Nocturnal BiPAP Therapy Improved Chronic Respiratory Failure in Chilaiditi Syndrome.

BACKGROUND: Chilaiditi syndrome includes the Chilaiditi sign, a radiological finding where a segment of bowel loop or small intestine has wandered between the liver and diaphragm and also any symptoms related to intestinal interference. CASE PRESENTATION: We report the case of a 79-year-old man with Chilaiditi syndrome whose dyspnea and sleep disturbance improved after nocturnal biphasic positive airway pressure (BiPAP) therapy implementation. DISCUSSION: BiPAP therapy may contribute to thoracic pressure elevation and reduce diaphragmatic compression due to intestinal enlargement, thus leadi

Response to noninvasive home mechanical vEntilation in Myotonic Dystrophy type 1: The multicenter REMeDY study.

Myotonic dystrophy type 1 (DM1) frequently leads to chronic respiratory failure, yet the effectiveness of noninvasive home mechanical ventilation (HMV) remains understudied. Our objective was to assess the effects of HMV on gas exchange, health-related quality of life (HRQL), and daily functioning, and to explore baseline predictors of treatment response. A prospective multicenter study was conducted in which clinical data, pulmonary function tests, blood gas analysis, polysomnography and overnight pulse oximetry with transcutaneous CO2 monitoring, and validated questionnaires were collected a

From Promise to Practice: Harmonizing Telemedicine in Pediatric Chronic Respiratory Diseases.

Telemedicine has the potential to substantially improve the care of children and adolescents with chronic respiratory diseases, including asthma, cystic fibrosis, bronchiectasis, and chronic respiratory failure. Digital health interventions-such as remote monitoring, virtual consultations, adherence-support tools, and educational platforms-can enhance disease control, continuity of care, and access to specialized services. Despite these opportunities, the implementation of telemedicine in pediatric respiratory care remains fragmented and uneven across healthcare systems. A central barrier to p

Emphysematous gastritis with portal venous gas: conservative management in a high-risk patient.

Emphysematous gastritis is a rare, often fatal condition characterized by gas within the gastric wall, usually in patients with significant comorbidities. We describe a 75-year-old man with incomplete tetraparesis, chronic respiratory failure and type 2 diabetes who presented with abdominal pain, vomiting and hematemesis. Initial computed tomography (CT) was reported as non-obstructive small-bowel dilatation with possible gallstone ileus, but subspecialty radiology review identified intramural gastric gas with portal venous gas consistent with emphysematous gastritis. He was managed non-operat

Real-world use of theophylline in critically ill patients with sinus pauses: a case series.

BACKGROUND: A sinus pause is is defined as a delay in sinus node depolarization exceeding 3 s following atrial depolarization. In patients with pauses greater than 5 s accompanied by hemodynamic compromise, the primary treatment is permanent pacemaker (PPM) implantation. However, in the presence of an active infection, alternative strategies such as temporary transvenous pacing or pharmacologic agents may be more appropriate. This report highlights the use of theophylline as a non-invasive therapeutic option in two patients with asymptomatic sinus pauses who had contraindications to PPM placem

ECMO use in infants with bronchopulmonary dysplasia: a literature review.

Bronchopulmonary dysplasia (BPD) is a leading cause of chronic respiratory failure in infants. Severe BPD is associated with prolonged mechanical ventilation, pulmonary hypertension, and high morbidity and mortality. As disease severity increases, extracorporeal membrane oxygenation (ECMO) is being reconsidered as a potential rescue therapy. Historically, ECMO has been reserved for neonates with reversible respiratory failure and those who required mechanical ventilation for less than 14 days, while infants with BPD were excluded. However, advancements in ECMO technology, anticoagulation strat

Enhancing Respiratory Support in COPD Patients with Nasal High Flow Using an Asymmetrical Cannula Interface: A Randomized Cross-Over Study.

PURPOSE: Nasal high flow (NHF) is increasingly used in COPD patients with chronic respiratory failure. An asymmetrical cannula interface (AI) may enhance these benefits by combining increased airway pressure with reduced rebreathing from anatomical dead space. The aim of this study was to compare the effects of NHF using an AI versus a conventional symmetrical interface (SI) on tidal volume (Vt), respiratory rate (RR), minute ventilation (MV), gas exchange, and neuro-respiratory drive (NRD). METHODS: Following a 15-minute baseline period, COPD patients recovering from acute exacerbation under

Effectiveness of exercise training in people with non-cystic fibrosis bronchiectasis with and without COPD.

BACKGROUND AND AIM: Non-Cystic Fibrosis (CF) bronchiectasis is associated with reduced exercise tolerance and symptoms such as dyspnea and fatigue, impairing functional capacity and limiting physical activity. In addition to airway clearance techniques, pulmonary rehabilitation (PR), including aerobic and resistance training, is recommended in these individuals. This retrospective, multicentric study covering a ten-year period compared in non-CF bronchiectasis people with and without COPD the effectiveness of PR, including exercise training, on exercise tolerance (primary objective) and other

Respiratory Syncytial Virus Infection Triggering a Pulmonary Hypertensive Crisis in a Boy With Prader-Willi Syndrome-Associated Sleep-Disordered Breathing.

Prader-Willi syndrome (PWS) is a multisystem genetic disorder frequently complicated by sleep-disordered breathing, including obstructive sleep apnea. Respiratory infections may precipitate acute decompensation in vulnerable patients with PWS. A four-year-old boy with genetically confirmed PWS receiving growth hormone therapy developed respiratory distress and cyanosis. He was found to be respiratory syncytial virus-positive and presented with acute respiratory acidosis and markedly elevated aminotransferases with coagulopathy. Echocardiography showed right-sided cardiac dilation consistent wi

Mapping the Evidence on Abdominal Weight Training for Respiratory Muscle Strength: A Scoping Review.

This scoping review synthesized evidence on abdominal weight training (AWT), a non-invasive method for respiratory muscle rehabilitation that increases diaphragmatic load, to evaluate its clinical significance. We specifically summarized its effects on respiratory muscle strength, measured by maximal inspiratory pressure (MIP) and maximal expiratory pressure (MEP), diaphragmatic function, and patient outcomes, including ventilator weaning. A systematic search of MEDLINE, CENTRAL, and Web of Science databases conducted on March 13, 2025, identified five studies meeting the inclusion criteria: o

Automatic alveolar ventilation-targeted NIV mode vs polysomnographic titration of fixed pressure in naive patients with chronic respiratory failure.

STUDY OBJECTIVES: Polysomnographic titration of fixed pressures of non-invasive ventilatory support (NIV) is recommended for patients with chronic hypercapnic respiratory failure (CHRF). Here we compared the effects of NIV titrated manually by PSG vs. fully automatic mode (iVAPS AE) in sleep quality, respiratory events manually scored and estimated by built-in software, nocturnal hypoventilation, daytime arterial blood gases and patient comfort. METHODS: We studied 26 adult patients (13 female) with CHRF due to COPD (n = 13), neuromuscular disease (n = 8), obesity hypoventilation syndrome (n

[Annual advances in clinical research on mechanical ventilation in management of respiratory failure 2025].

This review summarizes key advances in mechanical ventilation for respiratory failure reported in 2025. In patients with acute respiratory distress syndrome receiving invasive ventilation, the emphasis has shifted from standardized protocols to individualized lung-protective strategies, with positive end-expiratory pressure (PEEP) titration increasingly guided by respiratory mechanics and multimodal physiological parameters. Airway pressure release ventilation has emerged as a promising tool for improving dorsal ventilation-perfusion matching, while prone positioning assessment has moved towar

Mortality impact of long-term home non-invasive ventilation in COPD patients with versus without obstructive sleep apnea.

RATIONALE: Coexisting chronic obstructive pulmonary disease (COPD) and obstructive sleep apnea (OSA) is known as overlap syndrome. This may represent a distinct clinical phenotype that shows different responses after being initiated on noninvasive ventilation (NIV) for hypercapnic chronic respiratory failure. However, current data remain scarce. OBJECTIVE(S): This study estimated the impact of overlap syndrome versus COPD without OSA on transitions between 3 states (without/recovery from severe exacerbation, severe exacerbation and death) in patients initiated on domiciliary NIV therapy. Paediatric Long-Term Home Respiratory Support: Recommendations of the Swiss Society of Paediatric Pulmonology.

The number of paediatric patients evaluated for long-term home respiratory support has risen sharply in recent years, reflecting both improved survival of children with complex chronic conditions and expanded therapeutic indications. This population is characterized by substantial clinical heterogeneity, encompassing obstructive, restrictive, or central ventilatory disorders. Contributing comorbidities - such as severe obesity, chronic aspiration with secondary lung disease, neuromuscular weakness, impaired central respiratory drive, and dynamic upper airway obstruction - further complicate di

Organization and delivery of home-based care for chronic obstructive pulmonary disease patients receiving long-term home non-invasive ventilation: a systematic review.

BACKGROUND: Chronic obstructive pulmonary disease (COPD) contributes substantially to morbidity, mortality, and healthcare costs. For patients with chronic respiratory failure, long-term home non-invasive ventilation (LTHNIV) is an effective therapy, yet existing guidelines offer little direction on the most effective initiation and organization of care. We review current LTH-NIV implementation and management strategies and factors that facilitate or hinder successful treatment. METHODS: A systematic review of studies investigating the organization and implementation of LTHNIV for COPD patien

Intraoperative Ventilation Failure Due to a Fenestrated Tracheostomy Tube in a Three-Month-Old Infant with a Laryngeal Mass: A Case Report and Literature Review.

Pediatric tracheostomy is a common procedure, with a substantial proportion of cases performed on infants, often due to chronic respiratory failure or complex airway anomalies. This case report details a critical intraoperative event involving a three-month-old infant undergoing tracheostomy for a laryngeal mass. The patient experienced immediate and profound ventilation failure following the insertion of a fenestrated tracheostomy tube. Mechanical obstruction was ruled out by passing a suction catheter through the tube. Furthermore, switching ventilatory modes to pressure-controlled volume-gu
