In order to study the effect of the decrease in P-Pi caused by low pH on hemoglobin-oxygen affinity, we measured P-Pi,2,3-diphosphoglycerate (2,3-DPG), and oxygen tension at 50% saturation (P50) in 36 cases with acute exacerbation of chronic respiratory failure with hypercapnia. Home oxygen therapy (HOT) is important in the management of patients with chronic respiratory failure. I evaluated 159 HOT cases in our hospital during the period of 1988-1989. According to the national annual survey during the same period, HOT cases with tuberculosis sequelae were 46% of total national sanitorial HOT cases and 26-30% of all Japanese ones. A 27 year old white woman with a history of chronic eczema and episodes of serious infection of the chest, skin, and bone presented with acute respiratory failure. She was found to have a spontaneous right pneumothorax and a pneumatocele in the left upper lobe. Despite a left upper lobectomy she was left with chronic respiratory failure, bullous lung disease, and bilateral bronchiectasis. Tracheobronchomegaly is a distinct clinical and radiologic entity characterized by marked dilatation of the trachea and bronchi. In this report we describe a Marfanoid patient who developed TBM in his adulthood with rapid deterioration, leading to hypercapnic respiratory failure. This case represents an acquired form of TBM in a patient with Marfanoid features which has not been previously described. Tracheobronchomegaly should be considered as a cause of chronic respiratory failure in a young adult and can be diagnosed easily with chest roentgenogram and CAT scan.

We reported five cases of listeriosis (sepsis and meningitis) in the elderly in our hospital during the last 4 years, where no cases of listeriosis had been found. A case of a 48-year male patient with chronic cor pulmonale is presented. Exacerbation of the chronic respiratory failure was caused by pneumonia. The patient was treated with artificial ventilation for 22 days and stimulating aggressive antibacterial therapy. An improvement was achieved.
