTO. Patients and control subjects were compared based on the following parameters: blood gases, FEV1, vital capacity, hospitalizations, and survival. Symptoms, Karnofsky function score, and clinical evolution were also m

[Lung volume reduction in severe emphysema].

BACKGROUND: Lung volume reduction (LVR) surgery is an effective and organ-preserving treatment option for patients suffering from severe dyspnea due to endstage emphysema. METHOD: Resection of functionally inactive lung parenchyma reduces over-inflation and restores the elastic recoil of the lungs. Thus it results in improvement of dyspnea, mobility and pulmonary function. Patient selection is crucial. Of simliar importance is pulmonary rehabilitation, as well as sufficient expertise in the treatment of endstage chronic respiratory failure. RESULTS AND CONCLUSION: The in-hospital morbidity a

[Outcomes of noninvasive positive-pressure ventilation in patients with acute hypercapnia complicating chronic respiratory failure].

We used noninvasive positive-pressure ventilation to treat hypercapnea due to acute exacerbations of chronic respiratory failure (21 episodes in 19 patients; COPD, 4; pulmonary tuberculosis sequelae, 4; silicosis, 3; silicotuberculosis, 3; bronchiectasis, 3; others, 2). All patients had acute onsets of severe hypercapnea (PaCO2 > 45 Torr), acute decreases in pH (< 7.35), and tachypnea, paradoxical breathing or both. During the first 2 to 4 hours of bi-level positive airway pressure, PaCO2 decreased from 72 to 61 Torr (p < 0.0005), pH increased from 7.26 to 7.31 (p < 0.001), and respiratory rat

Non-invasive ventilation in the management of respiratory failure.

Non-invasive positive pressure ventilation (NIPPV) has been used increasingly to treat various forms of respiratory failure, with benefits in terms of gas exchange improvement, avoidance of endotracheal intubation and a decreased mortality. This review will focus on the recent developments and recommendations in the use of NIPPV in the treatment of acute and chronic respiratory failure, the methodology in the application of NIPPV, and briefly on its proposed mechanism of action.

Impairment of muscle energy metabolism in patients with sleep apnoea syndrome.

Impairment of muscle energy metabolism has been demonstrated in normal subjects with chronic hypoxaemia (altitude chronic respiratory failure). The purpose of this study was to verify the hypothesis that a comparable condition could develop in patients with sleep apnoea syndrome (SAS), considering that they are exposed to prolonged and repeated hypoxaemia periods. Muscle metabolism was assessed in 11 patients with SAS performing a maximal effort on cycloergometer. In comparison with normal subjects, SAS patients reached lower maximal loads [144 +/- 7 vs. 182 +/- 10 W (P < 0.005)] and lower pea

Evaluation of a movement detector to measure daily activity in patients with chronic lung disease.

This study was designed to evaluate the performance of movement detectors (pedometers) in measuring daily activity of patients with chronic lung disease. Three groups of subjects were studied: group 1: 25 patients with stable nonhypercapnic chronic obstructive pulmonary disease (COPD) (forced expiratory volume in one second (FEV1) = 47+/-9% predicted) studied twice, one month apart; group 2: 25 patients with chronic respiratory failure studied before and three months after nasal nocturnal mechanical ventilation; and group 3: 25 normal healthy subjects studied once. The median level of activity

[Noninvasive ventilation in the acute care hospital--a cost factor?].

BACKGROUND: Noninvasive ventilation as well established in treatment of chronic respiratory failure. Many announcements and our own experience give evidence that this method of treatment is useful for patients with acute respiratory failure too. Also the actual situation of our health system requires increasing attention to financial points of view. PATIENTS AND METHOD: We analyzed a number of 185 patients who needed mechanical ventilation in our intensive care unit in 1995. 80 of these 185 needed mechanical ventilation due to pulmonary and cardiopulmonary diseases (e. g. cardiac failure, exa

Causes of death in patients with COPD and chronic respiratory failure.

Although the factors associated with mortality, such as forced expiratory volume in one second (FEV1), arterial oxygen tension (Pa,O2) and pulmonary arterial pressure, have been well described, there is limited information on the circumstances of death in patients with chronic obstructive pulmonary disease (COPD). The aim of this study was to investigate the causes and circumstances of death in patients with COPD and chronic respiratory failure (Pa,O2 < 8.0 kPa (60 mmHg) breathing air), treated with long-term oxygen therapy (LTOT). Ten European centres participated in the study and data were c

Noninvasive mechanical ventilation in acute on chronic respiratory failure: determinants of success and failure.

Mechanical ventilation (MV) has been indicated in the treatment of acute respiratory failure (ARF) if conservative treatment fails. The recent innovations of noninvasive methods of mechanical ventilation (NMV) make it possible to avoid the complications of invasive MV, at the same time ensuring a similar degree of efficacy. A review of the literature from 1989 to 1996 shows that use of NMV in ARF has been reported in several studies involving more than 500 patients, mostly with chronic obstructive pulmonary disease (COPD). NMV was successful in 51-91% of cases, the severity of ARF being widely

Noninvasive mechanical ventilation and acute respiratory failure: indications and limitations.

Noninvasive mechanical ventilation (NMV) now represents the first step in the management of acute on chronic respiratory failure (A/CRF). During the last 5 yrs, many studies have confirmed the feasibility of NMV in an acute setting, either by facial or nasal interface, used in addition to volumetric or barometric respirators, to manage A/CRF. The best indications for NMV are slowly progressive A/CRF, frequently represented by chronic obstructive pulmonary disease (COPD), or restrictive pulmonary disease. The criteria to initiate NMV in such patients are worsening of respiratory status and arte

[Post hydatid chronic cor pulmonale].

We report a case in a patient aged 28 admitted with haemoptysis and dyspnoea. The chest x-ray showed multiple disseminated hydatid cysts throughout the lung fields. Computerized tomography and an abdominal echo found a hydatid cyst of the liver in contact with inferior vena cava which was probably responsible for the secondary pulmonary dissemination. Subsequently there was a multiplication of the lesions with rupture and infection of several cysts. After two years the disease progressed into chronic respiratory failure with the appearance of chronic cor pulmonale. The respiratory state rapidl

[Hospital admissions for respiratory diseases in the aftermath of the great Hanshin earthquake].

A questionnaire was sent to 30 hospitals and medical institutes in and around the Hanshin district, where the Great Hanshin Earthquake, occurred on January 17th, 1995. The questions concerned patients who were admitted from the day of the earthquake until 10 weeks later. Answers were obtained from 18 of the 30 facilities and the total number of patients admitted with respiratory diseases was 148. Patients with lobar pneumonia or focal pneumonia, or both, accounted for 58.8% of the total, and those with upper respiratory tract infection made up 19.6%. Patients with acute-on-chronic respiratory
