[A study on erythrocyte membrane band 3 protein anion transport function in chronic respiratory failure patients].

The changes of the structure, content and anion transport function as well as the blood gases and electrolytes inside and outside the erythrocytes were investigated in 3 groups of subjects: type I respiratory failure patients (group 1, n = 40); type II respiratory failure patients (group 2, n = 40) and controls (group 3, n = 37). The results showed that (1) anion transport function impairment of erythrocyte membrane band 3 protein and HCO3-/Cl- exchange restrain may be the reasons that aggravate CO2 retention and respiratory acidosis in cor pulmonale patients with type II respiratory failure.

[Home days ratio in patients treated with long-term oxygen therapy].

The home days ratio (days at home/observation period) is an important index for evaluating the efficacy of therapy for chronic disease. To compute the home days ratio in patients treated with long-term oxygen therapy, we retrospectively studied the records of 72 consecutive patients with chronic respiratory failure who received long-term oxygen therapy from the Department of Pulmonary Medicine, Ogaki Municipal Hospital. These patients were representative of those treated with long-term oxygen therapy in terms of age, cause of hypoxemia, blood gas values, and pulmonary function. The home days r

Elevated serum hepatocyte growth factor/scatter factor levels in inflammatory lung disease.

Hepatocyte growth factor/scatter factor (HGF/SF) plays an important role in tissue repair in liver and renal damage. The clinical significance of this growth factor in these diseases has also been reported. The lung is one of the major sources of HGF/SF; because of this, we investigated serum HGF/SF levels in 26 patients with inflammatory lung disease (15 with interstitial pneumonitis [IP], 11 with bacterial pneumonia [BP]) by enzyme-linked immunosorbent assay. As controls, we measured HGF/SF in the serum of 13 stable outpatients with chronic respiratory failure. All patients had no significan

[Chronic respiratory failure--survival for nine years with home mechanical ventilation].

We report the case of a woman, now 58 years old, with chronic respiratory failure due to spinal progressive muscular atrophy. She first noticed gradual progressive muscular weakness in her extremities in 1973. She started to complain of dyspnea on exertion in 1978. Chronic respiratory failure due to spinal progressive muscle atrophy was diagnosed in 1983. Home oxygen therapy was begun, but CO2 narcosis and exacerbation of chronic respiratory failure occurred at the end of that year. A tracheotomy was done and mechanical ventilation was begun. As her general condition improved and she could bre

Insulin resistance is not exhibited by advanced chronic obstructive pulmonary disease patients.

We have previously reported increased blood glucose concentrations and skeletal muscle glycogen depletion in severe COPD patients with chronic respiratory failure. In order to see if insulin resistance exists in severe COPD, we investigated nine patients with advanced COPD with chronic hypoxaemia and seven healthy control subjects of similar age, using the euglycaemic hyperinsulinaemic glucose clamp technique. We could not demonstrate a subnormal intravenous glucose requirement in response to insulin when maintaining euglycaemia in the COPD patients with chronic hypoxaemia. This indicates that

[Effectiveness of intermittent self-ventilation after ventilator weaning].

The essential cause for long-term mechanical ventilation with unweanability from respirator is chronic failure of the inspiratory muscles. Principally two different causes exist for chronic respiratory failure: Primary pulmonary diseases with overload or load imbalance of primarily uncompromised respiratory muscles, and neuromuscular diseases with a significant decrease in respiratory muscle capacity. Intermittent nocturnal ventilation (INV) leads to recovery by unloading the respiratory pump. In the present retrospective study we examined the value of INV in the "post-weaning-phase" for previ

Home treatment for chronic respiratory failure in children: a prospective study.

Home treatment for children with chronic respiratory failure (CRF) is increasing. However, the causes of CRF in children and the details of their home treatment are not well-known. The aim of this study was to describe the causes of CRF in the paediatric population and the treatments that the patients received at home. We surveyed all children (aged < or = 18 yrs) entering the Association Nationale pour le Traitement à Domicile de l'Insuffisance Respiratoire chronique (ANTADIR) for home treatment of CRF between March 1992 and March 1993. Two hundred and eighty seven children (178 boys, 62%) st

[Evaluation of a respiratory care unit on a 45-bed hospital ward].

With a limited number of beds for patients undergoing medical and surgical treatment for respiratory diseases, we set aside 6 of 45 beds on one floor to be used as a respiratory care unit. During the past 5 years, 1820 patients (1225 medical and 595 surgical) were admitted to the respiratory care unit; they were treated for an average of 5.02 days. Of the patients being treated medically, 451 received mechanical ventilatory support. The majority of those patients had acute exacerbations of chronic respiratory failure due to emphysema or to sequelae of pulmonary tuberculosis. Acute respiratory

[Mechanical ventilation and long-term respiratory care in the intensive care unit of a general hospital].

We studied the need for mechanical ventilation in 265 patients with respiratory failure who came to our medical ICU over the past 3 years. The time required for weaning from mechanical ventilation and the percentage of patients who needed oxygen therapy or mechanical ventilation at home after their condition was no longer acute were also studied. Of the patients treated in the medical ICU, 143 (54%) required mechanical ventilation; 104 (39%) had acute respiratory failure and the others had acute exacerbations of chronic respiratory failure. Some causes of acute respiratory failure were aspirat

[Long-term mechanical ventilation in Japan, with special reference to home mechanical ventilation].

Some patients in respiratory care units are difficult to wean from mechanical ventilation, and may be candidates for home mechanical ventilation (HMV). HMV may improve these patients' quality of life and decrease medical expenses. Since 1987, six nationwide questionnaire surveys were done to study the status of long-term mechanical ventilation (defined as mechanical ventilation for at least 90 days) and HMV in Japan. In 1994, an additional questionnaire regarding opinions about HMV was sent to physicians, patients undergoing HMV, their families, and people in companies that deal with home vent

[The effect of furosemide and valium on band 3 protein anion transport function in chronic respiratory failure patients].

In order to explore the effect of drugs on band 3 protein anion transport function and its clinical significance, several common drugs such as furosemide, hydrochlorothiazide, etc, were tested. The results demonstrated that furosemide, hydrochlorothiazide and valium could inhibit rapidly and reversibly the anion transport function of band 3 protein and led to HCO-3/Cl- exchange restriction. The mechanism of action of the drugs was thought to be an interaction between drugs and the two sites of band 3 protein anion transport system. The reason that pulmonary encephalopathy was induced by diuret

Combined lung and liver transplantation in patients with cystic fibrosis. A 4 1/2-year experience.

Patients with cystic fibrosis who have end-stage respiratory failure and associated liver cirrhosis have been considered poor candidates for lung transplantation because of high morbidity and mortality resulting from hepatic insufficiency after the operation. Since April 1989, our policy has been to combine heart-lung or lung and liver transplantation in this group of patients. Between June 1990 and March 1995, among 25 patients accepted in the program for combined transplantation, nine died awaiting transplantation and 10 underwent one of the following procedures: heart-lung-liver transplanta

Long-term nocturnal mechanical ventilation in patients with kyphoscoliosis.

1) to evaluate the long-term efficacy of nocturnal IPPV either via tracheostomy (tIPPV) or a nasal mask (nIPPV) as a means to improve alveolar ventilation in patients with chronic severe hypercapnia caused by kyphoscoliosis; and 2) to assess the effect of MV on hospitalizations and life-style. Twenty six patients with kyphoscoliosis in chronic respiratory failure were enrolled in the study. Patients were divided into two groups. The first group comprised 13 subjects who had been clinically stable for at least 1 month (arterial carbon dioxide tension (Pa,CO2) 81 +/- 1.5 kPa (60.8 +/- 10.9 mmHg)

Outcome of domiciliary nasal intermittent positive pressure ventilation in restrictive and obstructive disorders.

BACKGROUND: Nasal intermittent positive pressure ventilation (NIPPV) is a new technique which has rapidly supplanted other non-invasive methods of ventilation over the last 5-10 years. Data on its effectiveness are limited. METHODS: The outcome of long term domiciliary NIPPV has been analysed in 180 patients with hypercapnic respiratory failure predominantly due to chest wall restriction, neuromuscular disorders, or chronic obstructive lung disease. One hundred and thirty eight patients were started on NIPPV electively, and 42 following an acute hypercapnic exacerbation. Outcome measures were

[Long-term oxygen therapy at home. Compliance with the medical prescription and observance of the daily duration of at least 15 hours].

Long-term domiciliary oxygen therapy in patients with chronic respiratory failure significantly improves both survival and quality of life. These therapeutic objetives are only achieved by daily oxygen therapy of more than 15 hours. For a period of 3 months, we have prospectively measured the duration of oxygen therapy in 930 patients with chronic airflow obstruction. This is carried out by reading the meters on the oxygen concentrators, or for liquid oxygen by checking the weight of the cylinders at each delivery, making allowances for the flow rate and also for natural loss from evaporation.

Nutrition care of patients with acute and chronic respiratory failure.

Nutrition is intimately linked to pulmonary function and an understanding of these relationships have therapeutic utility. Malnutrition is known to be associated with impaired mechanical function of the lung in both chronic and acute respiratory insufficiency. Refeeding results in improvement in functional characteristics and may be critical in the weaning of patients from mechanical ventilation. In contrast, overfeeding may result in an increased ventilatory demand resulting in the inability to wean from respiratory support. This article considers the background as well as recommendations for
